A Study on the Prediction of Bone Remodeling of Plated-Human Femur using Stress Analysis (응력해석에 의한 골절판이 부착된 인체 대퇴골의 골재형성 예측에 관한 연구)

  • The stress distribution of bone is altered by the rigid bone plate, sometimes resulting in unfavorable osteoporosis. The rigidity and the biocompatibility are important factors for the design of prosthesis, however, it is also necessary to consider the effect on the bone remodeling. In this paper, it is attempted to establish an approximate and simple method to predict the trend of the configuration of surface bone remodeling upon a bone plate using stress analysis. Thus, three dimensional finite element model of plated-human femur is generated and simulated. In addition, the stress difference method (SDM) is introduced and attempted to demonstrate the configuration of surface bone remodeling of the plated-human femur.

  • 골다공증은 뼈의 화학적 조성에는 변화가 없고 단위 용적 내의 물량 감소를 초래해 경미한 충격에도 쉽게 골절을 일으키는 질환으로 뼛속에 구멍이 많아져 골밀도가 저하된 상태를 말한다. 우리 몸의 뼈는 소아기와 청소년기를 거쳐서 점차 단단해지고 20~30대에 생애 중 최대골량을 형성하며, 나이가 들수록 골량이 서서히 줄어들다가 폐경기 이후 여성에서 매우 빠른 속도로 골량이 감소한다. 이러한 골다공증은 대부분 증상 없이 조용히 진행되다가 골절이 나타난 후에야 발견된다. 골다공증이 진행된 후에는 허리가 구부러지고 경미한 충격에도 골절을 일으키는데 주로 척추, 대퇴골 늑글 등의 골절이 흔히 발생한다.

  • This paper presents an estimating method for local property changes and failure prediction of composite materials experiencing large shear deformation during draping process. The bone plate for the metaphyseal femur fracture was chosen to apply the presented method because it has complex geometry. The local property changes due to macro-/microscopic deformations of fabric composites during draping process were evaluated by various tests and the result was applied to predict static/fatigue behaviors of the bone plate. This paper was expected to present useful information on the design of composite structures with complex geometry and their performance evaluation.

  • After closed reduction and internal fixation with proximal femoral nail for intertrochanteric fracture, some patients complained lateral hip pain. We report two cases of lateral hip pain due to tendinopathy confirmed by ultrasonography.

Limb Salvage Surgery with Tumor Prosthesis for the Malignant Bone Tumors Involving the Proximal Femur (대퇴골 근위부 악성 골종양 환자에서 종양 대치물을 이용한 사지 구제술)

  • Purpose: As well as patient survival, the restoration of postoperative function such as ambulation is important in limb salvage operations for treatment of malignant bone tumors involving the proximal femur. The authors analyzed clinical outcomes of limb salvage operations using tumor prostheses for metastatic or primary malignant bone tumors in the proximal femur. Materials and Methods: From February 2005 to January 2014, 20 cases (19 patients) with malignant bone tumor involving the proximal femur with pain or complicated pathologic fracture were treated with segmental resection and limb salvage operations with tumor prostheses. Mean age was 63.1 years (range 35-86). Fourteen patients were male and six ones were female. The mean follow-up period was 20 months (1-94 months). There were 15 cases of metastatic bone tumor, 4 cases of osteosarcoma, and 1 case of multiple myeloma. The primary tumors of the metastatic bone tumors included 4 lung cancers, 3 hepatocellular carcinomas, and 3 renal cell carcinomas. Other primary tumors were breast cancer, thyroid cancer, colon cancer, prostate cancer, and malignant spindle cell tumor, each in 1 case. Modular tumor prostheses were used in all cases; (Kotz's$^{(R)}$ Modular Tumor prosthesis (Howmedica, Rutherford, New Jersey) in 3 cases, MUTARS$^{(R)}$ proximal femur system (Implantcast, Munster, Germany) in 17 cases). Perioperative pain was assessed with Visual Analogue Scales (VAS). Postoperative functional outcome was assessed with Musculoskeletal Tumor Society (MSTS) grading system. Results: Out of 20 cases (19 patients), 11 cases (10 patients) survived at the last follow-up. Average postoperative survival of the 9 deceased patients was 10.1 months (1-38 months). VAS score improved from pre-operative average of 8.40 (5-10) to 1.35 (0-3) after operation. Average postoperative MSTS function score was 19.65 (65.50%, 7-28). The associated complications were 2 local recurrences, 3 hematomas, 3 infections, 2 scrotal swellings, and 1 dislocation. There was no case of periprosthetic fracture or loosening. Conclusion: Limb salvage operation with tumor prosthesis is an appropriate treatment for early pain reduction and functional restoration in malignant bone tumors in the proximal femur with pain an/or complicated pathologic fractures.

A Biomechanical Study on a New Surgical Procedure for the Treatment of Intertrochanteric Fractures in relation to Osteoporosis of Varying Degrees (대퇴골 전자간 골절의 새로운 수술기법에 관한 생체역학적 분석)

  • This study investigates the biomechanical efficacies of various cement augmentation techniques with or without pressurization for varying degrees of osteoporotic femur. For this study, a biomechanical analysis using a finite element method (FEM) was undertaken to evaluate surgical procedures, Simulated models include the non-cemented(i.e., hip screw only, Type I), the cement-augmented(Type II), and the cemented augmented with pressurization(Type III) models. To simulate the fracture plane and other interfacial regions, 3-D contact elements were used with appropriate friction coefficients. Material properties of the cancellous bone were varied to accommodate varying degrees of osteoporosis(Singh indices, II∼V). For each model. the following items were analyzed to investigate the effect surgical procedures in relation to osteoporosis of varying degrees : (a) von Mises stress distribution within the femoral head in terms of volumetric percentages. (b) Peak von Mises stress(PVMS) within the femoral head and the surgical constructs. (c) Maximum von Mises strain(MVMS) within the femoral head, (d) micromotions at the fracture plane and at the interfacial region between surgical construct and surrounding bone. Type III showed the lowest PVMS and MVMS at the cancellous bone near the bone-construct interface regardless of bone densities. an indication of its least likelihood of construct loosening due to failure of the host bone. Particularly, its efficacy was more prominent when the bone density level was low. Micromotions at the interfacial surgical construct was lowest in Type III. followed by Type I and Type II. They were about 15-20% of other types. which suggested that pressurization was most effective in limiting the interfacial motion. Our results demonstrated the cement augmentation with hip screw could be more effective when used with pressurization technique for the treatment of intertrochanteric fractures. For patients with low bone density. its effectiveness can be more pronounced in limiting construct loosening and promoting bone union.
