Utilization and Expenditure of Health Care and Long-term Care at the End of Life: Evidence from Korea (장기요양 인정자의 사망 전 의료 및 요양서비스 이용 양상 분석)

  • Purpose: This study empirically investigates the utilization and expenditure of health care and long-term care at the last year of life for long-term care beneficiaries in Korea. Methods: This study used National Health Insurance and Long-term Care Insurance claims data of 271,474 LTCI beneficiaries, who died from July 2008 to December 2012. Their cause of death, place of death, health care costs, and the provision of aggressive care were analyzed. Results: Cardio-vascular disease(29.8%) and cancer(15.3%) were reported as their major cause of death, and hospital(64.4%), home(22.0%), social care facility(9.2%) were analyzed as the place of death. 99.3% of subjects used both health care and long-term care during the last 1 year of life. The average survival period were 516.2 days after they were LTCI beneficiaries. The health care expenditure gradually increased near the death, and the last month were three times more rather than the first month. Furthermore, 31.8% experienced some aggressive cares(CPR, blood transfusion, hemo-dialysis, etc.) at the last month of life. Conclusion: The results of this study suggest that it is important to develop the end of life care policies(for example, hospice, advanced care directives) for the LTCI beneficiaries. They might contribute to the improvement of quality of life and the reduction of health care expenditure of the elderly at the end-of-life.

Effects on the Functional Status Changes of LTC(Long-Term-Care) Services (노인장기요양보험 급여이용이 기능상태 변화에 미치는 영향)

  • The study was performed to identify the functional status change of beneficiaries of Long-Term Care Insurance and its related factors. We conducted the logistic regression with 17,652 beneficiaries during August and September in 2008. As a result, activities of daily living(ADL), behavioral changes, rehab, instrumental activities of daily living(IADL) and cognitive function, followed by nursing care area were improved in a greater degree. For the institutional service, level-1 beneficiaries was significantly improved in rehab area and level-2 beneficiaries was improved in ADL. For the home-visit care service of in-home services, level-1 beneficiaries was improved in ADL, level-2 beneficiaries was improved in ADL and rehab area, level-3 beneficiaries was improved in ADL, cognitive function and behavioral changes. For the day-and-night care service, level-1 beneficiaries was improved in ADL, IADL, behavioral changes and rehab area, level-2 beneficiaries was improved in behavioral changes, level-3 beneficiaries was improved in cognitive function and behavioral changes. For the short-stay service, level-3 beneficiaries was improved in behavioral changes. By the above results, there was a difference in a functional improvement by level and used services. Therefore, government need to provide the personalized service system based on the objective and comprehensive understanding for health and functional status of beneficiaries.

Analysis of Labor Market Activation Policy and Its Employment Outcomes: the Effects of Employment and Tailored Social Service Provision (한국의 노동시장 활성화 정책 특성과 취업 성과: 취업지원서비스와 맞춤형 사회서비스 중심 정책의 비교)

  • This article aims to analyze main features of activation policy and compare major programmes in terms of their impact on employment performances in Korea. Since the introduction of National Basic Living Security Act in 2000, a series of activation policies have been in place for social assistance recipients, low-income employees, and youth unemployed by means of providing tailored employment and social services via in-depth counselling and case management. These activation policies carry both enabling and demanding elements: requiring programme participation in exchange for public assistance benefit receipts on the one hand, and providing various social services to remove barriers to employment through case management on the other hand. Therefore, it merits attention to analyze how various features of activation programmes affect employment outcomes, the effectiveness of delivery system and policy instruments as well. In analyzing employment outcomes of activation policy of which main characteristics lie in provision of employment and social services, this article points out the features and policy instruments of the activation policy that contribute to labor market entry of public assistance recipients and low income employees. In addition, it also delineates the determinants of exit from benefit receipts. Results from statistical analysis show that activation policy with intense employment service helps both benefit recipients and low income employees enter into the labor market at a faster rate. However, tailored social service provision enables social assistance recipients to exit from benefit receipts. These results suggest theoretical and policy relevant implication in regards to redesigning the delivery mechanisms and service instruments of activation policy.

