Shape and Incidence of Rib Variations in Chest Radiographs (단순흉부촬영상 관찰되는 늑골 변이의 양상 및 빈도)

  • Background: The literature on variations of rib is limited. Very little has been written in the radiological journal of this country on the subject. It seemed of interest to investigate the nature and incidence of congenital variations in a series of routine chest roentgenograms. The topic of rib variations has not been covered extensively in the radiological journals in Korea. This has presented an opportunity to investigate the nature, type, shape and incidences of congenital rib variations in normal Korean adults from a series of routine roentgenograms. Methods: Chest radiographs of 5,000 adults, who visited our hospital for a routine check-up or for employment physical examinations from January 1996 to September 1998, were consecutively reviewed. The sex distribution consisted of 2,827 male males and 2,173 female females(ratio of 1.3:1) with the age range aged between 19 and 65 years(mean age: 34.6 years) were included. The chest PAs were analyzed for the presence, type, location, and shape of the rib variations. From this data the incidence of each type of variations was calculated. Results: Seventy-six of the 5,000 adults(1.52%), 63 male(2.23%) and 13 female(0.6%), showed 88 cases of rib variation(Table 1). The most common variation was the bifid rib(n=35), followed by hypoplasia of the rib(n=22), flaring of the rib(n=18), bridging of the ribs(n=7), cervical ribs(n=3), and fusion of between ribs(n=3)(Table 2). The bifid rib(Table 1) was found most frequent in the right fourth rib(12/35, 34.3%), followed by the left fifth rib(6/35, 17.1%) and right third rib(6/35, 17.1%). Hypoplasia of the rib was common in first rib(20/22, 90.9%). Flaring of the rib was common at fourth rib(8/18, 44.4%, right and left combined), and bridging between ribs was common between first and second rib(3/7, 42.9%). Conclusion : The percentage of incidence of rib variations in adults was 1.52%. Bifid rib was the most common variation, followed by hypoplasia, flaring, bridging, cervical rib, and fusion of ribs, in decreasing order.

Radiation Induced Rib Fractures on Bone Scan after Breast Cancer Surgery and Radiation Therapy (유방암 절제술 후 방사선치료를 시행한 환자의 골스캔에서 보이는 방사전 유발 늑골골절)

  • Purpose: This study is to evaluate rib fractures on bone scan in breast cancer patients treated with breast cancer surgery and radiation therapy and to evaluate its relation with radiation therapy and operation modality. Materials and Methods: Two hundred seventy cases that underwent serial bone scan after breast cancer surgery and radiation therapy were enrolled. Bone scan and chest a findings of rib fracture were analyzed. Results: The rib uptake was seen in 74 of 270 cases (27.4%) on bone scan and 50 cases (18.5%) were confirmed to have rib fracture by chest CT. The rate of modified radical mastectomy in patients with rib fracture was significantly higher than that in patients without rib fracture (66.0% vs. 27.0%, p = 0.000). The rate of additional radiation therapy to axillar or supraclavicular regions in patients with rib fracture was significantly higher than that in patients without rib fracture (62.0% vs. 28.6%, p =0.000). Rib fracture was seen most frequently at 1-2 years after radiation therapy (51.9%) and single rib fracture was seen most frequently (55.2%). Of total 106 rib fractures, focal rib uptake was seen in 94 ribs (88.7%) and diffuse rib uptake was seen in 12 ribs (11.3%). On one year follow-up bone scan, complete resolution of rib uptake was seen in 15 ribs (14.2%). On chest a, the rate of fracture line in ribs with intense uptake was significantly higher than that in ribs with mild or moderate uptake (p = 0.000). The rate of presence of fracture line in ribs with focal uptake was significantly higher than that in ribs with diffuse uptake (p = 0.001). Conclusion: Rib fracture in breast cancer patients after radiation therapy was related to radiation portal and operation modality. It should be interpreted carefully as a differential diagnosis of bone metastasis.

Giant Cell Tumor of the Rib: A Case Report and Review of the Literature (늑골에서 발생한 거대세포종: 1예 보고 및 문헌 고찰)

  • Giant cell tumor (GCT) of the rib may present as a posterior mediastinal mass when it involves the posterior arc. Only 4 cases of GCT of the rib presenting as a posterior mediastinal mass have been reported. We report a case of a 38-year-old man with GCT of the rib. Computed tomography revealed a well-defined, multi-lobulated, heterogeneous mass in the right superoposterior mediastinum, which appeared to invade the right third rib and thoracic vertebra. It was thought to be a posterior mediastinal ganglioneuroma or its malignant transformation. Grossly, the tumor mass arose in the posterior arc and showed substantial growth out of the rib. Microscopically, the tumor consisted of interspersed multi-nucleated giant cells and stromal mononuclear cells, compatible with GCT. For GCT, a wide excision with elective radiotherapy should be considered. GCT must be differentiated from posterior mediastinal ganglioneuroma that can be treated by surgical excision alone.

