기고 - 전자담배에 대한 최근 연구동향

  • 최근 국내에서도 전자담배 이용자 수가 늘면서 사업장 금연지도자 보건관리자들로부터 전자담배와 관련한 문의를 부쩍 많이 받고 있다. 필자는 2009년 전자담배 안전실태조사에 전문가 자문을 맡아 참여하였는데, 현 삼성병원 산업의학과 그 연구진행 과정을 대한금연학회에 발표하였고 작년 12월 '전자담배 현황과 대책' 심포지엄에서 주제발표를 하여 전자담배의 안전성과 정부의 관리체계에 대한 문제점을 지적한 바 있다. 올해 전자담배에 대한 언론이 더욱 증폭되고 있는 상황에서 정부관련 부처에서도 전자담배의 관리방안에 대한 연구용역을 추진하고 있는 상황이다. 여기서는 사업장의 금언지도자 보건관리자들에게 전자담배 논란에 적절하게 대응할 수 있는 근거 있는 자료를 제공하기 위해 전자담배와 관련한 국내 외의 최근 연구동향과 성과들에 대해 체계적으로 살펴보았다.

The effectiveness of a Autonomous Regulation Improvement Smoking Cessation Program on the Amount of Daily Smoking, Perceived Motivation, Cotinine in Saliva, and Autonomous Regulation for Girls High School Students who Smoked (자율성 증진 금연 프로그램이 흡연 여고생의 일일 흡연량, 타액 코티닌, 지각된 동기 및 자율성에 미치는 효과)

  • This study examined the effectiveness of a autonomous regulation improvement smoking cessation program in girls high school students. The study was carried out with a nonequivalent control group pretest-posttest design. The subjects of the present study were 47 girls high school students in G and K city. The subjects were divided into an experimental group (n=23) who participated in the autonomous regulation improvement smoking cessation program, and a control group (n=24) who did not participate. Data were collected from 1 April 2013 through 26 April 2013. Collected data were analyzed using SPSS PC+ 18.0 with the Chi-square test, Fisher' exact test, Mann-Whitney U test, t-test. The experimental group had significantly different in cigarettes smoked per day(p=.044), cotinine in saliva(p=.048), perceived motivation(p=.001) and autonomous regulation(p=.027) in comparison to the control group. The autonomous regulation improvement smoking cessation program, when delivered to girls high school students who smoked, was effective in discouraging the smoking habit, and can be utilized as an effective nursing intervention for girls high school students who smoke.

Factors related to the willingness of chronic patients to quit smoking among adult smokers in Korea: Focusing on the data of the 7th National Health and Nutrition Survey (우리나라 성인 흡연자 중 만성질환자의 금연의지 관련 요인: 국민건강영양조사 제7기 자료를 중심으로)

  • This study aims to confirm the association with the willingness to quit smoking in adult smokers with chronic diseases, and the research method was analyzed using data from the 7th National Health and Nutrition Survey (2016-2018). The subjects were frequency analysis, chi-square test, and logistic regression analysis of 960 chronically ill patients aged 19 years or older, which were analyzed using the SPSS 26.0 Program. As a result of the study, depending on the level of education, high school graduation (OR=1.328, 95%CI=1.004-1.757), college graduation or higher (OR=1.556, 95%CI=1.167-2.075), the more stressed you are (OR=1.602, 95%CI) =1.217-2.109), there was an attempt to quit smoking (OR=5.263, 95% CI=4.287-6.462), which was associated with the willingness to quit smoking. It is necessary to prevent chronic diseases by reducing the smoking rate through active participation in smoking cessation programs targeting groups with high willingness to quit smoking, and by preparing measures to relieve appropriate stress.

The Effects of Smoking Cessation Coaching Program based on Motivation Stage to Stop Smoking of Patients at a Public Hospital (금연동기단계에 따른 코칭프로그램이 환자 금연에 미치는 영향)

