클램운동 시 허리불안정성 유무가 골반 돌림 및 엉덩관절 벌림근 근활성도에 미치는 영향 (The effect of whether the lumbar is instable or not and pressure biofeedback application location during clam exercise on pelvic rotation and hip joint muscle activity)

  • Background: The purpose of this study was to find out how the lumbar instability during clam exercise causes changes in pelvic rotation and hip joint abductor muscle activity. Design: Case-control study. Methods: Twenty male participated in this study. The amount of pelvic rotation was measured using myomotion during the clam exercise in a normal group and a group of patients with low lumbar instability, and the muscle activity of the muscle gluteus medius and the tensor fasciae latae using an EMG device. Results: The amount of pelvic rotation that occurred during the clam exercise was statistically significantly greater in the lumbar instability group than in the normal group (p<.05), and the ratio of muscle activity of the muscle gluteus medius/the tensor fasciae latae was statistically significantly higher in the normal group than in the lumbar instability group (p<.05). Conclusion: In order to stabilize the lumbo-pelvic and strengthen the strength of the hip joints, it is believed that it is necessary to apply exercise while controlling the lumbo-pelvic movement during clam exercises, and various studies will be needed.

가슴우리팽창과 가로막 호흡운동이 돌림근띠 복원술 환자의 통증과 기능에 미치는 효과 (Effects of Thoracic Expansion and Diaphragm Breathing Exercises on Pain and Function in Patients with Rotator Cuff Repair)

  • Purpose: The study aims to determine the effect of a breathing exercise on shoulder pain, range of motion (ROM), and forward head posture in patients with rotator cuff repair. Methods: In total, 25 patients with rotator cuff repair were included in this study. The experimental group (n = 13) underwent a breathing exercise, while the control group (n = 12) received traditional physical therapy. The visual analogue scales (VASs) for pain, flexion and abduction ROM, and the craniovertebral angle (CVA) of both groups were recorded at both pre- and post-intervention. Paired t-tests were used to determine significant changes in the post-intervention compared with the pre-intervention period, and independent t-tests were used to analyze differences in dependent variables between the two groups. Results: After the two-week intervention, the experimental group experienced a significantly decreased VAS (p < 0.05) and significantly increased ROM and CVA (p < 0.05), while the control group experienced a significantly decreased VAS (p < 0.05). Further, the experimental group that underwent the breathing exercise showed greater improvements in flexion and abduction ROM and in the CVA than the control group (p < 0.05). Conclusion: The results suggested that a breathing exercise can reduce shoulder pain and enhance ROM and posture in patients with rotator cuff repair.

  • Background: This study was to investigate the relationship between scapular downward rotation ratio (SDRR), lower trapezius (LT) muscle, and rounded shoulder angle (RSA) on each side in subjects with scapular downward rotation syndrome (SDRS). Design: Cross-sectional Study Methods: Fifteen subjects have participated in this study. The RSA and SDRR were assessed using a tape measure in standing posture. The RSA was computed by the angle made by two lines: one was the distance between the root of the scapula and the acromion, and the other was the distance between the acromion and the horizontal line in the root of the scapula. The SDRR was computed by two horizontal lines: one was the distance between the mid-line and root of the scapula, and the other was the distance between the mid-line and inferior angle of the scapula. LT muscle strength was performed in a prone position by the hand-held dynamometer. Results: There was a positive correlation between SDRR and LT strength in the less affected sides (r=.59; p=.02), however, there was no correlation between RSA and LT strength in the more affected sides (p>.05).

자가 등 가동운동이 어깨 통증 및 기능에 미치는 효과 (The Effect of Self Thoracic Mobilization Exercise on Shoulder Pain and Function)

  • 본 연구는 자가등 가동운동(STME, Self Thoracic Mobilization Exercise)이 어깨 기능장애 환자에게 어깨통증과 장애정도, 어깨의 기능에 미치는 영향을 알아보고자 하였다. 어깨 기능장애 환자 32명을 대상으로 무작위로 STME와 대조군으로 할당하였으며, 시각 상사척도와 어깨기능장애, 어깨의 가동범위 평가를 중재 전-후 실시하였다. 16명의 STME 중재 그룹은 폼롤러를 활용하는 방법과 네발자세에서 이용하여 굽힘, 폄을 자가로 실시하였고, 16명의 대조군은 전기치료 패드를 이용한 플라세보 효과를 활용하였다. 두 집단은 20분 동안 주당 3회를 4주간 실시하였다. STME군이 대조군 보다 통증과 기능장애에서 모두 유의한 향상을 보였으며(p<0.05), 어깨의 가동범위는 STME을 이용한 그룹이 플라세보 그룹 보다 굽힘, 벌림, 가쪽 돌림에서 유의한 효과를 보였다(p<0.05), 하지만 폄과 안쪽돌림에는 유의한 효과를 보이지 못했다(p>0.05). 따라서 STME은 어깨통증과 기능장애 해결에 효과적이며, 향후 어깨 통증 환자에게 보다 쉬운 접근방법의 중재방법을 교육하고 효과적인 치료를 할 수 있을 것이다. 나아가 사회적 치료비용 또한 경감시킬 수 있을 것이다.

