Factors Associated with Ruptured Intracranial Aneurysm in a Hospital (일개 병원의 뇌동맥류 파열의 위험요인)

  • This study was conducted to identify the factors associated with the ruptured intracranial aneurysm (RIA) among demographic, admission to hospital, clinical, aneurysym itself and lifestyle characteristics. Medical records of RIA patients and un-RIA patients which had been admitted to undergo treatment including surgery (From January to December 2016) were included into data analysis. Multiple logistic regression showed that two and more than warning signs (14.14 (CI: 1.25-159.40)) indicated the greatest odds ratio with RIA, was followed by headache more than 3 scores (13.95 (CI: 3.68-52.83)), the admission via emergency room (13.62 (CI: 4.85-38.26)), single marital status (9.72 (CI: 2.22-42.49)), 1 mmHg increased systolic blood pressure (1.04 (CI: 1.01-1.08)), 1 score increased GCS (0.58 (CI: 0.37-0.90)), arrythmia finding in electrocardiogram (3.70 (CI: 1.22-11.22)) and increased age (0.95 (CI: 0.91-0.99)). The risk groups having factors associated with RIA were identified. Preventive activities including routine assessment should be done before developing the rupture and urgent care should be needed after developing the rupture for risk groups.

The Comparison of Usefulness between MDCT Angiography and DSA in the Diagnosis of Intracranial Aneurysm (뇌 동맥류 진단에 있어 디지털 감산 혈관조영술과 다중검출기 전산화단층촬영 혈관조영술의 유용성 비교)

  • The purpose of this study was to compare the image between DSA and MDCT Angiography and to examine whether MDCT Angiography could be useful as a screening test for the diagnosis of cerebral aneurysm in patients who were diagnosed with cerebral aneurysm on DSA. Of patients who were diagnosed with cerebral aneurysm DSA at University Hospital, 194 patients who concomitantly underwent MDCT Angiography were enrolled in the current retrospective study. The methods for analyzing cerebral aneurysm were to analyze the presence of cerebral aneurysm on DSA and MDCT Angiography. In cases in which it exceeded 1, the corresponding cases were classified as narrow-neck aneurysms. In otherwise cases, they were classified as wide-neck aneurysms. Thus, a comparative analysis could be performed to ascertain if cases were narrow-neck or wide-neck aneurysms. As compared with DSA, the sensitivity of MDCT Angiography for cerebral aneurysm was measured to be 97.4%. The degree of consistency between narrow-neck and wide-neck aneurysms was 90.2% and the proportion of undetectable an at MDCT Angiography was 2.54%. mean size was 2.4 mm. It is expected that a non-invasive diagnostic modality for a screening test for cerebral aneurysm, MDCT Angiography might be a very useful regimen as compared with an invasive one, DSA.

Characteristic of Coils According to Volume in Case of Embolization of Cerebral Aneurysm (뇌동맥류에 대한 색전술 시 체적에 따른 코일 특성)

  • This study presents the assessment results of coil length characteristics according to the volume of cerebral artery among the inpatients that received angiography and coil embolization following intracranial aneurysm from March, 2010 to September 2011. The volume rendering method was applied to the patients that received embolization to their cerebral arteries to obtain volume measurements. After coil embolization, the volume ratios were calculated with the volumes and lengths of coils. The embolic volume ratios were $43.11{\pm}3.11%$, $36.07{\pm}2.03%$, 40.91%, and 38.25% when the aneurysm sizes were 6mm or less, 6~10mm, 10~15mm and 20mm or more, respectively, being similar to the recommended volume ratios. Regardless of the types of aneurysm, the coil length according to volume was 0.65cm per $1mm^3$ of 20~$100mm^3$ when one type of 0.25mm diameter coil was used. They were 0.62cm per $1mm^3$ of 20~$150mm^3$ when one type of coil was used in the aneurysm volume of the saccular type and 0.60cm per $1mm^3$ of 20~$90mm^3$ when one type of coil was used in the aneurysm volume of the multi-lobulated type.

