Delayed Diaphragmatic Injury with Massive Hemothorax Due to Lower Rib Fracture (하부늑골 골절에 의한 지연성 대량혈흉을 동반한 횡격막 손상)

  • Simple rib fracture is one of most common injury after blunt thoracic trauma found in approximately 7% to 40% of cases. Delayed traumatic diaphragmatic injury with massive hemothorax after rib fracture is rare but a potentially life-threatening condition. We present a rare case of a 79-year-old male with delayed diaphragmatic injury with massive hemothorax due to fracture of the lower ribs. Under thoracoscopy, hemothorax was evacuated, diaphragmatic rupture was identified and repaired, and the lower ribs were fixed with metal plate (s). Although simple lower rib fractures may be the only clinical finding, close observation and monitoring are required because of the possibility of diaphragmatic and/or intraabdominal organ injury.

  • Patients with Horner's syndrome exhibit a variety of symptoms, including miosis, palpebral ptosis, and anhidrosis. This syndrome is caused by interruptions of the sympathetic neural pathways. This paper describes two cases of patients with Horner's syndrome who experienced a first rib fracture after crushing injuries.

  • We experienced a case of giant cell tumor arising from the right 2nd rib in the diagnostic and therapeutic thoracotomy at our department of Thoracic and Cardiovascular Surgery, Chungnam Univ. Hospital in May 3rd, 1991. The reports of this tumor are very rare and are known to have the incidence of 1 percent or less of all giant cell bone tumor. The tumor of the right 2nd rib, infant head-sized, covered with parietal pleura was invaded into the vertebra on operation finding. The tumor was resected partially[three fourths] and revealed to be the giant cell tumor on pathologic finding. At now, the patient is taking the radiotherapy to achieve the therapeutic goal to the remnant tumor.

  • Chondroblastoma is a benign bone tumor accounting for less than 1 % of bone tumor. It infrequently involves the flat bones, among which ribs are particularly rare. On fine needle aspiration cytology, the diagnostic smear consists of chondroblasts, osteoclast-like giant cells, and chondroid matrix. The cytologic hallmark to differentiate from other giant cell-containing lesions is chondroblasts. We experienced a case of chondroblastoma in a 13-year-old female. Fine needle aspiration cytology from the 5 th rib revealed dispersed chondroblasts and osteoclast-like giant cells on hemorrhagic background. Chondroblasts had round to oval nuclei with fine, evenly distributed chromatin and distinctive grooves or indentation. Their cytoplasm was well-defined.

  • As the size of tanker increase, the analysis and strength prediction of the transverse web frames in a tanker have become important problems. Therefore, several papers dicussed the subject and various method of analysis have been presented. Most of these studies are based on the elastic framework analysis. Framework analysis is carried out by the matrix methods. The matrix methods used most frequently are the displacement method, force method and the transfer matrix method. In this paper, the analysis is carried out by the transfer matrix method. The program has been tested by IBM 1130 and the results of example show good agreements with those by the program of stress analysis, STRESS, which was developed in M.I.T.

  • Giant cell tumor(GCT) occurs very unusually in the rib(less than 1% of GCT). We present the cytologic features of GCT of the rib. It showed multiple cellular clusters composed of characteristic, benign looking osteoclast-like multinucleated giant cells and fibroblast-like mononuclear cells. The multinucleated giant cells contained numerous nuclei (average, 30 to 40 per cell, which were closely packed. The nuclei in giant cells were remarkably uniform and round to oval. The mononuclear, neoplastic stromal cells were elongated and spindle-shaped. There was no cytologically malignant portion in the tumor.

  • Chondroblastoma is a rare benign bone tumor most frequently found in the epiphyseal region of long bones. The most common locations are the knee and proximal humerus,but the rib is an unusual site. Second decade is the most common age group. The ossification centers for the rib appear in the second fetal month and ossify in a caudal-to-cranial direction. There are also epiphyseal centers at the head and tubercle of the rib that appear at puberty and ossify in the third decade. The epiphyseal plates of the head and tubercle may be the site of origin of the posterior chondroblastomas; the anterior lesions may arise from the costochondral junction. The histologic features consist of polygonal chodroblast, small foci of chondroid production, osteoclast-like giant cell, and are diagnostic of chondroblastoma.We experienced a case of the chondroblastoma arising from the rib, and its clinical and histologic features are discussed.

  • Huge chondrosarcoma is a rare form of primary malignant tumor of the chest wall. We operated on a 60 year old female patient who had a huge anterior chest wall mass with local invasion into the pericardium and satellite tumors on the visceral pericardium of the heart. En-bloc resection of the huge tumor including both upper 3 ribs, both clavicles, manubrium of the sternum, pleura, and pericarium, was followed by complex chest wall reconstruction using a Gore-tex soft tissue patch and latissimus dorsi musculocutaneous free flap.

  • Subclavian vein thrombosis in thoracic outlet syndrome is an uncommon disease. Thrombolysis, venoplasty with a balloon and stent insertion are needed for treating this condition. Sometimes, trans-axillary first rib resection is also needed. We report here on a case of subclavian vein thrombosis that was successfully treated with the medial calviculectomy, internal jugular vein transposition and stent insertion.

  • Simple bone cyst (SBC) is very rare in adult ribs. The diagnosis basically relies on conventional radiography and occasionally on a. There has been no earlier publication on PET/CT diagnosis of SBC. We report a case of adult costal SBC diagnosed by positive $^{18}F$-fluorodeoxyglucose (FDG) uptake. Histology showed the FDG uptake to be associated with reactive woven bone formation and nonspecific chronic inflammation. Correlation of PET, CT, plain radiography and sonography are also described.
