Analysis on Femoral Neck Fractures Using Morphological Variations (파라메트릭 형상모델을 이용한 근위 대퇴골의 경부 골절 영향 해석)

  • It has been reported that the femoral morphology has a major correlation to femoral neck fractures(FNF). Previous studies to analyze these correlations have relied on mechanical testing and finite element methods. However, these methods have not been widely applied to various femur samples and models. It is because of the availability of the samples from both patients and cadavers, and also of the geometric limitations in changing the shape of the models. In this study we analyzed femoral neck fractures using a parameterized femoral model that could provide flexibility in changing the geometry of the model for the wide applications of FNF analysis. With the parameterization a variety of models could be generated by changing four major dimensions: femoral head diameter(FHD), femoral neck diameter(FND), femoral neck length(FNL), and neck-shaft angle(NSA). We have performed FEA on the models to compute the stress distributions and reaction forces, and compare them with the data previously generated from mechanical testing. The analysis results indicate that the FND is significantly related with the FNF and the FHD is not significantly related with the FNF.

Analysis of Bone Mineral Density According to the Biochemical Variable Markers in Adults (생화학적 표식자에 따른 성인들의 골밀도 분석)

  • To evaluate the bone mineral density (BMD) and biochemical markers. We evaluated the BMD of femoral neck and lumbar spines of 998(male 568, female 430) persons who took a regular health screening in Woosuk University Hospital from September 2007 to March 2008 by dual energy bone mineral densitometry. Results of BMD are different in terms of biochemical markers. Especially aged people showed osteoporotic change progressively. Degree of osteoporosis increases with age. A steep decrease of BMD can be found in postmenopausal women who have low level of female hormone. More persistent effort is needed to find out the factors that can reduce BMD values for prevention of problems by osteoporosis. In essence, research on factors related to other biochemical markers must be studied continuously.

Normal Korean Femoral Neck Anteversion, Acetabular Anteversion and Combined Anteversion Measured with Computed Tomography (컴퓨터 단층 촬영으로 계측한 정상 한국 성인에서의 대퇴 경부 전경사, 비구 전경사 및 합산 전경사)

  • Purpose: The authors measured the anteversion of the femoral neck and acetabulum and the sum of the two values in normal Korean people by computed tomography. The authors examined the normal range of the values to analyze the difference in sex and sides as well as the relationship between the femoral neck and acetabular anteversion. Materials and Methods: The authors measured the anteversion of the femoral neck and acetabulum in 118 normal Korean adult males and 114 females aged between 21 and 49 on both the right and left sides by computed tomography and calculated the sum of anteversion. The authors analyzed the mean and standard deviation, and investigated the sex differences and side differences, as well as the relationship between the acetabular anteversion and femoral neck anteversion statistically. Results: The anteversion of the acetabulum in males was 15.3°±6.1° on the right side and 15.3°±6.6° on the left side. The anteversion of the femoral neck in males was 5.3°±7.6° on the right side and 1.5°±9.2° on the left side according to the Hernandez et al. method. The anteversion of acetabulum in females was 16.8°±5.4° on the right side and 16.3°±5.8° on the left side. The anteversion of femoral neck in females was 10.3°±8.2° on the right side and 7.9°±8.2° on the left side according to Hernandez et al. method. No difference in acetabular anteversion, and a significant difference in the femoral neck anteversion on both the right and left sides were observed between males and females. No difference of acetabular anteversion was observed between the right and left sides, but a significant difference in femoral neck anteversion was noted between the right and left sides measured by either the Hernandez et al. method or Weiner et al. method. The Pearson coefficient revealed no correlation between the femoral neck anteversion and acetabular anteversion. Conclusion: No difference in the acetabular anteversion was observed, but there was a significant difference in femoral neck anteversion between males and females. A significant difference in femoral neck anteversion was observed between the right and left sides. No correlation was noted between the anteversion of the femoral neck and acetabulum.

