Ultrasound-Guided Injections in the Lumbar and Sacral Spine (요추 및 천추부에 대한 초음파 유도하 중재 시술)

  • Study Design: Literature review. Objective: Ultrasound-guided injections are a common clinical treatment for lower lumbosacral pain that are usually performed before surgical treatment if conservative treatment fails. The aim of this article was to review ultrasound-guided injections in the lumbar and sacral spine. Summary of Literature Review: Ultrasound-guided injections, unlike conventional interventions using computed tomography or C-arm fluoroscopy, can be performed under simultaneous observation of muscles, ligaments, vessels, and nerves. Additionally, they have no radiation exposure and do not require a large space for the installation of equipment, so they are increasingly selected as an alternative method. Materials and Methods: We searched for and reviewed studies related to the use of ultrasound-guided injections in the lumbar and sacral spine. Results: In order to perform accurate ultrasound-guided injections, it is necessary to understand the patient's posture during the intervention, the relevant anatomy, and normal and abnormal ultrasonographic findings. Facet joint intra-articular injections, medial branch block, epidural block, selective nerve root block, and sacroiliac joint injections can be effectively performed under ultrasound guidance. Conclusions: Ultrasound-guided injections in the lumbar and sacral spine are an efficient method for treating lumbosacral pain.

Dynamic MR Imaging in Gastric Cancer : Comparison Between Precontrast and Postcontrast Images (위암의 역동적 자기공명영상: 조영증강 전 후 영상의 비교)

  • Purpose: To analyze contrast-enhancement pattern of stomach cancer on dynamic MRI and to verify the efficacy of intravenous contrast agent in the evaluation of stomach cancer. Materials and Methods: Twelve patients with proven stomach cancer underwent dynamic MRI. By using l.OT scanner, we obtained precontrast FLASH images, and 30, 60, 90 second delay FLASH images after intravenous contrast injection. All patients ingested one liter of water and had intramuscular injection of Buscopan just before MR study. For quantitative analysis we measured signal to noise ratio (SiN) of stomach cancer on each image, and signal difference to noise ratio (SD/N) between cancer and intraluminal fluid, cancer and the pancreas. For qualitative analysis two radiologists evaluated lesion conspicuity on each image by grading system(grade 0, 1, 2, 3: poor, fair, good, excellent). Results: SiN of stomach cancer increased gradually by time(precontrast, 30, 60, 90 second delay: 38.7, 42.5, 57.4, 65.7). SD/N between cancer and intraluminal fluid significantly increased after contrast enhancement(l.24, 25.01, 39.30, 45.89). SD/Ns between cancer and the pancreas were 10.5, 9.33, 9.99, 10.66, respectively. In qualitative analysis, precontrast images were better than postcontrast images for delineation of stomach cancer. Postcontrast images showed clear endo-Iuminal side of stomach cancer, but outer margin of stomach cancer was more distinct on precontrast images. Conclusion: Precontrast MR images are better than postcontrast MR images in the depiction of stomach cancer. Intravenous contrast agent is not imperative in the evaluation of stomach cancer.

Immuno-specificity of Egg Yolk Antibodies against Bovine Rotavirus and Bovine Coronavirus causing Calf Diarrhea (송아지 설사병 주요원인체인 소로타바이러스와 소코로나바이러스에 대한 난황항체 생산 및 면역특이성 분석)

  • This study was performed to produce specific egg yolk antibodies against bovine rotavirus (BRV) and bovine coronavirus (BCV) that are major pathogens causing diarrhea in calves. Chickens were immunized with BRV and BCV intramuscularly in the breast muscle by injection 5 times at two weeks interval. At 6 weeks after the first immunization of BRV or BCV, cross reactivity of each serum derived from BRV- or BCV-immunized hens was tested. Each serum antibody against BRV or BCV was reacted with only specific BRV or BCV antigens. Serum and egg yolk-antibody titers of hens against BRV or BCVwere highest at 8~12 weeks after first immunization. Specific serum and egg yolk-antibody titers against BRV were about 104,000 and 107,000, respectively, and those against BCV were about 145,000 and 155,000, respectively. Hemagglutination inhibition titers in the immunized egg yolk antibodies against BRV and BCV were 5,120 and 1,280, respectively, and were ${\geq}8$ times higher than that in non-immunized control. These results suggested that the immunized egg yolk antibodies could effectively neutralize BRV and BCV.

