The Review of Environment, Food and Exercise on Allergy Anaphylaxis (환경, 음식 및 운동 알레르기 반응에 대한 고찰)

  • Exercise-induced anaphylaxis (EIA) is a physical allergy, sometimes severe, triggered by exertion following specific food intake. It was defined for the first time in 1980. EIA is associated with different kinds of exercise. The clinical manifestations progress from itching, erythema and urticaria to some combination of cutaneous angioedema and vascular collapse. Mast cell participation in the pathogenesis of this syndrome has been proved by the findings of an elevated serum histamine level during exhaustive exercise. As predisposing factors of EIA, a specific or even nonspecific sensitivity to food has been reported. Food-dependent exercise-induced anaphylaxis (FDEIA) is a distinct form of food allergy induced by physical exercise. It is typified by the onset of anaphylaxis during exercise which was preceded by the ingestion of the causal food allergens. The diagnosis of FDEIA is heavily dependent on clinical history. Allergy tests may need to be performed using a broad panel of food and food additives. As with food allergies, FDEIA diagnosis is based on interview, biological test and skin test. Prophylaxis aims to prevent a recurrence; the patient should be given an emergency kit to deal with any recurrent episodes. After the food allergen has been identified, it should be avoided for at least 4 to 5 hours before any exercise. Two cases of EIA are presented (EIA to circumstances; FDEIA) in this paper, The diagnosis, pathophysiology and therapy of FDEIA are also reviewed.

Consideration of Adverse Reaction to MDCT Contrast Media (MDCT에 사용되는 조영제의 부작용에 대한 고찰)

  • In this experiment, we investigated 82 patients who suffered adverse reactions due to contrast medium. We selected the subjects out of 21,178 people who had an intravenous injection of contrast medium to undergo MDCT examination at one university hospital in Busan in 2007. As a result, the largest groups of the patients were as follows. 52.4% of the patients were male when classify by gender; 28.0% of the patients were 50's by age; 45% of the patients got when it was spring(April and March); 75.6% of the patients had a side effects when the speed of injection is 2.5mL/sec; 58.5% of the patients were suffered when the volume of injected contrast medium is over 130mL. Urticaria was the main symptom of side effect as 26.8%. And the main treatment for the effect was alleviating the symptoms before making patients to return home. Thus, practical preventive measures are needed as follows : use the OCS system to observe warning signs at risky patients, secure warming spaces for patients to cope with season changing, prepare enough emergency kits for the patients in danger, and establish CPR call systems, explain the risk of contrast medium and get agree about using contrast medium.

A Case of Urticaria occurred to a Breastfeeding Patient in Postpartum Period Treated with Korean Medicine (모유 수유 중인 산욕기 환자의 두드러기 치험 1례)

  • Objectives : The aim of this study is to report the effect of herbal medicine and acupuncture on a breastfeeding woman's postpartum urticaria which was unresponsive to a 1st generation antihistamine and a topical steroid. Methods : Wheals and pruritus occurred to a 36-year-old breastfeeding woman 2 days after the parturition. Symptoms were unresponsive to about a week of systemic antihistamine and topical steroid treatments. GamiSamul-tang herbal decoction was administered three times a day for the first 21 days and twice a day for the next 10 days of the treatment. Acupuncture treatment was applied to 12 acupoints (both sides of LI04, LR03, ST36, LI11, SP10, SP6) for 15 minutes once a week, 5 times in total. The patient's symptom was evaluated with Weekly Urticaria Activity Scale (UAS7) and Visual Analogue Scale for Worst Pruritus within 24 hours ($VAS_{Pruritus}$) each time the patient visited out-patient department. Results : The scores of UAS7 and $VAS_{Pruritus}$ at the baseline were 38 and 8.2 respectively. 10 days after the herbal medicine and acupuncture treatment began, both decreased to 0 and wheal, pruritus, erythema didn't appeared again for the rest of the follow up periods. Conclusions : This study shows the possibility of using GamiSamul-tang and acupuncture to treat postpartum urticaria.

