Development of PVDF sensor and system to detect breathing sounds during deep sedation (진정 마취 시 호흡음 검출을 위한 PVDF 센서 및 시스템 개발)

  • Respiration is one of the important vital signs to determine the condition of the patient. Especially during deep sedation, since the patient's apnea and hypopnea are difficult to detect without continuous monitoring, there is a need for a continuous respiration monitoring method that can accurately and simply determine the patient's respiratory condition. Currently, respiration monitoring methods using various devices have been developed, but these methods have not only late response time but also low reliability at the clinical stage. In this study, attachable sensor using PVDF(polyvinylidene fluoride) film and a monitoring device which could detect abnormal symptoms of breathing in early stage during deep sedation. The results of this study can be used in various medical fields including not only in the area of remote monitoring for respiration related sleep monitoring but also in routine monitoring during deep sedation.

  • Generally an operator would take notice at putting a patient under anesthesia. If the operation is executed in mistake, the patient is exposed to danger. The object of this Paper is that a system is developed for an accuracy of system and a convenience of user interface to prevent an operation of several elements of risk by mistake. The part of electrical system particularly is made for convenience of a manipulation using electrical switch and encoder. A real-time monitoring system is developed for an airway pressure and a gas concentration of carbon dioxide of patient using graphic LCD(liquid crystal display). Moreover, this flow control system could be developed control with accuracy by feedback control method. This is implemented using flow control valve and flow sensor. The implemented system gives convenience and precision of a manipulation of variable value using developed technique. This system shows guaranteed stabilization and confidence of anesthesia ventilator by notifying us that patient's state and information in case of being out of alarm range of variable value.

Effect of anesthesia nursing practice training program under perianesthesia nurses in Korean

  • This study is an experimental design study before and after an unequal control group to confirm the effect of the anesthesia nursing practice education program on the patient safety culture and organizational communication satisfaction of the perianesthesia nurse. Data was collected from 56 perianesthesia nurses nationwide through a questionnaire. The data were analyzed using the SPSS 27.0 program to analyze with descriptive statistics. As a result, the patient safety culture interaction between time and the experimental group (F=6.04, p=.017) and the organizational communication satisfaction score interaction were significant (F=4.97, p=.030). This study is of nursing significance in that a clinical practice education program for anesthesia nursing of perianesthesia nurses was developed and applied to the practical field. Based on the results of this study, it is suggested to develop and apply various practical adaptation programs to improve the practical capabilities of working nurses in the PACU using rapidly developing medical equipment.

Analysis stages of anesthesia with Bispectrum Coherence and DFA algorithm of the EEG (뇌파신호의 바이스펙트럼 Coherence와 DFA 알고리듬을 이용한 마취단계 분석)

  • Due to the anesthesia process is inappropriate on the operation, awakening state was appeared. To prevent the state, it is necessary to monitor the patients by measuring the depth of anesthesia. In this study, we investigate the possibility of the development of actual surgery available quantitative indicators. The DFA which is included the correlation property of the EEG is used to analysis the depth of anesthesia and bispctrum index. In the results, at the pre-operation, the peak of bispectrum was widely distributed, DFA value was decreased. At the during operation, bispectrum was concentrically appeared in the low frequency area. At the post operation, bispectrum and DFA was both returned to the pre-operation state. We confirmed to be close correlation between the peaks of the bispectrum and DFA value.

To prevent the state, it is necessary to monitor the patients by measuring the depth of anesthesia. In this study, we investigate the possibility of the development of actual surgery available quantitative indicators. The DFA(detrended fluctuation analysis) which is included the correlation property of the EEG is used to analysis the depth of anesthesia and bispctrum index. Convergent factors affecting length of stay in the postanesthesia care unit among elderly from general anesthesia patients (노인 전신마취 환자의 회복실 체류에 영향을 미치는 융합적 요인)

  • This study is a descriptive investigation for understanding factors influencing the length of stay in the postanesthesia care unit of elderly patients. Retrospective investigation was conducted on 300 patients aged 65 or older among those who had received an operation under general anesthesia and treated in postanesthesia care unit of hospital C located in Gyeonggi-do, from January 1 to December 31, 2014. The patients' average postanesthesia care unit stay was found to be 48.4 minutes and the 30-59 minute section accounted for the highest part with 58.0%. The postanesthesia care unit stay time according to intra-operative factors showed significant differences depending upon muscle relaxants, transfusion, ABGA, body temperature and total hours under anesthesia. Concerning the post anesthesia care unit stay length according to post-operative factors, significant differences were observed depending upon complications, PCA device, and circulatory drug use. In order to find out factors influencing the post anesthesia care unit stay length, the multiple regression analysis was conducted. As a result, the circulatory drug use and intra-operative lower body temperature were found to have an effect on the post anesthesia care unit stay length with the total explanatory power of 13%. Based on these findings above, it is deemed helpful to carefully monitor factors related to the post anesthesia care unit stay length and provide swift response accordingly for shorter post anesthesia care unit stay time of elderly general anesthesia patients.

