Detection of Hidden Proximal Caries using Q-ray view in Primary Molars (Q-ray view를 이용한 유구치의 숨은 인접면 우식증 탐지)

  • The purpose of this study was to evaluate the ability of Q-ray view (All-in-one Bio, Seoul, Korea) in detection of proximal caries in primary molars with sound marginal ridges. Thirty two children aged 3-9 years (average $5.6{\pm}1.3$ years old) were chosen, and two examiners evaluated 100 proximal surfaces of primary molars with sound marginal ridges. The teeth were examined with; (a) visual examination, (b) Q-ray view, (c) DIAGNOdent (KaVo, Biberach, Germany) and (d) digital periapical radiography. Kappa statistic was used to assess the agreement between each examination method and the degree of caries progression. The kappa values for enamel caries were 0.15 (visual examination), 0.10 (Q-ray view), 0.25 (DIAGNOdent) and 0.68 (digital periapical radiography). The kappa values for dentinal caries were 0.34 (visual examination), 0.56 (Q-ray view), 0.44 (DIAGNOdent) and 0.70 (digital periapical radiography). Although Q-ray view showed low diagnostic ability in detection of enamel caries, it was effective in detection of hidden proximal caries extended into dentin. Q-ray view would be a useful and simple device which could aid pediatric dentists in detection of hidden proximal caries in primary molars especially when examining uncooperative children or disabled persons.

  • 3차원적으로 잘 충전된 근관은 치근단 누출과 재감염을 방지하며, 조직이 잘 치유될 수 있는 생물학적 환경을 제공해준다. 이 때문에 근관계의 완전한 충전은 근관치료의 중요한 목표 중의 하나이다. 본 연구의 목적은 4가지 방법으로 근관충전 후 디지털 방사선 사진을 촬영하여 근관충전의 질을 평가하고 투명표본을 제작하여 색소침투범위를 측정함으로써, 근관충전방법에 따른 치근단부 폐쇄능을 비교 평가하고자 하는 것이다. 직선형의 단근관을 갖는 80개의 전치를 선택하여 ProFile$^{\circledR}$ 니켈-티타늄 회전식 기구를 사용하여 근관을 성형한 후 무작위로 20개씩 4군으로 나누었다. 사용된 근관충전법은 다음과 같다:MicroSeal$^{\circledR}$(Group A), Thermafil$^{\circledR}$(Group B), Continuous wave 충전법(Group C), 측방가압충전법(Group D), 각 군에서 10개 치아는 Sealapex를, 나머지 10개 치아는 AH26$^{\circledR}$을 충전용 시멘트로 사용하였다. 근관충전이 끝난 치아는 충전의 질과 길이를 평가하기 위해, 근원심과 협설방향으로 디지털 방사선 사진을 이용하여 촬영하였다. 각 치아의 치근단부 2mm를 제외한 나머지 부위는 두겹의 nail varnish를 바르고, 2% methlylene blue용액에 48시간동안 침잠시켰다. 흐르는 물에 깨끗이 세척한 후, 투명치아를 만들었다. 선상의 색소침투를 관찰하고 치관측 최대깊이를 입체현미경하에서 40배율로 평가하여 다음과 같은 결과를 얻었다. 1. 충전방법에 따른 근단부 폐쇄효과를 비교시, 실험군 모두 비교적 양호한 근단부 폐쇄효과를 보였고 통계학적으로 유의성이 없었다. 2. 충전용 시멘트에 따른 근단부 폐쇄효과를 비교시, AH26$^{\circledR}$을 사용한 군에서 Sealapex를 사용한 군보다 더 적은 색소침 투를 나타냈다(p<0.05). MicroSeal$^{\circledR}$을 이용한 실험 1군내에서 AH26$^{\circledR}$을 사용하였을 때 미세누출이 더 적었고(p<0.05), 다른 군내에서는 통계학적으로 유의성이 없었다. 3. 근단부 충전상태에 따른 미세누출 비교시, 저충전, 과충전과 색소침투간에는 상관관계가 없었다. 4.충전방법에 따른 근단부 충전상태 평가시, Thermafil$^{\circledR}$을 이용한 실험 2군에서 과충전이 많이 나타났다(p<0.05). 이상의 결과로, 기존의 측방가압법 및 여러 열가소성 충전법이 유사한 근단부 폐쇄효과를 나타낸 바, 방법에 따른 술자의 숙련도, 충전시간, 재근관치료의 편이성 등을 고려하여 근관충전방법을 선택하는 것이 합리적일 것이라고 사료된다.

