Feature of cerebral infarction with tsutsugamushi disease (쯔쯔가무시병과 동반된 뇌경색의 특징)

  • This study was performed on 16 patients diagnosed with tsutsugamushi disease and cerebral infarction from January 2007 to December 2015. An acute cerebral infarction was diagnosed by brain MRI and MRA. Tsutsugamushi disease was diagnosed using a polymerase chain reaction. To distinguish the difference between the generalized cerebral infarction and infarction with tsutsugamushi disease, the blood pressure and body temperature were measured uponadmission. In general, the blood pressure increases during an acute cerebral infarction. Interestingly, in this study, 12 patients showed a systolic blood pressure less than 130 mmHg uponadmission. The location of the cerebral infarction and whether single or multiple cerebral infarction were examined. Thirteen patients had a cerebral infarction in anterior circulation and 3 patients developed in posterior circulation. To evaluate the coagulation disorders, prothrombin time (PT), activated partial thromboplastin time (aPTT), D-dimer, fibrinogen, fibrin degradation product (FDP). D-dimer, which is generally known to increase in an acute cerebral infarction, showed a significant increase in the 13 patients. Fibrin degradation products (FDP) showed a significant increase in 15 patients. The pathophysiological mechanism of tsutsugamushi disease is known as vasculitis, which may result in an endothelial cell injury and proliferation of the endothelial wall, which may lead to a cerebral infarction accompanied by coagulopathy. Without endothelial cell damage and proliferation, a vasospasm caused by vasculitis may cause vasoconstriction and cerebral infarction.

The effect of immediate and delayed recognition memory on event-related potential(ERP) (즉각적 재인 기억과 지연 재인 기억이 사건과련전위에 미치는 영향)

  • The effect of immediate and delayed recognition memory on event-related potential (ERP) was studied using a continuous recognition memory task and event-related potential (ERP). Among 240 stimulus words 40 words were not repeated. 100 were immediately repeated and 100 were repeated after 5 intervening words. All words presented only once during the experiment were referred to as new words. Subjects responded faster and more accurately to words repeated immediately than to new words a and to those repeated after intervening words. In terms of ERP results the immediately repeated words were associated with large P300 amplitude, early P300 latency and a absence of N400. while words repeated after a delay were associated with small P300 a amplitude. late P300 latency and the presence of N400. N400 was elicited only to new w words and to those repeated after a delay. The general morphology of the waveform was s similar for three stimulus-presentation conditions until around 3l0ms after the onset of stimulus. These results indicate that immediate and delayed recognition memory could be dissociated into two distinct processes possibly being mediated by different cerebral mechanism, and the dissociation between two types of recognition memory emerges around 3l0ms poststim'ulus. The immediate and delayed recognition memory for words are considered in terms of template matching and memory searching.

  • Impaired processing of facial information is one of the broad ranges of cognitive deficits seen in patients with schizophrenia. The purpose of this study was to elucidate the differences in brain activities involved in the process of facial working memory between schizophrenic patients and healthy comparison subjects. Ten patients with schizophrenia were recruited along with matched healthy volunteers as a comparison group. Functional magnetic resonance imaging (fMRI) was used to assess cortical activities during the performance of a 1-back working memory paradigm using images of neutral faces as mnemonic content. The patient group performed the tasks with reduced accuracy. Group analysis revealed that left fusiform gyrus, right superior frontal gyrus, bilateral middle frontal gyri/insula, left middle temporal gyrus, precuneus and vermis of cerebellum and showed decreased cortical activities in the patient group. On the other hand, an increased level of activation in lateral prefrontal cortex and parietal lobule was observed from the patient group, all in the right hemisphere. A decreased level of activity in the left fusiform gyrus among the patient group implicates inefficient processing of facial information. An increased level of activation in prefrontal and parietal neural networks from the patient group confirms earlier findings on the impaired working memory of patients with schizophrenia.

High Resolution Time Resolved Contrast Enhanced MR Angiography Using k-t FOCUSS (k-t FOCUSS 알고리듬을 이용한 고분해능 4-D MR 혈관 조영 영상 기법)

  • Purpose : Recently, the Recon Challenge at the 2009 ISMRM workshop on Data Sampling and Image Reconstruction at Sedona, Arizona was held to evaluate feasibility of highly accelerated acquisition of time resolved contrast enhanced MR angiography. This paper provides the step-by-step description of the winning results of k-t FOCUSS in this competition. Materials and Methods : In previous works, we proved that k-t FOCUSS algorithm successfully solves the compressed sensing problem even for less sparse cardiac cine applications. Therefore, using k-t FOCUSS, very accurate time resolved contrast enhanced MR angiography can be reconstructed. Accelerated radial trajectory data were synthetized from X-ray cerebral angiography images and provided by the organizing committee, and radiologists double blindly evaluated each reconstruction result with respect to the ground-truth data. Results : The reconstructed results at various acceleration factors demonstrate that each components of compressed sensing, such as sparsifying transform and incoherent sampling patterns, etc can have profound effects on the final reconstruction results. Conclusion : From reconstructed results, we see that the compressed sensing dynamic MR imaging algorithm, k-t FOCUSS enables high resolution time resolved contrast enhanced MR angiography.

