Recently, functional MRI has been used to investigate the neurobiological mechanisms of acupuncture and the specificity of acupoint. The group data tend to be regarded as more important than the individual data in the most of previous studies. This study was designed to investigate the effect of the variability of individual data on the group results. A functional MRI (fMRI) of the whole brain was performed in fifteen healthy subjects during placebo and acupuncture stimulations at the ST36 acupoint. After remaining at rest for 30 seconds, the acupuncture was inserted and twisted at the rate of 2 Hz for 45 seconds and then the acupuncture was removed immediately. This process was repeated three times. Individual and group analyses were performed by voxel-based analyses using SPM2 software. Visual inspections of the activation and deactivation maps from individual sessions have shown the large variability across fifteen subjects. This means that the group data reflected the brain activation responses of only a few subjects. We suggest that the individual data should be presented to demonstrate the effect of acupuncture.
Individuals with problematic hypersexual behavior (PHB) evince the inability to control sexual impulses and arousal. Previous studies have identified that these characteristics are related to structural and functional changes in the brain region responsible for inhibitory functions. However, very little research has been conducted on the functional connectivity of these brain areas during the resting state in individuals with PHB. Therefore, this study used functional magnetic resonance imaging devices with the intention of identifying the deficit of the functional connectivity in the executive control network in individuals with PHB during the resting state. Magnetic resonance imaging data were obtained for 16 individuals with PHB and 19 normal controls with similar demographic characteristics. The areas related to the executive control network (LECN, RECN) were selected as the region of interest, and the correlation coefficient with time series signals between these areas was measured to identify the functional connectivity. Between groups analysis was also used. The results revealed a significant difference in the strength of the functional connectivity of the executive control network between the two groups. In other words, decreased functional connectivity was found between the superior/middle frontal gyrus and the caudate, and between the superior/middle frontal gyrus and the superior parietal gyrus/angular gyrus in individuals with PHB. In addition, these functional Connectivities related to the severity of hypersexual behavior. The findings of this study suggest that the inability to control sexual impulses and arousal in individuals with PHB might be related to the reduced functional connectivity of executive control circuits.
Purpose : There is debate concerning the observation of metabolite changes on MRS at the designated cortex during some tasks. The purpose of this study is to assess the change of the lactate content at the motor cortex during hand-grasping tasks with performing real-time fMRI-guided fMRS. Materials and Methods : Seven healthy volunteers (23-28 years old) underwent realtime fMRI during right hand grasping tasks with using a 1.5 T system. After confirming the activating area, single voxel MRS was preformed at 1) the baseline, 2) during the task and 3) after the task on the activating cortex. The three consecutive spectra were compared for observing the changes of the lactate content by the tasks. The Cho/Cr, NAA/Cr and Lac/Cr ratios were calculated manually from those spectra. Results : MRS during the tasks revealed the lactate peaks at the 1.33 ppm resonance frequency with great conspicuity at the activated area, which was identified on the real-time fMRI. After the task scan, the lactate peaks completely disappeared and the spectra recovered to the values of the baseline scan in all volunteers. At baseline, during the task and after the task, the Cho/Cr ratios were 0.81, 0.76 and 0.77, respectively, and the NAA/Cr ratios were 1.68, 1.65 and 1.72, respectively, and the Lac/Cr ratios were 0.28, 0.41 and 0.30, respectively. During the task, Lac was significantly increased by 46%. Conclusion : We observed prominent lactate peaks on MRS during hand-grasping tasks at the activated area, as was shown on the real-time fMRI. We suggest that fMRS can be used as a sensitive tool for observing the metabolite changes of the functioning brain.