Determination of Haloperidol Serum Levels in Psychiatric Patients with Gas Chromatography-Nitrogen Phosphorus Detection (GC/NPD를 이용한 정신분열증 환자의 혈중 Haloperidol 정량분석)

  • Analytical method of haloperidol (HAL) in serum which has been widely used in therapy of schizophrenic disorders is developed. Gas chromatography/nitrogen-phosphorus detection (GC/NPD) was used for this study. Bromoperidol was used as an internal standard and diethylether as a solvent of three-step extraction. The extraction yield in this procedure was $67.5{\pm}1.9%$ at 15 ng/mL. A good linear response in the range of 1~40 ng/mL was obtained with correlation coefficient of $r^2=0.999$. Detection limit was 0.5 ng/mL when 2 mL of serum was used. This method was applied for the analysis of HAL in serum of schizophrenic patients. After HAL decanoate (HD) was intaken as intramuscular route, HAL levels were determined at second week and forth week. From the result, the concentration of HAL at forth week appeared to 29.6% lower than those at second week. The present method showed low detection limit and high selectivity. Therefore it can be applied for the trace analysis of HAL in serum and the monitoring.

Clincal Studies on the Gangrenous Mastitis Caused by Mircrococus pyogenes var aureus in Goats (Micrococcus pyogenes var aureus에 의한 유산양(乳山羊)의 괴저성유방염(壞疽性乳房炎)에 관한 임상학적(臨床學的) 연구(硏究))

  • Six cases of gangreous mastitis of goats infected spontaneously were observed clinically and pathologically in Daegu and Daejeon district. and with strain isolated purely from the infected goats, the artificial infection to the animal was examined, the sensitivity of strain to the antibiotics was tested and clinical treatment was carried out. The results obtained are summarized as follows: 1. In the six cases approximately same clinical findings were observed as the previously published literatures on gangrenous mastitis of cattle, sheep and goats. 2. The micrococcus pyogenes var aureus was highly virulent strain which was the causative organism for the gangrenous mastitis by inoculating in the udder. 3. The gangrenous mastis was probably occured by the formation of thrombosis in veins of udder. 4. In the sensitivity test, the micrococcus pyogenes var aureus resited for penicillin in 2 cases among the 6 strains, but sensitived for streptomycin, chloromycin, oxyteracycline, erythromycin, achromycin, neomycin and kanamycin in other 4 and in all case. 5. The treatment for gangrenous mastitis may be extirpated the gangrenous region surgically in the case of unilaterally or locally affected, treated by muscle injection or teat-operation in the case of severely or diffusely affected and infused antibiotics up to teat canal or treated by mammary tissue injection in the case of slightly affected.

Effects of Coriandrum sativum L. on Lipid Contents in Streptozotocin-Induced Diabetic Rats (식이내 고수(Coriandrum sativum L.)의 첨가가 당뇨성 흰쥐의 혈장과 간의 지질함량에 미치는 영향)

  • This study was designed to investigate the effects of Coriandrum sativum L. on lipid metabolism in nondiaetic and stretozotocin-induced diabetic.Thirty two mael Sprague-Dawley rats weighting 108$\pm$13 g were divided into four groups as follow: control (C), leaf(L), seed (S) and root (R) groups. For four weeks of experimental periods, the C group were fed basal diet and the L, S and R groups were fed Coriandru sativum L. diets containing 5% dried leaf, seed and root of Coriandrum sativum L, respectively. diet intakes, weight gain and food efficiency were not significantly different among the four groups. The liver and heart weights of L, s and R groups were significantly different amount the control group. The levels of plasma insulin was significantly increased but the plasma glucose levels was not decreased in all Coriandrum group. The levels of plasma lipids was not significantly different among the four groups. the levels of plasma total cholesterol was significantly decreased in R groups. The levels of plasma and liver TBARS values were significantly decreased in R groups. The levels of plasma vitamin E were tendency to decrease in all coriandum groups. These results indicate feeding diet with coriander did not show the hypolipidemic effect but show the antioxidative effect.

