Purpose: The purpose of this study was to identify effects of a strengthening program for the lower back in older women with chronic low back pain. Methods: The research design was a nonequivalent control group pretest-posttest experiment. The experimental group consisted of 16 older women and the control group, 14, all of whom had experienced low back pain for at least 3 months. The strengthening program for the lower back included lumbar stabilization exercises and education on pain management in daily living. For an 8 week period, exercises were done 3 days a week and on one day education was also given. Results: Pain and disability scores decreased significantly in the experimental group compared to the control group. Flexibility, life satisfaction and lumbar muscle strength scores increased significantly in the experimental group compared to the control group. Conclusion: Low back pain and disability can be relieved, and flexibility, muscle strength, and life satisfaction increased through a program to strengthen the lower back. It is suggested that a program to strengthen the lower back would be an effective nursing intervention for older women with low back pain.
Purpose: The purpose of this study is to understand the level of fatigue and daily living activities and to confirm their relationship in patients with chronic liver disease. Method: The sample consisted of 182 patients with chronic liver disease with a mean age of 42.4 years, who visited the D University Hospital and a S clinic from February, 2004 to March in 2004. Data were collected by a self reporting questionnaire on the selected variables such as fatigue and daily life activities. Collected data were analyzed by using SPSS program for descriptive statistics, Pearson's correlation coefficients, t-test, ANOVA. Results: Subjects showed the mild level of fatigue and slightly high level of daily living activities. They also showed a negative relationship between fatigue and daily living activities. Conclusion: It is necessary to develop nursing intervention program to reduce the level of fatigue for improving the level of daily living activities in patients with chronic liver disease.
Purpose: This study was done to investigate the correlation among symptom experience, self-efficacy, depression, and medication adherence in patients with chronic obstructive pulmonary disease (COPD). Methods: Participants were 100 patients with COPD recruited in one general hospital in Seoul. A structured questionnaire was used to measure the study variables. Data were analyzed using descriptive statistics, t-test, ANOVA, and Spearman correlation coefficient. Results: Symptom experience and depression were positively correlated (r=.41, p<.001), symptom experience and self-efficacy were negatively correlated (r=-.21, p=.035). Depression was negatively correlated with self-efficacy (r=-.60, p<.001) and medication adherence (r=-.48, p<.001). Self-efficacy and medication adherence were positively correlated(r=.76 p<.001). Conclusion: Findings from this study indicate that depression and self-efficacy are important variables related to medication adherence in patients with chronic obstructive pulmonary disease. Therefore, depression and self-efficacy should be assessed, and customized nursing interventions should be provided in order to increase medication adherence.
Purpose: To develop and test the validity and reliability of the Korean version of the Family Management Measure (Korean FaMM) to assess applicability for families with children having chronic illnesses. Methods: The Korean FaMM was articulated through forward-backward translation methods. Internal consistency reliability, construct and criterion validity were calculated using PASW WIN (19.0) and AMOS (20.0). Survey data were collected from 341 mothers of children suffering from chronic disease enrolled in a university hospital in Seoul, South Korea. Results: The Korean version of FaMM showed reliable internal consistency with Cronbach's alpha for the total scale of .69-.91. Factor loadings of the 53 items on the six sub-scales ranged from 0.28-0.84. The model of six subscales for the Korean FaMM was validated by expiratory and confirmatory factor analysis (${\chi}^2$ <.001, RMR<.05, GFI, AGFI, NFI, NNFI>.08). Criterion validity compared to the Parental Stress Index (PSI) showed significant correlation. Conclusion: The findings of this study demonstrate that the Korean FaMM showed satisfactory construct and criterion validity and reliability. It is useful to measure Korean family's management style with their children who have a chronic illness.
The purpose of this study was to discover the experience of hemodialysis which clients confronted hemodialysis at the same time as CRF diagnosis and to understand the nature and meaning of their crisis experience. The research subjects were 6 clients receiving hemodialysis in Seoul from Jan. 1996 to Jul. 1996. Data were collected by informal indepth interview and participant observation. Content Analysis(by Seaman & Verhonick, 1982; Woods & Catanzaro, 1988) was applied to collect similar contents and common experience in order to derivate concepts and categories for better understanding of their hemodialysis experience. As a result, 6 categories derivated to indentify their hemodialysis experience of the CRF clients-confronted hemodialysis at the same time as CRF diagnosis-were as follows: 1) The category of shocking crisis composed the concepts of shock, amagement and suffocation. 2) The category of denial composed the concepts of disease refusal and hemodialysis refusal. They repeatedly visited hospitals or didn't visit hospital in order to refuse disease, then depended on folk remedy or shamanistic method. 3) The category of severe anxiety composed the concepts of abandoned feeling, shame, resentment, neurosis and anger. 4) The category of depression composed the concepts of grief, suffering of unfairness, tearing, desire to death. 5) The category of powerlessness composed the concept of hopelessness. 6) The category of resigned acceptance composed the concepts of resignation and acceptance. In this study, the CRF clients who confronted hemodialysis at the same time as CRF diagnosis experienced six stages in accepting hemodialysis but these stages were mingled simultaneously and went on.