The Effects of Blood Flow on the Formation of Cerebral Aneurysms (혈류 유동이 뇌동맥류 형성에 미치는 영향)

  • 뇌동맥류는 뇌혈관의 일부가 풍선처럼 부풀어나는 혈관계 질환이며 뇌동맥류의 파열은 사망이나 심각한 후유 장애를 야기한다. 뇌동맥류의 다양한 발생 원인 중 혈관 내부의 혈류의 유동이 중요한 인자로 의심된다. 뇌동맥류의 형성에 미치는 혈류역학적 인자를 규명하기 위해 내경동맥에서 발생한 환자의 내경 동맥류 CT 사진을 이용하여 내경동맥류 모델을 제작하고, 모델 내부의 혈류유동장을 입자영상속도계를 이용하여 측정하였다. 동맥류가 발생한 내경동맥류 모델에서는 동맥류 원위부 목(distal neck)쪽과 반대쪽 내경동맥 벽에서 전단응력이 높게 나타났다. 동맥류 발생에 미치는 혈류역학적 인자를 규명하기 위해 동맥류를 제거한 내경모델을 제작하여 맥동유동에서 내부 유동장을 측정하였다. 심실수축기 동안 휘어진 내경동맥의 바깥쪽 벽에서 혈류의 혈관벽 부딪힘이 관찰되었으며 심실이완기 초반에도 이는 계속 유지되었다. 내경 동맥 내부의 부차적 유동특성을 연구하기 위해 동맥류 발생 위치에서 혈관 축과 수직인 평면의 유동장이 측정되었다. 혈관 단면에서는 휘어진 혈관의 바깥쪽에서 안쪽으로 시계방향의 와류가 형성되었으며, 이로 인해 혈관벽 바깥쪽과 시계방향으로 90도 정도 지역에서 전단응력이 높게 나타났다. 혈류 유동 특성과 동맥류 발생위치를 비교해 보면, 혈류의 혈관벽 부딪힘이 관찰되는 위치와 부차적 유동에 의해 전단응력이 크게 나타난 지역은 동맥류의 발생위치와 일치하였다. 따라서 혈류의 혈관벽 부딪힘과 부차적 유동에 의한 전단력이 동맥류 발생의 혈류역학적 요인으로 의심된다.

Surgical Experience of Paraclinoidal Aneurysms (상상돌기 주위 동맥류의 수술적 치험)

  • Objective : Paraclinoidal aneurysms termed that aneurysms arising from proximal internal carotid artery(ICA) between the site of emergence of the carotid artery from roof of the cavernous sinus and origin of the posterior communicating artery(PCoA). These aneurysms pose conceptual and technical surgical problems with regard to acquisition of proximal control and safe intracranial exposure. The efficiency of surgical technique according to the location of paraclinoidal aneurysm was studied for minimal exposure. Materials and Methods : Over the past four years, the authors treated surgically 171 cases of cerebral aneurysm, among them ten patients were paraclinoidal aneurysms with two patients unruptured aneurysms. Mean age was 47 years old, and all patients were female. Three patients were proximal posterior carotid artery wall aneurysms(one large, one giant), four patients carotid-ophthalmic artery aneurysms and three patients superior hypophyseal artery aneurysms. Results : There could be done clip in all cases, there were no deaths and no complication. And no patient developed sustained neurological deficits including visual function except hydrocephalus in one case. Four patients complained of visual disturbance but two patients had recovery after postoperation and two patient were not longer to bad. Conclusion : Our recent experience suggests that preoperative scrutiny of diagnostic angiography allows classification of all paraclinoidal aneurysms regardless of size and surgical technique which this classification has focused on operative approaches unique to each aneurysm projection was helpful to improve the operative outcome with good visual function and to shorten the operative time.

Ruptured Posterior Communicating Artery Aneurysm Causing Bilateral Abducens Nerve Paralyses - Case Report - (양측성 외전신경 마비를 보인 파열성 후교통동맥 동맥류 - 증례보고 -)

  • Arare case of bilateral abducens nerve paralyses after rupture of a left posterior communicating artery(PcomA) aneurysm with multiple unruptured aneurysms in a 46-year-old female is presented. Sudden left abducens nerve paralysis followed by progressive right abducens nerve paralysis were present without additional neuroophthalmological signs. Postoperatively, bilateral abducens nerve paralyses gradually recovered and disappeared in 2 weeks. The authors reviewed and discussed the possible mechanisms involved in this uncommon neuro-ophthalmological manifestation.