Effects of Woohwangcheongsim-won on Gene Expression in a Hypoxic Model of Cultured Rat Cortical Cells (배양한 흰쥐 대뇌세포의 저산소증 모델에서 우황청심원이 유전자 표현에 미치는 영향)

  • Objectives : The purpose of this investigation is to evaluate the effects of Woohwangcheongsim-won (WC) on the in vitro neuronal development and alteration in gene expression in a hypoxia model using cultured rat cortical cells. Methods : E/sub 18/ rat cortical cells were grown in a neurobasal medium containing B27 supplement and various concentration of WC. Initial development of growth cone was investigated by phase-contrast microscopy, while dendritic spine formation and synaptogenesis were investigated by immunocytochemistry with SynGAPα(a postsynaptic marker) and synaptophysin (presynaptic marker) antibodies. Alteration in gene expression was analyses by microarray using rat 5K-TwinChips. Results : WC suppressed the development of growth cones and WC increased the number of dendritic spines at 20 and 50㎍/mL concentration but there was no statistical significance. Instead, it significantly decreased the number at 100㎍/mL. The expression of anti-apoptosis gene Bcl2-like 1 (Bcl211) increased (Global M=0.46), while Akt1 decreased. Proapoptosis genes Bad and PDCD2 increased. The expression of hemoglobin alpha 1 (probably neuroglobin) increased (Global M=0.93). The expression of antioxidants such as catalase, heme oxygenase (HO), and PRKAG2 gene increased. The expression PKC gene increased. The expression of retinoic acid receptor alpha (RARα) increased significantly (Global M=1.0). Conclusions : These data suggest that WC trends to suppress cellular activity slightly in normoxia and increases the expression of apoptosis-, antioxidation-, oxygen capture-related genes in hypoxia, but increases Bcl111 that anti-apoptosis gene, on the other hand increases Bad, PDCD2 that pro-apoptosis genes, too..

Change of Cerebral Blood Flow during Tilt Tests in Children with Vasovagal Syncope (혈관미주 신경성 실신 소아에서 기립경사 검사 동안에 뇌혈류 변화)

  • Purpose : Syncope appears to be common. However, the mechanism of syncope is not clear. Increased vagal activity and withdrawal of sympathetic stimulation cause hypotension, bradycardia and finally loss of consciousness. The purpose of this study was to evaluate changes of cerebral blood flow velocity, blood pressure, and heart rate during tilt test in children with vasovagal syncope. Methods : Sixty four children with a past history of syncope were evaluated. The stand up test was performed for 15 minutes after a rest at supine position for 10 minutes, followed by an $80^{\circ}$ tilt test lasting 45 minutes. If presyncope(lightheadedness, nausea, blurred vision, or sweating) or syncope occurred, the study was discontinued. 12-lead electrocardiography, echocardiography, and electroencephalography were performed. Transcranial Doppler study was performed at the middle cerebral artery with 2 MHz continuous Doppler probe in 10 children with positive tilt test. Systolic, diastolic, mean cerebral blood flow velocity, integral, and pulsatility index were measured with blood pressure, heart rate, and $O_2$ saturation. Results : The positive rate of tilt test was 31.3%(20/64). Systolic, diastolic, and mean cerebral blood flow velocity decreased significantly in absence of hypotension or bradycardia during presyncope. Time velocity integral of cerebral artery also decreased significantly. Conclusion : Decreased cerebral blood flow velocity can predict the presyncope manifestation. Impairment of autoregulation of cerebral blood flow might play an important role in the pathophysiology of vasovagal syncope.

Evaluation of Traumatic Intracranial Hemorrhage Using Computed Tomography and Magnetic Resonance Imaging in Four Dogs (개에서 컴퓨터단층촬영술과 자기공명영상을 이용한 창상성 두개관내 출혈의 평가)

  • Four dogs with neurologic dysfunction resulted from intracranial hemorrhage by head trauma were referred to Veterinary Medical Teaching Hospital, Chungnam National University. There were no remarkable findings in survey radiography in four cases. CT and MRI scans were diagnostic in these cases. Three dogs underwent CT scanning. On CT images, the lesion was hyperdense and was not enhanced after intravenous contrast administration in dog 1 and dog 2. On CT of dog 4, there was no significant finding. All of four dogs were verified by dorsal, sagittal, and transverse T1-weighted (T1W) and T2-weighted (T2W) images. Appearance of the lesions in dog 1 and dog 2 was isointense (dog 2) or isointense with hyperintense rim (dog 1) on T1W images and hyperintense on T2W images. In dog 3 and dog 4, there were hypointense and hyperintense lesions on T1Wand T2W images respectively. The lesions in four dogs were located in the left intracerebral region, intracerebral and cerebellovestibular region, subdural space, and right cerebral hemisphere, respectively.

