Esophageal Stricture Treated with Endless Bougienage (무단소식자법에 의한 심한 식도협착증 치험 1 례)

  • Esophageal stricture due to various caustic agents has led to decrease markedly with the improvement of the way of life and socioenvironmental change, and can be prevented with adequate procedure and management. However, there are still sporadic incidents of esophageal stricture due to inadequate treatment and uncooperation of patients. Esophageal stricture was treated with various kinds of bougies ; peroral esophagoscopic bougie, Hurst or Maloney type weighted bougie, endless bougie, retrograde esophageal bougie and open surgery. Recently the authors had experienced a case of severe esophageal stricture after ingestion of HCl, which was treated by gastrostomy and endless bougienage with good result.

  • The incidence of decannulation difficulty included tracheal stenosis has markedly increased in recent years because of translaryngeal intubation and tracheostomy although advancing antibiotics and new treatment for these problems. Treatment has always been difficult but in mild cases, a new soft, flexible tracheal T-tube that designed to maintain an adequate tracheal airway as well as to provide support in the reconstructed trachea and in severe cases, transverse resection with subsquent end to end anastomosis has been used in recent years. Authors experienced 2 cases of tracheal stenosis and decannulation difficulty which developed after tracheostomy that was performed due to automobile accident and fall down respectively and using a silicone tracheal T-tube for 3 months good results were obtained. So authors reported with brief review of literatures.

Anesthetic Management of Corrective Operation of Tracheal Stenosis (기관절개후에 발생한 기관협착증 마취례)

  • A 25 years old male patient was experienced Trough method operation to correct of Tracheal stenosis following Tracheostomy. After admission, a No.3 Magill plastic portex tube had been placed in the previous tracheostomy opening. Stenotic narrowing portion was noted low trachea (about 3.5cm above carina) Despite of narrowing of the lesion, anesthetic course was uneventful with oxygen, halothane and non depolarized muscle relaxant by controlled respiration. Postoperative coures were also favorable With adequate respiratory cares and blood gas analyzed.

  • 부식성 식도염에서 속발한 식도협착의 비외과적 치료법으로 사용되었던 종래의 각종 소식자를 이용한 확장술은 그 효과가 일시적이며 소아에서 시행시 많은 문제점이 있다. 근래들어 관상동맥, 요관, 대장 등의 협착에 사용되어온 balloon Catheter를 이용한 식도 확장술은 비교적 안전하고 효과적이어서 소아나 정도가 심한 식도협착의 치료에 큰 도움을 줄 수 있다. 본 교실에서는 최근 2년간 부식성 식도염 후 속발한 식도협착 6 례를 Balloon catheter 확장법으로 치료하여 아래와 같은 결과를 얻었기에 보고하는 바이다. 1) 전체 6명 중 남자가 4명 여자가 2명으로 평균 연령은 45.7세였다. 2) 부식제의 종류는 가성소다가 5명 초산이 1명이였다. 3) 협착부위는 1례가 경부식도이며 5례가 흉부식도였고 이중 1례는 다발성 협착이 였다. 4) 확장 전 식도조영사진 상 협착부위의 평균 내경은 4.3mm였다. 5) 확장술을 시행한 횟수는 2회에서 5회( 평균 3.5회 )로 확장술이 끝난 후의 평균 내경은 13.5mm였다. 5) 확장술시 3례에서 식도파열이 발생하였으나 보존적 치료법으로 치유되었다.

A Case of Acquired Nasopharyngeal Stenosis (후천성 비인두 협착증 1례)

  • Nasopharyngeal stenosis is an obliteration of the normal communication between the nasopharynx and the oropharynx resulting from the fusion of the tonsillar pillars and soft palate to the posterior pharyngeal wall. It is a rare but serious problem. The most common etiology is currently the surgical trauma associated with uvulopalatopharyngoplasty or adenotonsillectomy. It can range in severity from a thin band to a complete obstructing cicatrix, Symptoms vary from mild hyponasal speech to almost complete nasal obstruction with oral breathing, We present a case of a 16 year-old male with nasopharyngeal stenosis after radiofrequency-assisted adenoidectomy in this paper. This patient was managed by synechiolysis, obturator and buccal mucosal graft.
