Myofascial Pain, and the Spraying and Stretching Technique (근막동통과 분사신장요법)

  • 근막동통은 발통점과 중추흥분효과가 나타나는 국소적 근육성 동통이다. 발통점은 근육의 일부나 건 부착점에 나타나는 부위로서 단단한 띠로서 느껴지고 촉진시 통증을 유발한다. 통증은 관련된 발통점의 위치에 따라 연관된 부위에 나타난다. 이러한 근막 발통점에 따른 증상은 약한 기능이상부터 일상생활이 어렵게 되는 정도의 심한 통증까지 다양하다. 이의 발생에는 국소적 근육이상, 심부 동통, 심리적 스트레스의 증가, 수면장애와 전신적인 요인들도 관여한다. 일반적으로 치료는 보존적인 처치로써, fluoro-methane 같은 vapocoolant spray를 사용하는 사용하여 분사신장하는 방법이 있으며, 국소적인 주사, 마사지, 초음파, 온열요법등이 있다. 여러 환자에서 근막동통을 가지는 환자에 분사신장요법을 시행한 결과 대부분 환자에서 증상의 개선을 보였다. 근막동통의 치료시 분사신장요법은 매우 보존적인 처치로 임상가의 적용과 접근이 쉬우며, 환자로 하여금 치료에 따른 불안감을 줄이면서 만족을 느낄 수 있는 치료로 사료된다. 그러나 만성 환자의 경우 분사신장요법의 치료결과에 대한 만족도가 높지 않았다. 분사신장 용법은 근막동통을 가진 환자 모두에서 효과적이지는 않으나 치료의 도입으로, 근막동통을 진단시에 일차적으로 사용할 수 있는 안정한 보존적인 용법으로 사료된다.

Botulinum Toxin type A injection Versus Lidocaine Injection for Myofascial Pain Involving upper Trapezius (승모근 근막동통에 대한 보툴리눔 독소와 리도카인 주사의 치료효과 비교)

  • The purpose of this double-blind study was to evaluate clinical effects of botulinum toxin type A (BTX-A) injection on myofascial pain syndrome (MPS) involving upper trapezius and compare with those of lidocaine injection. 21 patients presenting with active TrP1 and/or TrP2 in the upper trapezius over 6 months were selected for this study. The subjects were randomly divided into two groups; one group injected with BTX-A (15 unit of $Botox^{(R)}$ / 0.3 ml per trigger point (TrP)) and the other group injected with 0.5% lidocaine (0.3 ml /TrP). The clinical effects were evaluated by VAS and PPT at baseline, 2, 4, 6 and 8 weeks after treatment. BTX-A group showed persistent decrease of VAS values and increase of PPT values following treatment. While there was no significant difference in VAS values between BTX-A and lidocaine groups (p=0.347), there was significant difference in PPT values after treatment between two groups (p=0.000). The subjects received BTX-A showed noticeable improvement in PPT values after treatment, suggesting more reliable effect of BTX-A injection compared with lidocaine injection. The results of this study support that the direct injection of BTX-A to a TrP is an effective and safe treatment for MPS involving upper trapezius.

A Case Report of Referral Pain on Mandibular Toothache Originated from Myofascial Pain (근막동통으로 인한 하악 구치부 연관통의 임상증례)

  • Majority of toothache is caused by abnormality of pulpal or periodontal tissues. However, there are numerous nonodontogenic sources that may be responsible for pain felt in the tooth. Nonodontogenic toothache may result from muscle, maxillary sinus, neuropathic, neurovascular, even cardiac and psychogenic problems. Myofascial pain is one of the most common abnormality characterized by palpation of a hard band which acts as the trigger point that causes stiffness and fatigue of the muscle, referral pain in tooth, tension-type headache, and hyperalgesia. Masseter muscle particularly induce referral pain in maxillary and mandibular molars. This case reported the treatment of the pain in right mandibular molar originated from myofascial pain of the right masseter. The pain is improved by general and reversible treatments such as muscle exercise, physical therapy, and medication. Nonodontogenic toothache should undergo proper differential diagnosis in order to avoid unnecessary dental treatments, such as endodontic, periodontic treatment, and tooth extraction, which would fail to alleviate the symptom of the patient.

