What are Valuable Positive Signs of Supraspinatus Test for Diagnosis of Torn Rotator Cuff? - Comparison of Pain and Weakness in "Empty Can Test" and "Full Can Test" - (회전근 개 파열의 진단을 위한 극상근 검사에서 유용한 양성 징후는 무엇인가? - "Empty can test"와 full can test"에서 통증과 근력 약화의 비교 -)

  • Purpose: To investigate the validity of positive signs of supraspinatus test. Materials and Methods: The empty can test and full can test were performed on 200 shoulders which were diagnosed with magnetic resonance imaging or surgical findings as full thickness tear, partial thickness tear and no tear. Presence of pain, weakness, pain or weakness, and both pain and weakness were recorded as positive signs separately. The two tests with positive signs were compared and analyzed. Results: Pain and weakness were severity-dependent, and the empty-can test had a higher incidence of pain. The sensitivities of the two supraspinatus tests in all positive signs were higher when including partial-thickness tears in the tear group; however, their specificities were higher when excluding partial-thickness tears. The sensitivities of an empty-can test in 'pain', 'pain or weakness', 'both pain and weakness' were higher than those of the full-can test, otherwise, the specificity of the full-can test for 'pain' and 'pain or weakness' were higher. Concordance rate between 'pain or weakness' and 'pain' was the highest in all categorization. Conclusion: Both empty can and full can test were valuable for detecting torn rotator cuff.

  • 본 연구는 초기 편마비 환자의 손목에서 표면근전도 분석을 통해 근수축 개시 및 종료의 특성들을 알아보고, 임상적인 치료방법의 기초를 제안하고자 실시하였다. 연구대상자는 원주기독병원에 뇌졸중으로 입원한 환자 중 발병 후 3개월 미만인 13명과 원주시에 거주하는 대조군 7명이었다. 근수축 개시 및 종료의 지연은 표면근전도를 이용하여 손목굽힘근과 손목펴짐근에서 손목관절의 굽힘과 폄동작 시 3초의 근전도 신호음에 따라 가장 빠르고 강하게 최대 등척성 수축과 이완을 하여 신호를 수집하였다. 그 결과 편마비 환자의 환측은 건측과 대조군에 비해 손목관절 굽힘과 폄동작에서 근수축 개시및 종료가 유의하게 지연되었으며, 개시보다 종료가 더 유의하게 지연되었다. 따라서 초기 뇌졸중 환자의 근약화는 근육의 개시 및 종료의 반응시간 지연에 영향을 준다고 볼 수 있다. 앞으로 운동조절과 연관되어 기능적인 회복을 유도할 수 있는 근수축의 민첩성 훈련과 근섬유 동원의 효율성을 증진시키기 위한 치료방법들이 연구되어야 할 것이다.

Equine Polysaccharide Storage Myopathy in an American Paint Horse (American Paint Horse에서의 다당류저장성근질환)

  • A 4-year-old, American Paint horse gelding with recurrent episodes of exertional rhabdomyolysis was diagnosed with equine polysaccharide storage myopathy (EPSSM). The common clinical signs were muscle weakness, loss of muscle mass, exercise intolerance, difficulty holding up limbs for farrier, sensitivity of grooming, cramping with reluctance to move and the inability to rise. Through histological examination of skeletal muscle specimens, this case was confirmed of EPSSM.

  • 최근 산업사회의 급속한 환경변화는 근로자의 상병구조에서도 나타나 작업이 단순반복화 되며 다양한 작업공구를 반복적으로 사용하게 되어 작업속도와 작업량이 많아지고 운동부족으로 인한 근력의 약화 등에 의해서 근골격계질환이 급속히 증가하고 있다. 이로 인한 근로자의 삶의 질은 물론 결근에 의한 노동력 손실, 제품불량 증가, 산재보상비용 지출증가 등으로 성장을 둔화시키고 사회$.$경제적으로 문제의 심각성을 더해주고 있다. 최근 노동부가 발표한 산업재해통계를 보면 2002년도 산업재해 중 업무상질병자는 5,417명으로 전년대비 136명 감소하였으나, 이중 직업성 근골격계질 환자(신체부담작업 및 요통)는 1,827명으로 전년대비 193명(11.8%) 증가한 것으로 나타났고, 그 발생양상이 집단적이라는 특성을 갖고 있다. 이처럼 직업성 근골격계질환의 업무상 질병에서 차지하는 비율이 증가추세에 있고 대부분의 선진국에서도 과거에는 각종 화학물질에 의한 직업병이 상위를 차지해 오다가 근래에는 동 질환이 업무상질병의 상당부분 이상으로 차지하고 있다. 미국의 경우 1998년 한해동안 253,300건(64.2%) 발생하였고 이 가운데 자동차 관련업종이 가장 많은 비율을 점유하고 있다.(중략)

Medial Retracted Large Rotator Cuff Tears (내측으로 퇴축된 대범위 회전근 개 파열)

  • Medially retracted large-sized rotator cuff tears includes large-sized tears, massive tears and irreparable tears. Generally arthroscopic repair or open repair of rotator cuff tears is used in reparable tears. However, arthroscopic repair requires long period practice and endurance. In irreparable tears, arthroscopic debridement, partial repair, latissimus dorsi transfer and retrograde arthroplasty can be the option. Arthoscopic debridement gives temporal relief who experienced improvement in pain and increase in range of motion after subacromial local anesthetic injection. Also arthroscopic partial repair gives good results in irreparable cases, especially in suprascapular nerve traction neurapraxia. Tendon transfer can be used in mild to moderate muscle weakness in shoulder abduction for long term treatment. Pectoralis major transfer can be used in anterosupeior tears and latissimus dorsi transfer can be used in posterosuperior tears. Reverse shoulder prosthesis is used in extreamly weakened shoulder pseudoparalysis. The authors discussed the method of arthroscopic repair in irreparable tears. The debridement, partial repair, and tendon transfer could be used in medially retracted large-sized rotator cuff tears.

