Radiation Therapy for T2N0 Glottic Cancer (T2N0 병기 성문암의 방사선치료)

  • $\underline{Purpose}$: This study evaluated the results of definitive radiation therapy and the prognostic factors that affect survival rates for T2N0 glottic cancer patients. $\underline{Materials\;and\;Methods}$: Thirty patients with T2N0 glottic cancer who were treated with definitive radiation therapy at our institution between September 1986 and June 2004 were retrospectively reviewed. All patients were pathologically confirmed as having squamous cell carcinoma and were staged as AJCC T2N0. The age of the patients ranged from 39 to 79 (median 62) years and all were male. A total dose of $66{\sim}70\;Gy$ (median 66 Gy) was delivered with a 6-MV linear accelerator in $6.5{\sim}7$ weeks. The median follow-up period was 63 months. $\underline{Results}$: The actuarial disease-free survival rate for the entire group of the patients was 79% at 5 years. The five-year disease-free survival rates for patients without and with subglottic extension were 90% and 56%, respectively (p=0.03). However, anterior commissure involvement, supraglottic extension, and impaired cord mobility were not statistically significant prognostic factors. The five-year disease-free survival rates for patients with and without concurrent chemotherapy were 86% and 69%, respectively (p=0.47). $\underline{Conclusion}$: Subglottic extension can be considered a poor prognostic factor for T2N0 glottic cancer.

Analysis of Prognostic Factors Related to Survival Time for Patients with Small Cell Lung Cancer (소세포폐암 환자의 생존기간에 관련된 인자 분석)

  • Background : Small cell lung cancer represents approximately 20% of all carcinomas of the lung, and is recognized as having a poor long term outcome compared to non-small cell lung cancer. Therefore, this study investigated the prognostic factors in small cell lung cancer patients in order to improved the survival rate by using the proper therapeutic methods. Material and method : The clinical data from 394 patients who diagnosed with small cell lung cancer and treated from 1993 to 2001 at the Kosin University Gospel Hospital, were analyzed. Result : There were 314 male patients (79.7%), and 80 female patients (20.3%). The number of those with limited disease was 177 (44.9%), and the number of those with extensive disease was 217 (55.1%). Overall, 366 out of 394 enrolled patients had died. The median survival time was 215 days (95% CI : 192-237days). The disease stage, Karnofsky performance state, 5% body weight loss for the recent 3 months, chemotherapy regimens, and the additive chest radiotherapy were identified as being statistically significant factors for the survival time. The median survival times of the supportive care group, one anticancer therapy, and two or more treatment groups were 17 days, 211 days, and 419 day, respectively (p<0.001). These data emphasize the importance of anticancer treatment to improve survival time for patients. The group of concurrent chemoradiotherapy (30 patients) showed significantly longer survival time than the group given sequential chemoradiotherapy (55 patients) (528 days versus 373 days, p=0.0237). The favorable prognostic factors of laboratory study were groups of leukocyte =8,000/mm3, ALP=200 U/L, LDH=450 IU/L, NSE=15 ng/mL, s-GOT=40 IU/L. In extensive disease, there was no difference according to the number of metastatic site. However, the median survival time of patients with ipsilateral pleural effusion had longer than patients having other metastatic sites. According to the survey periods, three groups were divided into 1993-1995, 1996-1998, and 1999-2001. The median survival time was significantly prolonged after 1999 in comparison to previous groups (177 days, 194 days, 289 days, p=0.001, 0.002, respectively). Conclusion: Disease stage and 5% body weight loss for recent 3 months at diagnostic state were significant prognostic factors. In addition, the performance status, serum ALP, LDH, NSE, CEA levels also appear to be prognostic factors. The survival time of those patients with small cell lung cancer has been prologned in recent years. It was suggested that the used of the EP (etoposied and cisplatin) chemotherapy method and concurrent chemoradiotherapy for patients with a limited stage contributed to the improved survival time.

