Changes in Bronchoscopic Findings during Treatment-Course in Active Endobronchial Tuberculosis (활동성 기관지결핵에서 치료경과에 따른 기관지경소견의 변화)

  • Background: Endobronchial tuberculosis is classified into 7 subtypes as fibrostenotic type, edematous-hyperemic type, actively caseating type, tumorous type, ulcerative type, granular type and nonspecific bronchitic type by bronchoscopic features, and we make a prospective study to follow up how bronchoscopic findings change during treatment-course in each subtype of active endobronchial tuberculosis. Methods: We planned to do follow-up bronchoscopic examination every month until there was no significant change in endobronchial lesion, then every 3 months and at the end of the treatment in each patient with biopsy proven endobronchial tuberculosis from May, 1990 to August, 1993. Results: 1) This study included 66 cases, but bronchoscopic follow-up was completed as scheduled in 47 cases. 2) In actively caseating and edematous-hyperemic type, bronchostenosis occurred within 2 or 3 months of treatment in about 2/3 of total cases. 3) In fibrostenotic type, bronchostenosis did not improve in spite of the treatment. 4) In tumorous type, the changes in bronchoscopic findings were unpredictable because new lesions occured on other sites even 4 or 6 months after treatment in 2 cases and the size of initial mass increased 6 months after treatment in 1 case (among 7 cases). 5) Granular and nonspecific bronchitic type improved without significant sequelae within 2 or 3 months of treatment. Conclusion: It may be necessary to follow up the patient with bronchoscopy repeatedly 2 or 3 months after starting treatment in active endobronchial tuberculosis, and it is better to perform bronchoscopic examination at 6 months of treatment, especially in patients with tumorous type because there is possibility that new endobronchial lesion occurs. Aggressive therapeutic modalities such as stent-insertion, laser therapy or electrocautery should be considered to prevent bronchostenosis in cases with granulation tissue, fibrostenotic and tumorous types of endobronchial tuberculosis.

Treatment of Type 2 Distal Clavicle Fracture using Wolter Plate (Wolter 금속판를 이용한 제 2형 원위부 쇄골 골절의 치료)

  • Purpose: This study examined the clinical results of the treatment of type 2 distal clavicle fracture with using a Wolter plate. Materials and Methods: Between 2004 and 2007, 16 patient treated for type 2 distal clavicle fracture using a Wolter plate were included in this study. Their average age was 32.6 years and the postoperative mean follow-up period was 22.9 months. The reduction and union were qualified according to the immediate post-operative and final radiographs. The functional outcome was evaluated by Kona's system and the Constant score Results: By Kona's functional evaluation, there were 12 cases with excellent results, 3 cases with good results and 1 case of fair results and the average Constant score was 90. All 16 cases showed bony union. As complications, there was 1 case in which the protruded hook of the plate could be palpated at the skin, and 1 case showed an acromial fracture, but all the cases dispalyed successful bony union and there was no acromioclavicular joint arthritis, infection or any other complications. Conclusion: Wolter plate fixation for type 2 distal clavicle fracture is a reliable surgical method for satisfactory reduction and rigid fixation, a lower incidence of nonunion and excellent clinical result.

  • In this paper, the design procedure of substructure of the steel-type breakwater was described and the actual foundation design was performed for the test bed. The site investigation was executed at the Osan-port area, in Uljin, Gyeongbuk, where the steeltype detached breakwater is constructed. The foundation mainly depends on the lateral load and uplift force due to the wave force. Since the superstructure is stuck out about 9.0m from the ocean bed, the foundation must resist on the lateral force and bending moment. After considering various factors, the foundation type of this structure was determined by the steel pipe pile(${\varphi}711{\times}t12mm$). On the stability of pile foundation, the safety factors of the pile on the compressive, lateral and uplift forces were grater than the minimum factor of safety. The displacements of pile under the working load were evaluated as the values below the permissible ones. Based on the subgrade reaction method, we evaluated the relationship of subgrade reaction and displacement for the lateral and the vertical directions in the layers. The structural analyses along with the foundation were perfomed and the effect of pile foundations were compared quantitatively.

  • Within the Gyeongju Basin lies the central an ancient tombs (Wolseongbuk tombs), which are seen to be the core tombs of the Silla ruling class. An accurate understanding of the location of the ancient tombs, commonly known as a flatland area, provides a clue to understanding the contrast process and direction of the ancient tombs. This in turn requires an accurate understanding of the surrounding landscape, including where the ancient tombs are located. In other words, it must be possible to restore as much of the highland area as possible within the basin in which the ancient tombs are located. All data were analyzed as deeply as possible in order to identify the topographical features of the ancient tombs. As a result, it appears that the ancient tombs are located at the end of a fan or at the end of the line, and a large number of springs and wetlands are distributed around the area. This area is relatively low and unsuitable for generating high levels of moisture on the ground. These topographical features are directly related with the distribution of polymers, and solids were completely formed to avoid wetlands. Meanwhile, the ancient tombs are divided into several zones by springs and wetlands, and each area also has the characteristics of large groups where the boundaries are protruding and isolated. Also, this aspect was found to be true for the Oreung around Namcheon. After all, the location and distribution of Silla in the Daerungwon area are the result of the reflection of the fine geographical features of the Gyeongju basin, which are the key factors of springwater and wetlands.

