A Study Clinical Analysis of Cervical Spine disease patient's (경추 질환환자의 임상증례 분석연구)

  • The purpose of this study was to investigate in S city of Chung-Nam using direction radiation Cervical spine among patient enforcing AP view, lateral view, total 113 subjects(40-year-old low)by target examine and got following as: 1) Distribution of age 14~40 years old 57 men (50.4%), woman 56subjects(49.6%)be, and age group 30~40year old 80subjects(70.8 %), 20~29year old 27subjects(23.9%). 2) It appeared most by 81subjects(71.7%) that can not know cause though traffic accident, misstep, cause ignorantness were investigated to revealed by the chronic 87subjects(77%) appeared by thing which statistical significance is with occurrence cause. 3) 20 Among whole 113subjects 20~29years old 27 subjects(23.9%), 30~40years old 80 subjects(70.8%) manifestationacute form of a disease, the chronic all high distributionsee. 4) This investigator left shoulder region pain among 39 subjects whole 113 subjects (left to direction that could know that is each main pain in 34.5 %), cervical vertebral portion upside 31 subjects(27.4%), both shoulder region pain 20 subjects(17.7%), and becomes left side scoliosis 62 subjects(54.9%)by the most frequencies appear. 5) Appeared antespondylolisthesis by all curve 48 subjects(42.5 %) in curve direction and at Systolic blood pressure from 160mmHg to all 19 subjects (before total 69 subjects (61.0 %) at 16.8 %), Diastolic blood pressures curve 31 subjects(27.4 %) by 100 mmHg appear.

Effects of Application of Myofascial Release of Neck and Upper Trunk on the Pain, Insomnia and Sleep Disturbances in Patients with Chronic Neck Pain (경부 및 체간 상부 근막이완기법 적용이 만성 경부통 환자의 통증, 불면증 및 수면에 미치는 영향)

  • Purpose : Chronic neck pain negatively impacts the quality of life and causes various problems in daily life due to pain, insomnia, and sleep disturbances in patients with this condition. Therapeutic interventions to solve these problems in rehabilitation and physical therapy are being introduced; however, the evidence of the efficacy of myofascial release (MFR) is still insufficient. This study aimed to investigate the effects of applying MFR on pain, insomnia, and sleep disturbances in patients with chronic neck pain. Methods : Ten patients with chronic neck pain were randomly selected and grouped into the experimental group (n1 = 10) and control group (n2 = 10) by cross-over design. Pain was measured before and after MFR intervention. Moreover, insomnia was measured only after MFR intervention. Polysomnography was performed after MFR intervention. Wilcoxon signed rank test and Mann-Whitney U test were used for the visual analog scale (VAS). Independent sample t-test was separately performed to measure insomnia and sleep. Results : After MFR intervention, the VAS score of the experimental group (p = 0.005) significantly decreased than that of the control group (p = 0.002). The insomnia score of the experimental group significantly decreased than that of the control group (p = 0.001). The total sleep time (p = 0.001), sleep efficiency (p = 0.001), and sleep latency (p = 0.001) of the experimental group significantly increased than those of the control group in the polysomnographic measurement. Conclusion : The application of MFR of the neck and upper trunk may have a positive effect on pain, insomnia, and sleep disturbances in patients with chronic neck pain. It was also suggested that an objective and quantitative polysomnography can be used more often in the field of rehabilitation and physical therapy.

  • The purpose of this study investigated cervicocephalic kinesthetic sensibility in patients with chronic neck pain and the effect of a exercise treatment and transcutaneous electrical nerve stimulation. Fourteen patients with a chronic neck pain participated in this study. Subjects were divided into three groups, one group had undergone medicine, another medicine and TENS, the other medicine and TENS and exercise. the result of this research were as follow 1. No significant differences were found in medicine group and medicine and TENS during 4 weeks follow-up(P>.05). 2. Head reposition errors were significant in medicine and TENS and exercise during 4 weeks follow-up(P<.05). 3. In a period of treatment time, significant differences were found in each groups(P<.05).

The Effects of Therapeutic Exercise on the Grip Strength of Chronic Neck Pain Patients (치료적 운동이 만성경부통증환자의 악력에 미치는 영향)

  • Purpose:The purpose of this study was to investigate effects of therapeutic exercise on the grip strength of chronic neck pain patients. Methods:Sixteen chronic neck pain patients were recruited this study(8 females. 8 males) from 21 to 30 years of age(mean aged 24.44). The experimental group received contraction-relax exercise for more than 3-5 times with therapeutic massage for 15minutes per day and three times a week during 5 weeks period. Grip strength was measured by Grip Strength Dynamometer(Model: T.K.K, 5101, Japan) at pre, 3weeks, 5weeks and follow-up 2weeks. Statistical analysis was used repeated ANOVA to know difference within period. Results:The grip strength was significantly increased within treatment period(p<.05), among them significantly increased within pre-treatment and follow-up 2weeks(p<.05). Conclusion:These results indicate that therapeutic exercise increased grip strength of chronic neck pain patients. Grip strength was more increased at follow-up 2weeks. In the future, we suggest that studies of therapeutic exercise regarding chronic neck pain patient be further studied and an appropriate therapeutic exercise will be indicated.