The Relationship between Food Allergen Sensitization and Allergic Disease in Childhood (항원감작식품과 어린이 알레르기 질환과의 관계)

  • We have investigated the relationship between food allergen sensitization and allergic disease in 74 child (male 47, female 27) patients from 0 to 14 years of age diagnosed with allergic disease. The age distribution for the study was: newborn to 3 years old, 34 children; 4 to 6 years old, 24 children; 7 to 9 years old, 8 children and above 10 years old, 8 children. Of the 74 children, 10 children were allergic to 3 of the 21 types of foods tested, 21 children were allergic to 4 types and 15 children were allergic to 5 types. The results of specific IgE tests for class 2 (0.070-3.49 IV/mL, IgE density in serum) showed that 29 children were allergic to milk, 28 children to bean, 21 children to cheese, 7 children to egg, and 18 children to pork, while over class 2, 20 children were allergic to bean, 17 children to milk, 24 children to cheese, 20 children to egg, and 21 children to pork. A questionnaire was used to survey family allergy history and diet patterns for 40 child (male 22, female 18) patients with allergic disease. The frequencies of a family history of allergy were 45.5% for males and 50.0% for females. The allergic diseases included atopic dermatitis: 26.0%, atopic nasitis: 10.5%, atopic dermatitis + atopic nasitis : 31.5%, hives: 21.0%, and asthma: 10.5%. Children on diets of mixed breast feeding and infant formula were more allergic than those on either breast feeding or infant formula feeding. Eliminated allergenic foods were egg + milk: 12.5%, egg: 10.0%, and milk 2.5%.

The Effect of Gamisungmagalguntang(加味升麻葛根湯) on Chronic Urticaria Identified as "Wind Heat" Pattern (풍열형(風熱型) 만성 두드러기에 대한 가미승마갈근탕(加味升麻葛根湯)의 치료효과)

  • Objective : This case series study was designed to assess the effect of Gamisungmagalguntang for chronic urticaria identified as "Wind-Heat" pattern by retrospective methods. Methods : We treated 19 patients for chronic urticaria with Gamisungmagalguntang(加味升麻葛根湯), who visited to care chronic urticaria at the department of Ophthalmology, Otolaryngology and Dermatology of Korean Medicine at Kyunghee University Korean Medical Center from 1st January 2013 to 31st August 2013. We analyzed information and body conditions of 19 patients. And by using Urticaria Activicy Score(UAS), frequency and number of wheals, itch severity and total score on a 4-point(0-3) scale were assessed in 19 patients. The change of each criterion and total scores between baseline and follow up were analyzed using paired t-test and Wilcoxon test(p<0.05). Results : 1. UAS improvement was statistically significant(p<0.001). Effects of symptom improvement were excellent(31.58%) that means symptom remission and good(42.10%) that means symptom improved over 50%. 2. Heat sensitivity as suspected provocation factor could be a marker for the Wind-Heat pattern urticaria. 3. Among the Wind-Heat pattern urticaria patients, normal digestion(52.63%) and no thirst(78.95%) were higher than abnormal conditions. Whereas about sweat item, abnormal conditions(57.89%) were higher than normal. That means abnormal sweat conditions could be a marker for a defense qi(衛氣) dysfunction of the Wind-Heat pattern urticaria. Conclusions : Our findings suggest that Gamisungmagalguntang could be effective on the Wind-Heat pattern urticaria.

Clinical Analysis of Marine Telemedicine Cases for Ocean-Going Vessel Crew (원양선박 선원들의 해양원격의료 실태를 통한 임상분석)

  • The purpose of this study is to contribute to the study of the prevention of diseases and promotion of health of ocean going vessel crew members, through the medical diagnosis and disease classification efforts of this study. From the second half of 2016 to 2017, the diagnosis and health characteristics of 195 crew members were collected through counseling, treatment, and emergency care for about 1 year and 2 months. As a result, it is noted that the incidence of diseases was in the order of urticaria (5.6%), lumbar sprain (4.1%), acute gastroenteritis (3.1%) and anxiety (3.1%). In categorical review, the incidence of musculo-skeletal disease was the most common (25.1%) which was followed by skin disease (17.9%) and digestive disease (11.3%). In addition, the disease that was noted as was the most common in the under 30 years old category, and the incidence of the disease was high in the crew group. Finally, there was a difference between the pathogenesis (trauma vs disease, etc.) (p <.001) and the type of vessel (merchant ship and fishing vessel) (p <.005) as noted in this case.