Association of Bone Mineral Density with Physiological Characteristics and Lifestyles in Premenopausal Working Women (폐경전 성인직장여성의 골밀도와 생리적 특성 및 생활습관과의 관련성)

  • This study was peformed to assess the relationships among bone mineral density (BMD), Physiological characteristics and lifestyle factors in 61 premenopausal working women aged 30∼49 y in Busan. The BMDs of the lumbar spines (Ll∼L4), femoral necks (FN), ward's triangles (WT) and trochanters (TC) were measured by dual energy X-ray absorptiometry. Data for physiological characteristics and physical activity was assessed by questionnaire and usual intakes of coffee, green tea, alcohol, Coca cola by food frequency questionnaire. The BMDs of L14, FN, WT and TC were 1.02 g/$\textrm{cm}^2$, 0.76 g/$\textrm{cm}^2$,0.69 g/$\textrm{cm}^2$ and 0.66 g/$\textrm{cm}^2$respectively The BMD of FN was assessed as osteopenia by T-score. The BMD of WT was positively correlated with age of monarch (p<0.05) and the BMD of Ll4 was positively correlated with delivery number (p<0.05). The BMD of Ll4 was positively correlated with hours of outdoor activity per weekend and week (p<0.05, p<0.05). The BMDs of FN and WT (p<0.05, p<0.05) were positively correlated with intake of green tea per month and the BMD of FN (p<0.05) was positively correlated with intake of wine per month. But the BMD of Ll (p<0.05) was negatively correlated with intake of Coca cola per month. So nutritional education for increasing hours of outdoor activity and decreasing intake frequency of beverage contributing to diminishment of bone mineral density is needed for premenopausal working women to prevent osteoporosis.

A retrospective observational study of the BMD for 5-years in older men (성인 남성에서 5년간의 골밀도 변화 관찰)

  • To investigate the decrease of BMD by age and the risk factor of osteoporosis in Korean men. We describe the study of a five-years retrospective observational study with male patients. Eighty Korean men who visited hospital for health screening were assessed for this study from 2002 to 2006. We evaluated the BMD of the femoral neck and L-spine, and the preferences and habits in the life. The data were collected for 5 years, and we analysed the five-years change of BMD and the relations between BMD and other factors. Subjects were divided into 3 group by 1st assessment of femoral neck BMD, and were compared with each other. The age of subjects was $43.15{\pm}4.82$ and BMD of femoral neck was $-0.61{\pm}0.97$ and BMD of L-spine was $-0.67{\pm}1.10$ in the first year assessment. The femoral neck BMD of 4th and 5th assessment was decreased significantly compared to that of 1st assessment. The L-spine BMD of 2nd assessment was decreased significantly compared to the 1st assessment. There was no significant correlation between the changes of BMD and preferences or habits-drinking, smoking, eating habit, exercise. The femoral neck BMD of 5th assessment was decreased significantly compared to that of 1st assessment in the high femoral neck BMD group. And there was no significant change of femoral BMD and L-spine BMD in other groups. Low BMD group in the 1st assessment showed lowest BMD in the 5th assessment and high BMD group in the 1st assessment showed highest BMD in the 5th assessment. We can guess that the young men who has low BMD could have high risk of osteoporosis when he became older. And the femoral BMD should be considered important in anticipating the changes of BMD in middle aged men.

A Study on the Change of Bone Mineral Density(BMD) by Life Habit and Physical Condition (성인의 신체조건 및 생활습관에 따른 골밀도 변화에 대한 연구)

  • Purpose: To evaluate the correlation between BMD and life habit such as drinking. exercise smoking or physical condition such as age, sex, height, weight, body mass index(BMI). Materials and Methods: I evaluated the BMD of the femoral neck and L2-L4 spines of 321 persons who took a regular health screening in Woosuk university oriental medical hospital from February to April in 2006 by dual energy bone mineral densitometry. Results: The age of persons ranged from 20 years to 75 years(mean $45.10{\pm}11.54$) and there were 160 males and 161 females. In males, BMD of the femoral head was highest at 2nd decade, BMD of the spine was highest at 4th decade, and BMD of both femoral head and lumbar spine was lowest at 6th decade. In fenales, BMD of both femoral head and lumbar spine was highest at 4th decade and lowest at 6th decade. Among the various physical conditions, only height of persons showed significant correlation with BMD in both males and females. BMD was increased according to increasing height. In males, BMD of persons who had habit such as drinking, exercise or smoking did not show significant change statistically. But in females, drinking group showed high BMD relative to non-drinking group in both femoral head and lumbar spine. Conclusion: BMD was different according to age, sex, height and life habit. Especially aged people showed osteoporotic change progressively. More persistent effort is needed to find out the factors decreasing BMD for prevention of problems by osteoporosis.

