Primary Pulmonary Carcinoma Ex-pleomorphic Adenoma of the Salivary Gland Type (일차성 폐의 침샘형 악성 다형선종)

  • Pleomorphic adenoma is also called a mixed tumor and it most commonly occurs in the salivary gland. This neoplasm has a low grade malignant potential, but it may also show aggressive clinical behavior like recurrence or metastasis. We report here on a case of a tumor that was confirmed to be primary pulmonary carcinoma ex-pleomorphic adenoma by the pathologic examination after complete resection, and it had the characteristics of malignant neoplasms, such as multiple metastases.

  • Mucoepidermoid carcinoma is most frequent malignant tumor of parotid gland. However, mucoepidermoid carcinoma arising from pleomorphic adenoma has been disputed and rarely reported. 54-year-old woman with pleomorphic adenoma underwent superficial parotidectomy. And 15 years later, she diagnosed mucoepidermoid carcinoma at the same side. We report the case with a review of literature.

  • The differential diagnosis of lesions in buccal area include lipoma, neurofibromas, epidermoid cyst, salivary ducts calculus, hemangioma, lymphadenopathy. Accessory parotid glands is defined as salivary gland tissue adjacent to the parotid duct, but separated from the body of parotid and it may be found in approximately 20% of human parotid glands. The appearance of an accessory parotid tumor is rare, with a reported frequency of 7.7% of all parotid neoplasm. Angiomyoma, which is also termed angioleiomyoma, is a rare solitary subcutaneous tumors arising from the vascular smooth muscle. It often occur in the extremities and is rarely found in buccal area. We present 2 cases of rare tumor in buccal mass and resected surgically without facial nerve palsy.

Invisible Parotid Gland Pleomorphic Adenoma on Contrast Enhanced-Computed Tomography (조영증강 전산화단층촬영검사에서 보이지 않는 이하선의 다형선종)

  • Pleomorphic adenomas is the most common tumor of parotid gland and usually located and confined in superficial lobe of parotid gland. Computed tomography (CT) is commonly used to initially evaluate salivary gland lesion, but contrast-enhanced CT may sometimes fail to reveal lesions in spite of a high clinical suspicion. For this reason, ultrasonography (US) can be used as the first-line image work-up in some cases of parotid gland benign tumors. We experienced a case of a 60-year-old woman without underlying disease presenting a palpable parotid mass in which the initial CT examination was reported as 'no obvious mass detected'. However, the lesion was revealed in US and histologically confirmed as pleomorphic adenoma. The patient underwent superficial parotidectomy through modified facelift incision. To the best of our knowledge, this is the first report of invisible major salivary gland tumor on CT in the Korean literature.

Immunohistochemical Characteristics of Primary and Recurrent Pleomorphic Adenoma (원발성 다형선종과 재발성 다형선종의 면역조직화학적 특성)

  • Background and Objectives: When the pleomorphic adenoma(PA) recures, the tumor tends to become a multinodular mass that infiltrates into the normal tissue which is not a common condition for a benign tumor. This manifestation is probably due to the difference in cell biology of the recurrent tumor compared to that of the primary PA. The aim of this study is to assay the immunhistochemical characteristics of the recurrent PA compared to the primary PA and to evaluate whether this property can be used for developing a method that can select the patients who have higher risk to recur. Materials and Methods: Thirteen patients were enrolled in the primary PA group and 15 patients who had a recurrent PA were enrolled in the recurrent PA group. To evaluate the cell biology of the tumor, immunohistochemical stainings of Ki-67, bcl-2 and p53 were performed. Results: There was no difference in the expression of Ki-67 (p=0.117, p=.208) and p53 (p=.430, p=.328). The extent stained by bcl-2 was significantly larger in the recurrent PA group (p=.033, p=.014). The expression of bcl-2 did not increase while time passed. Conclusion: The high expression of bcl-2 seems to be a property of the recurrent PA group which can be found even during first operation before recurrence. By this immunhistochemical characteristic, we would be able to sort out the patients who have higher risk to recur.