Concurrent Chemoradiotherapy Results in Patients with Anal Cancer (항문암의 동시 화학 방사선 요법 치료결과)

  • Among the 63 patients with histopathologically proven primary squamous cell anal cancer who were managed in Presbyterian Medical Center and Yonsei University Cancer from Jan. 1971 to Dec. 1991, 34 patients, who were managed with surgery alone(abdominoperineal resection) or post-operative radiotherapy and concurrent chemoradiotherapy were analysed. With mean follow up time of 81.3 months, 30 Patients(88$ \% $) were followed up from 17 to 243 months. In methods, 10 patients were treated with surgery alone. 9 Patients were treated with combined surgery and postoperative radiotherapy(50$\∼$60 Gy in 28$\∼$30 fractions). 15 patients were treated with concurrent chemoradiotherapy. Chemotherapy (Mitomycin C 15 mg/squ, bolus injection day 1;5-FU, 750 mg/squ, 24hr infusion, day 1 to 5) and radiotherapy started the same day. A dose of 30 Gy was given to the tumor and to the pelvis including inguinal nodes, in 15 fractions. After 2 weeks a boost of radiotherapy(20 Gy) to the ano-perineal area and second cycle of chemotherapy completed the treatment. The overall 5-year survival rate was 56.2$ \% $. Concurrent chemoradiotherapy group was 70$ \% $ and surgery alone group was 16.7$ \% $. According to the cox proportional harzard model, there was significant difference between survival with concurrent chemoradiotherapy and surgery alone(p=0.0129), but post-operative radiotherapy was 64.8$ \% $, which was not stastically significant(p=0.1412). In concurrent chemoradiotherapy group, the anal funtion Preservation rate was 87$ \% $ and the severe complication rate(grade 3 stenosis and incontinence) was 13.3$ \% $. In conclusion, we conclude that the concurrent chemoradiotherapy may be effective treatment modality in patients with anal cancer.

Long-term Survival of Recurrent Pancreatic Cancer Treated with Tumorectomy and Stereotactic Body Radiation Therapy (수술 후 재발한 췌장암에서 종양절제술과 정위적 체부 방사선치료로 장기간 생존을 보인 환자)

  • A 70-year-old female diagnosed with pancreatic ductal adenocarcinoma was treated by pylorus-preserving pancreaticoduodenectomy (PPPD) and adjuvant concurrent chemoradiotherapy with 5-fluorouracil. Pancreatic ductal adenocarcinoma pT3N0 (stage IIA) was pathologically confirmed. Abdominal computed tomography (CT) findings 14 months after PPPD showed 10 mm sized solitary liver metastasis in segment 3. After 12 cycles of gemcitabine and 9 cycles of capecitabine plus oxaliplatin, the metastatic nodule increased in size to 27 mm. Tumorectomy at segment 3 of liver was done. 25 months after tumorectomy, chest CT showed 23 mm sized cavitary nodule in right upper lobe of lung. The result of percutaneous biopsy favored metastatic adenocarcinoma. Two sets of stereotactic body radiation therapy were done and the patient has survived without further disease progression for 6 years after initial diagnosis. This case suggests that selected population of recurrent pancreatic cancer patients with solitary liver or pulmonary metastasis can be treated by resection of metastatic site and ablative therapies.

