The Prevalence and Factors of Falls among the Community-Dwelling Elderly (재가 노인의 낙상 경험률과 관련 요인)

  • Purpose: The purpose of this study was to investigate the prevalence, characteristics, and factors related to falls occurring among the community-dwelling elderly. Methods: The subjects were 299 community-dwelling-elderly in Jeju Province. Data were collected via in-person interviews by five visiting nurses in October, 2012. Fall risk assessment tools developed by the CDC, K-ADL, HDS-K, and GDS were used for data collection. Results: A significant number of subjects (34.1%) had experienced a fall at least once within the last six months. Living arrangements, strokes, urinary incontinence, main daily activities, and depression were significant factors related to these falls. Other risk factors included heavy use of medications, difficulties in walking and standing, needing arms to stand up from a chair, and hearing loss. Conclusion: The results reveal critical factors related to falls among the community-dwelling elderly. These information should be used to develop and implement fall prevention programs in communities.

A Study on the Relationship between Body Function and Prelusive Movement to Falls to Promote Wellness in Chronic Stroke Patients (만성뇌졸중 환자의 웰니스 증진을 위한 신체기능과 낙성전조동작의 관련성 연구)

  • This study was conducted to investigate the effects of a participatory rehabilitation program on sit-rise and rise-to-walk test performances, and perception and motor skills in adults with medically vulnerable individuals and, adults with developmental disabilities in particular. Seventeen adults with developmental disabilities participated in a participatory rehabilitation program using resistance bands and exercise balls, for 60 minutes once weekly over 13 weeks. Their performances were measured before and immediately after the intervention, and 12 weeks after. The findings were as follows. In the sit-rise test, the number of times rising from sitting posture increased after the intervention versus before, but the difference was not statistically significant. In the rise-to-walk test, the performance showed statistically significant difference over time, and the post-hoc test showed a significant effect after the intervention versus before. There was no significant difference in perception and motor skills. In sum, the participatory rehabilitation program positively influenced dynamic balancing related to functional activities but had no significant effect on perception and motor skills, which is related to motor control and motor learning. It is suggested that to increase the participation in community activities, reduce fall risk, and improve dynamic balancing abilities in adults with developmental disabilities, participatory rehabilitation programs should be utilized to promote the physical wellbeing.

  • 본 연구는 방문건강관리사업 대상자 중 취약한 노인여성과 노인증후군 관련요인에 대한 인과관계 모형을 규명하여 취약한 노인 여성의 보건의료서비스 제공 시 효율적인 방안제시를 위한 기초자료로 활용하고자 시행하였다. 연구대상자는 2008년 7월 1일부터 9월 30일까지의 기간 동안 전국 보건소에 등록된 방문건강관리사업 대상자 중 동의된 65세 이상 노인여성 1,350명을 선정하여 훈련된 조사원이 직접면담을 통해 조사하였다. 수집된 자료 중 불충분한 29명을 제외한 1,321명을 최종 분석대상으로 하였다. 수집된 자료는 SPSS 17.0 프로그램을 이용하여 빈도분석 및 카이제곱 검정, t-검정, ANOVA, 사후검정은 Tukey를 이용하였으며, 유의한 변수들을 선정하여 로지스틱 회귀분석을 이용하여 분석 하였다. 연구결과를 요약하면 다음과 같다. 첫째, 허약과 관련 있는 노인증후군위험요인은 우울, 요실금, 낙상력, 일상적 수행활동(ADL)으로 나타났다(p<0.05). 둘째, 허약에 영향을 미치는 요인으로는 연령이 75세 이상의 고령일수록, 우울과 요실금의 증상이 있을 때, 최근 낙상경험이 있을 때 허약의 증가 위험이 높은 것으로 나타났다(p<0.05). 셋째, 노인증후군의 위험요인 증가에 영향을 미치는 요인으로는 평생 동안 담배를 5갑 이상 피웠을 때, 당뇨병이 있을 때로 나타났다(p<0.05). 이상 결과를 볼 때, 취약 노인 대상으로 노인증후군, 허약의 향상을 위해서는 만성질환관리 프로그램 뿐 만 아니라 체계적인 근력강화 운동 및 정신건강관리 프로그램의 충분한 확대보급이 반드시 필요하다.