Comparative study on the radiopacity of different resin-based implant cements (레진계 임플란트용 시멘트의 방사선 불투과성에 대한 비교연구)

  • This study was aimed to compare the radiopacity of four kinds of currently available resin based implant cements using digital radiography. Materials and Methods: Four resin-based implant cements((Estemp $Implant^{TM}$ (Spident, Incheon, Korea), $Premier^{(R)}$Implant (Premier, Pennsylvania, USA), $Cem-Implant^{TM}$ (B.J.M lab, Or-yehuda, Israel), $InterCem^{TM}$ (SCI-PHARM, California, USA)) and control group (Elite Cement $100^{TM}$ (GC, Tokyo, Japan) ) were mixed and cured according to the manufacturer's instructions on the custom made split-type metal mold. A total of 150 specimens of each cement were prepared and each specimen (purity over 99%) was placed side-by-side with an aluminum step wedge for image taking with Intraoral X-ray unit (Esx, Vatech, Korea) and digital X-ray sensor (EzSensor, Vatech, Korea). For the evaluation of aluminum wedge equivalent thickness (mm Al), ImageJ 1.47 m (Wayne Rasband, National Institutes of Health, USA) and Color inspector 3D ver 2.0 (Interaktive Visualisierung von Farbraumen, Berlin, Germany) programs were used. Result: Among the 5 cements, Elite cement $100^{TM}$ (control group) showed the highest radio-opacity in all thickness. In the experimental group, $InterCem^{TM}$ had the highest radio-opacity followed by $Premier^{(R)}$ Implant $Cement^{TM}$, $Cem-Implant^{TM}$ and Estemp $Implant^{TM}$. In addition, $InterCem^{TM}$ showed radio-opacity that met the ISO No. 4049 standard in all the tested specimen thickness. Cem-Implant on 0.5 mm thickness showed radiopacity that met the ISO No. 4049 standard. Conclusion: Among the implant resin-based cements tested in the study, $Premier^{(R)}$ Implant Cement and Estemp $Implant^{TM}$ did not show appropriate radio-opacity. Only $InterCem^{TM}$ and $Cem-Implant^{TM}$ 0.5 mm specimen had the proper radiopacity and met the experiment standard.

Bone changes around the maxillary posterior teeth opposing the implants in mandible: a clinical study (하악 임플란트에 대합되는 상악 구치의 주변골 변화에 대한 임상연구)

  • Purpose: When the edentulous area is restored by implant prostheses, the opposing hypofunctioned teeth will receive physiologic mechanical stimuli. This study evaluated the bone changes around the maxillary teeth opposing an implant restoration installed in the mandibular posterior area. Materials and Methods: Radiographs of the opposing teeth were taken at prostheses delivery (baseline), 3 and 6 months later. A customized film holding device was fabricated to standardize the projection geometry for the serial radiographs of the opposing teeth. The gray values of the region of interest of each digital image were compared according to time. Repeated measured analysis of variance was performed at the 95% significance level. Results: The gray values of the alveolar bone around the antagonist teeth of implants increased with time. The changes in gray values of the middle area were greater than those of the crestal area. However, the gray values of the mesial and distal areas were not different. The changes in gray values were different according to the unloaded time. Conclusion: A change in bone tissue will occur if a proper physiologic load is again applied to the bone tissues around a hypofunctioned tooth.

  • Purpose: To evaluate the diagnostic accuracy of occlusal and proximal caries detection using CCD, CMOS, PSP and film system. Materials and Methods : 32 occlusal and 30 proximal tooth surfaces were radiographed under standardized conditions using 3 digital systems; CCD (CDX-2000HQ, Biomedysis Co., Seoul, Korea), CMOS (Schick, Schick Inc., Long Island, USA), PSP (Digora/sup (R)/FMX, Orion Co./Soredex, Helsinki, Finland) and I film system (Kodak Insight, Eastman Kodak, Rochester, USA). 5 observers examined the radiographs for occlusal and proximal caries using a 5-point confidence scale. The presence of caries was validated histologically and radiographically. Diagnostic accuracy was evaluated using ROC curve areas (Az). Results: Analysis using ROC curves revealed the area under each curve which indicated a diagnostic accuracy. For occlusal caries, Kodak Insight film had an Az of 0.765, CCD one of 0.730, CMOS one of 0.742 and PSP one of 0.735. For proximal caries, Kodak Insight film had an Az of 0.833, CCD one of 0.832, CMOS one of 0.828 and PSP one of 0.868. No statistically significant difference was noted between any of the imaging modalities. Conclusion: CCD, CMOS, PSP and film performed equally well in the detection of occlusal and proximal dental caries. CCD, CMOS and PSP-based digital images provided a level of diagnostic performance comparable to Kodak Insight film.