암의 발생기작

  • 종양은 아직 그 발생 원인과 기작이 정확히 밝혀져 있지 않으므로 정확한 정의를 내리는 것은 어려우나 일반적으로 종양이란 정상 세포가 갖고 있는 세포 분열의 특이성을 상실하여 일어나는 조직의 자율적인 과잉 성장을 말한다. 이와 같은 세포의 비정상적인 증식에 의한 종양을 임상 및 병리 형태학적으로 양서 종양과 악성종양으로 분류한다. 양성 종양을 일으키는 종양 세포는 정상 세포와 비슷할 뿐 아니라 그 주변 세포들과 확실한 경계를 이루고 있으며 증식도 느리며 다른 부위로의 전이가 없다. 이에 반해 악성종양은 일반적으로 증식도 빠르고 이형의 세포로서 주변의 조직으로 확산, 전이될 뿐만 아니라 최종적으로 숙주인 개체를 사망시킨다. 악성종양은 다시 상피 조직에서 유례한 암, 비상피 조직에서 유래된 육종, 백혈구에서 유래된 백혈병 등으로 구별하지만 그의 본질은 거의 같으며 모든 악성종양은 통속적으로 암이라고 불리운다. 종양의 발생원인으로 크게 3가지로 나눌 수 있는데 화학물질, 바이러스 및 유전적 요인에 의한 것으로 알려졌다. 최초의 발암물질로 알려진 benzopyrene에 의한 발암 등 연구에 의해 화학적 발암원들은 직접 발암 물질로 작용하는 것이 아니라, 일단 체내에서 대사된 후 이들 대사 산물일 DNA 등에 작용함으로써 발암이 유도 되는 것으로 밝혀졌다. 이와 같이 화학적 변화를 거친 후에야 DNA에 영향을 미치는 것외에 다른 화학물질들은 또 다른 기작을 통해 암을 유발하는데 쥐의 피부에 benzopyrene을 한 번 처리하면 암을 유발하지 않으나 여기에 phorbol ester를 처리하면 높은 빈도로 암이 형성된다. 여기서 benzopyrene과 같이 세포의 DNA에 돌연변이를 일으키는 작용을 하는 발암물질을 발암개시제라 하고 phorbol ester처럼 그 자체로는 발암능이 없으나 발암개시제에 노출된 세포에 영향을 미쳐 발암능을 크게 강화시켜 주는 것을 발암촉진제라고 한다. 암은 세포증식 제어계에 DNA가 이상을 일으킨 현상을 말하는 데 이와 같은 DNA의 변형된 유전정보에 의해 암과 관련된 단백질을 합성하므로 이 DNA를 암유전자라 부르며 바이러스에서 유래된 것을 V-one 그리고 세포에서 유래된 것을 c-one이라 한다. 암유전자는 본래 암을 형성하기 위한 것이 아니라 증식제어 유전자로서 변이나 비정상으로 활성화 됨으로써 암을 유발시키므로 proto-oncogene이라 부른다. 또한 고등동물의 유전자 중에는 세포성장을 억제하는 유전자들이 있으며 이들은 세포의 성장 생존 분화를 조절하는 것으로 생각되고 있다. 따라서 이들 유전자는 세포의 암변형을 억제하는 기능을 가지고 있다. 이들 유전자의 이상으로 세포성장 억제기능이 상실되면 세포의 과잉 성장이 초래되면 결과적으로 암으로 유발하는 것으로 추측되고 있다. 최근의 연구에 의하면 암세포에서 암억제 유전자의 이상과 암유전자의 활성화가 함께 발견 되면서 정상세포가 암으로 변형되는 과정에는 암억제 유전자의 이상과 암유전자의 활성화가 동시에 관여한 가능성이 높은 것으로 알려져 있다. 정상 세포가 암을 유발하기 위해서는 발암의 다단계설에서와 같이 여러 단계의 변과가 필요한데 여러 가지 요인에 의해 정상세포의 염색체가 변화되어 정상세포들이 가지고 있는 세포분열의 특이성을 상실하고 증식이 빠르고 저항력이 강한 세포가 선택 되어지고 비정상 서ㅔ포으 과잉 분여러에 의해 종양이 형성되며 이어서 혈관의 신생을 촉진하는 맥관형성, 전이 등이 과정을 거쳐 신체의 다른 부위로 전이된다. 20세기 초까지는 암은 노화와 함께 자연발생적으로 일어나는 피할 수 없는 질병으로 여겨졌으며 그 치료도 조기에 발견된 암환자에게 외과적인 치료를 하는 것이 최선의 방법 이었다. 그러나 현재에는 암환자의 80% 이상이 환경적 요인에 의해 암이 발생 된다고 믿어지고 있다. 과거 치료에 중점을 둔 것에서 점차 예방의 가능성과 그 방법의 모색에 관심을 갖게 되었으며 치료적인 면에서도 외과적 수술 이외에 방사선 치료, 항암제의 투여 등 약물요법, 면역요법의 이용 이외에 더 나아가 gene theraph 및 tumor vaccines 개발에 대한 관심도 증가되고 있다. 국제암연구협회에서는 인간에게 발암이 가능한 물질의 종류를 정기적으로 발표하고 있는데 지금까지 발암 가능성이 높다고 널리 알려진 위험요인을 크게 나누어 보면 다음과 같다. 흡연, 음주, 식이요인, 호르몬 및 기타 요인으로 약물, 자외선 등을 들 수 있는데 현재까지 이들 요인에 의한 발암 기작이 완전히 규명된 것은 아니지만 이들에 의한 발암의 확률이 높다는 것은 사실이므로 이 위험요인에 노출되는 것을 방지함으로써 암발생을 예방할 수 있을 것이다. 암발생의 예방법으로는 암발생 자체를 예방하는 것과 이미 발생한 암환자를 조기 발견하고 치료하는 방법이 있을 수 있다. 현재까지의 여러 연구 결과들을 보면 유전적인 요인을 제외한 대부분의 발암 위험인자들은 개개인의 생활습관과 밀접히 관련되어 있음을 알 수 있다. 이를 바탕으로 1992년 대한 암협회에서는 '암 예방 14개 권장 사항'을 발표하여 국민 홍보활동을 하고 있는데 그 내용의 반 이상이 식생활 습관과 관련되어 있을 정도이므로 암예방에 있어서의 식품의 역할이 매우 크다 할 수 있다. 따라서 건전한 생활습관과 더불어 적절한 식품의 섭취는 암예방을 위한 기본이 될 것이다.

