Factors Affecting Unmet Medical Care in Elderly Hypertensive Patients

  • The purpose of this study is to investigate factors affecting unmet medical care in elderly hypertensive patients. The purpose of this study was to understand the reasons for unmet medical care in elderly hypertensive patients over 65 years of age and to identify the characteristics of each type to improve the medical use system and to contribute to policy making so that unmet medical care does not occur. Using data from the 7th period of the National Health and Nutrition Examination Survey (2016, 2017), 23 sample households were selected using the phylogenetic extraction method among appropriate households and analyzed using the SPSS 18.0 Program. As a result of the study, the unmet medical care of those who had suicidal thoughts compared to those who had not had suicidal thoughts in the past year, when females were higher than males, had worse subjective health status, those who did not subscribe than those who had private insurance, and those who had smoked in the past year. The experience rate was high. This is considered to be able to contribute to improving the medical service system and making policy decisions so that unmet medical care does not occur in the future.

Mortality Prediction of Older Adults Admitted to the Emergency Department (응급실 방문 노인 환자의 사망률 예측)

  • As the global population becomes aging, the demand for health services for the elderly is expected to increase. In particular, The elderly visiting the emergency department sometimes have complex medical, social, and physical problems, such as having a variety of illnesses or complaints of unusual symptoms. The proposed system is designed to predict the mortality of the elderly patients who are over 65 years old and have admitted the emergency department. For mortality prediction, we compare the support vector machines and Feed Forward Neural Network (FFNN) trained with medical data such as age, sex, blood pressure, body temperature, etc. The results of the FFNN with a hidden layer are best in the mortality prediction, and F1 score and the AUC is 52.0%, 88.6% respectively. If we improve the performance of the proposed system by extracting better medical features, we will be able to provide better medical services through an effective and quick allocation of medical resources for the elderly patients visiting the emergency department.

A Study on the Convergence of Spatial Equity of Medical Welfare Facilities for Older Persons and Services (노인의료복지시설과 서비스의 공간적 형평성 융합 연구)

  • The study aims to measure and analysis the spatial equity of Medical welfare facilities for older persons and services, and, based on this, to seek the plan to secure the fairness. To this end, the research was carried out by converging the studies of geography and regional development for the equity of social welfare studies and space arrangement on types and functions of Medical welfare facilities for older persons. The main results of the study showed that, first, in case of the spatial arrangement(desire-to-service), Medical welfare facilities for older persons are located in all areas of cities(Si) and counties(Gun) but mostly existing in cities. Second, in case of the equity of regional distribution of Medical welfare facilities for older persons, it can say the equity in Gun is higher than Si, comparing the regions of Si and Gun. Third, in case of spatial equity of sanatorium for older persons, the spatial equity of care facilities for older persons showed statistical difference depending on the time required to reach the facility, but no difference on distance. This study made various suggestions based on the results of the above research, and suggested the necessity of convergence studies grafting technologies such as AI and the Internet of Things.

The Association of Social Networks and Medical Care Utilization of Elderly Living Alone Depending on Self-rated Health Status: Focusing on the Moderating Effects of Welfare Services for Elderly (주관적 건강수준에 따른 독거노인의 사회적 관계망과 의료기관 이용 간의 관련성: 노인복지서비스 참여의 조절효과를 중심으로)

  • The objective of this study is to examine the association of social networks and medical care utilization of elderly living alone depending on self-rated health status, and also to measure the moderating effects of welfare services for elderly. To this purpose, the data which was collected from the 2014 National Survey on Elderly in Korea was analyzed by Chi-square test and Poisson regression. According to the results, social networks and elderly welfare services affected to medical care utilization of elderly living alone, only who aware of their self-rated health status as bad. With this group, the frequency of contact with family and participation in lifelong education had a significant effect on increasing medical care utilization. In contrast, the frequency of contact with neighbors and the participation in elderly employment program were significant to reduce the utilization of medical care. Furthermore, the moderating effects of elderly welfare services were also measured that formal social gathering has an effect of increasing the medical care utilization of elderly living alone by expansion of their social networks. Based on these results, this study consequently has an implication for suggesting expansion of elderly welfare services, which might be an alternative to social networks of elderly living alone, and also for seeking the direction of political intervention that helps health promotion and disease prevention through the research of medical care utilization.