  • Pleomorphic adenoma is the most common benign neoplasm of salivary glands. Pleomorphic adenoma can metastasis without malignant transformation. Metastasizing pleomorphic adenoma is equal to pleomorphic adenoma histologically yet metastasis to distant sites. Most Metastasizing pleomorphic adenoma are diagnosed many years following the initial treatment. 45-year-old man was found to have an asymptomatic right submandibular mass. A right submandibular gland excision and selective neck dissection was performed and pathology confirmed metastasizing pleomorphic adenoma. We report this case with a brief literature review.

A Case of Adenocarcinoma Ex Pleomorphic Adenoma with Tumor Thrombus of Submandibular Gland (악하선에 발생한 종양 혈전을 동반한 다형선종유래 선암종)

  • Carcinoma ex pleomorphic adenoma is rare, aggressive, poorly understood malignancy that occurs in the salivary glands. The patient was 70-year-old male presenting ovoid elevated submandibular mass that has been present for 30 years and rapid growing for 6 months. The initial cytologic finding by fine needle aspiration biopsy showed highly malignant cells and the radiologic finding revealed a $3.6{\times}3.7{\times}4.8cm$ sized mass with cervical lymphadenopathy. After wide excision of submandibular gland and modified radical neck dissection, the histologic examination revealed that tumor was composed partly of a benign pleomorphic adenoma and partly of an adenocarcinoma component with areas of calcification and frequent angiolymphatic invasion. We present a rare case of adenocarcinoma ex pleomorphic adenoma with tumor thrombus in the submandibular gland.

A Case of Extra-Submandibular Gland Pleomorphic Adenoma in Upper Lateral Neck (상측경부에 발생한 악하선 외 다형선종 1예)

  • Pleomorphic adenoma (PA) is a benign tumor which usually originates from major and minor salivary glands. This tumor arising outside submandibular gland (SMG) is extremely rare. To author's knowledge, only four cases have been reported so far in English literature. Its pathogenesis is still unclear, but it can be explained by embryologic theory of major salivary gland. A 68-year-old man with an incidental mass on left upper lateral neck visited to our clinic. The radiologic findings showed well-margined round mass outside left SMG. The excisional biopsy revealed a pleomorphic adenoma. We report the rare and unique case with a brief literature review.

A Case of Benign Parotid Tumor Misdiagnosed for Parotid Cancer on Preoperative Cytology (수술 전 세침흡인세포검사에서 악성으로 의심되었던 이하선 양성 종양 1예)

  • Fine needle aspiration cytology as a diagnostic workup of parotid gland tumor is a simple and useful method. Although fine needle aspiration cytology could not predict accurate diagnosis in all cases, it is usually helpful in differentiating malignancy and benign lesions. A 35-year-old female was found to have a parotid mass for 1 year. Preoperative evaluation including computed tomography and magnetic resonance imaging were non-diagnostic, but, fine needle aspiration cytology on parotid mass showed the suspicion of a low-grade mucoepidermoid carcinoma. Superficial parotidectomy and selective neck node dissection were done based on cytology. However, final pathological examination confirmed benign pleomorphic adenoma. Here, the diagnostic accuracy and cautions in interpretation of result of fine needle aspiration cytology is discussed with respect to the case.

Lumpectomy as a Surgical Treatment of Primary Benign Pleomorphic Adenoma of the Parotid Gland (귀밑샘에 생긴 다형선종의 수술적 치료로써의 제한적 국소절제술)

  • Purpose: Pleomorphic adenoma is the most common benign neoplasm in parotid gland. Superficial parotidectomy was usually used to remove the pleomorphic adenoma. But, this method has to remove tumor with normal parotid tissue. Authors did lumpectomy to remove pleomorphic adenoma in parotid gland, because pleomorphic adenoma is wrapped in a capsule as it grows. The purpose of this study is to evaluate the efficacy of lumpectomy as a treatment of pleomorphic adenoma in parotid gland. Method: From 2002 to 2008, 8 patients underwent the lumpectomy of the pleomorphic adenoma in parotid gland. Occurrence of the complications and recurrance were evaluated. Result: Patients were followed-up for a mean 45 months. There were no recurrance or no complication after lumpectomy. Conclusion: Authors suggest that the lumpectomy lead to decrese complications, recurrane and can be used as a procedure for the resection of pleomorphic adenoma in parotid gland.