Effects of Dental Fear on Depression Symptom and Stress in Adults (일부 성인의 치과공포가 우울 증상과 스트레스에 미치는 영향)

  • The purpose of this study was to determine the association of some adult dental fear and depression, and stress. Among the patients who came to the dental clinic for periodontal treatment, 68 patients who agreed to participate in the study and who met the selection criteria were included. Dental fear, depression symptom questionnaire, salivary cortisol, which can objectively evaluate blood pressure, pulse, and stress. The results of the analysis showed that depressive symptom was related to 'delayed treatment promise', 'cancellation of treatment appointment', 'increased muscle tension', 'rapid breathing when entering dentistry', 'fear when waiting in the waiting room', 'fear when you smell the hospital', 'fear when looking at the dental clinic', 'fear when seeing the anesthetic needle', 'fear when tooth whistle was heard', 'fear when teeth were removed', and 'total fear of dentistry'(p<0.05). Stress symptoms were higher in 'having sweated in the dentist', 'faster pulse', 'fear of entering the dentist' and 'total fear of the dentist'(p<0.05). Based on these results, it is necessary to study the effective methods to control the depressive symptoms and stress of patients who visit dental clinics, and it seems that continuous management is needed to prepare methods such as relaxation therapy for dental fear patients.

The Localization of the Excretory, Purified and Infected Antigenic Protein in the Tissue of Trichinella spiralis Larval Worm (선모충(Trichinella spiralis) 유충의 조직 내 배설, 분리 및 감염항원 단백의 분포)

  • In order to observe the localization of excretory, purified and infected antigenic protein in the tissue of Trichinella spiralis larvae, immunogoldlabeling methodology using IgG and protein A-gold complex was implemented. T. spiralis larvae obtained from rat muscle were initially cultured in medium, and secreted excretory antigen was collected for 1 or 3 days. Purified antigenic protein was obtained from homogenized T. spiralis larvae. Rabbits were then immunized with 1 or 3 days secreted excretory protein and purified 45 kDa protein, and IgG was purified from collected serum. Serum, against infected antigen, collected from rat on 1 and 4 weeks after infection with T. spiralis larvae, and IgG was purified from collected serum. T. spiralis larvae were embedded in Lowicryl HM20 medium. Then they were finally treated with immunized IgG and protein A-gold complex (particle size; 15 nm) and observed under electron microscope. In T. spiralis larvae tissue, the tissue antigen reacted with rabbit IgC antigen Day 1 secreted excretory protein, infected antigenic protein and purified 45 kDa protein. But different distribution pattern of labeled gold particles were observed. When Day 1 secreted excretoy protein was used, gold particle labeling was observed specifically on the cuticle, basal layer, esophagus interstitial matrix (EIM) and ${\alpha}_0,\;{\alpha}_1$ granules of stichocyte of the worm. In a separate group of tissue, the antigen reacted with rabbit IgG against Day 3 secreted excretory protein. Labeled gold particles were specifically distributed on the surface layer of cuticle, EIM and ${\alpha}_0$ granules of stichocyte of the worm. In case of using infected antigenic protein, gold particle labeling was specifically distributed on the cuticle and EIM of the worm. When purifed 45 kDa protein was used gold particle labeling was specifically distributed on the cuticle, basal layer, EIM and ${\alpha}_0,\;{\alpha}_1$ granules of stichocyte of the worm. Therefore, excretory antigens appeared to originate from the cuticle and ${\alpha}_0,\;{\alpha}_1$ granules of stichocyte for the first day but the cuticle layer associated with globular proteins and ${\alpha}_0$ granules of stichocyte after 3 days and infected antigens appeared to originate from the cuticle for 1 and 4 weeks after infection. These results suggest that excretory and infection specific antigens are secreted into the cuticle, basal layer, EIM and ${\alpha}_0,\;{\alpha}_1$ granules of stichocyte and 45 kDa protein may be contained these specific antigens.