초음파 펄스 에코 패턴으로 용접 결함 식별 방법 연구 (Study of the Weld Defects Identification Method by Ultrasonic Pulse Echo Patterns)

  • 본 연구에서는 초음파 탐상시험의 펄스 반사법으로 각각 용접결함에 따른 초음파 펄스파형모형을 연구하였다. 균열은 예리하고 선명한 신호들을 발생한다. 탐촉자를 결함주위로 이동하면 에코높이는 변한다. 긴 균열에서는 탐촉자가 결함 주위를 원형으로 목돌림주사법을 사용하여 탐상하면 에코높이는 급격히 감소한다. 그 에코 봉우리는 바늘과 같이 얇고 날카롭다. 기공은 단일 결함으로부터 발생하는 에코는 예리하고 선명하다 하지만 집단의 기공들은 다수의 반사들이 중첩되고 트레이스가 들쭉날쭉한 에코가 발생한다. 슬래그 개재물은 크랙과 슬래그 결함위치에서 각각 목돌림 주사법을 사용하여 탐상하면 그 에코형상은 어느 정도 차이를 볼 수 있었다. crack은 그 에코높이가 급격히 변하는 반면에 슬래그 개재물은 증가${\rightarrow}$감소${\rightarrow}$증가${\rightarrow}$감소된다. 또한 다수 밀집된 기공의 위치에서 결함은 대표적 에코형상과 같은 잡다한 에코형상은 슬래그에서는 볼 수 없었다. 용입불량은 결함의 에코형상은 크랙과 같이 날카롭고 예리하게 나타났고, crack과 비슷한 에코형상은 갖고 있었다.

주짓수 선수의 허리 통증 유무에 따른 엉덩관절 돌림 가동범위 비교 (Comparison of Hip Rotation Range of Motion in Jiu-Jitsu Athletes With and Without Low Back Pain)

  • Background: A limited hip rotational range of motion (ROM) has been considered to be one of characteristics of low back pain (LBP) in athletes. Although LBP frequently occurs in jiu-jitsu athletes, no previous has compared hip rotational ROM between jiu-jitsu athletes with and without LBP. Objects: The aim of the study was to compare ROM for hip internal rotation (IR) and external rotation (ER), and total hip rotation between jiu-jitsu athletes with and without LBP. Methods: Jiu-jitsu athletes were recruited for the LBP group ($n_1=15$) and control group without LBP ($n_2=15$). IR, ER, and total rotational range of hip joint were measured using a goniometer. Analysis of variance was used to compare the ROM between groups and sides. Results: The LBP group showed a significantly lower range of passive hip IR, passive total rotation, active IR, active ER, and active total rotation than the control group (p<.05). Dominant side of passive hip IR and active IR had a significantly lower ROM than non-dominant side (p<.05). In passive ER ROM, non-dominant side was significantly greater than dominant side (p<.05). Conclusion: Compared to jiu-jitsu athletes without LBP, athletes with LBP exhibit a loss of hip rotational ROM. Based on these results, clinicians and athletic trainers should measure hip rotational ROM when designing the management plan for jiu-jitsu athletes with LBP.

월 슬라이드와 슬링 슬라이드 운동이 어깨뼈 아래쪽 돌림 증후군의 어깨뼈 주변 근육의 활성도에 미치는 영향 (Effects on Muscle Activities around Scapula for Wall Slide and Sling Slide Exercises in Subjects with Scapular Downward Rotation syndrome)

  • Background: Evidence for effective management of scapular downward rotation syndrome is limited. The present study was performed to compare the scapular muscle activation through 4weeks wall slide exercise and sling slide exercise in subjects with scapular downward rotation syndrome. Methods: Twenty-two subjects with scapular downward rotation syndrome participated in the study. Surface electromyography data were collected from the upper and lower trapezius, serratus anterior and pectoralis major during shoulder flexion of $60^{\circ}$, $90^{\circ}$ and $120^{\circ}$ in the sagittal plane. The alignment of the scapula was measured using radiographic analysis. Subjects were assessed pre and post a 4 weeks exercise (wall slide, sling slide). The significance of the difference in pre- and post-exercise within each groups was assessed using a paired t-test. The significant difference between wall- and sling-exercise was used a independent t-test. Results: In the wall slide group, the muscle activity of upper trapezius decreased significantly during shoulder flexion at $60^{\circ}$, $90^{\circ}$ and $120^{\circ}$ after 4 weeks, and the muscle activity of serratus anterior increased significantly at all angles. Also, the muscle activity of pectoralis major decreased significantly at $90^{\circ}$ and $120^{\circ}$. Conclusions: Based on such results, it can be said that wall slide exercise is effective than sling slide exercise for the subjects with scapular downward rotation syndrome.