Factors Associated with the Lumbar Spine and Femoral Neck Bone Mineral Density in Korean Elderly Women (한국 여성 노인의 대퇴경부 및 요추 골밀도 관련 요인)

  • This study was conducted among Korean women over aged 60 who participated in Korea National Health and Nutrition Examination Survey (KNHANES) (the 2nd and 3rd year at the 4th survey, and the 1st year at the 5th survey). The purpose of the study was to figure out a relation between femoral neck and lumbar spine bone mineral density and anthropometry, life style, diet, fracture history, family history of osteoporosis, medical history, menstrual history and reproductive factor. To express the strength of the associations, percent differences were calculated from multiple linear regression models using the formula ${\beta}{\times}$(unit/mean BMD). Unit for continuous variables were chosen to approximate 1 standard deviation(SD). In women aged 60 or older, lean mass(B: 0.257) and fat mass(B: 0.237) greatly influenced bone density in the femoral neck and lumbar spine. Therefore it will be an effective way to prevent osteoporosis for elderly women by increasing lean mass and maintaining proper weight.

일부 폐경전 성인여성의 골밀도와 신체계측 및 식이인자에 관한 연구

  • 본 연구는 폐경전 성인여성들의 골격상태를 알아보고 신체계측 및 식이섭취실태가 골밀도에 미치는 영향을 평가하기 위해서 성인여성 61명을 대상으로 신체계측을 실시하고 24시간 회상법으로 영양소 및 주요 식품군의 식품섭취실태를 조사하고 DEXA를 사용하여 요추와 대퇴부 3부위의 골밀도를 측정하였다. 1) 대상자들의 평균 연령은 37세였으며, 평균 신장과 체중은 158.17cm, 54.55kg였으며, 평균 BMI와 WHR은 21.82, 0.78로 대상자들의 비만도는 정상이었다. 2) 평균 골밀도는 요추(L24) 1.04 g/$\textrm{cm}^2$, 대퇴경부 0.76g/$\textrm{cm}^2$, 대퇴전자부 0.66g/$\textrm{cm}^2$, 와드삼각부 0.69g/$\textrm{cm}^2$이었다. 3) T-score로 판정시 요추(L24)의 경우 골다공증군 3.28%, 골감소증군 14.75%, 대퇴경부의 경우 골다공증군 9.84%, 골감소증군 52.46%, 대퇴전자부의 경우 골다공증군 1.64%, 골감소증군 34.43%, 와드삼각부의 경우 골다공증군 6.56%, 골감소증군 45.90%였다. 4) 영양소중 칼슘(78.75%), 철분(69.75%) 및 비타민 A(92.17%) 영양소의 1일 평균 섭취량은 영양권장량보다 낮은 수준이었다. 5) 신체계측 및 영양소섭취량과 골밀도와의 상관관계분석에서 신체계측에서 체중(p < 0.05), 엉덩이둘레(p < 0.01), 제지방함량(p < 0.05), 총수분함량(p < 0.05)이 높을수록 요추골밀도가 유의하게 높았으나, 영양소섭취량은 골밀도와 유의한 상관성을 보이지 않았다. 6) 주요 식품군의 식품섭취실태와 골밀도와의 상관관계분석에서 버섯군의 식품섭취횟수(p < 0.05, p < 0.05)가 많을수록 요추(L3, L24)의 골밀도가 각각 유의하게 높았으며, 유지류군의 섭취식품수(p < 0.05)가 많을수록 와드삼각부의 골밀도가 유의하게 높았으나 곡류군의 섭취식품수(P < 0.01, p < 0.05, p < 0.05)가 많을수록 대퇴부 3부위 (대퇴경부, 대퇴전자부 및 와드삼각부)의 골밀도가 각각 유의하게 낮은 것으로 나타났다. 이상의 결과에서 폐경전 성인여성의 경우 요추보다는 대퇴부 3부위의 경우 골다공증이나 골감소증의 비율이 상대적으로 높았으며, 특히 대퇴경부에 있어서 골다공증이나 골감소증의 비율이 가장 높았음을 알 수 있다. 또한 요추와 대퇴부 3부위의 골밀도는 영양소섭취량과 유의한 상관관계를 보이지 않았으나 체중, 엉덩이둘레 등의 신체계측요인과 곡류군, 버섯군 및 유지류군 등의 주요 식품군의 식품섭취실태와 밀접한 상관관계를 보이고 있음을 알 수 있다. 따라서 성인여성에 있어서 골밀도손실을 예방하기 위해 평상시 적절한 체중유지와 아울러 식생활지침에 있어서 주요식품군별로 적절한 섭취식품의 수, 섭취량 및 섭취빈도에 대한 영양교육이 필요한 것으로 생각된다.