  • This study examined the effects of a smoking cessation coaching program based on the motivation stage applying the Transtheoretical Model to stop the smoking of patients in terms of the amount of smoking, nicotine dependence, CO level, and urine cotinine. The study design was a multi-repeat multiple repeat intermittent time series study with one-group, a pre-post design. The participants were 47 smoking patients (44 males and 3 females), who were treated at a public hospital in N city. The participants were 4 (8.51%) subjects in the action stage and 43 subjects (91.49%) in the preparation stage of the motivation stage. The coaching program intervention was conducted at the first day, second week, and 6th week. The smoking cessation maintenance of the subjects was checked at the 12th week. A chi-square test and t-test were used to analyze the data. The subjects in the action stage were kept under the condition of no smoking and nicotine dependence. After the program of subjects in the preparation stage, the amount of smoking, nicotine dependence, and CO level were significantly lower compared to the pre-test (p<.001). The findings suggest that the coaching program based on the motivation stage was effective in improving the smoking cessation for patients who smoke. On the other hand, the patients in the smoking cessation program may require much more financial assistance than those of healthy people. A greater workforce and budget will be needed for patients to stop smoking.

Development of the Smoking Cessation Convergence Program App for HIV Infected Persons : Usability Evaluation (HIV 감염인을 위한 융합금연프로그램 앱 개발 : 사용성 평가)

  • The purpose of this study was to develop an application(app) on smoking cessation for patients with human immunodeficiency virus (HIV) infection and evaluate its usability. A methodological study design was applied. The program contents were developed by expert panel based on the theory of planned behavior, HIV infection, and smoking cessation and usability evaluation and then the application was developed by IT experts following usability testing composed of heuristic evaluation by 3 mobile experts and users evaluation was conducted using surveys and interviews by 10 individuals with HIV infection. Descriptive statistics and contents analysis were computed to evaluated the collected data. All participants were male and their age were ranged from 40s to 60s. The average of app usability was 4.2. The participants experienced no difficulties and responded somewhat satisfaction. The results showed that the app was feasible and useful on smoking cessation for HIV patients and could be used as an valid approach.

The Effects of Dental Hygiene Students' Self-efficacy and Smoking Knowledge on Their Perception on Guidance for Antismoking (치위생과 학생들의 자기효능감과 흡연지식이 금연지도 인식도에 미치는 영향)

  • The present study purposed to survey dental hygiene students who will play important roles in guidance and to utilize the data as basic information for setting the direction of dental hygienists' efficient guidance for antismoking and developing antismoking education programs. For this purpose, a survey was conducted with 400 students at the Department of Dental Hygiene in Daejeon, Jollabuk-do and Jollanam-do during the period from May 19 to June 14 in 2008, and drew conclusions as follows: As to correlations among self-efficacy, smoking knowledge, and perception on guidance for antismoking, positive correlation was observed between self-efficacy and smoking knowledge (r=0.102), between self-efficacy and perception on guidance for antismoking (r=0.272), and between smoking knowledge and perception on guidance for antismoking (r=0.352). As to factors affecting perception on guidance for antismoking, the effect was high in order of perception on smoking, smoking knowledge, and self-efficacy. That is, perception on guidance for antismoking was more positive in those who thought that smoking must not be allowed, those with high smoking knowledge, and those with high self-efficacy. For dental hygienists' clinical activities for efficient prevention of smoking and guidance for antismoking, it is necessary to develop curriculums containing various practical contents.

Study on Short Term Smoking Cessation Treatment in Dental Hospitals in Korea (국내 치과대학병원에서 시행한 단기 금연진료에 대한 조사)