Clinical study of Chronic Urticaria (만성두드러기에 대한 임상적 고찰)

  • Introduction Urticaria is widely spread disease. About 15-20 $\%$ of population has experienced at least once in a life time. The etiology of urticaria is uncertain till now, and the affcting factors are various chemical material, physical factors, alcohol, fever, exercise, and hormone, etc. The symptoms of urticaria are small or large wheal-erythema reaction and itching or tingling sense. Cause there is no probe to detect the urticaria clearly, the treatment of urticaria is symptomatic. And mostly urticaria can be chronic and very hard to treat it fundamentally. Nowadays, many trials to treat the urticaria in oriental medical way show good curability. In this paper I'd like to report the treatment rate and degree of urticaria patients mostly using anti-histamine medication. Subjects From the outpatients who visited Korean Hospital of Kyunghee Kangnam during 2000-4 and 2002-1, the 36 chronic urticaria patients, at least more than 4 weeks from onset and 2weeks of treatment period, were chosen. Methods I evaluated the results of treatment with the following scale. Very Good: The all symptoms are clearly disappeared or one third of symptoms remain and sometimes eruption is appeared. Good: Half of the symptoms are improved but most of symptoms remain. No Change: No change appears before and after treatment. Worse: The degree and duration of eruption get worse than pre-treatment state. Results For Acupuncture treatment, I chose the several Acu points like Hapgok(합곡), Taichung(대충), Gokji(곡지), Yanggok(양곡), Yanggea(양계), and lmeup(족임읍), and usually lasted it 15 minutes. With the acupuncture treatment and herb medicine, 2-3 times a week, the patients whose treatment period was 2 to 4 weeks were 17(47.2$\%$), 4 to 8 weeks were 11(30.5$\%$), 8 to 12 weeks were 3(8.3$\%$), 12 to 16 weeks were 3(8.3$\%$), longer than 16 weeks were 2(5.5$\%$) Collecting the statistics of the frequency of prescripted herb medicine, Hyangsosan(향소산) was prescripted 21 times(58.3$\%$), Hyangsapyungwisan(향사평위산) was 15 times(41.6$\%$), Hwapisan(화피산) was 9 times(25$\%$), Yangwitang(양위탕) was 6 times(16.6$\%$), Bojungikkitang(보중익기탕가미) was 4 times(11.1$\%$), Yongdamsagantang (용담사간탕가미) was 4 times(11.1$\%$). The result of the treatments, evaluating with mentioned rate scale, was 11 cases(30.5$\%$) were Very Good degree, 19 cases(52.7$\%$) were Good , 6 cases(16.6$\%$) were No Change. No cases were Worse degree. Conclusion According to this research, we could say that chronic urticaria can be treated with Oriental medical methods. But more precise probes in both Oriental and Western medicine to diagnose the chronic urticaria should be established and we need to make standards for testing and diagnosing the chronic urticaria.

The First Report on Clinical Manifestation of Cryopyrin-Associated Periodic Syndrome in Korean Children

  • Purpose: The aim of this study was to evaluate the clinical characteristics of children diagnosed as cryopyrin-associated periodic syndrome (CAPS) in Korea. Methods: Diagnosis was made based on clinical features and confirmed by a mutation in the cold-induced autoinflammatory syndrome 1 (CIAS1) gene. Especially, osteocartilaginous overgrowth in the patella or distal femur was so characteristic that its presence warranted a diagnosis of chronic infantile neurologic cutaneous and articular/NOMID. Results: We observed the clinical features of 9 Korean CAPS patients. All the patients suffered from an urticarial rash with recurrent fever. Among the 9 patients, 6 presented with rash and 4 with fever on the 1st or 2nd days of birth. Eight patients showed myalgia, and 7 patients showed arthralgia in the joints, and 6 patients showed radiologic findings of arthropathy including cupping of the metaphysis, excessive growth of the epiphysis, osteopenia or overgrowth of the cartilage. Four patients showed brain atrophy, enlarged ventricles or leptomeningeal enhancement on magnetic resonance imaging. Intellectual disability was observed in 1 patient. Five patients had eye involvement as conjunctivitis, uveitis, chorioretinitis, avascular area or papillary edema, and 3 patients showed progressive hearing loss. All 9 patients showed increased C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR). Conclusions: All the patients carried a mutation on exon 3 of the CIAS1 gene. After the anakinra (interleukin-1 receptor antagonist) therapy, the fever and rash immediately disappeared, and CRP and ESR were improved.

