The Relationship of Self-Efficacy, Perceived Barriers and Health Promoting Behaviors in Stroke (뇌졸중 환자의 자기효능감, 지각된 장애성과 건강증진행위간의 관계)

  • Purpose: The purpose of this study was to: (1) describe the self-efficacy, perceived barriers and health promoting behaviors and (2) determine the relationship of these variables in stroke survivors. Methods: A convenience sample of 93 stroke survivors who were enrolled in a neurosurgical outpatient clinic in Seoul participated in the study. Results: Among health promoting behaviors, non-smoking was the subdomain with the highest mean score and exercise subdomain had the lowest mean score. Significant factors correlated with health promoting behaviors were age, self-efficacy and perceived barriers. Conclusion: Findings from this study suggest that health promotion programs aimed at enhancing self-efficacy and decreasing perceived barriers would be beneficial for the stroke survivors.

The Effect of an Orofacial Exercise Program using PNF on Diadochokinesis in Subacute Stroke Patients with Dysarthria (PNF 구강안면 운동 프로그램이 마비말장애가 있는 아급성 뇌졸중 환자의 교대운동속도와 연속운동속도에 미치는 효과)

  • Purpose: The aim of the present study was to investigate the effect of a PNF orofacial exercise program on diadochokinesis in subacute stroke patients with dysarthria. Methods: Thirteen subacute stroke inpatients with dysarthria were recruited for the study. All participants were randomly assigned to two groups: an experimental group (n=7) and a control group (n=6). The experimental group performed a PNF orofacial exercise program, and the control group carried out an orofacial self-exercise. Both groups received the treatments 30 minutes per day, 5 days per week, for 4 weeks. The outcome measures were assessed before and after the intervention, including an alternating motion rate (AMR) and sequential motion rate for diadochokinesis. Results: After the intervention, both groups showed significant improvement in diadochokinesis (p<0.05). In the change values for both groups, the experimental group showed a more significant improvement than the control group did in /pə/ of AMR (p<0.05). Conclusion: These findings suggest that PNF orofacial exercise programs may have positive effects on the improvement of diadochokinesis in subacute stroke patients with dysarthria.

Effects of Biofeedback Training with Shaker Exercise on the Suprahyoid muscle Activity of Stroke with Swallowing Disorder (Shaker운동을 병행한 바이오피드백 훈련이 삼킴장애가 있는 뇌졸중환자의 설골상근 근활성도에 미치는 영향)

  • PURPOSE: The aim of this study was to determine how biofeedback training with Shaker exercise effects the activation of the cervical myocardial muscle in stroke with swallowing disorder. METHODS: From June 2018 to September 2018, 30 patients who were hospitalized at C hospital and K hospital in Daegu, and K hospital in Gumi were surveyed to meet the criteria for selection. The participants were classified into three groups, and Shaker exercise was applied three times a day, five times a week, for four weeks; visuoauditory biofeedback group (VABG), visual biofeedback group (VBG), and self-exercises group (SG). In addition, the suprahyoid muscle activity was performed three times (before intervention, after intervention, and after two weeks). RESULTS: The pre and post-test comparisons, revealed a significant difference between the three groups (p<.01); VABG had the highest suprahyoid muscle activity. The post-test and follow-up test produced similar results in, the three groups (p>.05). The mean comparison showed the smallest difference in VABG, indicating that muscle persistence was the best. CONCLUSION: Shaker exercise has an effect on suprahyoid muscle activation. Biofeedback training, which provides an input of multi-sensory information in swallowing disorder treatment is recommended because it has the greatest effect when combined with visuoauditory biofeedback.

Smoking Attributed Economic Burden of Stroke Using Disability Grades (장애등급 분포를 활용한 흡연에 의한 뇌졸중 질병부담비용 측정)

  • Objectives: The purpose of the study was to estimate the economic burden of stroke in Korea using post stroke disability grades and to measure smoking attributed economic burden. Methods: The disability grade of stroke patient was assessed for stroke patients who did not have a history of stroke before from a large cohort study in Korea. Treatment costs of patients were estimated by the National Health Insurance Service annual statistical report and care cost was estimated by the need of care defined by the assessed disability grades of patients. The population attributable fraction from WHO was used to calculate the proportion of cost due to smoking. Results: The study revealed that the cost of the stroke in 2015 was about 3.228 trillion won, while the care-giver cost was approximately 176 billion won. The cost of the smoking attributed stroke was about 724 billion won. Conclusion: This study is useful for estimating the cost of smoking considering the post-stroke disability and the results is important for achieving the national goal of extending healthy life from the Fourth National Health Promotion Plan.

  • Atherosclerosis is a chronic vascular inflammatory disease in which plaque builds up in the arteries and impairs blood flow. This can lead to heart disease and stroke. Since most people do not have any symptoms until the artery is severely narrowed, early detection of atherosclerosis is critical. In this paper, in order to effectively detect atherosclerotic lesions in tube-shaped blood vessels, polar conversion is applied to MRI images based on the vessel center. We then propose a SE-LSTMNet model using continuous signal information for each angle of a polar coordinate image. The trained model showed classification performance of 0.9194 accuracy, 0.9370 sensitivity, 0.8796 specificity, 0.8700 F1 score, and 0.9719 AUC on the validation data.

