  • 견갑하근 파열의 손상과 치료방식이 발전하면서 견갑하근의 중요성이 더욱 대두되었고 이에 견갑하근의 파열양상, 봉합방법 그리고 후상방 회전근 개 파열과의 관련성 등이 여러 저자들에 의해서 연구 되고 있다. 즉, 이전까지 관심의 대상에서 제외 되었던 견갑하근의 부분파열이 점차적으로 회전근 개 파열의 중요한 역할을 하며 상완 이두건초염과 오구상완 인대의 내측활차의 손상이 이러한 부분파열의 원인을 제공할 수 있음을 알 수 있다. 그러나 견갑하근 파열에 대한 정확한 원인 인자는 아직 불투명하다. 이에 본 교실에서는 견갑하근 파열의 치료와 견갑하근 파열과 동반 손상된 상완 이두건 손상및 탈구 등을 치료하고 이에 대해 문헌 고찰과 함께 손상 종류에 따른 치료방법을 살펴보고 이에 대해 알아보고자 한다.

Effect of Heat Stress of Extreme Heat Lever on Muscle functionand Muscle Injury Markers in Elderly Women (열 스트레스가 여성노인들의 근기능 및 근손상에 미치는 영향)

  • The purpose of this study was to examine the influence of extreme heat on muscle function and muscle injury marker in elderly women. The subjects of this study were eight post-menopausal elderly women without any metabolic disease. All eight subjects were asked to perform the knee joint isokinetic exercise using isokinetic equipment (cybex) in the laboratory and experimental temperature within laboratory was adjusted to two conditions: extreme heat temperature(33±0.5℃) and normal temperature(20±0.5℃) maintained in 50±3% humidity conditions. Each experimental exercise was monitored and analyzed the change of HSP70, LDH and CK. Muscular functions (peak torque, total work, percentage of peak torque body weight, fatigue index, average power and total work) were significant differences at exercise between temperatural conditions (p<.05). In extreme heat temperature, muscular injury markers (HSP70, LDH and CK) were increased, threfore resulted in significantly higher than normal temperature(p<.05). These results show that extreme heat temperature can decrease muscle function in elderly women.

Prediction of Long-term Residual Inter-laminar Shear Strength of Thermally Damaged GFRP Rebar (고온손상된 GFRP 보강근의 장기 잔존 계면전단강도 예측)

  • Mechanical properties of GFRP rebars significantly decrease due to high temperature as well as alkalinity of concrete. This study focuses on the long-term reduction of inter-laminar shear strength of pre-damaged GFRP rebars by high temperature. For this investigation, bare GFRP rebar specimens were exposed to $270^{\circ}C$ for 1hour and then immerged in alkali solution for several months and tested in shear. No thermally conditioned specimens were immerged and tested for the comparisons. In results, the reduction of thermally damaged GFRP rebars was greater than that of no thermally damaged ones. Based on the accelerated experimental test data, an polynomial equation is presented for prediction of long-term residual inter-laminar shear strength of GFRP rebars previously damaged by high temperature.

  • 목적: 축구는 현재 전세계에서 가장 대중화된 스포츠이며 육체적인 손상이 흔히 발생한다. 여러 손상 부위중에서 하지의 손상이 가장 흔한 것으로 알려져 있고 골, 근육, 건, 인대 등의 다양한 손상이 있을 수 있다. 저자들은 2002년 한국-일본 공동개최 월드컵 경기에 참가했던 8명의 선수에서 나타난, 하지의 다양한 근골격계손상의 분포와 MR소견에 대해 알아보고자 한다.