  • The aim of this study was to investigate the effect of combine exercise on the fall injury related index and myokine in elderly women. Sarcopenia leads to a loss of strength, alter on to a decreased functional status, impaired mobility, a higher risk of falls, and eventually an increased risk of mortality. Aerobic exercise characterized by rhythmic and repetitive movements of large muscles, for sustained periods that depends primarily on the use of oxygen to meet energy demands through aerobic metabolism, and that is structured and intended to generate improvements in cardiopulmonary fitness, body composition, and cardiorespiratory health. Resistance training has performance in the elderly. As combined exercise therapy can be used to enhance muscle functions and cardiopulmonary functions, it is being highlighted as an effective health management methods for the aged. The myokine has been regarded an important factor of exercise how muscle communicate adipose tissue, bone and muscle to exert beneficial effects at the whole body level.

Physical Restraints Use and Associated Factors Among Elderly Patients in Long-term Care Hospitals (요양병원 재원 노인의 신체적 억제대 사용과 관련 요인)

  • The propose od this study was to examine the extent of using physical restraint on elderly patients (over 65 years old) and who were patients in long-term care hospitals. The data was collected, from March 3 2018 to March 29 2018, from the electronic nursing records by using a recording tool, and clinical observation was also used for assessing the use of physical restraint and the related factors. Descriptive statistics, Pearson correlation coefficients and logistic regression were then performed. The usage rate for physical restraints in long-term care hospital was 83.7%, and the most common type of physical restraint was side rails. The use of physical restraints showed a positive correlation with the fall risk scores and a negative correlation with the MMSE. Logistic regression analysis showed that the Fall Risk Score (OR=1.02, 95% CI=1.01~1.03), MMSE (OR=0.94, CI=0.88~0.99) and the use of medical devices (OR=0.80, 95% CI=0.65~0.98) were related with using physical restraints. Therefore, it was confirmed that physical restraint was used in cases of a high risk of falling, severe cognitive impairment and the use of complex and fragile medical devices to treat the patient. Clinical nursing practice should be changed so that treatment alternatives can be applied for elderly patients rather than using too many physical restraints.

Influence of the Silver Line Dance Applied to the elderly on the Balance during Walking (라인댄스 운동이 여성노인들의 보행 균형성 요인에 미치는 영향)

  • The purpose of this study was to determine the effects of line dance programme on the balance ability during walking to reveal the exercise intervention for fall prevention. A 12-week line dance programme was applied to 18 elderly females who aged more than 65 years in the community. Balance ability during walking was evaluated by the range of center of pressure(cop), the velocity of cop, and free torque that calculated on the basis of ground reaction force data. The range and velocity of cop in the anterio-posterior were significantly reduced after performing(p<.01, p<.05, respectively), but change in those of cop in the medio-lateral and free torque were not found. It was demonstrates that 12-week line dance programme allows more effective in anterio-posterior stability of walking. It was suggested that the effect of fall prevention exercise should be studied more associate with fall frequency as future study.

A Study on the Elderly Patients Hospitalized by the Fracture from the Fall (낙상 사고에 의한 골절로 입원한 노인 환자에 대한 조사 연구)