The radiographic depth of approximal root cavities : A comparative study of conventional and digital radiographs (인접면 치근 인공우식병소의 깊이 평가: 구내 일반 방사선사진과 디지털 방사선사진과의 비교)

  • Purpose : To investigate the reproducibility of the conventional and digital radiographs to determine the depth of approximal root cavities. Materials and Methods : A total of 80 artificial root cavities were prepared in the approximal surfaces of premolars, maxillary and mandibular molars. Standardized radiographs were taken at the baseline (0° horizontal and 0° vertical) and at a horizontal angulation of 10° in both mesial and distal directions. Radiographic cavity depths were measured by both conventional and digital radiographs. Results: At 0° horizontal angulation, no statistically significant differences could be determined between the results of conventionally and digitally determined radiographic depths with respect to the actual cavity depths in all premolar, maxillary, and mandibular molar groups. All conventional and digital radiographic depths at both 10° mesial and distal angulations showed statistically significant increases in depth compared to the actual cavity depths (p < 0.05), with the exception of digital radiographic depth at 10° mesial angulation for premolars. There were no statistically significant differences between conventional and digital radiographic depths for all groups. Conclusion: The present study suggests that both conventional and digital radiographs provide reproducible assessment of the depth of the approximal root cavity. But horizontal X-ray beam movements are likely to result in increase in radiographic cavity depth.

Detectability of Ektaspeed Plus Film, Digitized and Digora Images for Artificial Periapical Bone Lesions (Ektaspeed Plus 방사선사진, 간접 디지털 영상 및 Digora 영상의 치근단 병소의 판독능 비교)

  • The comparative detectability of the artificial periapical defects among Ektaspeed Plus film. digitized and digora images was evaluated. The artifical defects were made in the cancellous bone and cancellous-cortical junction with the size of 1.0×0.8mm², 1.4×1.1mm² and 2.8×2.2mm². The defects in cancellous-cortical junction extended into cortical bone with the depth of 0, 0.5 and 1.0 mm. The results were as follows : 1. In junctional defects Ektaspeed Plus film for 2.8×12.2mm² defect showed the highest detectability. But significant difference were only found between Ektaspeed Plus films and digitized images (p<0.05). 2. Almost all defects within cacellous bone were not detected except a few digitized and Digora images for the size of 2.8×2.2mm². Digora images for them showed significant differences with Ektaspeed Plus films and digitized images (p<0.05). 3. The sensitinity of all imaging modalities were 0.9 or 1.0 in junctional defects for the size of 1.4×2.2mm² and 2.8×2.2mm². For cancellous defects, Digora image showed the highest sensitivity of 0.6 for the size of 2.8×2.2mm². 4. Significant differences for change of exposure time were found in most group of Ektaspeed Plus films and digitized images (p<0.05) . But there was no significant differences in Digora images for cacellous defects.

Effects of implant collar design on marginal bone and soft tissue (임플란트의 collar design이 변연골과 연조직에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the effects of implant collar design on marginal bone change and soft tissue response by an animal test. Materials and methods: Two types of Implant (Neobiotech Co. Seoul, Korea) that only differs in collar design were planted on two healthy Beagle dogs. The implants were divided into two groups, the first group with a beveled collar (Bevel Group) and the second group with "S" shaped collar (Bioseal group). Standardized intraoral radiographs were used to investigate the mesio-distal change of the marginal bone. Histological analysis was done to evaluate the bucco-lingual marginal bone resorption and the soft tissue response adjacent to the implant. Mann-Whitney test was done to compare the mesio-distal marginal bone change at equivalent time for taking the radiographs and the tissue measurements between the groups. Results: Radiographic and histological analysis showed that there was no difference in marginal bone change between the two groups (P>.05). Histological analysis showed Bioseal group had more rigid connective tissue attachment than the Bevel group. There was no difference in biological width (P>.05). Bevel group showed significantly longer junctional epithelium attachment and Bioseal group showed longer connective tissue attachment (P<.05). Conclusion: For three months there were no differences in marginal bone change between the Bevel group and the Bioseal group. As for the soft tissue adjacent to the implant, Bioseal group showed longer connective tissue attachment while showing shorter junctional epithelium attachment. There were no differences in biologic width.

