  • 당뇨병 환자에서 동맥 경화증은 가장 흔하고 중요한 합병증 중 하나이며 동맥경화증에 의한 관상동맥질환, 뇌혈관 질환 등 대혈관 질환은 당뇨병이 없는 사람보다 그 빈도가 2-4배 높으며 사망률 또한 높고 뇌졸중 중에서도 특히 뇌경색의 빈도가 높다.

  • 우리나라에서 만성 간질환으로 인한 사망자는 해마다 2만여 명에 가깝다. 이는 전체 사망자의 8.8%를 차지하는 것으로 단일 장기로 따지면 만성 간질환은 뇌혈관질환에 이어 사망 원인 2위다. 특히 40~50대 사망원인 중 3위를 차지한다고 한다. 간질환의 가장 큰 원인은 바이러스성 간염으로 B형 간염이 약 70%, C형 감염이 약 10~15%를 차지하고 있다.

Validation of the Nursing Outcomes Classification on Cerebrovascular Patients (뇌혈관질환자에게 적용가능한 간호결과 분류체계의 타당성 검증)

  • Purpose: The purpose of this study was to assess the importance and contribution of 9 nursing outcomes and their indicators that could be applied to cerebrovascular patients. Methods: Data were collected from 175 neurosurgical nurses working at two university affiliated hospitals and five secondary hospitals located in Gwang-ju. The Fehring method was used to estimate outcome content validity(OCV) and outcome sensitivity validity(OSV) of nursing outcomes and their indicators. Stepwise regression was used to evaluate relationship between outcome and its indicators. Results: The core outcomes identified by the OCV were Tissue Perfusion: Cerebral, Nutritional Status, Neurological Status, and Wound Healing: Primary Intention, whereas highly supportive outcomes identified by the OSV were Oral Health, Self-Care: ADL, and Nutritional Status. All the critical indicators selected for Fehring method were not included in stepwise regression model. By stepwise regression analysis, the indicators explained outcomes from 19% to 52% in importance and from 21% to 45% in contribution. Conclusion: This study identified core and supportive outcomes and their indicators which could be useful to assess the physical status of cerebrovascular patients. Further research is needed for the revision and development of nursing outcomes and their indicators at neurological nursing area.

Effects of Bed Angles and Bed Rest Time Combined with Hemostatic Methods on Discomfort and the Occurrence of Hemorrhagic Complications in Patients after Transfemoral Cerebral Angiography (뇌혈관조영술 후 침상각도와 지혈방법에 따른 침상안정 시간이 불편감과 출혈합병증에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of bed angles and bed rest time combined with hemostatic methods on discomfort and hemorrhagic complications in patients after transfemoral cerebral angiography. Methods: Data were collected from 93 inpatients following transfemoral cerebral angiography, from April 20 to September 23, 2016. Patients were grouped according to bed angle ($0^{\circ}$ vs. $30^{\circ}$) and bed rest time combined with hemostatic methods (4-hour bed rest after manual compression vs. 2-hour bed rest after applying vascular closure device). Results: There was a significant group differences on discomfort (F=46.44, p<.001). The post-hoc analysis showed the lowest score of discomfort in those with bed angle $30^{\circ}$ and 2-hour bed rest. There was no difference in hemorrhagic complications among 4 groups. Conclusion: The postangiograpy discomfort can be effectively reduced with the least hemorrhagic complications by bed angle $30^{\circ}$ elevation and 2-hour bed rest after applying vascular closure device for those underwent transfemoral cerebral angiography.

Effects of a Cardiocerebrovascular Disease Prevention Education Program for Postmenopausal Middle-aged Women (폐경 중년여성을 위한 심뇌혈관질환 예방교육프로그램의 효과)

  • Purpose: This study was conducted to examine effects of a cardiocerebrovascular disease (CVD) prevention education program on knowledge, self-efficacy and health behavior among postmenopausal middle-aged women. Methods: A non-equivalent control group pretest-posttest design was used. Participants were 53 postmenopausal middle-aged women who registered in two community culture centers in G metropolitan city. Experimental group (n=26) received a CVD prevention education program 8 times over 8 weeks. Knowledge, self-efficacy and health behavior of the participants were examined with self-report structured questionaries. Data were collected between October 15 and December 11, 2013, and were analyzed using chi-square test, Fisher's exact test, independent t-test, and analysis of covariance with SPSS/PC version 21.0. Results: After the intervention the experimental group showed significant increases in the knowledge of CVD symptoms (p<.001) and CVD risk factors (p<.001), level of self-efficacy (p=.028) and health behavior (p<.001) compared to the control group. However, no significant difference was found between groups for knowledge of CVD prevention (p<.133). Conclusion: Results suggest that a CVD prevention education program can be an effective strategy to improve knowledge on CVD symptoms and risk factors, self-efficacy and health behavior for postmenopausal middle-aged women.

