상악골에서 Veneer 골이식과 동반된 즉시 임프란트 매식

  • Patients who present with severe maxillary bone loss Secondary to trauma or resorption provide a unique diagnosis and difficulty in implant installation. To solve this problem, various bone grafting methods and graft materials can be used in these compromised cases. But for the patients required wide reconstruction, autogenous iliac bone has been used widely because of easy harvesting of much corticocancellous bone. Usually, implant installations are performed 6-12 months later after grafting. but this method necessary long treatment period and additional surgery. To avoid this disadvantages, bone grafting with immediate implant installations are frequently used. But special care is necessary to prevent postoperative complication in this method. We present three cases of veneer grafting with immediate implant installation. These patients had a knife-edge shaped alveolar bone due to labial alveolar bone loss. We reconstructed this alveolar bone with veneer grafting with iliac bone and performed immediate implant installation. There was no complications during healing periods in these three cases. Six months later, we found good survival of grafting bone and osseointegration of these implants, so we could perform prosthetic treatment successfully.

  • We analyse benefits and costs of investment on the assurance of aircraft design and development of related infrastructure. Although the discounted cash flow from the sales of aircrafts is found to be smaller than the amount of required investment, the net present value considering the effects of export increase, import substitution, and the accident cost reduction is very high. This justifies governmental investment, for the effects are not easily appropriated by private investors.

  • Kimura's disease is uncommon and chronic inflammatory disease of unknown etiology. It manifests as solitary or multiple subcutaneous nodule, primarily located on the cervical area with local adenopathies and salivary gland hypertrophy. Its clinical course is benign nature. Histologic findings are hyperplastic lymphoid tissue, inflammatory infiltrates rich in eosinophils and proliferations of capillary venules. Treatment options are steroid therapy, radiation therapy and surgical excision. We report two cases of Kimura's disease on medial aspect of upper arm.

  • Tracheal bronchus is an uncommon anomaly of the airway in which an ectopic bronchus arises from the trachea superior to its bifurcation. It is usually asymptomatic and no intervention is needed. However in the cases complicated with recurrent pneumonia, bronchiectasis or abscess, surgical excision may be needed. We report 5 cases of tracheal bronchus with or without complication or combined anomaly.

  • The report of massive intratumoral hemorrhage from vestibular schwannoma is rare. A 66-year-old female who had suffered from disturbance of hearing for one year developed severe headache and dizziness. Brain MRI showed crescent shaped mass in the left cerebellopontine angle. A left suboccipital approach revealed an $3{\times}3cm$-sized encapsulated mass. The tumor was totally extirpated together with clot. Histologically the tumor was schwannoma with massive hemorrhage. Postoperative course was uneventful. The authors report the rare case of vestibular schwannoma presenting with intratumoral hemorrhage with review of possible pathophysiology and associated factor.

  • The term corrected transposition of great arteries [hereafter referred to as corrected TGA] of the heart in which there is both a discordant atrio-ventricular relationship and transposition of the great vessels. Usually situs solitus is present, while the ventricles are inverted showing an l -loop. The great vessels are transposed and in the l-position so that the pulmonary artery arises from the right-sided morphological left ventricle and the anteriorly l- transposed aorta arises from the left-sided morphological right ventricle yielding an SLL pattern. In the majority of cases, associated lesions are common. The most frequent are ventricular septal defect, obstruction to the pulmonary outflow tract, tricuspid valve incompetence and atrio-ventricular conduction abnormalities. In the rare cases, no associated conditions are present and hemodynamic pathways are normal. In the report, we present one case of a 20 year-old male having corrected TGA associated with severe tricuspid valve incompetence, was corrected by tricuspid valve replacement, directly developed a supra-ventricular tachycardia but was controlled by calcium-entry blocker, verapamil, successfully.

  • An 11-month old girl with a feeding difficulty and recurrent aspiration pneumonia received surgical correction of complete vascular ring, which was formed by right aortic arch, aberrant left subclavian artery(LSCA) originating from Kommerell's diverticulum(KD) and ligamentum arteriosum. Through left posterolateral thoracotomy, the ligamentum arteriosum was divided to relieve the tracheo-esophageal bundle. KD was separated from the right descending aorta, and the left subclavian artery was severed from KD at its origin and trasfered to the side wall of left common carotid artery. Postoperative course was uneventful, and the patient has been followed up with a clinical improvement.

  • Tracheal diverticulum is relatively rare. It results from congenital or acquired weakness of the tracheal wall. Most cases are asymptomatic, but when symptoms are present, they are usually nonspecific. A 54-year-old man complained of sputum lasting for several months. Chest computed tomography showed an air-containing cystic structure in the trachea. Fiberoptic bronchoscopy demonstrated ostium arising from the right posterolateral wall at the trachea. Reported herein is a case of eosinophilic bronchitis associated with tracheal diverticulum.

  • Bilateral clavicle fractures caused by a strong external force acting at the same time is a rare case. While unilateral clavicle fracture with conservative treatment shows satisfactory clinical outcome, bilateral clavicle fractures with it has drawbacks such as severe pain, difficulty in rehabilitation and breathing. We experienced a rare case of bilateral clavicle fractures with multiple rib fractures, hemo-thorax and transverse process fracture of thoracic vertebra treated with surgical intervention promoted functional recovery and comfortable breathing. So, we report this case with article review.

  • A ganglion cyst is a soft tissue mass that is surrounded by a dense connective-tissue capsule. The capsule is filled with a viscous fluid that is rich in hyaluronic acid and other mucopolysaccharides. But, Ganglion cysts in the knee joint are rare. There are very few case reports of ganglion cysts related to the surface of the anterior cruciate ligament, Posterior cruciate ligament and medial meniscus. We are reporting a case of a ganglion cyst in the anterior aspect of the anterior cruciate ligament accompanying with discoid lateral meniscus in the right knee of a 46-year-old woman without any history of trauma. The cyst and discoid lateral meniscus were treated successfully with arthroscopic excision and partial meniscectomy.
