Ultrastructural Studies of Effect of Monosodium Glutamate on the Epiphyseal Plate of Femur in Young Chicken (Monosodium Glutamate가 초생추 대퇴골 근위골단에 미치는 영향에 관한 투과 및 주사전자현미경적 연구)

  • This study was carried out to investigate the ultrastructural changes of the proximal epiphyseal plate of the femur in young chickens that had been treated with monosodium glutamate(MSG). Eighty 1-day old broiler chickens(Hubbard strain) were divided into control and experimental groups. The experimental group received daily administration of MSG(3mg/g of body weight in 0.75% saline) per orally for 1, 3, 6, 9, 12, 15, 18 and 21 days, and were sacrificed with exanguination. The control group received an equal volume of 0.75% saline. For the transmission electron microscopy, the prehypertrophic cartilage zone of epiphyseal plate was cleaved, fixed with 2% glutaraldehyde(containing 0.2% ruthenium red), postfixed with 1 % osmium tetroxide, embedded in Epon 812, and stained with uranyl acetate and lead citrate. For the scanning electron microscopy, the calcified zone of epiphyseal plate was cleaved and coated with gold palladium. The results obtained were as follows; 1. On transmission electron microscopic examination, the sacculation decreased from 12 day to 21 day MSG administrated groups, and the vesiculation decreased in 18 and 21 day MSG administrated groups in rough endoplasmic reticulum of chondrocytes in prehypertrophic cartilage zone. The ruthenium red binding particles in pericellular rim, territorial matrix and interterritorial matrix increased from 9 day to 21 day MSG administrated groups, but the crystalloid materials decreased. 2. On scanning electron microscopic examination, the trabecular formation and calcospherites of calcification zone decreased in 18 and 21 day MSG administrated groups. The resorption cavities widened from 15 day to 21 day MSG administrated groups.

  • Clear cell chondrosarcoma is a rare, low-grade variant of chondrosarcoma that comprises approximately 2% of all chondrosarcomas. This tumor usually involves the epiphysis and epimetaphysis of long bones, especially the proximal part of the femur or humerus, whereas involvement of the scapula is rare. It occurs at any age, but the peak is third to fifth decade, and is rarely seen in the first and second decades of life. Histologically, tumor cells with abundant clear cytoplasm and benign giant cells are usually found. We report on a case of clear cell chondrosarcoma of the scapula in an 8-year-old girl.

  • Clinical outcome of malignant tumor heavily depends on early diagnosis and proper management of the patient. But it's very difficult to diagnose malignant tumor in early stage because of it's vague symptoms and inadequate evidences of malignancy in radiologic studies and biopsy. Malignant tumor mistaken for infection or fracture initially will undergo inadequate treatment and that will influence on it's prognosis. So it's very important to rule out the possibility of malignancy by repeated evaluation if it's diagnosis is not definite. We report 1 case of osteosarcoma initially mistaken for stress fracture and 3 cases of osteosarcoma, malignant lymphoma and Ewing's sarcoma initially mistaken for tuberculosis and review the literature.

Changes in Bone Mineral Density and Biochemical Markers of Bone Turnover in Young Women: A 1-Year Follow-up Study (청년기 여성의 골밀도와 골대사지표의 변화: 1년 추적조사)

  • This study investigated the changes in bone mineral density(BMD) and biochemical bone turnover markers over 1 year in healthy young college women. In comparison of changes in BMD at the lumbar spine, proximal femur (trochanter, femoral neck, Ward's triangle), and whole body over 1 year, there was only a significant difference in that at forearm. However, serum osteocalcin, a marker of bone formation, and urinary deoxypyridinoline, a marker of bone reabsorption, were significantly changed over 1 year. These findings indicate that although BMD does not significantly change in early young adult, the changes of bone metabolism markers seem to mean active bone turnover in early young adult women.