Purpose : The purpose of this study was to find the optimum TE value for enhancing $T_2^{*}$ weighting effect and minimizing the SNR degradation and to compare the BOLD effects according to the changes of TE in 1.5T and 3.0T MRI systems. Materials and Methods : Healthy normal volunteers (eight males and two females with 24-38 years old) participated in this study. Each volunteer was asked to perform a simple finger-tapping task (sequential opposition of thumb to each of the other four fingers) with right hand with a mean frequency of about 2Hz. The stimulus was initially off for 3 images and was then alternatively switched on and off for 2 cycles of 6 images. Images were acquired on the 1.5T and 3.0T MRI with the FLASH (fast low angle shot) pulse sequence (TR : 100ms, FA : $20^{\circ}$, FOV : 230mm) that was used with 26, 36, 46, 56, 66, 76ms of TE times in 1.5T and 16, 26, 36, 46, 56, 66ms of TE in 3.0T MRI system. After the completion of scan, MR images were transferred into a PC and processed with a home-made analysis program based on the correlation coefficient method with the threshold value of 0.45. To search for the optimum TE value in fMRI, the difference between the activation and the rest by the susceptibility change for each TE was used in 1.5T and 3.0T respectively. In addition, the functional $T_2^{*}$ map was calculated to quantify susceptibility change. Results : The calculated optimum TE for fMRI was $61.89{\pm}2.68$ at 1.5T and $47.64{\pm}13.34$ at 3.0T. The maximum percentage of signal intensity change due to the susceptibility effect inactivation region was 3.36% at TE 66ms in 1.5T 10.05% at TE 46ms in 3.0T, respectively. The signal intensity change of 3.0T was about 3 times bigger than of 1.5T. The calculated optimum TE value was consistent with TE values which were obtained from the maximum signal change for each TE. Conclusion : In this study, the 3.0T MRI was clearly more sensitive, about three times bigger than the 1.5T in detecting the susceptibility due to the deoxyhemoglobin level change in the functional MR imaging. So the 3.0T fMRI I ore useful than 1.5T.
Purpose : This study was to present the functional brain mapping of both functional magnetic resonance imaging(MRI) and transcranial magnetic stimulation(TMS) in a case of schizencephaly. Materials and methods : A 28-year-old man, who had left hemiplegia and schizencephaly in right cerebral hemisphere, was exacted with both functional MRI and TMS. Motor function of left hand was decreased whereas right hand was within normal limit. For functional MRI, gradient-echo echo planar imaging($TR/TE/{\alpha}$=1.2 sec/90 msec/90) was employed. The paradigm of motor task consisted of repetitive self-paseo hand flexion-extension exercises with 1-2 Hz periods. An image set of 10 slices was repetitively acquired with 15 seconds alternating periods of task performance and rest and total 6 cycles (three ON periods and three OFF periods) were performed. In brain mapping, TMS was performed with the round magnetic stimulator (mean diameter; 90mm). The magnetic stimulation was done with 80% of maximal output. The latency and amplitude of motor evoked potential(MEP)s were obtained from both abductor pollicis brevis(APB) muscles. Results : Functional MRI revealed activation of the left primary motor cortex with flexion-extension exercises of healthy right hand. On the other hand, the left primary motor cortex, left supplementary motor cortex, and left promoter areas were activated with flexion-extension exercises of left hand. In TMS, magnetic evoked potentials were induced in no areas of right cerebral hemisphere, but in 5 areas of left corebral hemisphere from both abductor pollicis brevis. Latency, amplitude, and contour of response of the magnetic evoked potentials in both hands were similar. Conclusion : Functional MRI and TMS in a patient with schizencephaly were successfully used to localize cortical motor function. Ipsilateral motor pathway is thought to be secondary to reinforcement of the corticospinal tract of the ipsilateral motor cortex.
The current study examined brain regions associated with aesthetic experience to fractal images using functional MRI. The aesthetic estimations of the images showed that there is a general consensus regarding the perception of beautiful images. Out of 270 fractal images, fifty images rated highest(beautiful images) and fifty images rated lowest(non-beautiful images) were selected and presented to the participants. The two conditions were presented using the block design. Frontal lobes, cingulate gyri, and insula, the areas related to the cognitive and emotional processing in aesthetic experience, were activated when beautiful images were presented. In contrast, the middle occipital gyri and precuneus, the areas associated with experience of negative emotions, were activated when non-beautiful images were presented. The conjunction analysis showed activations in temporal areas in response to beautiful images and activations in parietal areas in response to non-beautiful images. These results indicate that beautiful images elicit semantic interpretations whereas non-beautiful images facilitate abstract processes.