Miniaturization of UHF Planar Antenna Employing Slot-loading (Slot-loading에 의한 UHF 대역 평면 안테나의 소형화 설계)

  • Planar antennas are very efficient for the use of the out-door data transmission applications due to their low-profile characteristics. But the size of the antenna in UHF band blocks the practical usage. In this paper, a miniaturized planar antenna is investigated for the application of AMR system in UHF band. The designed antenna is fabricated in the form of the microstrip patch on the FR-4 substrate. The miniaturization is achieved by slot-loading on the patch. First, effects of simple slots on the size reduction are analyzed, followed by the design using more complex slots resulting in the reduction ratio of 37.9% in 425.6 MHz and bandwidth of 3.8 MHz. The effect of epoxy coating for the protection is also investigated.

Taxonomical Review for Tilingia tsusimensis (Apiaceae) (대마참나물(Tilingia tsusimensis, 산형과)의 분류학적 재검토)

  • It was revaluated about taxonomical position of the Tilingia tsusimensis (Yabe) Kitagawa (Apiaceae) collected from Mt. Gaya, Mt. Geumo, and Mt. Songni in Korea. Its diagnostic features include slightly dorsally compressed mericarps, acute-prominent ribs, ternate leaves, and dimorphic white petals. It is morphologically similar to Cymopterus and Pimpinella in having ternate leaves. It is distinguished from Cymopterus having asymmerty mericarps, 3-4 mericarp ribs, and winged dorsal ribs. Pimpinella differs in having slightly laterally compressed mericarps without ribs and conspicuous calyx teeth. Also, it looks like Angelica with obsolete calyx teeth and multilayer pericarps, but is distinguished by having its broadly winged lateral ribs and strongly dorsally compressed mericarps. Tilingia tsusimensis formed a monophyletic clade or sister group relationship with a genus Angelica based on ITS sequences. Based on the above observations, we conclude that this plant is here referred to T. tsusimensis (Yabe) Kitagawa.

Histological Study of the Early Gonadal Development and Sexual Differentiation in Rhynchocypris oxycephalus (버들치, Rhynchocypris oxycephalus 초기 생식소 발달과 성분화에 관한 조직학적 연구)

  • The early gonadal development and sexual differentiation of Rhynchocypris oxycephalus are described from the stage of hatching to 150 days after hatching. During this peroid, the average length of the body grew from 0.64 cm to 5.96 cm. the primordial germ cells (PGCs), which could be recognized at the time of hatching, began to protrude into peritoneal cavity at a standard total length of 1.91 cm. At a standard total length of 2.29cm, initial ovarian differentiation wasidentified by the transformation of PGCs to meiotic oocytes. Finally, at the standard total length of 5.96 cm, the female gonads gradually developed towards migratory nucleus oocytes, characterisiing the maturation. Oocytes proliferated rapidly after sex differentiation while the testis entered a period of quiescence, as they continued to multiply but did not undergo growth until the standard total length of 4.00 cm. At a standard total length of 4.00 cm, spermatocytes arrested in thephase of interkinesis, Sertoli-like cells and sperm duct formation, with signs of meiotic activity, were observed. Therefore it may be concluded that R. oxycephalus belongs to the differentiated type of gonochoristic teleosts.

  • The posterior root of medial meniscus maintains normal meniscal function by circumferential hoop tension and prevents extrusion of meniscus and progression of osteoarthritis. A complete tear of posterior root of medial meniscus leads to loss of hoop tension, it is important to repair it and preserve the function of the medial meniscus. Recently, a variety of arthroscopic assisted reduction and repair techniques have been used. We create an arthroscopic all-inside suture technique using a 18 gauge spinal needle and suture anchor that is easier and more convenient compared with the previous techniques. So we report this technique with a review of current literatures.

Notes on Three Species of the Laboulbeniales (Ascomycotina) Newly Collected from Korea (한국산 충생자낭균류의 미기록종에 관하여)

  • Three species belong to the genus Laboulbenia were collected on Carabidae (Coleoptera) and Harpalidae (Coleoptera) from several regions of Korea. Laboulbenia anoplogenii Thaxter was collected from Stenolophus quinquepustulatus Widemann. The characters of this species are the fourth layer of receptacle consisted of two or three cells and the distal end of its posterior cell projecting upward. The thalli were produced on posterior and anterior legs of the hosts. Laboulbenia habui Terada was collected from Chlaenius variicornis Bates. This species is characterized by the basal cells of the secondary appendage composed of short-cylindrical cells, arranged alternately on a double row. The thalli were produced from the elytra and anterior abdomen of the hosts. Laboulbenia pallida Thaxter was collected from Anisodactylus signatus Panzer. This species is distinguished from other related species by the rounded tip of perithecium and the outer appendage branched on the basal cell. The thalli were produced on the posterior elytra of the hosts.
