오약순기산(烏藥順氣散)이 허혈성 뇌손상 흰쥐의 인지 및 운동기능회복에 미치는 효과 (The Effects of Ohyaksungi-san(Wuyaoshunqi-san) on Cognition and Motor Function Recovery after Ischemic Brain Injury in Rat)

  • Objectives : Ohyaksungi-san(Wuyaoshunqi-san) has been used for many years as a treatment for cerebrovascular diseases in Oriental medicine. This study was designed to evaluate the effects of Ohyaksungi-san(Wuyaoshunqi-san) on cognition and motor function recovery after ischemic brain injury, and also the expression of BDNF in hippocampus. Methods : This study was designed with 4 subgroups to evaluate the effects of Ohyaksungi-san(Wuyaoshunqi-san). As control groups, group I has no treatment during 1 week after ischemic brain injury and group II has no treatment during 2 weeks after ischemic brain injury. As experimental groups, group III has been treated with Ohyaksungi-san(Wuyaoshunqi-san) during 1 week after ischemic brain injury and group IV have treated with Ohyaksungi-san(Wuyaoshunqi-san) during 2 week after ischemic brain injury. Each group has been examined by tests as follows, neurological motor behavioral tests, cognitive motor behavior test and histological test. Neurological motor behavior tests consisted of limb placement test, beam-walking test and horizontal wire test. Cognitive motor behavior test was performed by using Morris water maze. In the histological test, TTC(2,3,5-triphenylteterazolium chloride) staining, hematoxylin & eosin staining, and immunohistochemical staining were used. Results : 1. The tests for motor function recovery change had significantly good result in the experimental groups as compared with control groups(p<.05). 2. The Morris water maze test on cognition also had significantly good result in the experimental groups as compared with control groups(p<.05). 3. In the immunohistochemical staining for the expression of BDNF in hippocampus, more immune reaction was investigated in the experimental groups as compared with control groups. Especially group IV has the greatest immune reaction. Conclusions : Ohyaksungi-san(Wuyaoshunqi-san) has good effects on cognition and motor function recovery after ischemic brain injury, and also the expression of BDNF in hippocampus.

국소 허혈성 뇌손상 흰쥐 모델에서 환경강화 조건 하 수정된 건측억제유도 운동치료가 앞다리 운동기능 증진에 미치는 영향 (Effects of Improved Forelimb Sensorimotor Function on the Modified CIMT Applied Under the influence of Environmental Enrichment in a Focal Ischemic Brain Injury Rat Model)

  • Environmental Enrichment (EE) alone is not capable of enhancing the fine digit and the forelimb functions. Therefore, we applied modified constraint-induced movement therapy (mCIMT) under the influence of EE to assess its effect on promoting improved forelimb sensorimotor functions. Focal ischemic brain injury was produced in Sprague-Dawley rats (60 rats, $250{\pm}50$ g) through middle cerebral artery occlusion (MCAO). Before MCAO induction, all rats were trained in modified limb placing tests and reaching tasks for 1 week. Then they were randomly divided into three groups: Group I: application of standard environment (SE) after MCAO induction (n=20), Group II: application of EE after MCAO induction (n=20), Group III: MCAO+EE, mCIMT and task-oriented training that was initiated at 10th day after MCAO induction (n=20). We also applied mCIMT (between 9 AM and 5 PM/daily) which included restraining the forelimb ipsilateral to the lesion using the 'Jones & Schallert' method. We assessed the change of modified limb placing, single pellet reaching test and the immunoreactivity of BDNF by immunohistochemistry (pre, 1st, 5th, 10th and 20th day). Group I showed no improved outcome, whereas group II and III significantly improved on the use of the forelimb and the immunoreactivity. The qualitative analysis of the skilled reaching test, of group III showed the greatest improvement in the fine digit and the forelimb function. These results suggest that EE combined with mCIMT is more functional in promoting enhanced fine digit and forelimb functional movements.

