This study aims to evaluate psychometric properties of the korean version of the caregiver reaction assessment. A total of 253 family caregivers for older adults with physical or mental impairments are recruited using the purposive sampling method. The psychometric properties of the instrument are tested by item-analysis, exploratory factor analysis(EFA), and convergent and criterion-related validity. The results show that the internal consistency values for the five dimensions of the K-CRA are good. The convergent and criterion-related validity is empirically confirmed. The EFA with the principal component method reveals the factor structure in the observed variables. The findings of the study confirmed that the K-CRA was both reliable and valid. In conclusion, the K-CRA would be useful in clinical and research settings in South Korea.
This study intended to examine the willingness to use as well as necessary functions for ICT-based care assistance devices in older adults living alone. A total of 241 older adults living alone from a city participated in the study. Study results show that many older adults living alone are willing to use ICT-based care assistance devices at their home and the functions of care assistance devices that they perceived as necessary were diverse. In addition, there were differences in perception of functional necessity by socio-demographics and medical and functional characteristics of older adults living alone. In particular, there were significant differences in the functions perceived as necessary according to the presence of osteoporosis, depression, insomnia, and limitations in daily activities. Our study findings suggest that the functions of ICT-based care devices should be developed in various ways in order to help healthy aging in place. Findings also imply that it is necessary to provide individualized services and customized care assistance devices according to individual's medical and functional characteristics and needs.
The purpose of this study was to compare the health status, functional status, and home environment hazards by the fear of falling and explore the factors that contribute to the fear of falling. This was a descriptive study. Data were collected for two years with 973 older adults who enrolled in a health center at A city. Trained nurses visited subjects' home and collected data using Resident Assessment Instrument MDS-HC. The average age of the subjects was 76.8 (±7.6) and female was 74.8%. 20.7% of subjects reported fear of falling. In terms of the fear of falling, two groups were compared. There were significant differences in vision, urinary incontinence, pain that interrupts daily activity, unstable gait, comorbidities, frequency of depressive or anxious sign. The greater the age, IADL and ADL dependency, comorbidities, unstabled gait, the more fear of falling. In conclusion, this study reassured the importance of assessing the major factors related to fear of falling such as age, IADL and ADL, comorbities, gait among older adults. This study will contribute to establish strategies for preventing fall. Reducing the fear of falling through the comprehensive nursing intervention will ultimately improve the quality of life in the elderly.
Objective : The purpose of this study was to look at a systematic review on the effects of robot-assisted therapy on stroke rehabilitation in upper limbs. Through this comprehensive study, we have to compare the studies. Methods : We systematically examined papers published in journal from June 2012 to September 2012, using EMBASE, MEDLINE, etc. 12 studies(randomized controlled trials) were included in the analyses. Results : Selected 12 studies were Pedro score 2 or more. The primary outcome measure tools including tools for upper extremity function and activities daily of living were used. Through various intervention, Robot-assisted therapy maximizes the effectiveness of intervention. The effects of upper extremity function and functional ability(ADL) by robot-assisted therapy showed significant effect, but no difference exists between the robot-assisted therapy and conventional therapy groups in terms of upper extremity function and functional ability. Conclusions : Robot-assisted therapy has been widely used clinically for stroke patients to maximize the effect had been applied to various interventions. Robot-assisted therapy depending on the methods of intervention showed that the effect of difference. In the future study, the use of robot-assisted therapy applied to a variety of interventions is needed on occupational therapy.
As people age and their physical functions are declined, they stay longer in the home, thus being more affected by the home environment. Many studies have shown the association between the home environment and health. However, little is known about the effect of the home environment on disability and depression of older people with functional limitation. This study, therefore, aimed to examine the moderating effect of the home environment on the pathway between walking limitation and instrumental activity of daily living(IADL) disability, and the relationship between IADL disability and depression. We performed logistic regression and multiple linear regression analyses with data on 3,027 participants from the 2018 Seoul Aging Survey. As results, first, older people who lived home with poor conditions showed a higher risk of walking limitation(OR=1.487, 95% CI: 1.250 - 1.770), IADL disability(OR=1.594, 95% CI: 1.303 - 1.949), and depression(OR=1.943, 95% CI: 1.553 - 2.430). Second, the home environment moderated the relationship between walking limitation and IADL disability. Older people with walking limitation who lived in poor housing condition experienced more IADL disability than others. Third, after controlling for walking limitation, the home environment had significant moderating effect on the relationship between IADL disability and depression. Elderly with IADL disability living in poor home experienced higher risk of depression. Based on these results, the study suggests the necessity of home modification intervention targeting functional limitation of older adults as strategy for preventing disability and depression, and enabling aging in place.
Recent years, mirror therapy or mirror neuron therapy has been used to improve neural damage of upper extremities among stroke patients. This systematic review was conducted to assess the clinical effect of mirror therapy used for the treatment of upper extremities for patients with stroke. After literature search, researchers selected for 9 randomized controlled clinical trial studies registered up to October 2013 based on PubMed database, using the following search terms: mirror therapy, mirror neuron, stroke, paresis, hemiplegia, upper extremity. There were significant improves of distal upper extremity function in the use of mirror therapy groups but the research using range of motion(ROM) as outcome measure. In conclusion, mirror therapy was more effective than conventional therapies or sham therapies for upper recovery of distal upper extremity function among stroke patients.
Purpose: This study was to explore the factors affecting on Dietary Behavior Change of the vulnerable elderly based on the Stage of Change. Methods: This study was a secondary analysis of the data collected from 1,262 elderly who were participated in the case management program. Total 984 participants who responded all questionnaire were included in the analysis. The variables included general characteristics, health behavior, health status, and dietary behavior stage. The data collected were analyzed by descriptive statistics, trend test and multi-variate logistic regressions. Results: about twenty four percent of the vulnerable elderly subject was pre-contemplation stage and 46.1% of them was contemplation stage of dietary behavior. Having a depressive mood, dependance of Instrumental Activity of Daily living, and being medicaid affected on starting or on maintaining healthy dietary behavior in vulnerable elderly negatively. Conclusion: The tailored nutritional intervention depending on dietary behavior stage are needed for the vulnerable elderly to improve the health. The program which can maintain the function remained of elderly and give psychological support together will be more effective.

The purpose of this study is to provide basic data for improving the life satisfaction of elderly women with chronic diseases by analyzing the factors affecting life satisfaction. Using the 6th Korean Retirement and Income Study, 1846 elderly women aged 65 years or older with chronic diseases were included in the study. In order to investigate life satisfaction, instrumental activities of daily living and interpersonal relationship according to general characteristics, descriptive statistics, t-test and ANOVA were used. We used hierarchical multiple regression analysis to find out the factors affecting life satisfaction. The results showed that elderly women with chronic illness were more satisfied with life when they were younger, spouses, higher education, proper sleep, regular exercise, and regular health checkups. This study has limitations in that the number of chronic diseases, types, and psychosocial aspects are not considered. However, it is significant that the elderly panel with the representative of the whole nation was used to grasp the life satisfaction of the elderly women with chronic diseases. Therefore, in order to improve the life satisfaction of elderly women with chronic illness, appropriate sleep, exercise, instrumental daily life ability, interpersonal intervention will be needed.
