Recognition and Satisfaction fusion survey regarding health insurance coverage of dental scaling (치석제거 건강보험 급여화에 대한 일부 지역주민의 인식도 및 만족도 융합연구)

  • This study aimed to investigate recognition of dental scaling and recognition of and satisfaction with health insurance coverage of dental scaling and make an efficient healthcare policy for dental health insurance; to do this, a survey was conducted in 389 residents in North Jeolla Province, drawing the following conclusion: First, the respondents were most likely to suggest that it was desirable to get dental scaling 'twice a year' and to start it 'in their twenties'. Second, those who were female and who were more concerned about periodontal health were more likely to recognize coverage of dental scaling. Third, those who were female and who were more concerned about periodontal health were also more satisfied with the coverage. It is therefore necessary to make PRs by using teaching media positively as well as by giving correct information with the objective of improving recognition of dental scaling on a continuous basis.

Unequal distribution of family policy in Korea (한국 가족정책의 계층화)

  • This study analyzes the unequal distributional effect of threesome of family policy(child benefit, childcare services and parental leave) focusing on family income, mother's status in labor market in Korea. To measure the unequal distributional effect of family policies, this study used the quantile analysis. The results of this study are as follows. First, in terms of childcare service and parental leave, there is some difference of the rate of use by family income and mother's status in labor market. Second, total public fund for childcare services, child benefits and parental leave are high in fourth income quintile, and mothers work regularly. Third, public fund is high in fourth income quintile, dual earners, mothers work regularly, the family has many number of child, and is high educational level of parents. Finally, the results of quantile regression show the biggest factors of unequal distribution of family policy are mother's stable work and it deepens the inequalities and differences. Based on these results, this article suggests that more equal right to access and use family policy regardless of the type of employment, adequate minimum income through income transfer, and universal application of the policy.

서독(西獨) 및 일본(日本)의 의료보험제도(醫療保險制度)가 주는 교훈(敎訓)

  • 본고(本稿)의 목적은 서독(西獨) 일본(日本) 양국(兩國)의 의료보험제도(醫療保險制度)를 비교 검토하여 그들의 경험에서 배울 수 있는 교훈을 토대로 우리나라의 실정에 적합한 정책방안을 제시하려는 데 있다. 일반적으로 서독(西獨)은 제도내용이 효율성(效率性)보다는 형평성(衡平性)에, 일본(日本)은 비용효과(費用效果) 측면에 치중함으로써 서독(西獨)에서는 의료비 증가로 비용억제(費用抑制)가, 일본(日本)에서는 급여의 형평문제(衡平問題)가 중심과제로 되어있다. 이들 양국(兩國)의 경험을 토대로 한국제도(韓國制度)의 개선(改善)에 유용한 일련의 정책대안(政策代案)을 제시하면, 프로그램간 부담과 급여의 균등화를 추진하고, 정부(政府)의 보조금지원(補助金支援)은 목표인구(目標人口)를 대상으로 규모를 최소화하여 비용효과적으로 실시해 나가야 할 것이다. 소단위조합(小單位組合)의 지속적인 광역화(廣域化) 추진과 조합운영에 자율적인 기능을 부여해 나가는 한편 지역보험(地域保險) 보험료부과기준(保險料賦課基準)의 단순화 및 형평제고를 위해 자력조사(資力調査)를 조기에 실시할 것을 제안한다. 후송체계(後送體系)를 전국적으로 확대 적용시켜 나가면서 고소득계층에 대하여는 강제적용보다 임의적용으로 전환하여 민간보험시장(民間保險市場)의 개발을 꾀하고, 소득 있는 피부양자를 선별하여 보험료(保險料)를 부과(賦課)함으로써 보험재정(保險財政)의 안정을 도모하여야 할 것이다.

