Analysis of Whiplash by Rear End Collisions Using a Cervical Spine Model with Preloaded Muscles (근력을 적용한 경추 모델의 후방 충돌 해석)

  • Whiplash injuries often occur in motor vehicle collision accidents. This injury frequently occurs in the cervical region. However, the reason for this has not yet been clarified. In this study, a multi-body neck model with muscles was designed. Some muscles in the model were preloaded; these were previously determined using the concept of the follower load. Cervical spinal vertebrae, discs, and muscles were designed in accordance with the human cervical spine. The purpose of this study was to investigate the effect of preloads on muscles. The results imply that the whiplash model with preloaded muscles simulates practical situations more closely than models without preloads.

Treament of Sternal Dehiscence or Infection Using Muscle Flaps (근육편을 이용한 흉골열개 및 감염의 치료)

  • Background: Sternal infection or dehiscence after cardiac surgery through median sternotomy is rare. If suitable treatment is not performed for the complication, however, the mortality is high. For 12 patients with sternal dehiscence or infection, we performed wide excision of the infected and necrotic tissue and covered with muscle flap(s) to obliterate the mediastinal dead space. Material and method: Sternal infection or dehiscence occurred in 13 of patients who underwent cardiac surgery One patient, who died of cerebral infarction before the sternal complication was treated, was excluded in this study. The sternal wound complication occurred in 6 of patients with valve replacement and 6 of patients with coronary bypass surgery, respectively. Since 1991, 9 patients underwent definite surgical debridement and muscle transposition as soon as fever was controlled with closed irrigation and drainage. The necrotic tissue and bone was widely excised and the sternal dead space was eradicated with the single flap or the combined flaps of right pectoralis flap(turnover flap), left pectoralis flap(turnover flap or rotation-advancement flap), and right rectus muscle flap. Result : There was no mortality in 12 patients with coverage of muscle flap(s) for sternal infection or dehiscence The mean interval between the diagnosis of sternal complication and the myoplasty was 6.6$\pm$3.9 days. In 4 patients, one pectoralis muscle flap was used, and in 8 patients both pectoralis muscle flaps were used. For each 1 patient and 2 patients in each group, right rectus muscle flap was added. For the last 3 patients, a single pectoralis flap was used to eradicate the mediastinal dead space and the longer placement of the mediastinal drain catheter was needed. One patient, who had suffered from necrosis of left pectoralis flap(rotation-advancement flap) with subsequent chest wall abscess after coverage of both pectoralis flaps, was managed with reoperation using right rectum flap. Conclusion : Sternal dehiscence or infection after cardiac operation can be readily managed with wide excision of necrotic infected tissue(including bone) and muscle flap coverage after short-term irrigation of sternal wound. The sternal(mediastinal) dead space may be completely eradicated with right pectoralis major muscle flap alone.

The Analysis of Balance and muscle Activity according to Stair Height Gait Training in Adult Hemiplegia (성인 편마비환자의 계단 높이에 따른 보행훈련시 균형 및 근활성도 분석)

  • 본 연구는 뇌졸중으로 인한 성인 편마비 환자의 계단 높이에 따른 보행 훈련을 통하여 균형과 근활성도 변화를 분석하는데 목적이 있다. 뇌졸중으로 진단받은 편마비 환자 30명을 대상으로 5cm, 10cm, 15cm 높이의 계단 보행 훈련군으로 나누어 계단 보행 훈련을 실시하였으며, 임상적 균형능력 측정도구인 Berg Balance Scale(BBS), Timed up and go(TUG), Functional Reach Test(FRT)와 근활성도를 측정하였다. 연구결과 균형변화에서는 세 군간 계단 보행 훈련 전과 후의 균형 변화량이 통계학적으로 유의한 차이가 나타나지 않았으나(p>.05), 10cm 높이 계단 보행 훈련군에서 가장 큰 변화량을 보였다. 근활성도변화에서는 계단 오르기시 10cm 높이 계단 보행 훈련군에서 넙다리곧은근, 넙다리두갈래근, 장딴지근, 15cm 높이 계단 보행 훈련군에서 모든 근육들이 통계학적으로 유의한 차이를 보였으며(p<.05), 계단 내리기시 근활성도 변화에서는 5cm 높이계단 보행 훈련군에서 앞정강근와 장딴지근, 10cm 높이 계단 보행 훈련군에서 모든 근육들이, 15cm 높이 계단 보행 훈련군에서 넙다리두갈래근, 앞정강근, 장딴지근이 통계학으로 유의한 차이를 나타내었다(p<.05). 따라서, 높이에 따른 계단 보행 훈련은 편마비 환자의 임상적 균형 능력과 근활성도를 변화시키는데 영향을 줄 수 있으며, 특히 10cm 높이 계단 보행 훈련군에서는 다른 높이의 계단 보행 훈련군에서 보다 약간 상대적인 증가를 보였다.

