Pathophysiology of the Gastroesophageal Reflux Disease (위식도역류의 병태)

  • 이상에서 고찰한 위식도역류의 병태를 간략히 요약하면 다음과 같다. 위액에는 염산, 펩신, 담즙산과 췌효소등을 포함하고 있어 역류된 위액은 상부 기관식도관에 자극을 주거나 손상을 줄 수 있으며 개개인의 점막 상피의 저항도와 적절한 타액의 분비는 위액에 의한 손상의 정도를 결정하는 중요한 요소로 알려져 있다. 위식도역류로 인한 많은 증상이나 소견은 두가지의 기전으로 나타나서 첫째, 역류된 위액이 직접 조직에 영향을 주거나 둘째, 간접적으로 식도 하부에 분포된 미주신경에 의한 연관자극에 의해 일어나는데 위식도역류의 증상 중 두부, 경부, 심장 및 폐장의 증상은 이들 장기에 미주신경의 일부가 같이 분포하여 일어나는 간접적인 기전이다. 하부식도괄약근(lower esophageal sphincter)의 기능은 역류를 막는 가장 중요한 요소로 알려져 있어 간헐적으로 하부식도 괄약근의 압력이 떨어지면 역류가 일어나게 된다. 정상적인 식도의 연동운동은 식도에 역류된 위내용물을 위로 제거되는데 중요하며 위식도역류 환자에서 흔히 연동운동이 저하되거나 비정상적인 연동운동을 관찰할 수 있다. 또한 위내용물이 소장으로 비워지는 시간이 연장되어 위내용물이 위내에 축적되는 것도 위식도역류의 원인의 하나로 생각된다. 이러한 위식도역류의 병태를 연구하는 것은 이 질환의 이해에 도움을 줄 것으로 생각된다.

Effect of joint mobilization on improvement of knee pain, isokinetic strength, muscle tone, muscle stiffness in an elite volleyball player with knee injury (무릎손상 엘리트 배구선수에 관절가동운동이 무릎통증, 등속성 근력, 근긴장도, 근경직 개선에 미치는 효과)

  • This case study identified the effects of joint mobilization on knee pain, isokinetic strength, muscle tone, and muscle stiffness in an elite volleyball player with a knee injury. The subject had experienced cartilage defects of the left knee joint and underwent surgery to correct the condition. The patient complained of continuous pain in the left knee joint in daily life in addition to pain during exercise. The study was conducted from August 5 to 12, 2015 and joint mobilization was applied to the left knee joint for 15 minutes once a day for 8 days. Knee pain was measured using a visual analogue scale, and the concentric peak torque of the quadriceps and hamstring muscles was measured using an isokinetic muscular strength measurement device. The muscle tone and stiffness of the rectus femoris muscle, vastus medialis, and vastus lateralis on the injured side were measured using a myotonometer. All the measurements were conducted before and after the intervention. Joint mobilization was effective in reducing knee pain on the injured side, increasing the concentric peak torque of the quadriceps and hamstring muscles on both sides, and increasing the muscle stiffness of the quadriceps muscle on the injured side. Concentric peak torque of the quadriceps muscle on the injured side increased a great deal as the number of joint mobilizations was increased, largely diminishing the difference in concentric peak torque between the normal side and injured side. On the other hand, joint mobilization was ineffective in improving the hamstring to quadriceps strength ratio on the injured side. While this study suggests that joint mobilization can be an effective intervention to improve the knee pain, isokinetic strength, and muscle stiffness of elite volleyball players, it should be performed alongside training for an appropriate strength ratio.

  • Scapulothoracic dissociation is a rare injury caused by blunt force or massive traction to the anterolateral shoulder girdle with disruption of scapulothoracic articulation. This injury is characterized by massive soft-tissue swelling of the shoulder with intact skin, lateralization of damaged scapular. It frequently is accompanied by various musculoskeletal injuries and neurovascular injuries. 33-year-old man sustained a scapulothoracic dissociation without fracture and Vascular injuries during arresting. We report diagnostic method, clinical finding and non-operative management with literatures.

  • Skeletal muscle tissues feature cellular heterogeneity, including differentiated myofibers, myoblasts, and satellite cells. Thanks to the presence of undifferentiated myoblasts and satellite cells, skeletal muscle tissues can self-regenerate after injury. In skeletal muscle regeneration, the collective motions among these cell types must play a significant role, but little is known about the dynamic collective behavior during the regeneration. In this study, we constructed in vitro platform to visualize the migration behavior of skeletal muscle cells in specific conditions that mimic the biochemical environment of injured skeletal muscles. We then visualized the spatiotemporal distribution of stresses arising from the differential collectiveness in the cellular clusters under different conditions. From these analyses, we identified that the heterogeneous population of muscle cells exhibited distinct collective migration patterns in the injury-mimicking condition, suggesting selective activation of a specific cell type by the biochemical cues from the injured skeletal muscles.

  • Recent earthquake, such as the Northridge(1994), the Kobe(1995) and the Izmit(1990) earthquakes, gave serious damage in various buildings and bridges by the vertical seismic component. Most of the seismic designs neglect the vertical seismic component for usual frame structures. The purpose of this study is to evaluate the effects of the vertical seismic component and to compare the axial force of columns and plastic rotation angle of the analytical models in these effects. The vertical seismic component produced a large increment of axial force in columns. And the vertical seismic component caused a significant increase of the damage in the columns. As analysis result, increase of axial force cause the damage of columns and give possibility of story collapse mechanism of the structure system. Therefore, area that near fault ground motion is expected may be consider the effect of vertical component of seismic ground motions.

  • Iliacus muscle tears are a rare injury seen after the high-energy trauma or as a result of low-energy injuries in patients with a bleeding diathesis as coagulopathy, receiving anticoagulation therapy and hemophiliac. Femoral nerve palsy due to compression from a hematoma by iliacus muscle rupture are rarely reported. Routine evaluation includes MRI to confirm and define the pathologic abnormality supplemented by EMG and nerve conduction studies to evaluate patterns and extent of femoral nerve injury. Hematologic evaluation for bleeding diathesis may preceded, if suspicion of coagulopathy is present. We report the case of a healthy 32-year-old male with iliacus rupture and concomitant femoral nerve palsy sustained by kicking motion during soccer game. After 6 months of observation with non-operative treatment regimen, satisfactory results were obtained, so we report it with a review of the literatures.

The Effects of Hand Grip Force on Shoulder Muscle Activity in Two Arm Posture (파악력의 강도가 두 가지 팔 자세에서 어깨근육 근활성도에 미치는 영향)

  • This study aimed to investigate the effect of hand grip force on the activity of shoulder muscles in 2 arm postures. The muscle activity of the upper trapezius, lower trapezius, infraspinatus, anterior deltoid, and posterior deltoid muscles in 22 healthy subjects (11 men and 11 women) were measured using surface electrodes during 4 hand gripping tasks (0%, 30%, 50%, and 70% of maximum voluntary contraction) in 2 shoulder positions (neutral position and $90^{\circ}$ elevation position). Among changing grip force significantly differenced infraspinatus, anterior deltoid, and posterior deltoid muscles' activity in the shoulder neutral position(p<0.05). In the shoulder $90^{\circ}$ elevation position, anterior deltoid, posterior deltoid muscles' activity was significantly differenced(p<0.05). Hand gripping was found to alter muscle activation. The hand grip task activated the infraspinatus muscle in the neutral position and inhibited the deltoid muscle in the $90^{\circ}$ elevation position. This finding may prove useful for the development preventative measures and rehabilitation strategies for shoulder injuries.