Risk Factors of Cerebrovascular Disease According to the Level of Blood Pressure among the Prospective Tele-medicine Users (재택진료 잠재적 이용자의 혈압 수준에 따른 뇌혈관 질환 위험요인 분포)

  • Purpose: This study was undertaken to investigate bio-medical and behavioral risk factor of stroke according to the level of blood pressure among the prospective tele-health users. Method: A descriptive study was conducted using interviews with a structured questionnaire. Of the households in the village, 24 households were selected for inclusion in the study. Among all of the residents from these households, a total of 48 residents who completed all the blood tests as well as the questionnaire. Results: Among bio-medical risk factors including demographic characteristics, physiologic variables, and health history, only triglyceride(TG) was significantly different among normal, prehypertension, and hypertension groups(F=3.78, p<.05). However, regarding behavioral variables, those who were classified as prehypertension and hypertension group reported more frequent drinking, those who were in prehypertension group reported highest scores of stress and lowest frequency of exercise, and those who were in normal and hypertension group showed higher scores of interest on health and lower scores of perceived seriousness of disease and health knowledge than those who were in prehypertension group. Conclusion: Identification and implementation of modifiable risk factors of stroke according to the level of blood pressure are crucial for health care utilizing tele-medicine.

  • Purpose: To investigate factors related to cardio-cerebrovascular disease and groups disadvantaged by it in Korean middle-aged women, using the Korea National Health and Nutrition Examination Survey (KNHANES). Methods: The present study was conducted with 1,627 middle-aged women, aged 40 to 64 years, who participated in the 7th (2016) Korea National Health and Nutrition Examination Survey conducted by the Korea Centers for Disease Control and Prevention. Results: Cardio-cerebrovascular disease among middle-aged women was associated with quality of life, menopause, diabetes mellitus, body mass index, and family history of hypertension. The incidence of cardio-cerebrovascular disease in middle-aged women was found to be the most prevalent in women who have entered menopause, have a family history of hypertension, and have a body mass index greater than $30.0kg/m^2$. Conclusion: This study classifies the subjects according to the risk level of each disadvantaged group for cardio-cerebrovascular disease prevention and management in middle-aged women. The results provide evidence to support a tailored cardio-cerebrovascular disease prevention and management program based on the related factors of disadvantaged groups and to establish strategies in educational and practical aspects.

Effects of Information Provision on Anxiety, Blood Pressure & and Pulse in Cerebral Angiography Clients (정보제공이 뇌혈관조영술 대상자의 불안, 혈압, 맥박에 미치는 영향)

  • Purpose: This study was to evaluate the effects of information on anxiety, blood pressure and pulse in cerebral angiography clients. Methods: The data were collected from June to November 2014. The participants were 42 (21 each for experimental and control group) patients who to received cerebral angiography. The information developed from researchers' materials for cerebral angiography was provided only to experimental group. Measured variables were anxiety, systolic and diastolic blood pressure, and pulse rate. Research tools for anxiety were Spielberger's state anxiety inventory, and 10 point visual analogue scale (VAS). Results: The difference in mean systolic blood pressure after intervention between the experimental group ($129{\pm}15.34$) and the control group ($141{\pm}17.70$) was statistically significant (t=-2.28, p=.028). The differences between the two groups in state anxiety, VAS anxiety, diastolic blood pressure, and pulse rate after intervention were not statistically significant (p>.05). Conclusion: The information using educational material was effective in to decreasing systolic blood pressure in patients who received cerebral angiography. Therefore this study material could be used as a nursing intervention for patients in cerebral angiography.

The Effect of Discharge Education Program on Compliance with Sick-Role Behavior in Cerebrovascular Surgery Patients (퇴원교육 프로그램이 뇌혈관 수술환자의 역할행위 이행에 미치는 영향)

  • Purpose: The purpose of this study was to examine the effect of the discharge education program on compliance with the sick role behavior for patients having undergone cerebrovascular surgery. Method: Research was done using a posttest only design. The subjects were 60 patients who were admitted to the neurosurgery unit at C.N.U. Hospital in G. City and were divided into the experimental and control groups. The discharge education program were intervened two times in the experimental group by the researcher; the first one was at the time of discharge using a booklet about knowledge related to disease and compliance, and the other one was a telephone education session after a week from discharge. Data were collected two times by interview and telephone using questionnaires from January 19, to June 10, 2000. The first one was at hospital before discharge, and the other one was one month later from discharge. Data were analysed by $\chi^{2}$, t-test, ANOVA, and Pearson's correlation. Results: The experimental group showed a higher score of compliance(t=2.772, p=.008) than those of the control group, but knowledge about CVD was not significant between the two groups(p>.05). Conclusion: The discharge education program was effective on the compliance of the patients having undergone cerebrovascular surgery.