Trends of Using UAS7 in Chronic Urticaria Literature and Adherence of UAS7 in a Single Korean Medical Hospital (만성 두드러기의 환자 자기 평가 도구로서 UAS7 사용 현황에 대한 고찰 : 국내외 연구논문 분석 및 일개 한방병원 의무기록을 중심으로)

  • Objectives : The purpose of this study is to investigate the using trends of UAS7 over the last 5 years to analyze patients using UAS7 in a single Korean medicine hospital and to understand and suggest the actual status of application in clinical research. Methods : PubMed, RISS and OASIS were used to search clinical research papers related to chronic urticaria in the last 5 years. The adherence of UAS7 was assessed based on medical records in a single Korean medical hospital for 4 weeks. Results : Total 536 articles were selected, and 401(74.8%) articles used the assessment tool. UAS7 has been most commonly used assessment tool to evaluate the severity of chronic urticaria and DLQI has been most commonly used to evaluate the quality of life. The usage rate of UAS7 increased from 50% in 2016 to 85% in 2020. There were a total of 10 articles in traditional medicine research, of which 9 assessment tools were used and 4 articles used UAS7. To analyze adherence of UAS7, a total of 15 patients were selected. 6 patients (40%) did not record any value, and only 2 patients(13%) showed more than 80% adherence. Conclusions : UAS7 is most commonly used assessment tool in clinical research currently. However, since patient record adherence tends to be poor, it is needed to find ways increasing the UAS7 record adherence.

Herbal Medicine Treatment for Cold Urticaria: A Systematic Review and Meta-Analysis (한랭 두드러기에 대한 한약 치료: 체계적 문헌 고찰 및 메타분석)

  • Objectives : The purpose of this study is to evaluate the effects of herbal medicine treatment in patients with cold urticaria. Methods : We searched randomized controlled trials(RCTs) reporting the effects of herbal medicine for cold urticaria through domestic and international databases from their inception to September 2023. The results were summarized in tables. We assessed the risk of bias in included RCTs through Cochrane risk of bias tool and the data synthesis was conducted through RevMan version 5.4. Results : A total of 12 RCTs were included in this review and all trials compared herbal medicine alone treatment(treatment group) with western medicine alone treatment(control group). The total effective rate(TER) of treatment group was statistically higher than that of control group(RR: 1.49, 95% CI: 1.38 to 1.62, p<0.00001, I2=65%). On the other hand, when comparing except for 1 trial with different evaluation period, the TER of treatment group was statistically higher than that of control group and heterogeneity was very low(RR: 1.36, 95% CI: 1.26 to 1.47, p<0.00001, I2=0%). And when comparing 8 trials using the total symptom score(TSS) change index as an indicator of TER, the TER of treatment group was statistically higher than that of control group and heterogeneity was very low(RR: 1.38, 95% CI: 1.26 to 1.51, p<0.00001, I2=0%). The treatment group showed more statistically significant decrease compared to the control group in TSS(MD: -2.51, 95% CI: -2.63 to -2.40, p<0.00001, I2=99%). The relapse rate of treatment group was statistically lower than that of control group(RR: 0.19, 95% CI: 0.10 to 0.40, p<0.00001, I2=0%). Mild adverse events such as sleepiness, dizziness were reported in control group and gastric discomfort was reported in treatment group. In the risk of bias assessment, many cases were evaluated as 'Unclear risk'. Conclusions : This review found that herbal medicine alone treatment could more effective and safe than western medicine alone treatment for cold urticaria. But further well-designed researches are needed because of heterogeneity between trials and the quality of the included trials.