A Case Report on Snapping Hip Patient Treated by Chuna Manual Therapy for Meridian Sinew System (경근추나 치료 후 호전된 발음성 고관절 환자 치험례)

  • Objectives : This study is designed to evaluate the effect of conservative oriental medical treatment using Chuna manual therapy for meridian sinew system for snapping hip patient who have hip joint movement system impairment. Methods : 28-year old snapping hip patient who have hip joint movement system impairment was treated with conservative oriental medical treatment using Chuna manual therapy for meridian sinew system. The improvement of the patient was evaluated by Numerical rating scale(NRS), Snapping Sound Degree(SSD), Range of Motion(ROM). Results: After 13 times treatment, the patient had significant improvement in Numerical rating scale(NRS), Snapping Sound Degree(SSD), Range of Motion(ROM). Conclusions: If hip joint movement system impairment cause a snapping hip, we can treat with Chuna manual therapy for meridian sinew system.

  • The purpose of the study was to evaluate the effects of sling exercise program on muscle activity and cervical spine curvature of person with forward head posture. The subjects, 25 students, were divided into two groups. This experiment was intended to examine the muscle activity and cervical spine curvature while the subjects had performed the exercises to do for 2 times/week and 4 weeks. The factors of FHP were measured cranial vertical angle, cranial rotation angle, muscular activity and cervical spine curvature. Cervical spine curvature measured craniovertical, craniocervical, cervicohorizontal and upper cervical angle. Collected data were statistically analyzed by SPSS 10.0. After experiment for 4 weeks period, results were as following: Both group was significant difference of the results according to the CVA. Exercise group are more effective to increase muscle activity (p<.05), but cervical spine curvature was no significant difference. This result, we could find out there was a significant correlation between sling exercise and muscle activity, CVA and CRA.

Controversy in Pathophysiology of Rotator Cuff Tear: Degenerative Tear (회전근 개 파열의 병리 생태학에서의 논쟁점: 퇴행성 파열)

  • The pathophysiology of rotator cuff tears and the progression of asymptomatic tears to symptomatic tears are yet unclear and much controversy. It is likely to involve a number of factors such as a genetic predisposition, extrinsic impingement from structures surrounding the cuff and intrinsic degeneration from changes within the tendon itself. Degenerative changes in the rotator cuff with aging seem to be related to the anatomic and mechanical environment of the rotator cuff. The histopathologic appearance of rotator cuff tendon rupture specimens demonstrates a consequence of degenerative changes at the site of tendon insertion into bone. It weakens the tensile strength of the tendon. Is the process of degeneration intrinsic or extrinsic in nature? I suggest that degeneration is intrinsic and not caused by extrinsic factors. Even though, rotator cuff tear may be secondary to multiple factors, I believe that primary cause of rotator cuff tears is preexisting degenerative change.

Revision Rotator Cuff Repair (회전근 개 봉합술 후 재수술)

  • The primary purposes of revision repair for a failed rotator cuff repair are a relief of pain and functional improvement. Therefore, revision repair is most proper in patients with the functional deficit accompanied with the shoulder weakness as well as the persistent pain. The important factor that is considered in revision repair is a quality of torn cuff. Especially, Care must be taken to ensure that the revision repair is possible, considering the size of tendon defect, atrophy of the muscle, fatty infiltration and extent of the retraction of tendon. Revision repair of a failed rotator cuff repair is more difficult, and the functional results are less satisfactory than those of primary repair, because excessive bursal scarring and tendon retraction may be exhibited, a large or massive tear is often detected, tear has usually been present for a long time, and a quality of muscle-tendon may be poor. So, we discuss our experiences related to revision repair after a failed cuff repair that has been recently introduced through the articles.

Natural History of Rotator Cuff Disorders (회전근 개 질환의 자연경과)

  • Continued research needs to be devoted to understanding the natural history of rotator cuff tears. Recent studies have shown progression of tear, symptomatic flare-ups and irreversible change in rotator cuffs managed nonsurgically. These data allow the grouping of patients with rotator cuff tears into three categories based on risk-benefit ratios. Nonsurgical care should be maximized for patients with impingement symptoms only, with partial-thickness tears, with chronic tears, and for the elderly patients. The proper selection of candidates for nonsurgical management should lead to the high success rate quoted by multiple authors. Even though conservative treatment is necessary, early surgery should be considered for tears that are acute, small or medium, associated with shoulder loss of function, or occur in a younger patient. Prolonged nonsurgical care in these patients risks tear propagation and irreversible changes to the cuff, which may complicate rotator cuff repair.
