  • The aim of this study is to review the mannequin simulation. The mannequin simulation is widely used in the education of paramedic, fire fighter, nurse, dental school, and anesthesia. The wide scopes of mannequin simulation comprise the history, evolution, and application to problem-based learning.

  • Simulators were introduced in education as a tool to make advanced training standardized, less expensive, and without danger to those involved. In 1922 in the Unites States, Edward Link presented his homemade flight simulator, which became common place in both military and civilian aviation, known as the "Link Trainer". The development of mannequin simulators used for medical simulation education, training, and research is reviewed, tracing the motivations, evolution to commercial availability, and efforts toward assessment of efficacy of those for teaching cardiopulmonary resuscitation(CPR) for medical personnel and emergency medical technicians(EMT), cardiology skills, anaesthesia clinical skills, and crisis management. This study will provide a brief overview of simulators and trainers in several domains.

Convergence Effect of Simulation Training on Bleeding Amount Estimation in Trauma (시뮬레이션 교육이 외상성 출혈의 출혈량 추정에 미치는 융합적 영향)

  • The purpose of the study is to examine the visual evaluation of bleeding amount in hemorrhagic shock in paramedic students. Through manikin simulation training, paramedic students were able to have confidence with estimation of bleeding amount in the prehospital settings. Three rescue training manikins were placed in supine position and artificial blood was poured between the abdomen and pelvis. The bleeding evaluations of 700 mL, 1200 mL, and 1700 mL were performed before and after simulation training. Paramedic students underestimated the amount of bleeding in the trauma situation, and it was found that it was difficult for the students to evaluate the accurate amount of bleeding with a single simulation.

  • 본 연구는 병원전 단계 기도기 시뮬레이션 교육에 대한 연구로 연구대상은 응급구조학과 4학년 학생 22명으로 구성하였다. 교육시간은 10분 동안 3개의 기도유지기에 대한 전체 강의가 있었고, 10분 동안 마네킹을 이용하여 세 가지 기도기의 사용방법을 교육하고 실습을 시행하였다. 응급구조학과 학생을 대상으로 Airway management trainer Simman를 이용한 기도 내 이물 제거 모델에서 매킨토시후두경과 GlideScope and the AWS$^{(R)}$, McGrath$^{(R)}$ 비디오 후두경의 기관내 삽관 성공률과 삽관 시간, 시행자의 선호도를 비교한 결과 의미 있는 차이를 보였다. 마네킨을 이용한 병원전 기도기인 1차 시도에서는 유의한 차이를 보였지만 2차에서 숙련이 된 경우이라서 큰차이를 보이지 않았다. 기관내 삽관 기구의 선호도 조사에서는 Pentax AWS를 가장 선호하였다.

Efficacy and Usability of Patient Isolation Transport Module for CBRN Disaster : A Manikin Simulation Study (특수재난 대응 환자 격리 이송 장비의 효율성 및 편의성 평가: 마네킹시뮬레이션 연구)

  • In Chemical, Biological, Radiological and Nuclear (CBRN) disaster, integrated and optimized equipment package including stretcher, isolation unit, patient monitoring and treatment equipment is essential to achieve proper treatment and prevent secondary contamination. The purpose of this study was to evaluate the efficiency and ease of use of integrated CBRN disaster equipment package for disaster medical response. This study was a randomized crossover study using a manikin simulation for emergency medical technitian (EMT). All participants used the existing devices and prototype of integrated CBRN disaster equipment package alternately. Efficiency was measured by time from vital sign change to detection or treatment application. Ease was use was measured by questionnaires for each patient monitor, stretcher care and isolation unit. 12 EMTs were enrolled. hypoxia-detection time of integrated equipment group was significantly shorter than existing equipment group (4.9 s (3.8-3.9) vs 3.5 s (2.5-3.9), p < 0.05). There was decreasing tendency of ECG change detection and facial mask oxygen supply but no statistical significance was observed. Overall satisfaction of patient monitoring device in integrated equipment group was significantly higher than existing devices (4(3.5-5) vs 3(3-3), p < 0.05). The use of integrated CBRN disaster equipment package shortened the hypoxia detection time and improved usability of vital sign monitor compared to existing devices.

Comparison of Training Effectiveness for IV Injections: Intravenous (IV) Arm Model versus Computer Simulator (마네킹 모델과 컴퓨터 시뮬레이터를 이용한 정맥주사 실습교육의 효과 비교)

  • Purpose: The purpose of this study was to compare the effectiveness of training using an intravenous (IV) arm model versus a computer simulator for IV injections. Method: Study was a quasi-experimental study conducted with 106 nursing students. Participants were divided into two groups: the IV Arm Group using a mannequin arm model (control group) and the Computer Simulator Group using the Virtual IV demonstration (experimental group). Theoretical lectures and video presentations on IV injections were given to both groups. Each group went through the training practice using the IV arm or computer simulator. After the completion of training, questionnaires were given to the students to evaluate their learning attitudes and experiences, self-confidence in IV injection, and satisfaction with the training materials. Results: Student satisfaction with the training materials including the reality, usefulness, and educational effects showed notable differences between the two groups with the Computer Simulator group reporting more positive effects that the IV Arm group. However, there was no statistical difference between the two groups in the categories of learning attitude, learning experience, or self-confidence. Conclusion: While there was a differences in strengths and weaknesses of the two methods, both methods should be considered for practice and further study needs to be done on educational effectiveness.

The effect of portal compression sensor on the quality of chest compressions during cardiopulmonary resuscitation (CPR): A mannequin based simulation study (심페소생술 시행 시에 휴대용 압박 센서 활용이 흉부압박의 질에 미치는 영향: 마네킹 기반 시뮬레이션 연구)

  • This study is to collect a basic data of how Cardiopulmonary Resuscitation (CPR) procedure can influence to cardiac arrest patient with and without the Depth Device during the average transport time period. The data has achieved by comparing result sheet of CPR procedure by hands only versus with Depth Device by twenty 1st and 2nd class Emergency Medical Technician (EMT) from five different fire stations in city of Chong-Ju, and twenty Emergency Rescue major students who completed the BLS provide course. The experiment participators experienced loss of compression depth and rate increase over time. However, the CPR procedure with Depth Device shows that both EMT and students to allow maintaining both the compression depth and rate. The experiment leaves a positive result for CPR operators and considers being valuable domain for cardiac arrest patient.