감귤 부산물을 급여한 제주 재래돼지의 영양성분 및 기호성

  • 본 연구는 제주도에서 폐기되고 있는 밀감 부산물을 돼지에게 급여하여 폐기물의 이용과 기능성 돈육의 생산을 위하여 실시하였다. 돼지는 밀감 부산물을 급여하지 않은 등심육($JPN-T_0$) 및 육성기에 8% 급여한 후 비육기에 15% 급여한 등심육($JPN-T_1$)을 대상으로 여러 가지 영양성분 분석과 관능검사를 실시한 결과 수분, 조단백질, 조지방, 조회분, 열량 및 콜레스테롤 함량은 시료들 사이 에 차이가 없었다. 무기질은 칼륨, 인, 나트륨, 마그네슘 및 칼슘의 순으로 함유되어 있으며, 칼슘의 경우 대조구가 유의하게 높았고, 다른 무기질들은 유의한 차이가 없었다. 그리고 비타민 $B_1$$B_2$의 함량도 감귤 부산물을 첨가한 것과 하지 않은 돈육 사이에 유의한 차이가 없었다. 구성아미노산의 총량은 $JPN-T_0$가 13.57%이고, $JPN-T_1$이 14.80%였다. 그리고 glutamic acid, aspartic acid, lysine 및 leucine가 많이 함유되어 있었다. 유리아미노산 총량은 대조구와 감귤 부산물 급여구 사이에 유의한 차이가 없었으나, L-valine과 L-lysine은 감귤 부산물 급여구가 유의하게 낮았다(p<0.05). 대조구와 감귤 부산물 급여구에서 포화지방산은 palmitic acid와 stearic acid가 많았으며, 불포화지방산은 oleic acid와 linoleic acid가 많이 함유되어 있었다. 생육의 색깔은 감귤 부산물을 급여하지 않은 등심육이 유의하게 높았다(p<0.05). 가열육의 기호성 중에서 향기는 $JPN-T_0$$JPN-T_1$보다 유의하게 높았으나(p<0.05), 맛, 연도, 다즙성 및 전체적인 기호성은 유의한 차이가 없었다.자체를 악하다고 볼 수 없고 더구나 구원을 이 세상에서의 이탈로 볼 수 없다. 진정한 구원이란 원래 하나님이 보시기에 아름다웠던 그 세상으로의 회복을 포함한다. 이런 면에서 하나님 주권 신앙 하에서 구원이란 전 인격적인 구원, 전 우주적인 구원이 된다. 그렇기 때문에 성도는 세상의 삶과 학문, 예술, 정치, 경제, 사회를 포함한 모든 분야를 하나님의 뜻 가운데서 그 원래의 목적에 부합할 수 있도록 회복시키는 일에 적극 참여해야 한다.자체가 이를 주도하기는 사실 어려움이 있다. 그리고 대형유통점이 영업행위를 영업시간제한에서부터 출점제한에 이르기까지 규제하는 건은 심사숙고하여야 한다. 대형유통점이 국가경제 및 지역사회에 미치는 영향이 부정적인가 긍정적인가에 대해 국내외 학계와 업계에서 여전히 많은 논란이 있기 때문이다. 정부와 지자체에 의한 시장개입은 반드시 필요한 경우에 한해 합당한 방법에 의해 이루어져야 한다. 대형유통점에 대한 규제는 지역사회에 미치는 영향을 다면적으로 평가한 결과에 근거하여 이루어져야 할 것이다. 대부분의 지자체는 체계적인 평가시스템과 객관적인 통계 자료를 갖고 있지 못한 실정이다. 향후 가장 시급한 과제는 시장개방 이후 지난 10년간 대형유통점이 지역사회에 미친 영향에 관한 광범위한 통계자료를 수집하고 이를 체계적으로 분석하여 정책방향을 올바르게 설정하는 것이라 할 수 있다.i와 K. pneumoniae가 존재하며 확산 중임을 시사한다. 앞으로 CTX-M형 ESBL의 만연과 변종 CTX-M형 ESBL의 출연을 감시하기 위한 정기적인 연구와 조사가 필요한 것으로 생각한다., A2-1, B1-1, B2-1의 경우, 강우 일수 감소 이전과 연 유출량 변화는 거의 없었으나, 유사량과 영양물질 부하량은 다소 증가하는 것으로 나타났고 연강우일수를 약20일 감소시킨 A1-2, A2-2, B1-2, B2-2의 경우는 강우일수를 감소시키기 이전에 비해 유출량이 <