Effects of Vocalization on Upper Extremity Muscle Activity during Reaching Task in Patients with Hemiplegia (발성이 편마비 환자의 팔 뻗기 시 상지근육의 근활성도에 미치는 효과)

  • This study examined the effects of vocalization on upper extremity muscle activity during reaching task in patients with hemiplegia. Thirteen persons with right brain stroke performed reaching to a cup under four concurrent speech conditions of vocalizing the word "Ah" with hemiplegic side. These four conditions are self-vocalization, external vocalization, imaginary vocalization, and no vocalization. The muscle activity(Biceps brachii, Triceps brachii, Middle deltoid, and Upper trapezius) were measured using MP150. Muscle activity was significantly higher under self-vocalization and external vocalization conditions compared to the muscle activity under imaginary vocalization and no vocalization conditions on triceps brachii muscle.(p<0.05). Triceps brachii muscle was highly correlated with biceps brachii muscle(r=0.777, p<0.05). The results suggest that self-vocalization and external vocalization can be used in facilitating upper extremity movements in patients with stroke. When working with patients with right hemispheric stroke, therapists might explore possibilities of using patient's self-speech to enhance the quality of upper extremity movement performance.

  • 본 논문에서는 하지 편마비 환자의 편마비 정도를 측정하여 사용하지 않으려고 하는 근육을 강제적으로 사용하도록 하여 하지를 재활할 수 있도록 훈련하는 시스템을 제안한다. 제안하는 시스템은 체중 부하 및 하지 슬관절의 움직임을 측정하여, 이를 통해 환자가 자신의 편마비 정도를 인식할 수 있도록 화면에 출력함으로써 환자가 피드백을 통해 강제적으로 편마비 하지를 사용 하도록 훈련 시킬 수 있다. 하지 슬관절(Knee Joint)의 움직임은 기존의 방법과는 달리 적외선 필터를 장착한 단일 카메라를 통한 모션 캡쳐 기술을 사용하여 획득한다. 또한, 재활 시스템에 가상현실 기술을 도입하여 무릎을 동시에 굽혔다가 펴는 기립훈련과 양측 하지를 이용한 보행훈련을 입력으로 가상공간을 탐험할 수 있게 함으로써 환자가 흥미롭게 재활훈련을 받을 수 있도록 하였다.

  • Objectives : The purpose of this study is to investigate the effectiveness of McKenzie Exercise of cervical part Method : A case-controlled clinical trial was searched for the effect of McKenzie Exercise on the cervical part. From 2015 to June 2021, 8 studies were selected from the RISS database published in Korea. The selected studies included an experimental group with McKenzie Exercise and a control group with general physical therapy and stretching Results : McKenzie Exercise, ROM, muscle activion, pain, posture were improved. Conclusion : This study summarizes the results of applying McKenzie Exercise of cervical part. This study suggests for cervical function who wants to intervene in McKenzie Exercise of cervical part.

Association Between Temporomandibular Disorders and Cervical Muscle Pressure Pain (측두하악장애와 경부근육 압통 간의 상관성)