  • Purpose: To identify age, gender, medication, seasons and place of fall, and areas of the fractures from the fall among the hospitalized elderly patients in order to provide the basic data for future fall prevention program for the elderly. Methods: This study was conducted for 106 elderly patients admitted into a university hospital by fractures from the fall during the period from January 1, 1999 to December 31, 1999. Data on the age, gender, medication, season and place of the fall, areas of the fracture were collected based on their medical records. Result: The age range of the subjects were from 60 to 96 years old. The subjects were aged between 60-69 years old 49(46.2%), between 70-79 years old 31(29.2%), between 80-89 years old 24(22.6%), and over 90 years old 2(1.9%). Male patients comprised was 34(28.3%), while female patients comprised 76(71.7%). The fall occurred in Winter most frequently 34(32%). The place of the fall included room 81(76.4%), streets 13(12.3%), bathroom 6(5.7%), stair 4(3.8%), and mountain 2(1.9%). Twenty-two subjects (20.8%) had medication regularly, while 84 subjects (79.2%) had no medication. The areas of the fracture from the fall included upper extremities 20(18.9%) and lower extremities 86(81.1%). Radius fracture (7.5%) was the area where the fracture occurred most frequently in upper extremities and femur fracture (52.8%) was the area where the fracture occurred most frequently in lower extremities. A significant difference was found in the fracture area by age, season and place of the fall (p<.05). No significant difference was found in the fracture area by gender and medication. In all age groups, seasons and places of the fall, occurrence of fracture in lower extremity was significantly higher than that in upper extremity.

Factors Associated with Nurses' Activities for Hospital Fall Prevention (간호사의 병원낙상 예방활동에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to identify the factors affected with nurses' prevention activity against hospital fall. Methods: The data were collected from 325 nurses at C University Hospital in G City by using the structured questionnaires from February 21, 2011 to March 12, 2011. The data were analyzed by stepwise multiple regression. Results: The main factor associated with prevention activity against hospital fall was the attitude towards hospital fall (${\beta}$=.26, p<.001), the next one was the educational level (${\beta}$=.16, p=.002), and the last one was the frequency of fall prevention education (${\beta}$=.14, p=.009). The all factors could explain 11.1% of the variance in the nurses' prevention activities against hospital fall. Conclusion: Hospital managers need to make hospital culture to enhance the nurse's positive attitude about hospital fall prevention. In addition, educators need to develop educational programs including hospital fall prevention through academic curriculum and continuing education.

  • The purpose of this study was to evaluate the effects of line dance activity on the balance factors during static standing to reveal the exercise intervention for fall prevention. A 15-week line dance programme was applied to 16 elderly females who aged more than 65 years in the community. Balance ability during static standing was evaluated by the range of center of pressure(cop), the velocity of cop, and sway area that calculated on the basis of ground reaction force data, forces, and moments. The range and velocity of cop in the anterio-posterior were significantly reduced after line dance(p<.01, p<.05, respectively), but change in those of cop in the medio-lateral was not found. It was demonstrated that 16-week line dance activities allow more effective in anterio-posterior stability and sway area of static standing. It was suggested that the effect of fall prevention exercise should be studied more associate with fall from vestibular and sensory system as future study.

Nutrition and Psychosocial Factors were associated with Possible Sarcopenia in the Rural Elderly (농촌지역 주민의 영양결핍 및 사회심리적 요인과 근감소증 가능군과의 관련성)

  • Objective: This study aimed to provide basic data for preventing and managing sarcopenia by identifying the relationship between sarcopenia, malnutrition, and psychosocial factors among the elderly in the community. Methods: The study included 1,019 subjects aged 60 and over. "Possible sarcopenia" was defined by low handgrip strength with or without reduced physical performance. Nutrition was evaluated according to the mini nutrition assessment (MNA), and the psychosocial factors examined were self-efficacy, social isolation, fear of falling, and social capital (trust and participation). A logistic regression analysis was also performed on the relationship between risk of malnutrition, psychosocial factors, and sarcopenia. Results: MNA was significantly associated with social participation (OR = 1.747, p <0.001), fear of falling (OR = 2.905, p <0.001), and self-efficacy (high/low, OR = 0.654, p = 0.011). In model 3, which included both MNA and psychosocial factors, sarcopenia was significantly associated with MNA (OR = 2.529, p <0.001) and fear of falling (OR = 1.544, p = 0.045). Compared with the low self-efficacy group, the high group (OR = 0.589, p = 0.009) was significant. The factors related to possible sarcopenia include risk of malnutrition, fear of falls, and low self-efficacy. Conclusion: It will be necessary to improve self-efficacy so that individuals feel they can do activities of daily living themselves and to reduce their fear of falling through muscle strength and balance exercises. Finally, it is also necessary to increase regular participation in community social activities.