Effects of McKenzie Exercise on the Functional Recovery and Forward Head Posture of Chronic Neck Pain Patients. (만성경부통증 환자에 대한 McKenzie 운동이 기능 회복과 두부전방자세에 미치는 영향)

  • Purpose : To evaluate effects of McKenzie exercise on the functional recovery and forward head posture of chronic neck pain patients. Methods : The subjects were consisted of fifteen patients who had abnormal neck posture, mild neck pain (28 males, 17 females; mean aged 21.9) from 19 to 33 years of age(mean age 21.9). All subjects were received McKenzie exercise for 35 minutes with clinical massage per day three times a week during 4 weeks period. Neck disability index was used to measure functional disability level. Visual analogue scale(VAS) was used to measure subjective pain level. craniovetebral angle(CVA), cranial rotation angle(CRA) was used to measure forward head posture with digital camera. All measurements of each patients were measured at pre-treatment and after 2 week, after 4 week, post-treatment on 2 week. Result : The VAS of McKenzie exercise was significantly reduced between pre-treatment and post-treatment (p<.05) and effects of pain reduce was continued follow 2 weeks. The Neck Disability Index of McKenzie exercise was significantly reduced between pre-treatment and post-treatment (p<.05) and effects of pain reduce was continued follow 2 weeks. The CVA of McKenzie exercise was significantly reduced between pre-treatment and post-treatment (p<.05) and effects of pain reduce was continued follow 2 weeks. The CRA of McKenzie exercise was not significantly reduced between pre-treatment and post-treatment (p<.05). Conclusion : McKenzie exercise improved pain and function of Chronic neck pain patients.

  • Purpose : The purpose of this study was to examine the effect of therapeutic massage and Mckenzie exercise on pain and range of motion in chronic neck pain patient: a case study. Method : A Twenty-year old female subject with chronic neck pain was selected, she received therapeutic massage and Mckenzie exercise for 30 minutes a day, three days a week for five weeks. Pain was measured visual analog scale(VAS), neck disability index(NDI) and the Copenhagen neck functional disability scale (CNFDS). Range of motion was measured using a goniometer(shoulder joint) and the CROM Deluxe(cervical range of motion instrument). Result : VAS, NDI and CNFDS were decreased 3 score, 4 score and 6 score(respectively) than before the training. The range of motion was increased in shoulder joint and neck movement than before the training. Conclusion : The massage and Mckenzie exercise increased range of motion in shoulder joint and neck movement, and decreased neck pain in chronic neck pain patient. Additional research on therapeutic massage and Mckenzie exercise for improving pain and range of motion is need.

Effect of Craniocervical Flexion Exercise on Pain and Cross Sectional Area of Longus Colli Muscle in Workers with Chronic Neck Pain (두경부 굴곡 운동이 만성 경부통 근로자의 통증 및 경장근 단면적에 미치는 영향)

  • This study compared the effects of craniocervical flexion exercise with conservative cervical flexion exercise in workers with chronic neck pain. Subjects were randomly allocated to two groups: control (n=20) and experimental group (n=20), respectively. The conservative exercise for the control group consisted of cervical flexion exercise, whereas the experimental group performed a craniocervical flexion exercise. To compare the two groups, the visual analogue scale (VAS) for a pain, neck disability index (NDI) for a neck disability scale, and cross sectional area (CSA) of longus colli muscle using ultrasound image were assessed on pre-intervention, post-intervention, and 2 weeks follow-up. 2-way repeated ANOVA was used with Bonferroni post-hoc test. (1) There were significant main effects (within and between groups) (p<0.05) and interaction effect (p<0.05) in VAS. Post-hoc test revealed that there were significant differences in all pair-wise comparisons. (2) There were significant main effects (within and between groups) (p<0.05) and interaction effect (p<0.05) in NDI. Post-hoc test revealed that there were significant differences between pre-intervention and post-intervention, pre-intervention and 2 weeks follow-up. (3) There were significant main effects (within and between groups) (p<0.05) and interaction effect (p<0.05) in CSA of longus colli muscle. Post-hoc test revealed that there were significant differences in all pair-wise comparisons. The findings indicate that craniocervical flexion exercise are more effective for improving pain, neck disability, and CSA of longus colli muscle than cervical flexion exercise in workers with chronic neck pain.

