CT 영상에서의 간 영역 추출 및 간 종양 분석

  • 간세포암은 우리나라에서 전체 암사망자 중 17.2%로 3번째의 흔한 사망원인이며, 간암에 의한 사망률은 인구 10만 명당 약 21명에 이른다. 본 논문에서는 간 내부에서 발생하는 간세포암을 CT 영상에서 자동으로 추출하는 방법을 제안하여 간세포암의 보조진단으로서의 유용성에 대해 알아보고자 한다. 간 내부의 종양을 추출하기 위해 흉부의 윗부분에서 시작하여 2.5mm의 간격으로 약 45-50장 정도를 촬영한 CT 영상들을 대상으로 먼저 간 영역을 추출한다. 간 영역 추출은 먼저 관심이 없는 외부 영역을 갈비뼈를 중심으로 제거한 후 영상의 밝기 정보를 이용하여 각 기관의 영역을 분할 한다. 분할된 영역들은 위 아래로 인접한 영상에서의 분할 영역들과 밝기 값을 비교하여 적절하게 병합하는 3차원적 접근방법을 사용한다. 간 영역은 여러개의 영역들 중에서 간 영역의 구조 및 위치 등의 정보를 활용하여 추출한다. 추출된 간 영역에서 종양 판별과 추출을 위해 종양이 가지는 특징을 분석하여 종양을 추출한다. 전형적인 간세포암은 과혈관성 종양이므로 조영증강 CT 영상에서 주위보다 밝은 색으로 나타나며, 팽창 형성장을 보일 경우에는 구형으로 나타나는 특징이 있다. 이에, 주위 보다 밝은 색을 가지고 둥근형태를 가지는 영역을 종양의 후보영역으로 선정한 후, 그 영상의 위와 아래로 연결되는 영상에서도 같은 위치에서 같은 특징을 보이는 영역이 있으면 간 내부의 종양으로 판별하여 추출한다. 제안된 간 영역 및 간 종양 추출 방법의 정확성을 판별하기 위하여 CT 영상을 대상으로 실험하여 영상의학 전문의가 판단한 결과와 비교하였다. 간 영역 추출은 정확히 모두 추출되었으며, 간 종양 추출 및 판별은 전문의의 보조 진단도구로 활용할 수 있는 가능성이 매우 높다는 것을 확인할 수 있었다.emantic Similarity Measure 등을 단계적으로 수행하여 자동화되고 정확한 규칙식별을 하고자 한다. 이러한 방법들의 조합으로 인하여 규칙구성요소 추출이 되지 않을 후보 단어들의 수를 줄여서 보다 더 정확하고, 지능적인 규칙구성요소 추출 방법론을 제시하고 구현하여 지식관리자의 규칙습득에 대한 부담을 줄여 주고자 한다. 도움을 받을 수 있게 되었다.을 거치도록 되어있다. 교통주제도는 국가의 교통정책결정과 관련분야의 기초자료로서 다양하게 활용되고 있으며, 특히 ITS 노드/링크 기본지도로 활용되는 등 교통 분야의 중요한 지리정보로서 구축되고 있다..20{\pm}0.37L$, 72시간에 $1.33{\pm}0.33L$로 유의한 차이를 보였으므로(F=6.153, P=0.004), 술 후 폐환기능 회복에 효과가 있다. 4) 실험군과 대조군의 수술 후 노력성 폐활량은 수술 후 72시간에서 실험군이 $1.90{\pm}0.61L$, 대조군이 $1.51{\pm}0.38L$로 유의한 차이를 보였다(t=2.620, P=0.013). 5) 실험군과 대조군의 수술 후 일초 노력성 호기량은 수술 후 24시간에서 $1.33{\pm}0.56L,\;1.00{\ge}0.28L$로 유의한 차이를 보였고(t=2.530, P=0.017), 술 후 72시간에서 $1.72{\pm}0.65L,\;1.33{\pm}0.3L$로 유의한 차이를 보였다(t=2.540, P=0.016). 6) 대상자의 술 후 폐환기능에 영향을 미치는 요인은 성별로 나타났다. 이에 따른 폐환기능의 차이를 보면, 실험군의 술 후 노력성 폐활량이 48시간에 남자($1.78{\pm}0.61L$)가 여자(