Effect of Medetomidine and Combination of Medetomidine/tiletamine/zolazepam and Medetomidine/tiletamine/zolazepam/tramadol on Echocardiographic Cardiac Contractility in Dogs (개에서 Medetomidine, Medetomidine/tiletamine/zolazepam 합제, Medetomidine/tiletamine/zolazepam/tramadol 합제가 심장초음파 상 심장 수축력에 미치는 영향)

  • This study evaluated the myocardial performance on echocardiography after the sedation/anesthesia of medetomidine (D), the combination of medetomidine and tiletamine/zolazepam (DZ), and the combination of medetomidine, tiletamine/zolazepam and tramadol (DZT) in Beagle dogs. Ten healthy adult Beagle dogs (weighing $8.6{\pm}1.0kg$) were enrolled in this study. Heart rate (HR), fractional shortening (%FS), left ventricular ejection fraction (%LVEF), stroke volume (SV), cardiac output (CO), left ventricular internal diameter in systole (LVIDs) and left ventricular internal diameter in diastole (LVIDd) using M-mode echocardiography were measured prior to anesthesia, then every 10 min for 60 min. The HR, %FS, %LVEF, SV and CO were significantly decreased during sedation/anesthesia with D, DZ and DZT combination of anesthesia. Although those anesthetic protocols provided acceptable quality of sedation/anesthesia, levels of cardiovascular suppression were substantial and persistent and thus the continuous monitoring on vital signs should be accompanied in any situation. Close attention is required for dogs with pre-existing heart diseases, when those anesthetic protocols were applied.

Anesthetic and Cardiorespiratory Effects of Medetomidine-Ketamine-Butorphanol and Xylazine-Ketamine-Butorphanol in Dogs (개에서 Medetomidine-Ketamine-Butorphanol과 Xylazine-Ketamine-Butorphanol의 마취 효과 및 심폐에 미치는 영향)

  • This study examined the anesthetic and cardiopulmonary effects of xylazine or medetomidine in combination with ketamine-butorphanol in dogs. Five dogs were used in both the medetomidine-ketamine-butorphanol (MKB) group and the xylazine-ketamine-butorphanol (XKB) group. The procedures for the two groups were performed 4 weeks apart. MKB group showed a shorter duration for anesthesia than XKB group. Other factors were not statistically significant between the two groups. The MKB group showed signs of bradycardia, therefore cautious patient monitoring is necesessary. The XKB showed a longer anesthetic time and less adverse effects, however the MKB combination was more expensive and had less advantages. In conclusion, the results suggested the recommended use of both MKB and XKB in procedures that need approximately 50 minutes. If patients have a risk of bradycardia, one should be cautious of using a medetomidine-xylazine-butorphanol combination. Both MKB and XKB did not have much adverse effects; however MKB did not have advantages when compared to XKB. Therefore, XKB may be more effective when compared to MKB.

DENTAL TREATMENT OF A PATIENT WITH CATCH22 SYNDROME UNDER GENERAL ANESTHESIA: A CASE REPORT (CATCH22 syndrome 환아의 전신마취 하 치아우식 치료: 증례보고)

  • CATCH22 syndrome or DiGeorge syndrome is a medical acronym of cardiac defects, abnormal facial appearances, thymic hypoplasia, cleft palate, and hypocalcemia. Patients with CATCH22 syndrome are susceptible to infection due to an absent or hypoplastic thymus and often have difficulties in maintaining good oral hygiene, which may require dental treatment. We present a case of dental treatment for the uncooperative child with CATCH22 syndrome under general anesthesia. A 4-year-old, 14.8 kg boy with CATCH22 syndrome visited Seoul National University Dental Hospital for dental check up. The patient had multiple caries requiring dental treatment. He experienced the corrective heart surgery due to Tetralogy of Fallot a few years ago. General anesthesia was planned because his heart rate and vital sign had shown unstable during the previous conscious sedation procedure. Dental restorative treatments were successfully performed and no complications were observed during and after the procedure. Safe and effective dental management of the patients with CATCH22 syndrome could be performed with the help of general anesthesia and careful monitoring.