  • Smoking has been identified not as a major risk factor for circulatory and respiratory diseases but also as causes of various oral diseases. A number of clinical studies and regional health surveys have found an association between smoking and poor oral health status and between smoking and prognosis of dental treatments. However, there is few studies about status of smoking cessation treatment and policies in dentistry in Korea. The purpose of this study was to investigate the smoking patterns of outpatients and outcomes of short-term smoking cessation treatment in dental hospitals in Korea and, subsequently, to seek further smoking cessation services in dentistry. This study was sponsored by Korean Dental Association (KDA) and department of culture and welfare. 825 dental patients were voluntarily participated in a 4-week smoking cessation program with nicotine patch and 297 participants of them completed on smoking-related questionnaires. All participants were recruited from outpatients of 11 dental university hospitals (primarily in the department of oral medicine, oral surgery and periodontology) in Korea during 3-month period from October 2009 to January 2010. The Questionnaires included demographics, duration of smoking, heavy smoking index (HSI), number of thinking of quit smoking, duration of stop smoking and reasons to smoking, awareness of smoking effects on oral health, and their success rate after 4 weeks of nicotine patch program was investigated. The statistical analysis was carried by SPSS version 18.0 program and Chi-square test. According to the results of this study, male in their 30s to 50s were the most prevalent of all the participants and duration of smoking increased with age. Attempt rate to quit smoking (Quit smoking) was the highest in 30s and 40s with duration of quit smoking ranging 1 to 3 months. Emotional stresswas the most frequently reported reason for smoking, followed by habit and pleasure in order. All age group showed high HIS over 71% and awareness of smoking effects on oral diseases such as oral soft tissue diseases, periodontal diseases and dental caries was found relatively high (50~60%) Periodontal implant was the main reason for participation in the smoking cessation services in dental clinics and the success rate of, 4-week nicotine patch program of all the participants was 29.4%, extremely low compared to that of medical clinics. Systemic education for dentists to be able to provide interventions to quit smoking including counseling with the 5As'and development of available measures for smokers is needed as considered that the low success rate of the smoking cessation services in dentistry could be explained mainly by lack of dentists' strategies, experience and attention. Awareness and attention of dentists should be emphasized and their participation be encouraged by long-term, multidisciplinary policies such as establishment of insurance fee, which would made a considerable progress in preventing smoking-related oral diseases and promoting public oral health.

Factors Related Smoking Cessation Attempts among Teenage Smokers (청소년 흡연자의 금연시도 관련 요인)

  • The purpose of the study is to analyze the relationship between the warning picture on a cigarette pack and non-smoking attempt, which is expected to contribute to the negative perception of smoking as a research subject about smoking adolescents. An online survey data of the Youth Health Behavior in 2018 has been used, and 3,722 adolescents who are currently smokers were selected for the study. For the measurement of variables, demographic sociology, health-related, and smoking-related factors have been revised, and multivariate binomial logistic regression analysis has been performed. The perception rate of cigarette warning pictures among adolescents who smoke currently is 84.7%, and among them, the attempt rate to quit smoking is 72.8%. As a result of the multivariate binomial logistic regression analysis, there is a meaningful relationship between adolescent smokers' attempts to quit smoking and whether they perceived cigarette pack warning pictures, and school grade year, academic performance, stress perception, and ease of purchasing cigarettes have been also expressed as meaningful variables. To be based on the result, it is necessary to manufacture to design a cigarette pack warning picture that can be easily recognized by smoking adolescents in the future.

Cure Rate Model with Clustered Interval Censored Data (군집화된 구간 중도절단자료에 대한 치유율 모형의 적용)

  • Ordinary survival analysis cannot be applied when a significant fraction of patients may be cured. A cure rate model is the combination of cure fraction and survival model and can be applied to several types of cancer. In this article, the cure rate model is considered in the interval censored data with a cluster effect. A shared frailty model is introduced to characterize the cluster effect and an EM algorithm is used to estimate parameters. A simulation study is done to evaluate the performance of estimates. The proposed approach is applied to the smoking cessation study in which the event of interest is a smoking relapse. Several covariates (including intensive care) are evaluated to be effective for both the occurrence of relapse and the smoke quitting duration.

Cigarette price rise induced change in youth smoking rate (담배가격 인상에 따른 청소년 흡연율 변화)

  • This study investigated changes in the youth smoking rate and smoking cessation rates before and after a cigarette price increase in 2015. Specifically, the changes in smoking rate, smoking cessation attempt rate, selection rate and amount of smoking were investigated using primary data pertaining to Korean Youth Risk Behavior obtained from a web-based survey of youth general characteristics, mental health, subjective grade, drinking and smoking related characteristics before and after a cigarette price increase. To accomplish this, 800 middle school and high school students in 9th (2013), 10th (2014), 11th (2015) were surveyed. The current youth smoking rate was slightly decreased from 9.7% in 2013 to 9.2% in 2014, while the smoking rate in 2015 decreased significantly by 15.2% after the cigarette price increased to 7.8%. These results confirmed that increasing cigarette price is very effective at decreasing youth smoking rate, providing motivation to stop smoking and preventing smoking. The prevention of smoking and inducement to stop smoking during adolescence is very important because the majority of teenage smokers continue smoking into their adulthood. In addition to expanding a completely non-smoking area and a comprehensive ban on tobacco advertising, promotion and sponsorship should be urgently sought, and the effect can be maximized when the WHO FCTC faithfully implements the already-founded treaty.