Improvement of the disability benefit in NPS from the perspective of universalism, adequacy, and equity (국민연금 장애연금 급여의 개선방안에 관한 연구: 보편성, 적정성, 형평성을 중심으로)

  • This study investigates issues, which the current disability benefit of the national pension Scheme is facing, and seeks improvements concerning the universalism of coverage, the adequacy of benefit levels, and the equity of institution. The low universalism problem is caused by the coverage deficits and the strict disability assessment system of NPS and can be overcome by widening the disability category and changing the disability assessment system to workability test. In addition, the benefit level of the disability pension will be reduced stronger than the old age benefit in the long. The low benefit level due to the short contribution period and the low disbursement rate and can be improved by the enhancement of the standard contribution years and the disbursement rate. On the other hand, the main reason of the equality problem can be seen as the requirements for benefit, which are applied differently depending on the membership status. As policy measures, the unification of requirement on the basis of a recent payment, a payment in a certain percentage of life, or a hybrid of both criteria is investigated.

  • In accordance with the new healthcare policy of government (Moon Jae-In Care) to strengthen health insurance coverage, the National Health Insurance (NHI) coverage of brain magnetic resonance imaging (MRI), brain/neck MR angiography (MRA), and head and neck MRI have been expanded since 2018 in Korea. This article has been reviewed focusing on the "Detailed matter concerning criteria and method for providing reimbursed services in the NHI. Some revisions" regarding reimbursement for MRI, which was revised from October 2018 to April 2020 and is currently in effect. It included the MRI reimbursement system in Korea, recent adjustment of the reimbursement coverage for patients with headache or dizziness, and reimbursement coverage, standard imaging, and radiologic report of brain MRI, brain/neck MRA and head and neck MRI. This article could help radiologists gain knowledge on health insurance to protect the expertise of the radiologist and to play a leading role in the hospital. As the policy changes, detailed matter concerning criteria and method for providing reimbursed services in the NHI may be revised. Therefore, radiologists should update issues related to insurance reimbursement for MRI continuously.

A Study on the Relationship Between Child Poverty Rate and Family Policy Expenditure of Welfare States -Focused on OECD Countries- (복지국가의 아동·가족복지지출과 아동빈곤율의 관계 -OECD 국가를 중심으로-)

  • The main purpose of this study is to analyze the relationship between child poverty rate and family policy expenditure of welfare states (focusing on OECD countries). We analyzed not only the total social & family policy expenditures but the components of the family policy expenditure. OECD SOCX and calculated data from the LIS & OECD data were utilized for child and family policy expenditures and the poverty rate. One-way correlation and cluster analysis were employed for the analysis. The analytic results are as follows: Southern European and Anglo-Saxon countries' child poverty rates were higher and Scandinavian countries' child poverty rates were lower than any other clusters. The countries with high child poverty rate had higher child poverty rate than the entire nation's poverty rate, but Scandinavian countries' child poverty rate was lower. There was a strong correlation between family policy expenditure and child poverty rate. Especially the service expenditure and leave benefit expenditure were highly correlated with child poverty rate. On the other hand, cash expenditure was not significantly correlated with child poverty rate. We can suggest the policy implications from these results. Based on the analytic results, policy implications that the government should increase the family policy budget, especially the budget for family services and leave benefit to decrease child poverty rate and should make effort to support the employment of parents through policies such as active labor market strategies can be suggested.

Improvement of Permanent Disability Benefit System in Korean Worker's Compensation Insurance from the Perspective of Life Cycle (생애주기를 고려한 산재보험 장해연금 개선 방안 연구)

  • The permanent disability benefit(PDB) system whose purpose lies in helping affected workers with their self-realization through compensating them for their work-related injury or disease and facilitating their timely return to work is very important in terms of social welfare. Actually, the portions of PDB's in all WCI expenditures have been on the sharp increase. In spite of its positive aspects like this, however, an excessive amount of PDB's may have negative impacts on the affected workers' will to work or return to work, and increasing trends in the amount of PDB's payable in annuity will remain in an aging or aged society a potentially risky factor posing a threat to the soundness of WCI budgets. In this respect, the author attempted herein to identify the long-term reality of trends in the number of claimants entitled to PDB's and the amount of PDB's payable to them through making the long-term budget projections of PDB's; address problems with PDB's payable in annuity, which are or will potentially be one of the gravest burdens in securing the soundness of WCI budgets; and discuss how to optimize the amount of PDB's through studying & analyzing overseas cases and income replacement rates and what kinds of improvements are thinkable. It's recommended to reduce the absolute amount of benefits payable in annuity in a phased way considering claimants' life-cycle instead of sticking to the current system, which takes the form of lifelong pension plan.