N-6와 n-3 지방산이 풍부한 식이가 뇌졸중 유발 모델에서 뇌경색 크기 및 항산화 효소계에 미치는 영향 (Neuroprotective & antioxidant effects of diets high in n-6 and n-3 fatty acids in rat focal brain ischemia model)

  • This study was undertaken to investigate the effects of n-6(corn oil) & n-3(fish oil) fatty acids on infarction size and the cerebral activities of antioxidant enzyme in rat focal brain ischemia model. Weaning Sprague-Dawley rats were fed with either corn oil supplemented diet(COD, 14% corn oil) or fish oil supplemented diet(FOD, 14% menhaden oil) for 6 weeks. The right middle cerebral artery was occluded for 2 hours with a silicon rubber coated nylon surgical thread. After 24 hours of recirculation, the rats were sacrificed and brain sections were photographed using CCD camera after staining with 2, 3, 5-triphenyltetrazolium chloride for 60 minutes in room temperature. The infarcted area was measured and the volume of infarction was calculated. Catalase(CAT), superoxide dismutase(SOD) activities, and fatty acid composition in the brain were also measured. The total and corrected infarction volumes were not significantly different between FOD and COD group. The docosagexaenoic acid(DHA) and DHA content/arachidonic acid(AA) ratio of the cerebral cortex, an index of defense against lipid oxidation, were significantly increased in FOD group compared to those of COD group(p<0.05). In the left cortex(non-infarction side) as well as the right cortex(infarction side) of FOD group, CAT and Cu/Zn SOD activities were higher than those of the COD group(p<0.05). However, CAT and Cu/Zn SOD activities were not significantly different between the left cortex(non-infarction side) and the right cortex(infarction side) of both FOD and COD group. GPx activities were also not significantly different between two groups. Our results demonstrate that the brain infarction size in FOD and COD were not significantly different. However, cerebral lipid composition and antioxidant enzyme activities in FOD and COD group were different. Fish oil, a source of n-3 polyunsaturated fatty acid(PUFA) and corn oil, that of n-6(PUFA) may have a protective effect against oxidative stress induced via different mechanisms.

  • Objectives : Sopung-tang(Shufeng-tang) is a famous herbal prescription that treated ischemic brain injury. This study was designed to evaluate the effects of Sopung-tang(Shufeng-tang) on congnition and motor function recovery after ischemic brain injury in rats. Methods : Male rats were divided into 4 groups. Those rats caused ischemic brain injury by occlusion of MCA as Longa method. Control group I was per os normal saline for 7 days after ischemic brain injury. Control group II was per os normal saline for 14 days after ischemic brain injury. Experimental group I(Ex I) was taken with Sopung-tang(Shufeng-tang) for 7 days after ischemic brain injury. Experimental group II(Ex II) was taken with Sopung-tang(Shufeng-tang) for 14 days after ischemic brain injury. The author carried out neurological, cognitive motor behavior tests and histological assessment. Neurological motor behavior tests consist of limb placement test, beam-walking test and horizontal wire test. Morris water maze test was used for cognitive motor behavior test. In the histological assessment test, TTC(2,3,5-triphenylteterazolium chloride) staining, Hematoxylin & Eosin staining and immunohistochemical staining were experimented. Results : 1. In neurological motor behavior tests, motor function recovery was significantly increased in the experimental groups as compared with control groups(p<0.05). Especially Ex II was significantly increased as compared with Ex I(p<0.05). 2. In Morris water maze test, congnitive motor function recovery was significantly increased in the experimental groups as compared with control group(p<0.05). Especially Ex II was significantly increased as compared with Ex I(p<0.05). 3. In the immunohistochemical staining for the expression of BDNF in hippocampus, more immune reaction was investigated in the experimental groups as compared with control groups. Especially most immune reaction was experimented in the EX II. Conclusions : According to the above results, Sopung-tang(Shufeng-tang) can treat on the congnition and motor function recovery after ischemic brain injury in rats. And it is effective method in expression of BDNF in hippocampus.