사학연금제도 재정건전성 제고를 위한 재정추계 및 정책평가 방법 연구

  • 본 연구는 사학연금의 장기적 재정건전성을 확보하기 위한 재정추계 및 정책평가 방법에 관하여 논의한다. 먼저 재정안정화 시나리오의 설정에 있어서 기간간 예산제약식을 이용하여 장기적으로 균제상태(steady state)를 달성할 수 있는 납부율 및 급여율의 선정이 어떻게 이루어질 수 있는지 살펴본다. 또한 재정추계에 있어서 정책 변화에 따라 개인들의 행동 패턴이 달라질 가능성이 있음을 강조하고, 조기퇴직 결정에 관한 캘리브레이션을 통해 재정추계에서 내생성에 대한 고려가 어떻게 이루어질 수 있는지 예시한다. 마지막으로 힉스 보상의 개념에 기초하여 안정화 정책에 대한 평가가 연금 재정지표에 미치는 효과만이 아니라 가입자 / 수급자의 후생에 미치는 영향까지 같이 고려하여 이루어질 필요가 있음을 설명한다.

Pension Structure, Benefit Generosity and Pension Spending in the Retrenchment Period of Welfare States (복지국가 재편의 경로의존성 : 공적연금 제도 구조와 급여관대성 및 지출수준에 관한 비교연구)

  • This study investigated changes and determinants of public pension generosity and pension spending in welfare states during the last retrenchment period. Path-dependency thesis, industrialization theory and power resources model were examined with the twelve welfare states from 1980 to 2007. The main results are as follows. First, the developments of benefit generosity and pension spending have been differently presented according to pension structure. Second, the cross-national pooled-time series analysis confirmed that pension structure is the most significant factors to determine the level of benefit generosity and pension spending. Third, the positive effect of population ageing on pension spendings were proved even without any changes of pension generosity. New social risks, however, have restrained the pension spending. Fourth, the power of the left party and labor union did not affect the pension policy, which implies that power resources theory cannot explain the development of pension policy in this retrenchment period.

Parental Leave and Female Labor Supply in Korea (육아휴직 지원과 여성의 노동공급)

  • It is often argued that the parental leave should be extended in order to help employees achieve the balance of work and family life. However, one should be careful in designing the parental leave since there is a tradeoff between the continuity of employment after childbearing and the depreciation of human capital due to the time off the work. The paper investigates whether the parental leave payment introduced in Korea in 2001 helped women's take-up of the leave and employment after giving birth. The results are as follows. First, the increase in the payment indeed raised the female take-up of the leave. Second, women returned to labor market less often after giving birth in the short run, but the size of the effect tends to decrease in the long run. However, the negative effect on returning to the same workplace after the birth remains significant in the long run. Lastly, there is no evidence that the share of women of childbearing age at workplace changed due to the increase in the parental leave payment. Although there is a limitation in that the analysis is based only on the employment covered by the Employment Insurance, these findings imply that the increase in the parental leave payment may not be an effective policy tool for promoting female labor market participation. On the other hand, it should be noted that it is necessary to pay attention to child development as well as female employment as an objective in order to make an overall judgement on the parental leave policy.