운동프로그램 중재가 돌림근띠 복원술 환자의 근 활성도에 미치는 영향 (Effects of Exercise Program Intervention on Muscle Activity in Rotator Cuff Repair Patient)

  • Purpose: This study was conducted in order to examine how an effective rehabilitation exercise program influences the activity of shoulder muscles, and to help the clinical application of a rehabilitation program, for prevention and relief of pain, adhesion, and joint stiffness of patients who undergo rotator cuff repair. Methods: Nine test subjects were placed randomly into each group for a total of 27 subjects and exercise program interventions according to the group were conducted for six weeks, after which maximum voluntary isometric contraction (%MVIC) value was re-measured for supraspinatus muscle, infraspinatus muscle, serratus anterior muscle, and middle deltoid muscle in all groups in order to compare changes in muscle activity before and after the experiment in order to perform comparative analysis of changes in muscle activity between groups, based on which four experimental hypotheses were confirmed. Results: Changes in muscle activity according to %MVIC showed a statistically significant difference (p<0.01) (p<0.001) in all muscles, except the middle deltoid muscle, and post-verification results showed that changes in muscle activity according to %MVIC were greater in test groups I and II, compared with the control group, for the supraspinatus muscle, infraspinatus muscle, and serratus anterior muscle. Conclusion: Therefore, rehabilitation through use of the methods described above should be applied efficiently in clinical settings and more research in development of much more efficient rehabilitation program interventions must be conducted.

운동면과 어깨 관절 굽힘 각도가 어깨뼈 내밈 운동 시 어깨뼈 위쪽 돌림근에 미치는 영향 (Effect of Movement Plane and Shoulder Flexion Angle on Scapular Upward Rotator During Scapular Protraction Exercise)

  • PURPOSE: This study was to determine the effect of the plane of movement (sagittal plane vs. scapular plane) and shoulder flexion angle ($90^{\circ}$ vs. $130^{circ}$) during scapular protraction exercises in healthy subjects by investigating the elecromyographic (EMG) activities of the serratus anterior (SA), upper trapezius (UT), and pectoralis major (PM). METHODS: Twenty-one healthy subjects participated in this study. Subjects performed maximal scapular protraction at the $90^{\circ}$ or $130^{\circ}$ shoulder flexion angles in the sagittal or scapular planes. Surface EMG was recorded from the SA and UT, and PM muscles. Dependent variables were examined by 2 (plane) ${\times}$ 2 (angle) repeated measures of analysis of variance (ANOVA). RESULTS: Significantly increased EMG activities in the SA and UT were found during scapular protraction exercise at the $130^{\circ}$ shoulder flexion angle in the sagittal and scapular plane. Also, EMG activity of the PM significantly decreased at the $130^{\circ}$ shoulder flexion angle in the sagittal plane and the $90^{\circ}$ and $130^{\circ}$ shoulder flexion in the scapular plane. CONCLUSION: we recommend scapular protraction exercise at the $90^{\circ}$ shoulder flexion in the sagittal plane to selectively strengthen the SA muscle with limitation of upper trapezius activity and at the $130^{\circ}$ shoulder flexion in the scapular plane to selectively strengthen the SA muscle with limitation of pectoralis major activity.

배호흡운동과 가슴우리팽창운동이 돌림근띠 손상환자의 머리위치 및 어깨자세에 미치는 영향 (Effects of Abdominal Breathing and Thoracic Expansion Exercises on Head Position and Shoulder Posture in Patients with Rotator Cuff Injury)

  • PURPOSE: The aim of this study was to examine the effects of abdominal breathing and thoracic expansion exercises on craniovertebral and cranial rotation angles in patients with rotator cuff injury. METHODS: This study enrolled 19 patients with rotator cuff injury, and ten and nine of the patients were randomly placed in abdominal breathing and thoracic expansion exercise groups, respectively. After pain treatment in each group, breathing exercise was conducted thrice a week for four weeks. The patients were recorded by using a digital camera and Image J (version 1.46, National Institutes of Health, USA), an angle measurement program, was used to analyze changes in the craniovertebral angle, cranial rotation angle, and sagittal shoulder posture. RESULTS: Statistically significant differences in the craniovertebral angle were found in both the abdominal breathing and thoracic expansion exercise groups (p<.05). A significant difference in cranial rotation angle was found in the thoracic expansion exercise group only (p<.05). No statistically significant differences in sagittal shoulder posture were found in both groups (p>.05). CONCLUSION: Although abdominal breathing and thoracic expansion exercises did not effectively change sagittal shoulder posture, the exercises were effective in improving craniovertebral and cranial rotation angles. Therefore, abdominal breathing and thoracic expansion exercises are suggested as effective exercise programs for forward head posture.