귀비탕(歸脾湯)의 농도별(濃度別) 구강투여(口腔投與)가 백서(白鼠)의 허혈성(虛血性) 뇌손상(腦損傷)에 미치는 영향(影響) (The Dose-dependent Effects of Guibi-Tang on Focal Brain Ischemic Injury in Rats)

  • Objectives : The aim of this study was to investigate the effects of Guibi-Tang(GBT) on focal brain ischemic injury induced by intraluminal filament insertion in rats. Methods : The ischemia was induced by intraluminal filament insertion into middle cerebral artery. Sprague-Dawley rats were divided into five groups, normal group(n=8); control group was ischemia induced and no treatment(n=8); GBT 1X group was ischemia induced and administrated 42.2 mg/ml/kg of Guibi-Tang orally(n=8); GBT 3X group was ischemia induced and administrated 126.6 mg/ml/kg of Guibi-Tang orally(n=8); GBT 6X group was ischemia induced and administrated 253.2 mg/ml/kg of Guibi-Tang orally(n=8) for 21 days. mGluR5, Bax, Bcl-2 and cytochrome C were investigated to observe the effects of Guibi-Tang on apoptosis. The effects of Guibi-Tang on neuroprotective/apoptotic agents in cresyl violet, choline acetyltranferase(ChAT) with ischemic injury were investigated. Results : The intensity of mGluR5 mRNA in the hippocampal CA1 was increased in normal and GBT(Guibi-Tang) 1X groups compared with the control group. The intensity of Bax mRNA in the hippocampal CA1 was decreased in normal and GBT 1X groups compared with the control group. However it was increased unexpectedly in GBT 3X group. The intensity of Bcl-2 mRNA in the hippocampal CA1 was increased in normal and GBT 1X groups compared with the control group. The intensity of Bax/Bcl-2 ratio in the hippocampal CA1 was decreased in normal and GBT 1X groups compared with the control group. The intensity of cytochrome C protein in the hippocampal CA1 was decreased in normal, GBT 1X and GBT 6X groups compared with the control group. The density of neurons stained by cresyl violet and ChAT was increased in normal and GBT 1X groups compared with the control group. Conclusions : These results suggest that Guibi-Tang may have protective effect on vascular dementia.

갈근 에탄올추출물의 국소뇌허혈 모델에 대한 뇌신경보호 효과 (Neuroprotective effect of Puerariae Radix extract on focal cerebral ischemia in mice)

  • Objectives : The purpose of this study was to evaluate the neuroprotective effect of Pueraria lobata extract on focal cerebral ischemia in mice. Methods : Focal cerebral ischemia was induced by occlusion of the right middle cerebral artery using the intraluminal filament model. ICR male mice underwent 90 minutes of middle cerebral artery occlusion (MCAo) followed by 24 hours of reperfusion. Mice were administered Pueraria lobata extract orally at the dose of 300mg/kg just prior to reperfusion. Rotarod test and balance beam test were practiced to assess sensory-motor function 23 hours after MCAo. In rotarod test, the latency to fall on the accelerating rotarod was recorded for 5 min. In balance beam test, the score was graded according to number of slips and latency to cross. The infarct volume was measured 24 hours after MCAo using 2% 2,3,5-triphenyltetrazolium chloride (TTC) staining. Results : Pueraria lobata extract treated group showed significant reduction in infarct volume by 27.3% compared to control group (p<0.05). In rotatod test, it also showed significant extension of latency time compared to control group ($67.82{\pm}15.08$ vs. $5.62{\pm}1.06$, p<0.001). In contrast to performance in rotarod test, that in balance beam test did not improve with Pueraria lobata extract treatment. Conclusions : We conclude that Pueraria lobata extract has a significant neuroprotective effect and reduces damage of sensory-motor function in MCAo model. These findings suggest that Pueraria lobata could be a potent neuroprotective agent.