  • Health service quality is increasingly emphasized, and primary care service is no exception in this. This study identified patients' perception of quality considering convergence factors associated with increased utilization among patients over the age of 65 using community health centers. A total of 307 respondents were included in the analysis. The results showed that positive perceptions on facility and environment, patient-centered care, technical knowledge and skills of physicians, and emotional support with nurses and non-medical workers were statistically and significantly associated with higher utilization. In order to increase utilization of primary care service at community health centers, they need to manage and improve these factors.

Status of health and nutritional intake of the elderly in long-term care facilities: focus on Gwangju Metropolitan City (노인의료복지시설 입소 노인의 건강 상태 및 영양소 섭취 현황: 광주광역시를 중심으로)

  • Purpose: This study assessed the food intake and nutritional status of the elderly in long-term care facilities in order to provide adequate food services and improve the nutritional status. Methods: The survey was carried out from August 2019 to October 2019 for the elderly in long-term care facilities located in Gwangju Metropolitan City. The survey was conducted to collect data from 199 elderly persons (34 males and 165 females) aged over 65 years old. The food intake was assessed using a 1-day 24-hour recall method. Results: More than 90% of the subjects were over 75 years old. Forty five percent of the subjects were active, 44.2% of the subjects perceived themselves as not being healthy. Dementia and Parkinson's disease were the most common diseases, followed by hypertension, musculo-skeletal disease, diabetes, and stroke. Only 25.6% of the subjects had most of their teeth intact, and 44.7% of the subjects had difficulty in chewing and swallowing. The total food intake was 1,127 g in males and 1,078 g in females. The most frequently consumed foods were kimchi, cooked rice with multi-grains, soybean soup, cooked rice with white rice, yogurt, pumpkin porridge, soy milk, and duck soup. The average energy intake of the subjects was 1,564.9 kcal in males and 1,535.5 kcal in females. The overall nutritional status of the elderly in the long-term care facilities was poor. In particular, the intake of vitamin D and calcium, vitamin C, riboflavin, and potassium were very low. The intake of vitamin D was 5 ㎍, and 86.4% of the elderly were below the estimated average requirement, while the intake of sodium was high. Conclusion: The results of this study can be used to understand the health and nutritional status and to improve the food services and nutrition management for the elderly in longterm care facilities.

  • As the result of a significant improvement in the economic situation and development of scientific techniques in Korea during the last 20 years, the life expectancy of the Korean people has lengthened considerably and as a result the number of old aged people has markedly increased. Such an increase of the number of aged people brought about many problems. Authors would like to take a medical look at the "Problem of old aged people" presently facing us in Korea. Currently the number of people over 65 has increased rapidly and is 1,620,000, 4% of total population. But it is still much lower than 8.9% in Japan, 10.7% in U.S.A., and 14.9% in the United Kingdom. Over 25% of these aged people were found to have at least more than one disease which requires medical care. Diseases occur in the circulatory system, 30.9%, respiratory system, 17.1%, digestive system 8.6%, mental disorders, 8.4%, malignant neoplasms, 7.0%. About 51% of the aged over 65 are under medical security benefit, mostly with partial coverage plan. Their clinic visit rate was very low (2.0% in 1981), which might be due to financial reasons. Since diseases affecting the aged progress chronically, early detection and long term care are utter most important. However there is almost no special facility, long term care center or geriatric specialist. For proper management of medical problems in the growing population of the Korean eldery expansion of medical security coverage, greater number of specialized facilities, education of geriatric special manpower and efficient operating system should be established.

A Study on the Medical Environmental Factor and Distinction of Medical Facilities for the Elderly in Berlin (베를린 노인의료 환경요인과 시설특성에 관한 연구)

  • Nowadays, a proportion of healthcare service for the elderly has been improved due to the factors, such as a prolonged life expectancy, a growth of aging population and a change of member of family. For these factors, it is necessary for the elderly to receive treatment for multidisciplinary diseases, associated with psychological care of sociological concept. It is quite difficult in an acute-care hospital to accommodate the elderly patient because of the fact that an acute-care hospital is required to maintain a high level of medical care and technical standard. That is why specialized medical service is needed for the elderly. In the case of Germany, they are at the stage of the change from large scale facilities to specialized facilities applying the integrated concept. This paper addresses the medical environmental factor and distinction of medical facilities for the elderly through survey and analysis relating to all change in Germany. Therefore, it aims to suggest a fundamental resource for architectural planning and network of medical facilities for the elderly.
