Streptococcal Toxic Shock Syndrome Occurred during Postoperative Radiotherapy in a Cancer Patient with Preexisting Lymphedema and Chronic Illness -Case Report- (수술 후 림프부종과 만성질환을 동반한 종양 환자에서 방사선치료 기간 동안 발생한 연쇄구균독소충격증후군 예)

  • A case is reported of a man with malignant fibrous histiocytoma (MFH) in right thigh who developed streptococcal toxic shock syndrome (STSS) during postoperative radiotherapy. Before radiotherapy, a patient complained wax and wane lymphedema following wide excision of tumor mass which was confirmed as MFH. He took some nonsteroidal antiinflammatory drug (NSAID) for about one month. He suffered preexisting hepatitis C virus (HCV) infection, diabetes and well-controlled hypertension. The patient received conventional radiotherapy to right thigh with a total dose of 32.4 Gy at 1.8 Gy per day. At last radiotherapy fraction, cutaneous erythematous inflammation was suddenly developed at his affected thigh. At that time, he also complained of oliguria, fever and chills. The patient was consulted to internal medicine for adequate evaluation and management. The patient was diagnosed as suggested septic shock and admitted without delay. At admission, he showed hypotension, oliguria, constipation, abnormal renal and liver function. As a result of blood culture, Streptococcus pyogenes was detected. The patient was diagnosed to STSS. He was treated with adequate intravenous antibiotics and fluid support. STSS is one of oncologic emergencies and requires immediate medical intervention to prevent loss of life. In this patient, underlying HCV infection, postoperative lymphedema, prolonged NSAID medication, and radiotherapy may have been multiple precipitating factors of STSS.

  • Primary intestinal lymphangiectasia is a congenital lymphatic disorder in which intestinal lymphatic channels are dilated and ruptured resulting in loss of protein, lipid, and lymphocyte into the intestine or peritoneum. As a result, hypoalbuminemia, generalized edema, diarrhea are clinically manifested. We report a case of primary intestinal lymphangiectasia with generalized edema which occurred in a 7-year old boy who was treated with lipid restriction diet with medium chain triglyceride oil supplement.

Acupotomy and venesection in Upper Limb Lymphedema and Peripheral neuropathy following Breast Cancer Surgery (유방암 수술 후 발생한 림프부종과 말초신경병증에 대한 도침술과 정맥자락술 집중치료 증례보고)

  • Purpose: In order to estimate clinical effects of acupotomy and venesection in a patient with peripheral neuropathy and upper limb lymphedema following breast cancer surgery. Methods: From 17th August, 2009 to 29th August 2009, 1 female patient with peripheral neuropathy and upper limb lymphedema following breast cancer surgery was treated with general oriental medicine therapy(acupuncture, moxibustion, cupping, physical therapy, herbal medication) and acupotomy with venesection. Results: The patient's chief complaints- Lt hand numbness, Lt arm edema, Lt. wrist flexion limitation - were notably improved. Conclusions : This study demonstrates that oriental medical treatment with acupotomy and venesection therapy has significant effect in improving symptoms of peripheral neuropathy and upper limb lymphedema following breast cancer surgery, as though we had not wide experience in this treatment, more research is needed.

A Case Report of lymphedema due to axillary lymph node dissection (유방암 절제술 후 발생한 림프부종 환자의 치험1례)

  • Objectives : The purpose of this case is to report the improvement of a patient with lymphedema due to axillary lymph node dissection after acupuncture therapy and herbal medicine. Methods : We treated the patient with Dong-Qi acupuncture, herbal medication(木通大安湯), lymph absorption massage, compresses, abdominal breathing. We measured the circumference of the patient's elbow and wrist every day at 6pm. Results : Since being hospitalized, the circumference of the patient' elbow and wrist has been decreased and VAS score has gotten lower gradually. Conclusions : We considered Dong-Qi acupuncture, herbal medication had a useful effect on lymphedema due to axillary lymph node dissection with lymph absorption massage, compresses, abdominal breathing.

Two Cases of Cancer-induced Lymphedema Patients Treated with Oryeongsan (종양으로 인한 이차성 림프부종에 대한 오령산 치험 2례)

  • Objectives : The purpose of this study was to report two cases which suggest Oryeongsan as a pharmacological treatement option for cancer-induced secondary lymphedema. Methods : The cases were two female patients with peripheral edema. The first was diagnosed with cervical cancer with paraaortic lymph node metastasis and suffered from bilateral edema of the lower extremities. The second was a breast cancer patient with right arm lymphedema, which was aggravated after a weekly regimen of chemotherapy. Oryeongsan was orally administered for 3-4 days to both patients. Results : The symptoms in lower extremities and body weight of the cervical cancer patient decreased. The breast cancer patient did not lose body weight, but pain in upper extremities was reduced. Conclusions : Oryeongsan may potentially be an option for secondary lymphedemas, including cancer-induced lymphedema.