Effects of Scutellaria baicalensis GEORGI on Gene Expression in a Hypoxic Model of Cultured Rat Cortical Cells (배양한 흰쥐 대뇌세포의 저산소증 모델에서 황금(黃芩)이 유전자 표현에 미치는 영향)

  • Objectives : The purpose of this investigation is to evaluate the effects of Scutellaria baicalensis GEORGI on alteration in gene expression in a hypoxia model using cultured rat cortical cells. Methods : E18 rat cortical cells were grown in a Neurobasal medium containing B27 supplement. On 12 DIV, Scutellaria baicalensis GEORGI(20 ug/ml) was added to the culture media and left for 24 hrs. On 11 DIV, cells were given a hypoxic insult $(2%\;O_2/5%\;CO_2,\;37^{\circ}C,\;3\;hrs)$, returned to normoxia and cultured for another 24 hrs. Total RNA was prepared from Scutellaria baicalensis GEORGI-untreated (control) and -treated cultures and alteration in gene expression was analysed by microarray using rat 5K-TwinChips. Results : For most of the genes altered in expression, the Global M values were between -0.5 to +0.5. Among these, 1143 genes increased in their expression by more than Global M +0.1, while 1161 genes decreased by more than Global M -0.1. Effects on some of the genes whose functions are implicated in neural viability are as follows: 1) The expression of apoptosis-related genes such as Bad (Global M = 0.39), programmed cell death-2(Pdcd2) (Global M = 0.20) increased, while Purinergic receptor P2X(P2rxl) Global M = -0.22), Bc12-like1(Bc1211)(Global M = -0.19) decreased. 2) The expression of 'response to stress-related genes such as antioxidation-related AMP-activated protein kinase subunit gamma 1 gene (Prkag1) (Global M = 0.14), catalase gene (Global M = 0.14) and Heme Oxygenase(Hmoxl) increased. 3) The expression of Fos like antigen 2 (Fos12) expressed in neurons that survive ischemic insult increased (Global M = 0.97). Conclusions : these data suggest that Scutellaria baicalensis GEORGI increases the expression of antiapoptosis- and antioxidation- related genes in a way that can not yet be explained.

  • Constraint-Induced Movement Therapy(CIMT) is considered as one of the most interesting upper extremity rehabilitation in the field of neurorehabilitation. CIMT is an intensive training provided in the affected upper limb for 6 hours a day, 5 days a week for 2 weeks, while unaffected arm is restrained for 90% of waking hours. Recently, instead of CIMT, modified Constraint-Induced Movement Therapy(mCIMT) has been applied because of the clinical limitations of CIMT. CIMT or mCIMT studies have used various outcome instruments to measure different aspects of upper limb function after intervention. There are various kinds of evaluation tools to measure different aspects of upper limb function after CIMT intervention. It has been proven that Pediatric Motor Activity Log(PMAL), Quality of Upper Extremities Skills Test(QUEST), Melbourne Assessment of Unilateral Upper Limb Function(MAULF), Assisting Hand Assessment (AHA) are effective. The purpose of this study was to investigate the cortical change in children with hemiplegic cerebral palsy after CIMT. As a result, use-dependent cortical reorganization was revealed. Also, increased activity of the contralateral motor cortex and decreased activity of the ipsilateral cortex were found. It supports the mechanism of cortical reorganization, the principles of neural plasticity and specifically activation of the contralateral cortex, for improving upper limb function after CIMT.

Distribution and Characterization of the Neurosteroid Acyltransferase from the Bovine Brain (소의 뇌에서 Neurosteroid Acyltransferase의 분포 및 특성에 관한 연구)

  • The enzymatic properties as well as its distribution in the cerebral region and subcellular organells were investigated for the neurosteroid acyltransferase from the bovine brain, which synthesize the fatty acid esters of the neurosteroids. The cerebellum region was the highest in NSAT activity while the cerebrum was the lowest with 50% of the cerebellar activity. The NSAT was found to be mainly localized in the microsomal fraction. The optimal temperature and pH were $40^{\circ}C$ and 4.9, respectively. When $^3H-DHEA$ was utilized as substrate, the $K_m$ and $V_{max}$ was $32.6\;{\mu}M$ and 4.86 nmole/mg protein/h, respectively. Under the same condition pregnenolone$({\Delta}^5P)$ was a competitive inhibitor with $K_i=22.8\;{\mu}M$ and testosterone was a uncompetitive inhibitor with $K_i=22.8\;{\mu}M$. This may suggest that the NSAT has a different conformation in the acylation of the ${\beta}-hydroxyl$ group at C-3 and C-17.