Clinical Usefulness of Implanted Fiducial Markers for Hypofractionated Radiotherapy of Prostate Cancer (전립선암의 소분할 방사선치료 시에 위치표지자 삽입의 유용성)

  • Purpose: To assess the usefulness of implanted fiducial markers in the setup of hypofractionated radiotherapy for prostate cancer patients by comparing a fiducial marker matched setup with a pelvic bone match. Materials and Methods: Four prostate cancer patients treated with definitive hypofractionated radiotherapy between September 2009 and August 2010 were enrolled in this study. Three gold fiducial markers were implanted into the prostate and through the rectum under ultrasound guidance around a week before radiotherapy. Glycerin enemas were given prior to each radiotherapy planning CT and every radiotherapy session. Hypofractionated radiotherapy was planned for a total dose of 59.5 Gy in daily 3.5 Gy with using the Novalis system. Orthogonal kV X-rays were taken before radiotherapy. Treatment positions were adjusted according to the results from the fusion of the fiducial markers on digitally reconstructed radiographs of a radiotherapy plan with those on orthogonal kV X-rays. When the difference in the coordinates from the fiducial marker fusion was less than 1 mm, the patient position was approved for radiotherapy. A virtual bone matching was carried out at the fiducial marker matched position, and then a setup difference between the fiducial marker matching and bone matching was evaluated. Results: Three patients received a planned 17-fractionated radiotherapy and the rest underwent 16 fractionations. The setup error of the fiducial marker matching was $0.94{\pm}0.62$ mm (range, 0.09 to 3.01 mm; median, 0.81 mm), and the means of the lateral, craniocaudal, and anteroposterior errors were $0.39{\pm}0.34$ mm, $0.46{\pm}0.34$ mm, and $0.57{\pm}0.59$ mm, respectively. The setup error of the pelvic bony matching was $3.15{\pm}2.03$ mm (range, 0.25 to 8.23 mm; median, 2.95 mm), and the error of craniocaudal direction ($2.29{\pm}1.95$ mm) was significantly larger than those of anteroposterior ($1.73{\pm}1.31$ mm) and lateral directions ($0.45{\pm}0.37$ mm), respectively (p<0.05). Incidences of over 3 mm and 5 mm in setup difference among the fractionations were 1.5% and 0% in the fiducial marker matching, respectively, and 49.3% and 17.9% in the pelvic bone matching, respectively. Conclusion: The more precise setup of hypofractionated radiotherapy for prostate cancer patients is feasible with the implanted fiducial marker matching compared with the pelvic bony matching. Therefore, a less marginal expansion of planning target volume produces less radiation exposure to adjacent normal tissues, which could ultimately make hypofractionated radiotherapy safer.

Transmission Dose Estimation Algorithm for Tissue Deficit (조직 결손에 대한 투과선량 계산 알고리즘 보정)

  • Purpose : Measurement of transmission dose is useful for in vivo dosimetry. In this study, previous algorithm for estimation of transmission dose was modified for use in cases with tissue deficit. Materials and Methods : The beam data was measured with flat solid phantom in various conditions of tissue deficit. New algorithm for correction of transmission dose for tissue deficit was developed by physical reasoning. The algorithm was tested in experimental settings with irregular contours mimicking breast cancer patients using multiple sheets of solid phantoms. Results : The correction algorithm for tissue deficit could accurately reflect the effect of tissue deficit with errors within ${\pm}1.0\%$ in most situations and within ${\pm}3.0\%$ in experimental settings with irregular contours mimicking breast cancer treatment set-up. Conclusion : Developed algorithm could accurately reflect the effect of tissue deficit and irregularly shaped body contour on transmission dosimetry.