Nonpharmacologic Pain Relief with Oral 25% Dextrose or/and Pacifier for Newborn Infants (신생아에서 비약물적 통증조절을 위한 25% 경구 포도당과 인공 젖꼭지 사용의 효과)

  • Purpose: The goal of this study was to compare the efficacy of oral 25% dextrose treatment or/and pacifier for analgesia in healthy newborns during intramuscular injection of a hepatitis B vaccine. Methods: A prospective, randomized, partially blinded, clinical trial was performed in 132 healthy newborns. They were assigned randomly to 4 treatment groups: control group (2 mL distilled water), dextrose group (2 mL 25% dextrose), pacifier group, dextrose+pacifier group (pacifier coating with 25% dextrose) during intramuscular injection of hepatitis B vaccine. For all groups, Neonatal Infant Pain Scale (NIPS), Neonatal Facial Coding System (NFCS), Premature Infant Pain Profile (PIPP) scores were evaluated before the injection, during the injection, and at 2 minutes after the injection. Pain scores were compared among the 4 groups. Results: Maternal and neonatal characteristics were similar among the 4 groups. 25% dextrose treatment led to lower NIPS pain scores during injection (6.4${\pm}$0.9 vs. 5.5${\pm}$1.7, P=0.01) and after injection (1.6${\pm}$2.0 vs. 0.6${\pm}$0.9, P=0.01) and NFCS pain scores after injection (1.5${\pm}$2.3 vs. 0.7${\pm}$0.8, P=0.04) than control group. The number of neonates who feel the pain (indication of scores: NIPS${\geq}$4, NFCS${\geq}$3) decreased (9 (23.1%) vs. 0 (0%), P=0.04 via NIPS, 7 (17.9%) vs. 0 (0%), P=0.02 via NFCS). However, all treatment groups did not decreased PIPP scores, compared with the control group. Conclusion: Oral 25% dextrose is effective than distilled water or using pacifier with or without 25% dextrose in reducing pain during intramuscular injection of hepatitis B vaccinations. Further study based on this preliminary study need about nonpharmacologic management of pain in newborns.

Multiple Linear Analysis for Generating Parametric Images of Irreversible Radiotracer (비가역 방사성추적자 파라메터 영상을 위한 다중선형분석법)

  • Purpose: Biological parameters can be quantified using dynamic PET data with compartment modeling and Nonlinear Least Square (NLS) estimation. However, the generation of parametric images using the NLS is not appropriate because of the initial value problem and excessive computation time. In irreversible model, Patlak graphical analysis (PGA) has been commonly used as an alternative to the NLS method. In PGA, however, the start time ($t^*$, time where linear phase starts) has to be determined. In this study, we suggest a new Multiple Linear Analysis for irreversible radiotracer (MLAIR) to estimate fluoride bone influx rate (Ki). Methods: $[^{18}F]Fluoride$ dynamic PET scans was acquired for 60 min in three normal mini-pigs. The plasma input curve was derived using blood sampling from the femoral artery. Tissue time-activity curves were measured by drawing region of interests (ROls) on the femur head, vertebra, and muscle. Parametric images of Ki were generated using MLAIR and PGA methods. Result: In ROI analysis, estimated Ki values using MLAIR and PGA method was slightly higher than those of NLS, but the results of MLAIR and PGA were equivalent. Patlak slopes (Ki) were changed with different $t^*$ in low uptake region. Compared with PGA, the quality of parametric image was considerably improved using new method. Conclusion: The results showed that the MLAIR was efficient and robust method for the generation of Ki parametric image from $[^{18}F]Fluoride$ PET. It will be also a good alternative to PGA for the radiotracers with irreversible three compartment model.