Risk of Seizures after Operative Treatment of Ruptured Cerebral Aneurysms (뇌동맥류 파열 환자의 수술 후 경련발작의 위험인자)

  • Objective : Postoperative seizure is a well documented complication of aneurysm surgery. The purpose of the present study was to analyze risk factors for postoperative seizure. Methods : Between January 1990 and December 1996, we performed craniotomy for ruptured cerebral aneurysms in 321 patients. Among them 206 patients who could be followed up for more than 1 year(range, 1 to 4.6 years) were enrolled to present study. All patients were treated with anticonvulsants for 3 to 18 months postoperatively. We analyze the incidence of postoperative seizure in different sex and age groups, and risk factors associated with postoperative seizures following aneurysm rupture. For statistical processing chi-square test and Fisher's exact test were used. Results : In the follow-up period of 1 to 4.6 years(mean, 1.8 years) postoperative seizure appeared in 18 out of 206 patients(8.7%). Mean latency between the operation and the first seizure was 6 months(range, 3 weeks to 18 months). The age of the patients has significant influence on the risk of seizure, it occurred more often in younger patients(p =0.0014). Aneurysm location in the MCA was associated with a significantly a higher risk of seizure(p = 0.042). Eight patients(19%) out of 42 patients who suffered delayed ischemic neurologic deficit(DID) developed seizure. Delayed ischemic neurologic deficit was associated with significantly a higher risk of seizure(p =0.019). Infarct and hypertension were associated with significantly a higher risk of seizure(p <0.05). pre- or postoperative intracranial hematoma(intracerebral or epidural hematoma) was associated with significantly a higher risk of seizure(p <0.0001). H-H grade, Fisher grade, Glasgow Outcome Scale of patients and timing of operation after subarachnoid hemorrhage had no significant relation with the risk of seizure. Conclusion : Factors associated with the development of postoperative seizure were middle cerebral artery aneurysm, delayed ischemic neurologic deficit, infarct on late postoperative CT scan, hypertension, pre or postoperative intracranial hematoma(intracerebral or epidural hematoma). Identification of the risk factors may be help to focus the antiepileptic drug threapy in cases prone to develop seizures. Prospective evaluation is indicated.

The Analysis of Cerebral Aneurysm's Prone position Using Computed Tomography Equipment (전산화단층촬영장치를 이용한 뇌동맥류의 호발부위 분석)

  • This research studied 249 patients who has an aneurysm and went through computerized CT angiography at S University Hospital in Northern Kyung-sang area from October 1st 2008 to September 30th 2010, and the results are the following. Among total 249 of research objects, 159 women (63.9%) showed higher distribution than 90 men (36.1%) The order of the distribution of aneurysm classified by blood vessel is the following. Posterior communicating artery 34.9% as the highest, internal carotid artery 21.7%, middle cerebral artery of 15.7%, anterior communicating artery 14.5%, posterior cerebral artery and posterior cerebral artery channel 3.6% each, anterior cerebral artery and vertebral artery 2.4% each, and posterior inferior cerebellar artery 1.2%. As a result of analysis of distribution of aneurysm classified by gender, there was no case of posterior cerebral artery and posterior cerebral artery channel among men, and there was no case of posterior inferior cerebellar artery among women. Distribution of aneurysm classified by vessel according to gender showed no statistic significance. (p<0.05). And distribution of aneurysm classified by vessel according to the age, showed no statistic significance. (p>0.05), After applying post analysis to understand the group with age difference, the significance was the highest among the group of 61-70, (4.21), and the group of the age under 30 was the lowest. (2.0) There was statistic significance on the distribution of aneurysm classified by vessel according to the season. (p<0.05). After applying post-analysis in order to understand the groups with difference between seasons, it was found that fall was the highest (4.55) and spring was the lowest. (2.50)