Structural analysis of trabecular bone using Automatic Segmentation in micro-CT images (마이크로 CT 영상에서 자동 분할을 이용한 해면뼈의 형태학적 분석)

  • This paper proposes an automatic segmentation method of cortical bone and trabecular bone and describes an implementation of structural analysis method of trabecular bone in micro-CT images. The proposed segmentation method extract bone region with binarization using a threshold value. Next, it finds adjacent contour lines from outer boundary line into inward direction and sets candidate regions of cortical bone. Next it remove cortical bone region by finding the candidate cortical region of which the average pixel value is maximum. We implemented the method which computes four structural indicators BV/TV, Tb.Th, Tb.Sp, Tb.N by using VTK(Visualization ToolKit) and sphere fitting algorithm. We applied the implemented method to twenty proximal femur of mouses and compared with the manual segmentation method. Experimental result shows that the average error rates between the proposed segmentation method and the manual segmentation method are less than 3% for the four structural indicatiors. This result means that the proposed method can be used instead of the combersome and time consuming manual segmentation method.

  • Chondromyxoid fbroma (CMF) is a rare, benign tumor of the bone that represents fewer than $2\%$ of all benign tumors of bone. CMF is most often found in the long tubular bones, especially the tibia and femur near the knee joint. Less common sites included the pelvis, fibula, calcaneus and rib. A 54-year-old male patient presented to us with history of swelling and mild, intermittent local pain without any rise in overlying skin temperature in lateral portion of left 7th rib for one-month duration, which was diagnosed as benign rib tumor by plain chest X-ray and CT scan, and treated successfully by excision of rib with good result. Pathologic diagnosis of this tumor was CMF. Without any medical therapy, there was no evidence of recurrence after operation. We report this case and follow-up of the patient.

  • Developmental dysplasia of the hip broadly includes inadequate development of the hip joint involving the acetabulum or proximal femur, or both. Although ultrasonographic studies in neonates have greatly lowered the frequency of neglected or operatively treated cases, its sensitivity is less than desired. Hip dysplasia without subluxation is commonly diagnosed incidentally and strongly related to degenerative arthritis in females after the 4th decade. Hip dysplasia with subluxation shows symptoms through various periods, depending on its severity, especially for women with onset during pregnancy. A complete physical examination and early treatment for neonates are extremely important for obtaining satisfactory outcomes. To avoid underdiagnosis and to serve appropriate treatment on time, the authors recommend examining any suspicious hips in infants under two years of age. The study will discuss the diagnosis and primary treatment of developmental dysplasia of the hip.

Treatment of Osteoid Osteoma (유골 골종의 치료)

  • Purpose : The purpose of the current study was to report the results of curettage and en bloc excision as well as to introduce how to excise the nidus percutaneously with Halo-mill. Material and Methods : Twenty patients(14 men and 6 women) were evaluated, who had operative treatments after diagnosed as osteoid osteoma from March 1990 to January 1998. These patients ranged in age from 7 to 42 years(average: 20.8 years). Locations were 9 femurs, 6 tibias, 2 vertebras, 1 ulna, 1 maxilla and 1 skull. Nine femoral lesions included 5 proximal metaphysis, 2 neck and 2 diaphysis, while 5 tibial lesions included 3 diaphysis, 1 proximal metaphysis and 1 distal metaphysis. We used simple radiography, bone scan, CT and MRI for the accurate diagnosis and localization. As for surgical treatments, while excision and curettage had to need open-exposure of lesion, the percutaneous excision of nidus did not need openexposure : guided Halo-mill into K-wire inserted to nidus under image intensifier. Results : Simple radiography showed that 10 cases had typical nidus and others had only cortical sclerosis. Bone scan was performed at 14 cases and all had hot uptake except one case. We used CT in 10 cases and MRI in 4 cases as diagnostic methods, of which 1 case didn't reveal nidus at CT. Surgical treatment consisted of 6 curettages, 11 excisions, 2 percutaneous excisions with halo-mill and 1 total elbow arthroplasty. We used 7mm sized Halo-mill. During the follow-up period, all patient relieved symptoms and there were no recurrences. All had histologically typical findings except one which had hyperostosis without nidus. Conclusion : Complete removal of the nidus is the most important factor in the treatment. We could excise the nidus percutaneously in 2 cases with the minimal injury to surrounding soft tissues. If we could evaluate the precise location, size of nidus and percutaneous acccesibility, the percutaneous excision of nidus with Halo-mill could be an alternative method as a treatment of osteoid osteoma.