Surgical Treatment of Traumatic Rib Fracture with Judet's Rib Struts (Judet씨 Struts를 이용한 외상성 늑골골절의 치료)

  • Judet's rib struts which are designed for osteosynthesis are made of stainless steel This report describes clinical analysis of our experiences of 30 cases with the operative stabilization of multiple rib fractures with Judet's rib struts from December, 1995, to December, 1996 in Chungmoo Hospital, Chounan. Our indications for operative stabilization were as follows: 14 cases in flail chest, 8 cases in severe displacement of rib including segmental fractures, 7 cases in additional procedures during intrathoracic operation, and 1 case in other cause. Postoperative artificial ventilation is needed in only 1 case for 5 days and postoperative complications were few: 2 cases of hemopneumothorax, 2 cases of alcohol withdrawal delirium, and 1 case of postoperative hepatitis. Average duration of hospital admission who have limited thoracic injuries was 10.5 days. Though more comparative studies is necessary, we find this technique to be better than previously published m thods, since it provides better stabilization and immobilization of the ribs and shortening the duration of hospital admission.

  • Chondromyxoid fbroma (CMF) is a rare, benign tumor of the bone that represents fewer than $2\%$ of all benign tumors of bone. CMF is most often found in the long tubular bones, especially the tibia and femur near the knee joint. Less common sites included the pelvis, fibula, calcaneus and rib. A 54-year-old male patient presented to us with history of swelling and mild, intermittent local pain without any rise in overlying skin temperature in lateral portion of left 7th rib for one-month duration, which was diagnosed as benign rib tumor by plain chest X-ray and CT scan, and treated successfully by excision of rib with good result. Pathologic diagnosis of this tumor was CMF. Without any medical therapy, there was no evidence of recurrence after operation. We report this case and follow-up of the patient.

A Study for Web Frame Design on Engine Room Structure of Ship (선박기관실 구조의 특설늑골 설계에 대한 연구)

  • A design method for web frame scantling on engine room structures was described. The parameters determining the scantling of web frame were studied on the basis of the empirical ship's data. It is found that the parameters determining the scanting of web frame consist of tween deck height, main engine and propeller excitation frequency, scantling draft, web frame spacing, the number of decks and main engine BHP etc. And a formula proposed by empirical ship's data was established in view points of static and dynamic structural behavior. In this study, it can be shown that at initial design stage, a method for web frame scantling of engine room structure is provided as very practical design processes.

  • A 17-year-old-boy with a bilateral incomplete cervical rib, upon abduction of his left arm at 45 degrees, had immediately begun to show symptoms of severe tingling, claudication, pallor, and weakness of his left upper extremity. These symptoms were aggravated at 90 degrees, leaving him debilitated from his work in the printing office. Transfemoral positional subclavian arteriography revealed total occlusion of the subclavian artery immediately distal to a cervical rib during 90 degrees abduction. Resection of the anterior scalene and medial aspect of the middle scalene muscles, cervical and first ribs, and arteriolysis were performed via a combined supraclavicular and infraclavi cular approach. He has returned to work as a printer with marked relief of symptoms and has remained asymptomatic over follow-up periods of 10 months.

The Study of Effectiveness in a Modified Rib Oblique Projection View Using a Chest Phantom (흉부팬텀을 이용한 변형된 늑골 사방향 검사의 유용성에 관한 연구)

  • This study is filmed by applying the axial angle variation of the X-ray tube instead of the patient's position change during the perimetric examination of the ribs. A Reference image with the rib oblique examination using a chest phantom and experimental images applied with a six-phase variation in the axial angle by increasing $5^{\circ}$ tube angle each from $5^{\circ}{\sim}30^{\circ}$ from the vertical incident direction of the chest phantom to the right horizontal axis were obtained. For the quantitative comparative evaluation of the images, SNR and CNR were calculated for regions of interest in the experimental images based on the reference image. Also, the left-right rib ratio in the reference image and the left-right rib ratio in the experimental images are measured and compared. As a result of the study, the experimental images with a tube angle of $25^{\circ}$ were best shown in the measurements of the SNR, CNR and left-right rib ratio compared to the reference image with a standard examination method. The modified rib examinations will consider useful, if it is difficult to maintain the patient's examination position.

  • A 42 year old male was admitted for a bony mass on the posterior arc of the left $6^{th}$ rib, which was detected in a multiphasic health screening test. According to the chest computed tomography scan and bone scan, osteochondroma was suspected. He underwent VATS rib resection. There was no vessel or nerve injury. The patient was discharged without any complication on the $4^{th}$ post operative day. The pathological diagnosis was benign fibrous histiocytoma. Generally, posterolateral thoracotomy is needed for rib resection, but we found that there was no difficulty in doing this kind of surgery under a thoracoscopic approach, which has the advantage of better cosmesis.