Age-Related Physical Function(ADL, IADL) and its Related Factors of Elderly People in Korea (우리나라 고령자의 연령에 따른 신체적 기능(ADL, IADL)과 관련요인)

  • This study was performed to determine the levels of physical function (ADL, IADL) and to reveal its association with the related factors in the elderly people. The study subjects were 1,756 (male 872, female 884) people aged over 70 who received medical check-ups and long-term care services between 2009 and 2012 from the National Health Insurance Corporation. As a result, the distribution of impaired ADL and IADL increased significantly with age. Logistic regression showed that the risk ratio of impaired ADL was increased significantly in the following groups: female, urban, low weight, stroke history group, smoking, alcohol drinking, and not regular exercise group. The risk ratio of an impaired IADL were increased significantly in the group of females, low weight, smoking, alcohol drinking. On the other hand the risk ratio of an impaired ADL and IADL was similar in each age group. As above results, the levels of ADL and IADL in the study subjects are closely related to the socio-demographic characteristics and health related behaviors. In particular, they suggested that the levels of ADL and IADL were lower in the poor group of the health-related behaviors, such as smoking, alcohol drinking, and regular exercise.

  • When the eldery with limited mobility and disabled use a wheelchairs to move, it can cause decreased exercise ability like decline muscular strength in upper limb and lower extremities. The disabled people suffers with spinal cord injuries or post stroke hemiplegia are easily exposed to secondary problems due to limited mobility. In this paper, We designed intelligent wheelchair robot system for upper limb and lower extremities exercise/rehabilitation considering the characteristics of these severely disabled person. The system consists of an electric wheelchair, biometrics module for Identification characteristics of users, upper limb and lower extremities rehabilitation. In this paper, describes the design and configurations and of developed robot. Also, In order to verify the system function, conduct performance evaluation targeting non-disabled about risk context analysis with biomedical signal change and upper limb and lower extremities rehabilitation over wheelchair robot move. Consequently, it indicate sufficient tracking performance for rehabilitation as at about 86.7% average accuracy for risk context analysis and upper limb angle of 2.5 and lower extremities angle of 2.3 degrees maximum error range of joint angle.

Hemiplegic Migraine Presenting with Unilateral Facial Palsy: A case report (편측안면마비로 발현한 편마비편두통 1예)

  • Hemiplegic migraine (HM) is a rare subtype of migraine with aura and is accompanied by a fully reversible motor aura. HM can occur in two forms: familial or sporadic. Currently, three genes are related to familial HM. Typically, HM occurs in the first or second decade of life and involves gradually progressing aura symptoms in succession, accompanied by headaches. The aura includes visual, sensory, motor, aphasic and often basilar-type symptoms. Motor aura (weakness) is related to the regions where the sensory aura is involved, and it usually starts at the hand before spreading to the arm and face. Aphasia is a common form of speech aura, but does not typically present as a difficulty in understanding. In this case report, the sensory-motor aura started at the right face and then gradually progressed to the right leg without any symptoms in the ipsilateral upper extremity. To the best of my knowledge, there has been no previous case report for the presentation of a hemiplegic migraine, as in this case report. As there is a possibility of misdiagnosis of Bell's palsy at the early stage of this case, this case report suggests that a physician should consider the rare possibility of stroke or HM when a patient presents with unilateral facial palsy.

The Correlation Between Early Clinical State and Functional Outcome in Acute Stroke Patients (급성기 뇌졸증 환자의 상태와 기능회복도와의 상관관계)

  • Nowadays there were two tendencies of studies about prognostic factors in stroke. One way was to define prognostic factors according to the radiological features. And the other way was to define according to the mental state, recognition, perception, motors, language, urinary&bowel incontinence etc.. The former could be objectively investigated, while the latter was difficult. The purpose of this study was to determine which variables would be predictors of stroke and which factors would be affect predictions most. The subjects of this study were 32 patients who were admitted to the Dept. of Internal Medicine, Dongguk Univ. College of Oriental Medicine whthin 48 hours from attack, Medical records were reviewed FIM, CNS, NIH stroke scale. We compared each sub-items of FIM, CNS, NIH stroke scale about mental state, recognition, perception, motors, language, urinary&bowel incontinence with MBI score at 4 weeks from admission. Also, we analyzed the correlations of sub-items and groups which devided into 5 according to independence of MBI score. And we found out the most influent factors with multiple regression analysis. The major results were as follows; 1. In mean of MBI score at 4 weeks of each groups devided low, middle, high score at mental state, recognition, perception, motors, language, urinary&bowel incontinence items, there were statistical differences in all items. 2. The mental state and lim ataxia sub-items had no significant correlations with groups divided according to independence of MBI score. All the other items were significantly correlated. 3. The most influent factors was recognition. The second was sensory and the third was bowel incontinence. 4. The most influent scales was FIM, and the second was CNS, and NlH had no statistical significancy.

A Case Report : Hypersexuality and Obsessive-Compulsive Behaviors in a Stroke Patient with the Left Mesial Frontal Cortex and Both Basal Ganglia Lesion (좌측 정중 전두엽과 양측 기저절 경색 후 과잉성욕 증상과 강박 증상을 보인 환자 1례)

  • Hyposexuality after stroke has been frequently observed, but hypersexuality as a sequela of stroke has not been commonly documented. We report a patient who exhibited hypersexuality and obsessive-compulsive behaviors after stroke in the region of the left mesial frontal cortex and both basal ganglia. At 2 months after stroke, he visited psychiatric unit due to these symptoms. His motor function was almost full recovered. He was treated with fluvoxamine and perphenazine. With two-month medication, his hypersexuality and obsessive-compulsive behavior disappeared. This case may indicate that basal ganglia-thalamocortical circuit plays an important role in the mediation of sexual behavior and obsessive-compulsive behavior. Since changes in sexual activity may not be spontaneously reported, a systemic inquiry into patient's sexual functioning after infarction in frontal lobe or basal ganglia is warranted.