중추신경계통내 분포하는 Zinc의 조직화학적 동정 (Autometallography for Zinc Detection in the Central Nervous System)

  • Zinc는 인체 내에 철$(Fe^{2+})$다음으로 많은 trace element로서 200여개 효소의 기능에 필수적일 뿐만 아니라 신경계통내에서는 신경조절물질로 작용한다. 뿐만 아니라 허혈, 간질 및 퇴행성 뇌질환의 주요 병리기전에도 관여되어 있다. 그러나 대부분의 Zinc는 단백질에 결합되어 (bound form)신경세포의 세포질 및 핵질내에 존재하고, 10% 이하의 Zinc는 이온상태(free form, $Zn^{2+}$)로 신경종말 (Zinc enriched terminal)에 있는데 , 후자만이 조직화학법으로 가시화된다. 최근까지 새로 개발된 조직화학법으로 Zinc enriched(ZEN)neurons의 분포에 관한 연구가 각광받고 있으나, 국내에서는 이에 대한 연구가 전무한 실정이다. 이에 본 연구자는 고전적인 조직화학법의 기본 원리를 소개하고, 렛드 중추신경계통내 Zinc의 분포를 광학 및 전자현미경으로 관찰하고자 하였다. 본 연구에서 사용된 실험동물은 Wistar 계통의 랫드(10주령)와 BALB/c 마우스이며, 마취제로는 Pentobarbital(50mg/kg)을 이용하였다. 생체 뇌조직내 이온상태의 $Zinc(Zn^{2+})$를 침전시키기 위하여 selenium(10mg/kg, i.p.)을 처리하였고, 1시간 후 3% Glutaraldehyde액으로 관류고정하여 동물을 희생시켰다. 뇌와 척수를 꺼내어 sucrose에 가라앉을때 까지 담가두었다가 Dry Ice를 이용하여 얼리고, Freezing microtome위에서 $30{\mu}m$두께의 절편을 작성하였다. 조직절편내 $Zn^{2+}$을 동정하기 위한 조직화학법으로는 autometallography (AMG) (Danscher, 1985)를 이용하였다. 광학현미경하에서 밝혀진 Zinc의 분포는 해마복합체를 비롯한 종뇌의 여러부위에 고농도로 분포하였고, 척수에는 중간정도, 그리고 소뇌 및 뇌간에는 매우 낮은 농도로 분포하였다. 전자현미경에서 관찰된 AMG염색과립(silver grains)은 신경종말에 있는 연접소포에 국한되었으며, 이러한 ZEN terminals은 주위 여러 신경세포의 돌기(dendrites)및 세포체 (soma)에 특이한 연접을 이루고 있었다. 즉 후각망울을 포함한 종뇌에서는 주로 비대칭연접 (asymmetrical synapses)이 관찰되었던 반면에, 척수에서는 대칭연접(symmetrical synapses)을 이루고 있었다. 이상의 결과를 종합하면, 신경종말내 연접소포에 Zinc를 함유하고 있는 소위 ZEN terminals은 중추신경계통에 광범위하게 분포하고 있으며 또한 신경부위에 따라 다양한 분포와 미세구조의 차이를 보였다. 이러한 사실은 중추신경계통내에서 Zinc가 영위하는 신경생물학적 기능이 신경부위에 따라 다양할 것임을 시사한다.

한국 정상인에서 연령에 따른 뇌혈류분포와 혈류예비능의 변화: Tc-99m HMPAO SPECT에 의한 연구 (Change of Cerebral Blood Flow Distribution and Vascular Reserve according to Age in Koreans Measured by Tc-99m HMPAO Brain SPECT)