Purpose: The purpose of this study was to understand the degree of pain belief in musculoskeletal patients, and to identify the correlation with chronic pain, pain coping and pain disability. Methods: A total of 203 inpatients or outpatients with chronic pain in orthopedics agreed voluntarily to participate in this study and answer a questionnaire. Data were analyzed using SPSS/WIN 17.0 program with descriptive statistics, t-test, ANOVA, Scheffe test, Pearson's correlation coefficient, and simple linear regression. Results: The degree of pain belief in this study showed statistically significant differences depending on their age, education, job, health status, and pain duration. Among the variables correlating with pain belief in this study, there were positive correlations between pain and pain disability, pain and passive coping, pain belief and passive coping, pain belief and pain, pain belief and pain disability. The strongest correlation was passive coping and pain disability. Conclusion: From the results of this study, we concluded that it is necessary to develop the nursing intervention which can help reducing negative pain belief in patients with chronic musculoskeletal pain. Also we need to enhance the ways of coping to active or chronic pains for controlling them effectively.
Purpose: The purpose of this study was to examine the satisfaction level of care services depending on the activities of daily living and types of care services for inpatients with chronic disease. Methods: The participants consisted of 156 chronically ill patients. A structured questionnaire and personal interviews were used for collecting the data. The data were analyzed using SPSS (ver. 12.0) program for frequency, t-test, ANOVA, multiple regression analysis. Results: The chronically ill patients taken care of by family members were more satisfied with care services than those taken care of by paid caregivers. Also, the more able the patients were for their daily living activities, they were more satisfied with care services. In addition, type of care services was the most important factor in terms of affecting patients' satisfaction with care services most. Conclusion: The result of this study suggests that nurses need to pay more attention to those with low activities of daily living and taken care of by paid caregivers, who have the probability of low satisfaction of care services.
Purpose: The aim of this study was to test the effects of passive exercise on upper extremities muscle spasticity, finger edema, and depression for chronic stroke patients. Methods: A non-equivalent pretest-posttest design was employed for this study. The subjects were 30 elderly patients who were hospitalized to treat chronic stroke. 15 patients were assigned to the experimental and control groups respectively. Passive exercise for 20 minutes per day, five days a week for 8 weeks (total 40 times) was provided for the experimental group. Outcome measurements included manual spasticity test to measure upper extremities' muscle spasticity, the ring measurement method for finger edema and the Korean version of Short-form Geriatric Depression Scale (SGDS-K) for depression. Results: The upper extremities' muscle spasticity (Z=-2.52 p=.012) and the degree of depression (F=5.56, p=.006) in the experimental group were significantly reduced compared to those of the control group. But the degree of finger edema did not significantly differ between the two groups (F=1.46, p=.240). Conclusion: Passive exercise for upper extremities should be encouraged for elderly patients with chronic stroke to enhance the upper extremities' functional capacity as well as depression.
본 연구의 목적은 고혈압 및 당뇨병 환자의 건강 관련 의사결정에 대한 참여경험을 탐색하는 것이다. 당뇨병 또는 고혈압의 만성질환을 진단받고 1년 이상 보건소에 정기적으로 방문하였던 장년기 환자 11명이 참여하였다. 자료의 수집은 개별 인터뷰를 통해 시행되었으며, Braun과 Clark 가 제시한 주제 분석법을 적용하여 분석되었다. 연구 결과 6개의 하위주제와 3개의 주제가 도출되었다. 각각의 주제는 '짧은 면담시간을 효율적으로 활용하기 위해 노력함', '의료인에게 한정된 영역 안에서 응답함', '건강관리에 대한 지속적인 동기부여가 결여됨'이었다. 본 연구 결과는 만성질환자의 의사결정에 대한 참여를 독려하기 위해 정확하고 충분한 건강정보를 제공하고 지지적인 태도로 환자의 동기를 부여해야 할 필요성을 제시하였다.
Purpose: The purpose of this study was to compare the needs of case management between Medical aid beneficiaries with simple and multiple chronic diseases (SCD vs MCD). Methods: The study employed secondary analysis method using a cross-sectional data from 2009 case management service enrollees. Data on 35,862 beneficiaries who have chronic disease(s) were used in the description of chronic disease characteristics, and data on 20,392 beneficiaries, excluding those who have depression and/or disabilities, were used to compare the group differences. Results: Mean age was $68.8{\pm}11.63years$, and 73.3% were females. MCD group showed an older age, had more women than SCD group. Self-care ability and appropriateness of health care utilization were significantly different between the groups, but there was no difference in health-related quality of life. In subscales, there were significant differences in general health status, depression, symptom management, healthy lifestyle, hygiene and vaccination, and appropriateness of health utilization. Conclusion: Different characteristics between patients with simple and multiple chronic diseases indicate that different case management approaches are required for these groups. The study results could be used as a basis for the development of case management model tailored to the characteristics and needs of medical-aid beneficiaries.