Clinical Features and Management of Multidrug-Resistant Tuberculosis (다제 내성 폐결핵 환자의 임상상 및 치료에 대한 고찰)

  • Objectives: Although outbreak of MDR Tb has been a recent problem in western countries, it has been a longstanding problem in Korea. The poor outcome of MDR Tb is mainly due to poor compliance, high rate of side reaction of secondary drugs, and limitation in number of available drugs. Thus, to improve the outcome of MDR Tb, it is crucial to make individualized adequate prescription based on the knowledge of the patterns of resistance to each drugs in the community as well as the natural history. The purpose of present study is to evaluate the clinical features of Korean MDR Tb patients including patterns of drug resistance and success rate of treatment which was prescribed according to the sensitivity tests. Methods: Retroscpective analysis of 71 Korean patients with MDR Tb was made. All strains isolated from patients showed resistence to at least two first line drugs. Patients profile, previous treatment history, patterns of drug resistance, outcome of treatment was analysed. Initial treatment regimen was selected according to the previous treatment history and was modified according to the sensitivity reports. The regimen was composed to include at least 4 sensitive drugs when possible. Results: The patients showed resistance to 4.1 drugs on average. 90% of them were resistant to INH and RFP. Among 71 patients, 35 patients(49%) had cavitary lesions in CXR. Treatment outcome was analysed in 55 patients. 35 patients(67%) were improved after treatment and 18 patients(33%) showed treatment failure. 5 patients showed primary resistance. Treatment outcome could be evaluated in 4 of them and all showed improvement after treatment. 14 patients(20%) had to change their regimens due to drug side effects. The most frequent side effect was elevation of liver enzymes(6 patients). Others included dizziness, hyperuricemia, tinnitus, skin rash, GI troubles. More than 50% of side effects developed within 3 months. In repeated drug sensitivity test, the concordance rate of resistance to INH was 100% and RFP 98%. EMB, PZA showed 80% concordance rate. But in the other drugs, the concordances were less than 50%. Operation was done in 5 patient - 1 patients as a adjunctive means of chemotherapy -. In that case, negative conversion of sputum AFB was done. Conclusion: 2/3 patients of multidrug-resistant tuberculosis were improved by appropriate prescription and regular medication suggesting that more aggressive management and monitoring is indicated in multidrug-resistant tuberculosis.

Radiotherapy of Neck Node Metastases from an Unknown Primary Cancer (원발병소 불명암의 경부림프절 전이에서 방사선치료의 역할)

  • Purpose: The aim of this retrospective analysis was to analyze the results of radiotherapy administered to patients with neck node metastases from an unknown primary cancer, with or without neck dissection. Materials and Methods: From January 1986 to December 2005, 88 patients with neck node metastases from an unknown primary cancer were treated with curative intent. The age of the patients ranged from 35 to 74 years (median age, 59 years). There were 74 male and 14 female patients. Distribution of patients by N status was as follows: N1, 4 patients; N2a, 10 patients; N2b, 48 patients; N2c, 8 patients; N3, 18 patients. Fifty-one patients underwent neck dissection and 37 patients had only a biopsy (31 patients had fine-needle aspiration and 6 patients had an excisional biopsy). All patients received radiotherapy. The follow-up time ranged from 1 to 154 months, with a median time of 32 months. Results: The overall survival (OS) and disease-free survival (DFS) rate at 5 years were 43.9% and 41.7%, respectively. The factors associated with the OS rate were neck dissection, and a subsequent primary tumor. Factors associated with the DFS rate were N stage, neck dissection, and a subsequent primary tumor. Neck failure was noted in 15 patients, distant metastases in 18 patients, and a subsequent primary tumor in 8 patients. Conclusion: With comprehensive radiotherapy given to the bilateral neck and the potential mucosal sites, good survival rates can be obtained in patients with neck node metastases from an unknown primary cancer. However, considering the side effects, a randomized trial is required to determine the optimal radiotherapy volume.