  • 목적 : Tc-99m HMPAO 아세타졸아미드(이하, ACZ) 부하 뇌관류 SPECT(이하 중재적 SPECT)는 뇌혈관 질환에서 조기진단, 수술 적응증 선별이나 방법 결정, 그리고 수술 효과 판정과 경과 관찰시 매우 유용하게 사용되고 있다. 중재적 SPECT의 정화한 해석과 진단율을 높이기 위해서는 정상인의 CVR 자료가 필요하지만 국내엔 이런 자료가 없다. 본 연구에서는 연령에 따라 뇌혈류분포와 혈류예비능이 차이를 보이는지 알아보고자 하였다. 대상 및 방법: 정상 지원자 34명을 대상으로 하였고, 남자 20명, 여자 14명이었으며, 평균연령은 $40.3{\pm}24.9$세, 범위는 4세에서 82세였다. 이들은 신경학적 검사상 특이 소견이 없었으며, 뇌혈관질환, 당뇨, 고혈압, 허혈성 심질환, 그리고 두부외상의 병력은 없었다. 중재적 SPECT는 Tc-99m HMPAO와 이중 헤드 회전형 감마카메라를 이용하여 휴식 및 ACZ부하 SPECT를 연속적으로 시행하여 감산영상을 얻는 방법을 사용하였다. Tc-99m HMPAO는 휴식 및 부하 모두 925 MBq을 주사하였다. ACZ(성인: 1 g, 소아: $250{\sim}500 mg$) 주사 후 23분째에 두 번째 Tc-99m HMPAO를 주사하고 10분 후 부하영상을 얻었다. 동일한 관심영역을 전두엽, 두정엽, 측두엽, 후두엽, 기저핵, 시상, 소뇌반구에 설정하여 평균 방사능계수를 얻었다. 정량적 지표로 R/L, R/Cbll와 R/WB를 사용하였다. 감산영상에서 구한 평균방사능계수를 상응한 휴식기 계수를 기준으로하여 백분율을 구하여 CVR 평가지표로 사용하였다. 모든 통계적 검정에서 통계적 유의성은 P값이 0.05 미만인 경우만 차이가 있다고 인정하였다. 결과: 한국 정상인에서 뇌혈류 분포는 전 연령에서 R/L의 대뇌반구의 평균 1.004에서 1.018로 좌우 대뇌반구간의 차이가 5%미만이었다. 각 연령군 모두에서 소뇌의 측정치가 가장 높았고, 기저핵과 시상은 가장 낮은 혈류분포를 보였다. 연령이 증가할수록 전두엽과 측두엽은 유의하게 감소하였고 후두엽과 시상부는 유의하게 증가하였다. 모든 연령에서 남녀간에 혈류분포의 차이는 없었다. CVR은 $29.9{\pm}12.9%$이었고, 소아에서는 CVR이 연령에 따라 유의하게 증가하여 10대 후반에 가장 높았다. $20{\sim}49$세 사이에는 서서히 감소하는 경향을 보였고, 50세 이상의 연령군에서 양측 전두엽, 좌측 두정엽, 그리고 우측 기저핵에서 나이가 들어감에 따라 의의있게 감소하였다. 평균 CVR이 여자에서 남자보다 높은 경향을 보였으나 통계학적으로 유의한 차이는 없었다. 결론: ACZ와 Tc-99m HMPAO을 이용한 감산법으로 CVR를 정량적으로 평가할 수 있었고, 한국 정상인에서 뇌혈류 분포와 CVR이 연령에 따라 변한다는 사실을 알 수 있었다.

백서의 가역성 뇌허혈 모형에서 재관류 시간에 따른 뇌경색 크기의 변화 (Changes in Infarct Size after Reperfusion with Time in a Reversible Cerebral Ischemic Model in Rats)