Surface Electromyographic Characteristics of a Myofascial Trigger Point of the Temporalis Muscle: A Case Report (측두근의 근막동통 발통점의 표면 근전도 특성: 증례 보고)

  • Myofascial pain is a condition associated with regional pain and muscle tenderness characterized by the presence of myofascial trigger points. In this case report, a subject complaining of nighttime bruxism was clinically assessed, and a latent trigger point of the anterior temporalis muscle was identified with manual palpation. A surface electromyographic (SEMG) exam of the anterior temporalis muscle harboring the latent trigger point demonstrated several SEMG features, including post-contraction irritability, delayed relaxation following contraction and accelerated muscle fatigue. It was concluded that a SEMG exam may detect abnormal masticatory muscle function and, therefore, assist in the evaluation of myogenous temporomandibular disorders.

The Effect of Meridian Massage on the Reduction of Pain and Subjective Symptoms of Myofascial Pain Syndrome (경락마사지가 근막동통 증후군의 동통과 자각증상 감소에 미치는 효과)

  • Purpose: This research was for checking the effect of meridian massage on the reduction of pain and subjective symptoms of myofascial pain syndrome. Method: The method of the research was interruptive time series design. The research objects are 25 hospital workers with myofascial pain syndrome at B hospital in Busan from July 22, 2002 to August 18, 2002. SPSS Win 10.0 was used for data analysis, paired t-test and repeated measures ANOVA for hypothesis test. Result: The recipients of meridian massage felt less pain than before(F=12.587, p=.000). The recipients of meridian massage felt less often than before (F=6.705, p=.001). The recipients of meridian massage got lower score on subjective symptoms of myofascial pain than before(F=12.857, p=.000). The recipients of meridian massage had lower blood pressure than before(systolic blood pressure; t=4.697, p=.000, diastolic blood pressure; t=3.426, p=.002). The recipients of meridian massage did not get the lower number of pulse than before(t=0.33, p=.744). Conclusion: The above results show that meridian massage is effective on the reduction of pain and subjective symptoms of myofascial pain syndrome and makes stable the blood pressure. Therefore meridian massage can be apply as the effective intervention for the reduction of pain and subjective symptoms of myofascial pain syndrome.

  • Pain is a multidimensional experience that involves a complex interaction of sensory,affective and cognitive components. And especially, because of the emotional significance of orofacial pain, it is often a puzzing problem that clinicians are forced with from day to day. This case report describes differential diagnosis and management of dental and myofacial pain affected by psychological factors.

A Study of Microvascular Changes in Masticatory Muscles of Myofascial Pain Patients During Music Listening (근막동통 환자의 측두근에서 측정된 음악에 의한 혈액관류 변화)

  • The use of music as a means of inducing positive emotions and subsequent relaxation has been extensively studied by researchers. A great deal of this research has focussed on the use of music as a means of reducing feelings of anxiety and stress as well as aiding in the relief of numerous pathologies. The purpose of this study was to evaluate this effect of music using laser doppler flowmeter that monitors relative changes in the muscular blood cell perfusion, concentration of moving blood cells, mean velocity of the myofascial pain disorder patients. Total of 30 subjects were selected. They were monitored by LDF(Laser Doppler Flowmeter) during music listening for 3 minutes. LDF probe was placed over the temporalis muscle. The obtained results were as follows: 1. The average blood perfusion for 3 minutes of music listening at surface of temporalis muscles were elevated on both affected and unaffected parts compared to rest time. 2. The average of blood perfusion elevated during music listening at surface of temporalis muscles were higher in affected part than unaffected part. 3. Increase of blood perfusion during music listening is proportioin to that before music listening. In summary, these results may encouraging start to the elucidation of the hemodynamic events occuring during music listening and aimed to serve as a base for further studies.