The Reconstruction with Extracorporeal Irradiated Autograft for Osteosarcoma of Extremities (사지 골육종에서 체외 방사선 조사를 이용한 재건술)

  • Purpose: With advances in various treatment modalities, limb salvage surgery has been commonly used in osteosarcoma of extremities. An alternative method for skeletal reconstruction is reimplantation of the tumor bearing bone following extracorporeal irradiation (ECI). We report the long-term results of ECI autograft in aspect of the oncological and functional outcomes, and complications. Materials and Methods: We retrospectively reviewed 31 osteosarcoma patients who underwent reconstruction with ECI between July 1995 and January 2006. There were 24 males and 7 females with a mean age of 24 (7-74 years) and a mean follow-up of 117 months (17-177 months). Twenty-five cases were reconstructed with ECI autograft, 6 cases with ECI autograft-prosthesis composite. The pathologic subtypes were conventional in 29 cases, periosteal in 1 case, and parosteal in 1 case. The most common location of tumor was distal femur (15 cases) followed by humerus (3), proximal fibula (3) and proximal tibia (3). Musculoskeletal Tumor Society (MSTS) score was used for functional evaluation. The overall survival rate, local recurrence, complications were analyzed. Results: The overall survival rate was 80.6% and the disease-free survival rate was 64.5%. Five patients died of distant metastasis. One patient required above-knee amputation due to local recurrence. All of them, twenty-three complications occurred, which included nonunion in 7 cases, deep infection in 5 cases, joint instability in 4 cases, metal failure in 2 cases, Limb-length discrepancy (LLD) in 2 cases, periprosthetic fracture in 1 case, epiphyseal collapse in 1, local recurrence in 1 case. The mean MSTS functional score was 62.5%. Conclusion: Extracorporeal irradiated autograft can be achieved relatively good result in aspect of oncological and functional aspect, but is needed to be additional research about occurring many complications. The reconstruction with ECI after intercalary or fragmentary resection is effective reconstruction in aspect of oncological and functional result, complications.

MRI Findings of the Repaired Anterior Cruciate Ligament (전방 십자 인대의 일차 봉합술 후 MRI 소견)

  • Purpose: To investigate MRI findings of the repaired anterior cruciate ligament (ACL). Materials and Methods: Seventeen of arthroscopic ACL primary repair with sutures pull-out technique were followed for 21.4 months (range: 12 to 60 months). Stability was assessed with physical examination and KT-1000 arthrometer (MED metric, San Diego, CA) and postoperative MRI checked with time. The patients were divided into 2 groups according to the location of tear which was defined with the location of remained synovial sleeve. Group I (11 patients) comprised that the tear was located within proximal 1/3 of ACL substance and group II (6 patients) comprised below proximal 1/3. MRI findings of the repaired ACL were evaluated by its course, sharpness, thickness and signal intensity using 3 grade system and correlated with its location of tear. Results: In all cases, Lachman test and flexion-rotation drawer test were negative, pivot-shift test was less than grade 1 and the mean side-to-side difference by use of KT-1000 arthrometer was 1.4 mm (range: -1.0 to 2.5 mm). The overall continuity of the repaired ACL was well maintained in all cases. However, mild sagging was observed in 10 cases(58.8%), mild obscure contour in 6 cases (35.3%), increased thickness in 8 cases (47.1%) and slight increased signal intensity in 5 cases (29.5%). There was no statistical significance in all parameters between 2 groups. And a focal defect at the femoral attachment site in sagittal image was observed in 7 cases (41.2%) of all patients which comprised 2 cases (18.2%) of group I and 5 cases (83.3%) of group II. It was observed more frequently in group II with statistical significance (p=0.035). Conclusion: Some abnormal MRI findings such as mild sagged course, obscure contour, increased thickness and signal intensity, the focal defect at femoral attachment site could be observed even though the stability was well maintained clinically. We thought that the focal defect was affected by the location of tear of ACL.