Effects of Cervical Stabilization Exercise Using Sling on Hamstring Flexibility in Patients with Chronic Neck Pain (슬링을 이용한 경부 안정화 운동이 만성 목통증환자의 넙다리뒤근 유연성에 미치는 영향)

  • Purpose: The primary purpose of this study was to investigate the effects of cervical stabilization exercise (CSE) on hamstring flexibility in patients with neck pain. A secondary purpose of this study was to investigate the effects of cervical range of motion (CROM) and craniovertebral angle (CVA). Methods: This study was a single-blind, randomized, comparative trial. Twenty patients were allocated into either the cranio-cervical flexion exercise (CCFE) group or the CSE group. Before and after the intervention, we measured straight leg raise (SLR), popliteal angle (PA), CROM, and CVA in the sitting and standing positions. Fisher's exact test, the Mann-Whitney test, and Wilcoxon's signed-rank test were used to analyze our data. Results: Both groups showed significant improvements in the value of SLR, PA, cervical extension, cervical rotation, and CVA in the standing position (both, p<.05) after intervention. However, only the CSE group showed significant improvements in cervical right lateral flexion (z=-2.209; p<.01) and cervical left lateral flexion (z=-2.537; p<.05) after intervention. The CSF group showed more significant improvements in SLR, PA, both cervical lateral flexions, and both cervical rotations than the CCFE group. Conclusions: The results of this study will guide future research in identifying the effectiveness of CSE. In conclusion, it can be inferred that CSE has a positive effect on SLR, PA, CROM, and CVA in the standing position in patients with chronic neck pain.

Effects of Dynamic Balance Exercise on Pain, Functional level, and Psychosocial Level in Patients with Non-specific Chronic Neck Pain (비특이성 만성 경부통 환자에게 적용한 동적 균형 운동이 통증과 기능적 수준, 심리사회적 수준에 미치는 효과)

  • Background: Patients with neck pain develop instability due to muscle imbalance, decreased proprioception, and balance disorders. Studies have examined various exercise methods as treatment methods, but few studies have compared the effects of cervical stabilization exercise and dynamic balance exercise. The purpose of this study was to investigate the effects of dynamic balance exercise on pain, functional level, and psychosocial level in patients with non-specific chronic neck pain. Methods: Thirty-four non-specific chronic neck pain patients were randomly assigned to the experimental group (EG, n=17) and control group (CG, n=17); the cervical stabilization exercise and dynamic balance exercise program were applied to the EG; and only the cervical stabilization exercise program was applied to the CG. The intervention was conducted twice a week, for six weeks. Assessment items evaluated pain, dysfunction (Korean version neck disability index), range of motion, craniocervical flexion test, cervical deep flexor endurance test, and psychosocial level. Data analysis was performed using intention-to-treat analysis as assigned. To analyze differences in the items assessed in the two groups, we used a repeated measures analysis of variance with an interaction between group (EG, CG) and time point (baseline, 6 weeks, 12 weeks). Results: The endurance of the cervical flexor muscles between the group and the measurement point after intervention (p<.05). Both groups showed significantly improved endurance between time points after the intervention (p<.05), with the EG showing a greater change than the CG. None of the other measurement items differed in the pattern of change between measurement points. Conclusion: In conclusion, the EG applying a cervical stabilization exercise and a dynamic balance exercise experienced a significant difference in muscle endurance improvement compared to the CG. We propose an exercise intervention program that includes stabilization exercises and dynamic balance exercises for patients with chronic cervical pain who lack muscle endurance.

Osteoid Osteoma Around the Hip Joint (고관절 주위에 발생한 유골 골종)

  • Purpose: We experienced 8 cases of osteoid osteoma arising around hip joint which had some characteristics that differ from those arising on long bone. we reports the characteristics of osteoid osteoma arising around the hip joint. Materials and Methods: 8 cases of osteoid osteoma were diagnosed during 1985 to 2004 at hanyang university hospital. all cases were comfirmed patholgically. 6 cases were male, 2 cases were female patients. The mean age was 17 years old (ranged from 8 to 29). They occurred in intertrochanteric area (4 cases), subtrochanteric area (2 cases), acetabulum (1 case) and femoral neck (1 case). We used radiologic tools including magnetic resonance image, computed tomography, bone scintigraphy. clinicopathologic test including erythrocyte sedimentation rate and Creactive protein. Results: The patients expressed various symptoms including thigh pain, knee pain, low back pain and radiating pain respectively. 2 patients had experienced operation on knee joint. 3 patients showed limping gait. Aspirin relieved the pain in 3 patients. The difference in circumference was 1cm between both thighs in 2 cases. Conclusion: Patients with osteoid osteoma arising around hip joint which have various symptoms such as severe knee pain and claudication, differ from infectious disease by clinicopathologic test including erythrocyte sedimentation rate and C- reactive protein and had better diagnostic result in computed tomography.