A Comparative Study of Influencing BMD Factors in Postpartum and General Women in Their Twenties and Thirties ($20{\sim}30$대 여성의 골밀도 영향 인자에 대한 출산 후 여성과 일반여성과의 비교 연구)

  • This study is to evaluate the factors that affect to the bone mineral density(BMD) of postpartum women by comparing BMD between postpartum and general women who are ages from 20 to 39, and to identify correlation between various factors weight, height, body mass index(BMI), hemoglobin(Hb) and BMD. Study subjects were postpartum(n=159) and general(n=180) women. We checked the BMD of femoral neck and lumbar spine by using dual energy X-ray bone densitometry. The mean age was $30.69{\pm}3.32$ in postpartum care group and $31.22{\pm}5.66$ in general women group. In postpartum care group, the mean BMD of femoral neck and lumbar spine were -0.187 and -0.076. In general women group, the mean BMD of femoral neck and lumbar spine were -0.029, and -0.169. According to BMI level, each group was divided into two subgroups. One was 23 or more BMI subgroup, and the other was under 23 BMI subgroup. There was a significant difference in BMD of femoral neck and lumbar spine between two subgroups in general women group, but only a significant difference in BMD of lumbar spine is noted between two subgroups of postpartum group. According to Hb level, each group was divided into two subgroups. One was 11g/dl or more subgroup, and the other was under 11g/dl subgroup. There was not a statistically significant difference of BMD in all subgroups. BMD of femoral neck and lumbar spine showed a significant correlation with weight, height, BMI in both groups. There was a significant correlation between BMD and Hb level in postpartum care group, but a significant correlation was not noted in general women group. The postpartum women showed a significant correlation between BMD and various factors such as weight, height, BMI just like in general women. There was a significant correlation between BMD and Hb in postpartum women, but not in general women.

Comparison Measurement Error of T-score Between Lumbar and Femoral Neck According to Kind of DXA (DXA측정기 종류에 따른 요추부와 대퇴경부 골밀도 값의 측정오차 비교)

  • In this study, three different devices, Norland, Osteocore and Lunar were used to compare and analyze the measurement error by each bone density measurement device by classifying the physical characteristics into age, height and weight, the subject of total 300 sampling 100 persons for each device. Categorizing Lumbar region and Femoral neck as normal (T-score$\geqq$-1.0), osteopenia (-1.0>T-score>-2.5) and osteoporosis (T-score$\leqq$-2.5), the findings were observed as follows. Norland device showed the least measurement error in age and height, while Lunar showed the least in weight among the devices. And, the result of comparing the bone density measurement error based on the lumbar region showed that all of Lunar, Norland and Osteocore have the least variation of measurement error in osteopenia and the result of comparing based on the femoral neck showed that all of Lunar, Norland and Osteocore have the least variation of measurement error in osteoporosis. For each variable, the measurement error was observed to be vary upon the device. To solve this, standardized common Phantom should be used to compare and converge the measured value of each company and cross-calibration would be necessary when replacing the software.