Stomal Recurrence after Total Laryngectomy - A Critical Analysis of Etiology and Therapeutic Problems­ (후두전적출술 후 기공주변의 재발)

  • Stomal recurrence after total laryngectomy presents serious therapeutic problems despite aggressive treatment methods. The purpose of this study is to evaluate the critical analysis of etiology and treatment results and to clarify the treatment plans and prevention of stomal recurrence. Among 159 cases who had undergone total laryngectomy for cancers of larynx(135 cases) and hypopharynx(24 cases) during recent 10 years, stomal recurrence occured in 12 cases(1 case with type I, 2 cases with type II, 2 cases with type III, 3 cases of type IV and unclassified 4 cases according to Sisson's classification) and the retrospective analysis of results were as follows: 1) Average duration of stomal recurrence was $8.2{\pm}4.35$ months after cessation of primary treatment. 2) The overall incidence of stomal recurrence was 7.6%. 3) The suggested etiology in the pathogenesis of stomal recurrence could be inadequate surgical margin, delayed laryngectomy after initial tracheostomy and improper management of metastatic nodes. 4) Mean survival time was $7.3{\pm}5.61$ months and one case with type I underwent surgical salvage is still alive out of 7 cases with chemotherapy and radiotherapy and 5 cases with surgical salvage and adjacent therapy. In summary, aggressive surgical resection should be recommended in cases with high risks of stomal recurrence.

  • Minimally invasive coronary artery bypass grafting without using cardiopulmonary bypass (CPB) is a recently accepted modality of myocardial revascularization prcedures which is particularly suitable to the patients with lesions in the left anterior descending(LAD) and the right coronary arteries. Of the consecutive 35 patients of coronary artery bypass grafting performed at Sejong General Hospital from March to August 1996, six patients underwent minimally invasive coronary artery bypass grafting without CPB. All had stenotic lesions of the LAD more than 90%. Bypass grafting of the LAD was approached through midline sternotomy in one, through ministernotomy in two, and through limited left anterior thoracotomy in three patients, respectively. The internal mammary arteries were prepared without the use of thoracoscope. The mobilized mammary arteries were connected directly to the LAD in 5 patients, and the anastomosis required interposition of a segment of the radial artery in the remaining one. The diagonal branch was revascularized with the saphenous vein graft at the same time in one patient. No blood transfusion was necessary in 2 patients, and average blood required during surgery was 800ml in 4 patients. All patients were extubated from 4 to 14 hours(mean 9 hours) after operation. Early postoperative coronary angiography in 5 patients between 7 and 10 days after surgery has proved full patency of the grafts. With these limited clinical experiences, the clinical results demonstrated that minimally invasive coronary artery bypass grafting without CPB is an useful procedure especially in patients with isolated lesion in the proximal LAD.

  • Esophageal atresia and tracheoesophageal fistula may occur as separate entities but usually occur in combination.The first report of esophageal atresia with tracheoesophageal fistula was done by Thomas Gibson in 1696.In1941, Haight and Towlseg performed the first successful primary repair,recently we were experienced a case of esophageal atresia with distal tracheoesophageal fistula in infant patient who presented the symptoms of dyspnea and vomiting.The operation was performed transpleurally through right 4th intercostal space.Patient tolerated all the operative procedure and recovered uneventflly.He was well-being without other problem.

  • The glomus tumor is a distinctive benign neoplasm with a small painful nodule, occurs most commonly in extremities but may be found elsewhere in the body. Its occurrence in the trachea or lung parenchyme has been recognized with extreme rarity and there has not been any report, to our knowledge, of its occurrence in the main bronchial glomus tumor. We report a case of a glomus tumor in the left main bronchus in a 67-year-old man who was presented with blood-tinged sputum and dyspnea, which was completely relieved by surgical resection.

