A Report of Two Cases of Mucoepidermoid Carcinoma (폐에 생긴 점액표피양 암종 2예)

  • Mucoepidermoid carcinoma of the lung can be divided into low grade and high grade varieties, like their major salivary glands counterparts. This carcinoma comprises only. about 0.5% of all pulmonary neoplasma. Their clinical and biologic behaviors are closely related with histologic grade of carcinoma. We have experienced two cases of mucoepidermoid carcinoma. Case 1 occurred in a 16 years old male and showed the histologic features of low grade type. This carcinoma arised from the right upper lobe bronchus which was removed by the right upper lobectomy. Case 2 occurred in a 41 year old male was the high grade carcinoma located at the distal left main bronchus which was removed by the left pnemonectomy.

Endobronchial Leiomyoma - A case report - (기관지내 평활근종 -1례 보고-)

  • Endobronchial leiomyoma is extremely rare and accounts for less than 2% of benign tumors of the lower respiratory tract. Leiomyomas are predominantly found in the young and the middle aged : of the average age being 35 years for bronchial and lung parenchymal lesions and 40.6 years for tracheal lesions. The symptom depends on the location of the tumor, its size, and changes in the lung distal to the lesion. A 37-year-old woman was admitted to our hospital complaining of coughing. Bronchoscopy revealed complete obstruction of the right main bonchus at the carina by an oval-shaped, nonulcerative, smooth, and pinkish-tan tumor with a broad margin and extended to the left main bronchus. A biopsy was performed and showed a benign spindle cell tumor. A right pneumonectomy was performed because of chronic infection, and the lung could not expanded during aeration. The histological diagnosis of the resected specimen was leiomyoma. The postoperative course was uneventful.

Primary Endobronchial Actinomycosis (원발성 기관지 방선균증)

  • We report a case of a 20-year-old woman who presented with fever, dry cough and pulmonary consolidation at the left upper lobe on chest radiograph. Fiberoptic bronchoscopy revealed obstruction of the left upper lobar bronchus with exophytic mass and multiple nodular protruding lesions at the left main bronchus. Endobronchial actinomycosis was confirmed by demonstration of sulfur granule through the bronchoscopic biopsy of nodular lesion. Intravenous administration of penicillin G followed by oral tetracycline therapy for 5 months resulted in complete recovery of symptoms which had been present for 3 months prior to therapy. Infiltrative consolidation on the chest X-ray disappeared and all the lesions shown by bronchoscopy were nearly normalized after 6 months only to remain small nodular remnants at the left main bronchus. Endobronchial actinomycosis should be included in the differential diagnosis of endobronchial mass.

A Study on the Diagnostic Usefulness of Ultrasound and Magnetic Resonance Imaging for the Diagnosis of Shoulder Rotator Cuff Tear (어깨 회전근개 파열 진단을 위한 초음파 검사와 자기공명영상 검사의 진단적 유용성 연구)

  • Rotator cuff tears are a leading cause of shoulder pain in adults. Due to the increase in social activities, the number of patients complaining of shoulder pain is increasing, and interest in shoulder diseases is also increasing. With the development of ultrasound equipment, the sensitivity and specificity of diagnosis are high, and it is used to diagnose rotator cuff tears in musculoskeletal disease. Ultrasound is recognized as a complementary method to MRI examination in rotator cuff tears. Therefore, this study aimed to find out the diagnostic usefulness of ultrasound and MRI examinations in the diagnosis of shoulder rotator cuff tears.A retrospective analysis was performed on 262 patients who were diagnosed with final rotator cuff damage by arthroscopy after completing ultrasound and MRI examinations. Sensitivity, feature, positive predictive value, image predictive value, and touch were disassembled for the test results. In addition, the degree of clavicular tear was scored and recorded in 5 stages. Ultrasound examination was similar to MRI examination results for both full-thickness and partial tears, and there was no statistically significant difference. Partial tear test results showed higher positive predictive value and accuracy than MRI test. In conclusion, ultrasound can be fully utilized as a screening test for rotator cuff disease, and it is thought that it will be selected and used clinically according to the patient's constitution and situation.

Subjective and Functional Results after a Proximal Gastrectomy: Prospective Study for Comparison of Reconstruction Procedures (근위부 위절제술 후 재건 술식에 따른 경과 관찰)