Purpose: We studied whether brain perfusion SPECT is useful in the psychiatric disability evaluation of patients with chronic traumatic brain injury (TBI). Materials and Methods: Sixty-nine patients (M:F=58:11, age $39{\pm}14$ years) who underwent Tc-99m HMPAO brain SPECT, brain MRI and neuropsychological (NP) tests during hospitalization in psychiatric wards for the psychiatric disability evaluation were included; the severity of injury was mild in 31, moderate in 17 and severe in 21. SPECT, MRI, NP tests were peformed $6{\sim}61$ months (mean 23 months) post-injury. Diagnostic accuracy of SPECT and MRI to show hypoperfusion or abnormal signal intensity in patients with cognitive impairment represented by NP test results were compared. Results: Forty-two patients were considered to have cognitive impairment on NP tests and 27 not. Brain SPECT showed 71% sensitivity and 85% specificity, while brain MRI showed 62% sensitivity and 93% specificity (p>0.05, McNemar test). SPECT found more cortical lesions and MRI was superior in detecting white matter lesions. Sensitivity and specificity of 31 mild TBI patients were 45%, 90% for SPECT and 27%, 100% for MRI (p>0.05, McNemar test). Among 41 patients with normal brain MRI, SPECT showed 63% sensitivity (50% for mild TBI) and 88% specificity (85% for malingerers). Conclusion: Brain SPECT has a supplementary role to neuropsychological tests in the psychiatric disability evaluation of chronic TBI patients by detecting more cortical lesions than MRI.
The development of functional magnetic resonance imaging (fMRI) has significantly contributed to mapping brain functions and understanding brain networks during rest. This paper proposes a CNN-LSTM-based classification model to classify the progression stages of Alzheimer's disease. Firstly, four preprocessing steps are performed to remove noise from the fMRI data before feature extraction. Secondly, the U-Net architecture is utilized to extract spatial features once preprocessing is completed. Thirdly, the extracted spatial features undergo LSTM processing to extract temporal features, ultimately leading to classification. Experiments were conducted by adjusting the temporal dimension of the data. Using 5-fold cross-validation, an average accuracy of 96.4% was achieved, indicating that the proposed method has high potential for identifying the progression of Alzheimer's disease by analyzing fMRI data.
최근 MR영상을 허혈성 뇌졸중의 초급성기에 뇌조직의 관류 이상을 조기에 진단하려는 연구들이 진행되고 있으나 아직 일반적인 진단용 소프트웨어만 있을 뿐 영상 자료를 후처리하여 뇌조직의 구조 및 기능적인 정보를 제공하는 mapping 영상을 특수 소프트웨어는 실용화되어 있지 않다. 본 논문에서는 Gamma-variate 곡선 정합을 이용한 뇌관류 파라미터 영상 mapping의 알고리즘 구현에 관해 연구하였다. 관류 MR영상의 각 화소마다 측정된 시간에 따른 신호강도의 변화 곡선은 비선형적이어서 뇌관류에 관한 여러 가지 혈역학적 변수들을 보다 정확하게 계산할 수 없었다. 그래서 수렴속도가 빠르고 안정성이 높은 비선형 최적화 알고리즘인 Levenberg-Marquardt 알고리즘(LMA)을 활용하였다. 즉 시간에 따른 신호강도의 변화 곡선을 Gamma-variate 함수를 이용하여 곡선 정합한 후, CBV, MTT, CBF, TTP, BAT, MS의 여러 가지 혈역학적 변수를 LMA에 의해 계산하였다. 그 결과로 관류 MR영상으로부터 얻은 mapping 영상은 초급성 허혈성 뇌졸중에서 관류에 관한 혈역학적 변화를 평가함으로써 나중에 생길 뇌경색의 범위를 예견하는 데에 유용하였다.
Purpose : To evaluate the usefulness of functional MR imaging (fMRI) for language mapping and determination of language lateralization. Materials and Methods : Functional maps of the language area were obtained during word generation tasks and decision task in ten volunteers (7 right handed, 3 left-handed). MR examinations were performed at 1.5T scanner with EPI BOLD technique. Each task consisted of three resting periods and two activation periods with each period of 30 seconds. Total acquisition time was 162 sec. SPM program was used for the postprocessing of images. Statistical comparisons were performed by using t-statistics on a pixel-by- pixel basis after global normalization by ANCOVA. Activation areas were topographically analyzed (p>0.001) and activated pixels in each hemisphere were compared quantitatively by lateralization index. Results : Significant activation signals were demonstrated in 9 of 10 volunteers. Activation signals were found in the premotor and motor cortices, the inferior frontal, inferior parietal, and mid-temporal lobes during stimulation tasks. In the right handed seven volunteers, activation of language areas was lateralized to the left side. Verb generation task produced stronger activation in the language areas and higher value of lateralization index than noun generation task or decision task. Conclusion : fMRI could be a useful non-invasive method for language mapping and determination of language dominance.