A Case Report of Traumatic Tracheoesophageal Fistula (외상성 기관식도루 -수술체험 1례-)

  • Acquired, traumatic tracheoesophageal fistula [TEF] is rare and difficult problem to manage. This 55 years old man met with a roller accident of a tractor. During accident, he received a penetrating injury on the left upper sternal border. At local clinic, he received closed thoracotomy drainage [CTD]for relief of pneumothorax[left]. Three days after CTD, he complained abdominal pain and hematemesis. The endoscopy revealed large ulcer at the stomach, so he received subtotal gastrectomy. On 10th day post subtotal gastrectomy, he developed aspiration and coughing from a TEF. The esophagogram showed large TEF at the mid-trachea level. So he transfered to our hospital for operation. This patient was operated on for late TEF three weeks after injury. We have used absorble 4-0 Vicryl sutures to repair trachea. We repair all esophageal injuries with two layers of nonabsorbable silk suture. Where suture line on the esophagus, the strap muscle was interposed for reinforcement. And for feeding, the feeding jejunostomy was performed. Postoperatively the osteomyelitis of the manubrium site was developed, so on the 30th postoperative day, an ostectomy of manubrium, both clavicle and fight 1st, 2nd ribs, and the pectoralis major musculo-cutaneous flap coverage were performed.

  • Esophageal atresia with tracheoesophageal fistula may occur as separate, but usually occur in combination. First described by Thomas Gibson in 1696, esophageal atresia was not successfully treated until 1939 when the first two survivors of staged correction were described by Ladd and Leven. In 1941, Haight and Towsley performed the first successful one-stage primary repair. We report three cases of esophageal atresia with tracheoesophageal fistula of which were treated with one-stage surgical repair method. The operation was performed tra-nspleurally through right 4th intercostal space. The fistula in the trachea was closed with interrupted 5-O prolene sutures and esophagoesophageal anastomosis was performed with 3-O prolene single layer sutures in all cases. All patients tolerated the operative procedures and recovered uneventfully. On follow-up study, anastomotic stricture was developed in one patients, so esophageal dilatation was performed for it with the Griintzig balloon catheter and the result was satisfactory. The other patients were well-being without any complication.

  • Congenital esophago-bronchial fistula without atresia is very rare and usually demonstrate more insidious clinical effects, and patients may reach adult life before the diagnosis. We had been experienced a typical case of esophago-bronchial fistula without atresia and document the case. A 42-year-old women was admitted to our hospital due to frequent choking symptom and attacks of pneumonia, and patient status was in chronic pulmonary infection status such as most probably bronchiectatic state of right middle & lower lobe including significant destructive changes. We had confirmed esophago-bronchial fistula without atresia with esophagoscopy, esophagogram, and chest CT, and performed esophago-bronchial fistulectomy and right middle & lower lobectomy. Its belongs to type II of Braimbridge`s classification for congenital esophago-bronchial fistula. Postoperation course was smooth and uneventful.

  • Leiomyoma of the trachea is a rare benign tumor. A case of leiomyoma of the thoracic trachea is described in a 46-year-old woman. The patient complained of productive cough, dyspnea and blood-tinged sputum since July 1993. Plain chest radiographs were not helpful, but computed tomography of the chest showed an intraluminal tracheal mass just above the carina. Fiberoptic bronchoscopy revealed a broad based, nodular mass rising from the posterior tracheal wall, just above the carina. The tumor was excised by sleeve resection and end-to-end anastomosis of the trachea was performed. The patient`s postoperative course was uneventful. She was discharged on the 20th postoperative day.

Tracheal Reconstruction Using Femoro-Femoral Bypass -A Case Report- (우측 소매 전폐 적출술 후 발생한 기관 협착증의 체외 순환을 이용한 수술치험 1례)

  • Resection and reconstruction of distal trachea or carina have posed tremendous technical challenges for surgeons. Successful outcome depends on thorough preoperative evaluation, careful anesthetic management,strict attention of surgical technique and postoperative care. We report a successful case of revision of tracheal stenosis using femoro-femoral bypass on a 13~year-old boy. The patient complained severe dyspnea about I month following right sleeve pneumonectomy. Preoperative CT scan and intraoperative bronchoscopy showed pin-point tracheal stenosis at a tracheo-bronchial anastomosis site about 1.2cm in length.At operation the lesion was severely adhesed and the lumen was nearly obstructed. The stenotic segment was resected and direct end-to-end anastomosis was done under femoro-femoral bypass for adequate oxygenation. The patient was discharged at postop. 16 days without specific complications and has continued to do well.