Analysis of Health Insurance Standards and Utilization of MRI in Korea: Based on Health Insurance Claim Data (한국의 MRI 건강보험 급여기준 및 진료이용에 관한 연구)

  • This study analyzed the current status of MRI (frequency, amount of treatment) based on the history of application of the MRI health insurance benefit standard and health insurance claim data. MRI examinations began as a health insurance benefit in 2005. In 2005, the indications were restricted for some diseases, but coverage for benefits in 2010, 2013, 2016, and 2018 was expanded. In 2021, the Ministry of Health and Welfare decided to apply health insurance for all MRI examinations. From 2010 to 2017, the number of MRI examinations increased by 86.7% in 2017 compared to 2010, and the amount of treatment increased by 53.5%. According to general characteristics, the number of MRI examinations was higher in women than in men. By age, the number of examinations was the highest among ages 70-79. Outpatient examinations were more frequent than inpatient examinations, and the number of examinations in the tertiary hospitals was the highest among the types of hospitals. The number of brain MRI examinations was the highest in each exam site. In December 2013, the standard of MRI was expanded for heart disease and Crohn's disease, the number of cardiac MRI and abdominal MRI examinations increased in 2014 compared to 2013. However, the number of examinations is small and not associate with the disease, it would be difficult to say that it affected the increase in the total number of MRI examinations. To assess health insurance sustainability and policy effectiveness, monitoring will be necessary.

Development and Limitation of Income Maintenance Policy after the Financial Crisis (외환위기 20년, 소득보장정책의 발전과 한계)

  • The purpose of this study is to investigate the development process and current status of income maintenance policies after the financial crisis, to sort out key issues or problems, and to explore future plans or directions that can overcome the limitation. In order to develop intensive argument, the scope of research is limited to the National Pension, Basic Pension, and the National Basic Livelihood Security System. The research also focused on two values of the 'universalism' and the 'adequacy' based on the 'SPF(social protection floor)' in the overall narrative process. Additionally, this paper briefly summarized the related contents released by the Moon Jae-in Government and presented an affirmative strategy and a transformational strategy centered on social allowance and basic income respectively, to establish an integrated income guarantee scheme. Although some improvements to the present system are also required, it is an effort to expand the practical effect of social benefits through the full and comprehensive reform of benefit structure and method.

  • 매년 사업주와 근로자로부터 상담한 내용을 돌아보면 우리 사회 노동시장의 현안이 무엇인지, 인권수준은 어떠한지, 노동행정의 방향이 어떠한지를 확연히 체감하게 된다. 그 중 하나가 육아휴직제도이다. 20년 전만 하더라도 육아휴직은 거의 허용되지 않았다. 결혼 또는 임신하면 퇴직이 관행이던 시절이었다. 관행에도 불구하고 임신한 근로자가 퇴직하지 않으면 출 퇴근이 어려운 지방으로 배치전환하거나, 직무를 변경하고, 담배연기 가득한 방 가운데 여성근로자를 근무시키는 비인간적인 처사도 행해졌다. 2~3년 전만 하더라도 "우리 회사는 육아휴직은 안된대요. 그럴바엔 그만두래요. 아이 맡길 곳도 만만치 않은데 어쩌면 좋아요"라는 여성근로자들의 상담이 한달에도 몇 건씩 쏟아져 나왔었다. 초기 여성노무사가 소수이다 보니 워킹맘들의 노동법률상담을 할 기회가 많았다. 때론 사업주들을 설득하고, 중재하고, 처벌조항을 들어 압박하기도 하면서 육아휴직을 권장했지만 기존의 관행과 문화가 일시에 변화하기는 어려운 일이다. 여성의 경제활동율이 늘어나고 출산율이 줄어들면서 정책적으로 출산 및 육아휴직을 장려하는 정부의 감독행정 강화, 그간 노동계 및 시민사회단체의 노력 등으로 인해 법률에 규정된 육아휴직제도가 비로소 현실적으로 보장되어가고 있음을 목도한다. 이즈음에 육아휴직자의 연차휴가 사용, 퇴직금, 육아휴직급여 등에 대한 문의가 많은데 그만큼 육아휴직자가 늘어났다는 것을 반증하는 결과이다. 따라서 이번호에서는 출산휴가 및 육아휴직자들의 연차휴가 사용과 퇴직금 산정방법, 출산 육아휴직급여에 대해 알아보고자 한다.