Analysis of Treatment Failure after Curative Radiotherapy in Uterine Cervical Carcinoma (자궁경부암에 있어서 방사선치료 후의 치료실패 분석)

  • Purpose : The aim of this study is to analyze the treatment failure patterns and the risk factors for locoregional or distant failure of uterine cervical carcinoma treated with radiation therapy. Materials and methods . A retrospective analysis was undertaken of 154 patients treated with curative radiation therapy in Gyeongsang National University Hospital from April 1989 through December 1997. According to FIGO classification, 12 patients were stage IB, 24 were IIA, 98 were IIB, 1 were IIIA, 17 were IIIB, 2 were IVA. Results : Overall treatment failure rate was $42.1\%$ (65/154), and that of complete responder was $31.5\%$ (41/130). Among 65 failures, 25 failed locoregionally, another 25 failed distantly, and 15 failed locoregionally and distantly. Multivariate analysis confirmed tumor size (>4 cm) as risk factor for locoregional failure, and tumor size (>4 cm), pelvic lymph node involvement as risk factors for distant failure. Conclusion : On the basis of results of our study and recent published data of prospective randomized study for locally advanced uterine cervical carcinoma, we concluded that uterine cervical carcinoma with size more than 4 cm or pelvic lymph node involvement should be treated with concurrent chemoradiation.

  • Purpose: Brain metastases are present in approximately 10-16% of small cell lung cancer patients at diagnosis. Brain metastasis is an important clinical problem associated with increasing the survival rate, with a cumulative incidence of up to 80% in patients surviving 2 years. Prophylactic cranial irradiation(PCI) reduces the incidence of brain matastasis and may prolong survival in patients with limited small-cell lung cancer who achieved complete remission. This study was performed to analyze the incidence of brain metastasis, survival and clinical aspects after PCI in patients with limited small-cell lung cancer who achieved complete remission. Methods : Between 1989 and 1999, forty-two patients with limited small-cell lung cancer who achived achieved complete remission after therapy were enrolled into this study retrospectively. All patients received etoposide and cisplatin(VPP) alternating with cytoxan, adriamycin, and vincristine(CAV) every 3 weeks for at least 6 cycles initially. All patients received thoracic radiotherapy: concurrent(38.1%) and sequential(61.9%). All patients received late PCI. Results : Most patients(88.1%) were men, and the median age was 58 years. The median follow-up duration was 18.1 months. During the follow-up period, 57.1% of the patients developed relapse. The most frequent site of relapse was chest(35.7%), followed by brain(14.3%), liver(11.9%), adrenal gland(44%), and bone(2.2%). With the Kaplan-Meier method, the average disease-free interval was 1,090 days(median 305 days). The average time to development of brain relapse after PCI and other sites relapse(except brain) were 2,548 days and 1,395 days(median 460 days), respectively. The average overall survival was 1,233 days(median 634 days, 21.1 months), and 2-year survival rates was 41.7%. The average overall survival in the relapse group was 642 days(median 489 days) and in the no relapse group was 2,622 days(p<0.001). The average overall survival in the brain relapse group was 928 days(median 822 days) and in the no brain relapse group was 1,308 days(median 634 days)(p=0.772). In most patients(85.7%), relapse(except brain) or systemic disease was the usual cause of death. Brain matastasis was the cause of death in 14.3% of the cases. Conclusions : We may conclude that PCI reduces and delays brain metastasis in patients with limited small cell lung cancer who achieved complete remission. We found decreased survival in relapse group but, no significant survival difference was noted according to brain matastasis. And relapse(except brain) or systemic disease was the usual cause of death. In order to increase survival, new treatment strategies for control methods for relapse and systemic disease are required.

A Case of Tuberculosis Presented with Pseudoaneurysm of the Aorta and Acute Respiratory Distress Syndrome (복부 대동맥 가성동맥류와 급성호흡곤란증후군으로 동시에 발현한 결핵)

  • We experienced a case of tuberculosis presented simultaneously with pseudoaneurysm of the aorta and ARDS. A 63-year-old man was admitted with complaints of fever and right upper quadrant abdominal pain for a week. Chest X-ray on admission showed a normal finding. Sub-diaphragmatic abscess on abdomen CT scan was suspected and serosanguinous fluid was aspirated from the abscess pocket. On day 2, he was getting more dyspneic and chest X-ray revealed extensive bilateral infiltration. Aspirated fluid revealed AFB. Later, follow-up abdomen CT scan revealed a leakage of dye from the aneurysmal sac of the descending aorta which was previously diagnosed as sub-diaphragmatic abscess. An aortic stent was placed, covering the opening into the aneurysm. The sputum also showed positive AFB. The patient was successfully weaned from the ventilator, and discharged with anti-tuberculous medication on day 42. Follow-up abdomen CT scan 6 months later showed that the aneurymal sac was completely disappeared.