  • Objective : The purpose of the present study was to determine the appropriate time of clinical intervention by observing and analyzing the changes in the size of infarct, penumbra and cerebral edema and the extend of neurological deficit due to reperfusion damage according to time in a reversible cerebral ischemic model of reperfusing blood flow after inducing ischemia by maintaining middle cerebral artery occlusion for 2 hours(h) in rats. Methods : The rats were divided according to reperfusion time into control group(0 h reperfusion time) and experimental groups(0.5, 1, 2, 3, 4, 5, 6, 12, and 24 h of reperfusion time). Results : Changes in the size of infarction due to reperfusion damage were 0.93, 1.48 and 1.16% at 0.5, 1 and 2 h after reperfusion, respectively, and although a statistical significance was not present compared to 1.35% of the control group, damages increased drastically up to 6 h(6.64%), and the size increased were 6.65 and 6.78% at 12 and 24 h, respectively. Also there was no significant difference after 6 h up to 24 h in the size of infarction. In the areas where infarction occurred, reperfusion damage increased significantly with time in cortex than in subcortex. Accordingly, the size of penumbra area also showed a statistically significant decrease from 2 h up to 6 h after reperfusion, and 6 h after reperfusion, the area almost disappeared, becoming permanent infarction. Thus, reperfusion damage showed a significant increase from 2 h up to 6 h after reperfusion, and became steady thereafter. As for the mean ratio of the extend of cerebral edema, the control group and reperfusion 0.5 h group were 1.073 and 1.081, respectively ; up to 2 h thereafter, the ratio decreased to 1.01 but increased again with time ; and in reperfusion 12 h and reperfusion 24 h, the ratios were 1.070 and 1.075, respectively, showing similar size with that of control group. As for neurological deficit scores, the score of the control group was 2.67, that of reperfusion 2 h was 2, those of reperfusion 3 h and 6 h groups were 3.2 and 3.8, respectively, and those of reperfusion 12 h and 24 h groups were 4.2 and 4.6, respectively. Thus, as for the test results, the neurological deficit increased with time 2 h after reperfusion, and in reperfusion 12 and 24 h groups, almost all the symptoms appeared. Conclusion : As shown in these results, although the changes in the size of infarction due to reperfusion damage did not increase up to 2 h after reperfusion in the experimental groups compared to the control group, damage increased significantly thereafter up to 6 h, and the size remained about the same from 6 h to 24 h after reperfusion, becoming permanent infarction ; thus, the appropriate time of intervention according to the present study is at least 6 h before after maintaining reperfusion, including the time of cerebral artery occlusion.

  • 목 적: 최근 신생아 중환자실 입원 환자 중 여러차례 수혈을 받았던 환아에서 괴사성 장염, 간질환, 신장기능 부전 등이 더 많았으며 특히 4회 이상 수혈을 받았던 환아에서는 사망률도 의미있게 높았다는 보고들이 있어, 본원 신생아 중환자실에 입원한 환자를 대상으로 혈소판 수혈 횟수에 따른 기저 또는 동반 질환 및 사망률의 차이를 알아보고자 하였다. 방 법 : 1996년 8월부터 2001년 7월까지 본원 신생아 중환자실에 입원한 환자 3,955명 중 혈소판 수혈을 받았던 환아 72명(1.8%)을 대상으로 하여 1회 수혈을 받은 군(I군)과 그 이상 수혈을 받은 군(II군)으로 분류하여 양 군간 신생아 패혈증/파종성 혈관내 응고병증, 호흡 곤란증, 뇌실내 출혈, 동맥관 개존, 만성 폐 질환, 괴사성 장염, 간 신 질환, 폐출혈, 저산소성 허혈성 뇌증 빈도 및 사망률을 비교하였다. 결 과 : 1회 수혈을 받았던 I군은 29명(40.2%), 2회 이상 수혈을 받았던 II군은 43명(59.7%)이었으며 16명(22.2%)은 4회 이상 수혈을 받았다. I군관 II군 사이에 재태 주령, 출생 체중, 성별 및 산모 병력에 유의한 차이는 없었고 제왕 절개술 빈도만 II군에서 많았다. 혈소판 수혈을 필요로 했던 환아 72명 중 패혈증이 66명(91.7%)으로 가장 많았다. 1회 수혈군과 2-3회 수혈군, 그리고 4회 이상 수혈군 사이에 기저 또는 동반 질환의 빈도는 서로 비슷하였고, 사망률은 오히려 II군에서 낮았으나 통계학적으로 의의는 없었다. 결 론 : 신생아 패혈증/파종성 혈관내 응고병증, 호흡 곤란증, 뇌실내 출혈, 동맥관 개존, 만성 폐 질환, 괴사성 장염, 간 신 질환, 폐출혈 등이 있는 경우 혈소판 수혈이 필요하였지만 수혈 횟수와 기저 또는 동반 질환 빈도 및 사망률에는 차이가 없어, 본 연구 결과로는 다량의 혈소판 수혈이 필요할 환자를 예측할 수 없었으며 향후 더 많은 대상으로 연구되어져야 할 것으로 사료된다.