  • Aims: The aims of this study were to identify the association between cervical muscle pain and TMD by pressure pain response, and to find cervical muscles showing moderate to severe pressure pain that are correlated with masticatory muscle pain. Methods: Patients(n=129, female 65.9%, mean age 28.8 years) answered a TMD questionnaire asking about headache, neck pain, emotional stress, sleep disturbance, parafunction habits, and pain intensity. A clinical examination of the masticatory system was performed. Of the neck muscles, (1) the upper sternocleidomastoid, (2) the middle sternocleidomastoid, (3) the upper trapezius, (4) the splenius capitis, (5) the semispinalis capitis, (6) the scalene medius, and (7) the levator scapulae muscles were examined by palpation. Pressure pain or tenderness of all palpation sites was scored from 0 to 3 according to the pain response. The variables of sum of pressure pain scores were calculated from pressure pain scores and were used for statistical analyses. Results: Eighty patients(62.0%) answered that they suffer from neck pain in the TMD questionnaire. More than 40% of sternocleidomastoid and upper trapezius examination sites showed moderate to severe tenderness in the cervical muscles, and 36% of middle masseter in the masticatory muscles. For the 129 patients, the sum of cervical muscle pain scores(mean=12.88, SD=8.06) and the sum of TMD pain scores(mean=5.36, SD=5.10) were moderately correlated($\rho$ = 0.502, P < 0.001). The sum of TMD pain scores tends to increase as the sum of cervical muscle pain scores increases(Y = 0.395${\cdot}$X, $R^2$ = 0.659, P < 0.001). In the patients with masticatory muscle disorders, the sum of sternocleidomastoid and upper trapezius pain scores(mean = 8.67, SD = 4.95) and the sum of temporalis and masseter pain scores(mean = 3.37, SD = 3.56) showed moderate correlation($\rho$ = 0.375, P < 0.001). Those two variables were in a proportionate relationship(Y = 0.359${\cdot}$X, $R^2$ = 0.538, P < 0.001). In a partial correlation analysis of the sum of unilateral pain scores, the sum of right cervical muscle pain scores and the sum of left cervical muscle pain scores showed the highest correlation(r = 0.802, P < 0.001). The sum of right TMD pain scores and the sum of left TMD pain scores were moderately correlated(r = 0.481, P < 0.001). For the twenty patients with unilateral TMD pain, the partial correlation coefficient between the sum of ipsilateral cervical muscle pain scores and the sum of contralateral cervical muscle pain scores was the largest(r = 0.597, P = 0.009). A partial correlation between the sum of primary TMD side pain scores and the sum of ipsilateral cervical muscle pain scores was 0.564(P = 0.015). Conclusions: TMD is associated with cervical muscle pain on condition of pressure pain response to palpation. Of the cervical muscles, sternocleidomastoid and upper trapezius frequently exhibit moderate to severe pressure pain, and they are closely related to the masticatory muscle pain. The characteristic of symmetric involvement of pain is prominent in cervical muscles; however, TMD can affect the level of cervical muscle pain to modify its symmetric nature.

Effects of Biofeedback Exercise Training in Hemiplegic Patients after Stroke (바이오휘드백을 이용한 운동훈련이 재가 편 마비 환자의 상지둘레, 악력, 근육강도, 관절운동범위, 일상생활활동에 미치는 효과)

  • Purpose: The purpose of this study was to investigate the effects of biofeedback exercise training on muscle activity and activities of daily livings (ADL) in hemiplegic patients. An experimental group consisting of 17 people, was given biofeedback exercise training for 30- 60 minutes per week for 5 weeks, while a control group consisting of 18 people, was given normal exercise with quasi-experimental design. Result: The results of the study show that biofeedback exercise is effective for improving muscle activity in hemiplegic patients, especially in the hemiplegic limbs. However, this study found no significant differences in ADL and IADL between the experimental and the control groups. It implies that ADL and IADL may not be improved for a short period of time, such as 5 weeks, for people with more than five years of hemiplegia. The study suggests that the effect of biofeedback exercise on ADL and IADL should be determined in hemiplegic patients in acute stage.

Management of Infected Median Sternotomy Wound by Muscle Flap -5 cases- (근육편에 의한 정중 흉골절개 감염의 치료 -5례 치험-)

  • Infection of a median sternotomy incision may result in a large, unsightly,unstable,and potentially fatal wound. During the past 8 years, 5 consecutive patients [ 4 male and 1 female ] had repair of infected sternotomy wound. We describe our current preferred techniques and the results we have achieved with them. As soon as the sternal infection was shown, operative wound was opened and irrigated more than 4 times a day with 0.5% Betadine iodine solution until the exudate became clean with no growth of bacteria. Operation was performed in one-stage, which consisted of aggressive debridement of the infected bone and muscle transposition. Reconstruction was with one-side or both pectoralis flaps in all patients and rectus abdominis in 2 patients. There was no mortality or morbidity within 30 days postoperatively. We conclude that early aggressive debridement and muscle transposition remain the treatment of choice for most patients with infected median sternotomy wounds.