  • Purpose: A proximal gastrectomy is performed for gastric cancer in the upper part of the stomach to preserve the function of the stomach after surgery. An esophagogastrostomy is one of the common reconstruction methods for a proximal gastrectomy, but this method results in a high incidence of reflux esophagitis. This study was undertaken to compare subjective and functional results between esophagogastrostomy and jejunal interposition reconstructions. Materials and Methods: From June 1998 to December 2002, proximal gastrectomies were performed in 33 patients with tumors in the upper third of the stomach; 8 had reconstruction using jejunal interposition between the esophagus and the remnant stomach (JI group) while 25 had reconstruction using esophagogastrostomy (EG group). The postroperative courses of the patients were reviewed in terms of symptoms, weight changes, and endoscopic findings. Results: The mean age of the patients was 59 years; 26 were men and 7 were women. There were no significant differences in general complications, operating times, or histologic features between the two groups. Fifty-two percent (52%) of the EG group complained of dysphagia, and 16% them experienced heartburn and acid belching. Twelve percent (12%) of the JI group complained of dysphagia, but heartburn and acid belching did not occur. Incidences of reflux esphagitis (36%) and balloon dilatation for anastomotic stricture (16%) were more common in the EG group than in the JI group (0% and 12%). Conclusion: To prevent or minimize complications, such as reflux esophagitis and postoperative symptoms, a proximal gastrectomy with a jejunal interposition is an alternative method as an organ-preserving surgical strategy to improve quality of life for patients. Thoracicl Actinomycosis Associated with Broncholithiasis -Report on 2 cases- (기관지 결석증을 동반한 흉부 방선균증 -2예 보고-)

  • We report here on two eases of a 48-year old woman and a 46-year-old man who both presented with broneholithiasis and obstructive pneumonitis. Removal of the broncholithiasis failed with bronchofibroscopy, and so right middle lobectomy of the lung were done in the 2 patients. The histopathologic diagnosis was thoracic actinomycosis associated with broncholithiasis. Thoracic actinomycosis associated with broncholithiasis is a very rare condition, so we report here on these two cases of thoracic actinomycosis associated with broncholithiasis.

Persistent Hymen and Pyocolpos in a Female Shih-Tzu Dog (시츄 개에서 발생한 처녀막 잔존증과 질축농증)

  • A 9-year-old intact female Shih-Tzu was presented with abdominal pain and abdominal distension. Since the animal's birth, the owner had never observed proestrous bleeding in the patient. Ultrasonography revealed segmental enlargement of the genital system. An ovariohysterovaginectomy was performed to remove the genital mass. The vagina was grossly dilated as a result of being filled with a solid black-green substance. A vaginogram was performed. Based on the surgical findings and vaginogram results, an imperforate hymen was diagnosed. One month from the first surgery, a stump pyocolpos developed between the sutured site and the obstructed portion. The persistent hymen was removed by endoscopy. However, peritonitis and sepsis developed after the procedure, and the dog died. Therefore, During operation of persistent hymen, a cruciate incision over the hymen strongly recommended to avoid additional post operative complications.

  • Plastic bronchitis is a rare disorder characterized by the formation of extensive, obstructing endobronchial casts. It is associated with asthma and complex cardiac defects such as those requiring the Fontan procedure. The treatment of plastic bronchitis comprises conventional therapy involving spontaneous expectoration and bronchoscopic removal and specific therapy with several new drugs. Herein, we describe the cases of 2 patients diagnosed with plastic bronchitis accompanied with a different underlying disease, which were treated with inhaled corticosteroid and low-dose oral clarithromycin.

Laceration of Left Main Bronchus and Azygos Vein Following Stab Wound - 1 case report - (자상에 의한 기정맥 및 좌측 주 기관지 열상 - 치험 1례 -)

  • Tracheobronchial injuries are uncommon. Except for the cervical region, most tracheobronchial injuries are due to blunt chest trauma in Korea. The depth of the tracheobronchial trees renders these structures relatively safe from stab wound. We experienced a case of left main bronchial laceration with azygos vein tear following stab wound in the back of right chest firstly in Korea. The patient was a 24 years old male. A routine chest radiography showed a knife in chest at emergency room. We didn't remove the knife at emergency room. This patient was carried to operation room in 30 minutes after arrival of our hospital without computed tomography and bronchoscopy. The operation was performed through standard right posterolateral thoracotomy and then the knife was removed. The left main bronchus and azyos vein were lacerated obliquely. The penetrated azygos vein was ligated and the laceration of the left main bronchus was repaired. Postoperative course was uneventful.

A Case of Chest Traumatic Bronchial Rupture, Manifested by Bronchial Web in Bronchoscopy (Web 양상으로 발현한 외상성 기관지 파열 1예)

  • The incidence of traumatic rupture of the tracheobronchial tree has been increased considerably with advent of widespread mechanization and high speed era. Rupture of the bronchus is an unusual result of nonpenetrating trauma to chest. Early diagnosis and primary repair not only restore normal lung function but also avoid difficulties and complications associated with delayed diagnosis and repair. These complications are pneumonia, atelectasis and lung abscess secondary to the bronchial obtruction. We experienced a case of partial rupture on left main bronchus caused by nonpenetrating blunt chest trauma with rib fractures 1 year ago. He was suffered from progressively developing dyspnea on exercise and treated as bronchial asthma at other hospital. Bronchoscopic finding was the narrowed lumen of left main bronchus at 1cm from carina by web-like membrane. We confirmed by bronchogram and repaired by end to end anastomosis, which is rare delayed finding in bronchial rupture without pulmonary complications. We report a case of nonpenetrating traumatic bronchial rupture, manifested by bronchial web in bronchoscopy.