The Influence of 4 wks Complex Therapeutic Exercises on Visual Analog Scale of Pain and Range of Motion for Middle-Aged Women with Breast Cancer-Related Lymphedema (4주간 복합 운동치료가 유방암 림프부종 중년여성의 통증, 견관절 가동범위에 미치는 영향)

  • PURPOSE: This study aimed to investigate the effect of complex theraputic exercise on visual analog scale of pain, shoulder' range of motion for middle-aged women with breast cancer related lymphedema. METHODS: The 14 middle-aged women involved voluntary in this study and then they were divided into two groups(n =7 per group). The complex exercise program was implemented over 4 weeks, 60 minutes per day, with 3 types of exercise for stimulation whereas the control group was performed a classical decongestive physiotherapy in a same day. For data analysis, the mean and standard deviation were estimated; 2 way repeated measures ANOVA was carried out. RESULTS: First, The level of VAS was significantly reduced on time, interaction effect in the group. Second, most factors of ROM were significantly increased on time, interaction effect whereas extension was not significantly increased. CONCLUSION: In conclusion, Our results showed that complex therapeutic exercise could improve or maintain VAS and ROM of shoulder joint for middle-aged women with breast cancer related lymphedema.

A Case of Malignant Lymphedema in a Dog (개에 있어서 악성 림프부종 일례)

  • A five-year-old, female Great Dane dog with edema, localized trauma, mild pain, and lameness of the right hind limb was referred to the Veterinary Medical Teaching Hospital of Chungbuk National University. This dog had a history of mammary tumor excisions 6 months ago. Abnormal changes were not seen in the values of complete blood count and serum biochemical tests. But pedal direct lymphangiography using aqueous-based radiographic agent showed the obstructed lymph flow in right popliteal lymph node. Based on these observations, the dog was suspected as lymphedema resulted from lymph drainage flilure without any other possibilities of inflammation or other causes. Although recommended chemotherapy and physiotherapy had been applied for resolvinr presented problems for one month, there was no improvement on edema of damaged region and any other clinical signs. Therefore, the necropsy was performed after euthanasia under agreement of the owner of patient. In histopatholofical examination, the most characteristic lesions in the mass of femoral region were diffuse edema fibrosis and neoplastic cells in the lymphatics. Also, the neoplastic cells were very similar to those found in the tumor mass of mammary gland, which had diagnosed as fibrosing carcinoma. These facts suggested that the cause of obstructed lymph flow was the neoplasia in lymphatics of the right hind limb. With these results, a diagnosis of malignant lymphedema was made in this dog.

Effects of Aromatherapy Massage on Edema Reduction in the Treatment of Lymphedema (아로마 마사지가 림프부종에 미치는 영향)

  • The purpose of this study was to determine whether aromatherapy massages with manual lymph drainage (MLD) are significantly more effective than sham massages with MLD in reducing edematous limb volume in lymphedema. This study was performed on 46 patients who had developed unilateral upper or lower lymphedema. Twenty-three patients served as the experimental group and were treated with complex decongestive physiotherapy (CDP) applications including aroma massages with MLD, multi layered compression bandages, remedial exercises and skin care. Twenty-three patients in the control group were treated with CDP applications including sham massages with MLD, bandages, exercises and skin care. Patients undergo a therapy program once a day; 5 days a week for 2 weeks. Percentage excess volume (PEV) and body mass index (BMI) were recorded before and after treatment. PEV and BMI were significantly decreased before and after treatment in the experimental as well as in the control groups (p<.05). However, the percentage excess volume and BMI were not significantly improved after treatment between the two groups (p>.05). This study there suggests that aroma massages is not effective in the edema of patients with lymphedema.

Analysis of Domestic Manual Lymph Drainage (MLD)-related Research Trend (국내 림프드레나지(MLD) 연구에 관한 논문 동향 분석)

  • This study attempted to investigate the research trend of MLD studies which were issued from 2001 to March 2022 by research period/area/method/purpose through the analysis of academic theses and journals and use the results for the growth and of MLD. The study results found the followings: First, in terms of the results by research period, a total of 84 academic papers and journals were published: Phase 1 (15), Phase 2 (47), Phase 3 (22). Second, when analyzed by research area, 'cosmetology' was the highest with 19 papers in terms of the number of studies. In fact, the field of study accounted for the largest portion in all three phases. Third, when analyzed by research method, quantitative research showed the largest percentage in all three phases. Fourth, when analyzed by research purpose, a practical approach was dominant with 81 papers out of the total number of 84 (3 papers in theoretical approach). Concerning a domestic MLD-related research trend, it is an essential step for MLD study establishment and qualitative improvements. Such research analysis of MLD studies is meaningful in that it has not been attempted before. In addition, it is anticipated that the study results would be helpful in searching a direction for the academic growth and development of MLD.

Rapid Lymphedema Progression in Breast Cancer Patient with Previous Forearm Fracture (전완 골절 과거력이 있던 유방암 환자에서 비전형적 림프부종 발생 1례)

  • Fracture is uncommon cause of lymphedema. The mechanism of lymphedema progression is still unknown, but disruption of the lymphatic system during and after fracture might be involved. In contrast, breast cancer surgery is a common cause of lymphedema and is usually caused by the removal of axillary lymph nodes. Sentinel node biopsy (SNB) has been adopted in early breast cancer to reduce the risk of lymphedema. Thus, the incidence of lymphedema in SNB was lowered. However, less than 10% of SNB patients are still diagnosed with lymphedema, but it is known that it took years to diagnose. Recently, we encountered atypical lymphedema occurred after breast cancer surgery with SNB. Symptoms started earlier than usual and were more severe. Interestingly, she has a history of a proximal radial fracture on the same side of SNB. We thought there could be a relationship between the acceleration of breast cancer-related lymphedema and fracture.