  • Acute traumatic shoulder injuries related to motor vehicle accidents and industrial accident has shown a steep increase recently. In regard to the causal relationship and the previous illness, the rotator cuff tear and SLAP lesion are the mostly debated among shoulder injuries related to trauma. Both the possibility of spontaneous occurrence related to their degenerative etiology and the discordance between the extent of injury and the symptom of the rotator cuff tear and SLAP lesion, make it difficult and obscure to estimate the extent of involvement of accident. Therefore, the Insurance Committee of Korean Shoulder and Elbow Society performed a questionnaire for the shoulder specialists to investigate their criteria about deciding the treatment modalities and SLAP lesion and reviewed literatures regarding the causal relationship between the accident and the rotator cuff tear and SLAP lesion. The committee recommended the diagnostic criteria to judge contribution of the accident on traumatic shoulder injuries, and to offer a guideline for disabilities.

  • The purpose of this study was to examine the effect of kinesio taping on muscle strength and changes of muscle fatigue and damage. 10 male subjects participated in 1-RM and isometric cybex muscle strength tests with and without taping application. Muscle strength (bench press, leg press) and extension (knee, shoulder) strength were significantly increased after taping, but there was no significant difference in flexion (knee, shoulder) strength. The concentration of fatigue factors (ammonia, phosphorous), muscle damage index substances (CK, LDH), IGF-I and creatinine were reduced after taping, but there were no significant differences.

  • Purpose: The purpose of this study is to investigate the relationship between rotator cuff tear and nerve injury, and prevalence of nerve injury using electromyographic study. Materials and Methods: From May 2004 to Feb. 2005, 19 cases, who underwent surgery for full-thickness rotator cuff tear, were evaluated for nerve injury using electomyogram instruments preoperatively. Rotator cuff tears caused by acute high energy trauma were excluded in this study. Mean age was 59 (range, 45-87) years and mean duration of symptoms was 45 (range, 1-360) month. Results: There were six nerve injuries (31.6%). All of them were incomplete brachial plexus injuries, and mainly postganglionic lesions. Four cases among them had minor trauma history. There were no significant differences in terms of cuff tear size, range of motion, pain score and functional score between groups with and without nerve injury. Conclusion: This study showed high prevalence (31.6%) of nerve injury in full-thickness rotator cuff tear. So careful physical examination and evaluation for nerve injury are needed in rotator cuff tear.

  • The hamstring muscle injury is a quite common sports injury which has a wide spectrum of treatment option that simple muscle strain can be treated conservatively otherwise total rupture of muscle or musculotendinous junction must be treated surgically. Among them, rupture of proximal hamstring muscle near at its origin site is quite uncommon and it should be treated surgically because it could cause several complications if not treated appropriately. In this case report, the authors report successful surgical treatment of acute rupture of proximal hamstring muscle and also would like to review the clinical presentation of this type of injury.

  • It has been reported that there are many cases of chronic shoulder injuries of golf players, however acute injuries at dominant tide are relatively rare. In this paper, we show a case of acute rotator cuff tears at dominant side due to bad swing in middle-aged woman. We suggest that golfers should be cautious with playing at winter to prevent acute rotator cuff tears by starting with a warm-up and correcting faulty swings.

Optical Microscopic Image Analysis for Damaged GFRP Rebar by Alkali and High Temperature Exposures (알칼리와 고온노출에 의한 GFRP 보강근 손상에 대한 현미경분석 연구)

  • In this experimental study, the characteristic of damages on GFRP rebar exposed to high temperature only and immerged in alkaline solution after the exposure to high temperature was analyzed through microscopic image analysis. The found microcrack and pores in resin matrix were quantitatively compared if there was effect of pre-exposure to high temperature. The damages, such as microcrack and pores in resin matrix, by alkali exposure were mainly found in rebar surface. On the other hand, the pores caused by high temperatures were extensively found in a section and had greater width than those caused by the alkali exposure. In results of the quantitative comparison, the accumulated length and widths of microcrack and pores in resin matrix in pre-exposed GFRP rebar to high temperature were respectively 1.5 and 1.4 times of those in the GFRP rebar only immerged in alkali solution. Therefore, the deterioration of resin matrix by the alkali exposure could be accelerated due to the pre-exposure to high temperature.
