MICROLEAKAGE OF RESILON: EFFECTS OF SEVERAL SELF-ETCHING PRIMER (Resilon을 이용한 근관충전 시 수종의 치면처리제에 따른 미세누출 평가)

  • The purpose of this study was to compare the apical micro leakage in root canal filled with Resilon by several self-etching primers and methacrylate-based root canal sealer. Seventy single-rooted human teeth were used in this study. The canals were instrumented by a crown-down manner with Gate-Glidden drills and .04 Taper Profile to ISO #40. The teeth were randomly divided into four experimental groups of 15 teeth each according to root canal filling material and self-etching primers and two control groups (positive and negative) of 5 teeth each as follows: group 1 - gutta percha and $AH26^{(R)}$ sealer: group 2 - Resilon, $RealSeal^{TM}$ primer and $RealSeal^{TM}$ sealer: group 3-Resilon, Clearfil SE $Bond^{(R)}$ primer and $RealSeal^{TM}$ sealer group 4-Resilon, $AdheSe^{(R)}$ primer and $RealSeal^{TM}$ sealer. Apical leakage was measured by a maximum length of linear dye penetration of roots sectioned longitudinally by diamond disk. Statistical analysis was performed using the One-way ANOVA followed by Scheffe's test. There were no statistical differences in the mean apical dye penetration among the groups 2, 3 and 4 of self-etching primers. And group 1, 2 and 3 had also no statistical difference in apical dye penetration. But, there was statistical difference between group 1 and 4 (p < 0.05). The group 1 showed the least dye penetration. According to the results of this study, Resilon with self-etching primer was not sealed root canal better than gutta precha with $AH26^{(R)}$ at sealing root canals. And there was no significant difference in apical leakage among the three self-etching primers.

  • Endodontic management of an immature permanent tooth with dens invaginatus poses a challenge to efficient treatment planning for the clinicians. Because it is difficult to shape, disinfect, and seal the canal space effectively, teeth with complex root canal structures often require particularly extensive and thorough treatment approaches. The purpose of this case report was to share clinical insight from the results of short-term follow-ups after regenerative endodontic treatment with a dens invaginatus. Two immature maxillary lateral incisors with Oehlers type I and III dens invaginatus and infected necrotic pulp were treated using regenerative endodontic procedures. For the type III dens invaginatus case, an unusual approach toward redesigning the complex internal structure was taken, in order to have sufficient infection control and sealing. Cone-beam computed tomography (CBCT) and a surgical operating microscope were used to aid visualization and treatment. As a result, regenerative endodontic treatment appears to be effective for managing immature permanent teeth with complex dens invaginatus, and can lead not only to clinical and radiographic resolution, but also increased thickness of the dentinal walls.

A COMPARISON OF THE IRRIGATION SYSTEMS IN CALCIUM HYDROXIDE REMOVAL (근관세정 방법에 따른 수산화칼슘 제재의 제거 효율 비교)

  • The purposes of this study were to compare the efficacy of irrigation systems by removing a calcium hydroxide $(Ca(OH)_2)$ paste from the apical third of the root canal and the effect of the patency file. Sixty single rooted human teeth were used in this study. The canals were instrumented by a crown-down manner with .04 taper ProFile to ISO #35. $Ca(OH)_2$ and distilled water were mixed and placed inside the root canals. The teeth were divided into 6 groups according to the root canal irrigation system and the use of patency file as follows: group 1 - conventional method: group 2 - $EndoActivator^{(R)}$: group 3 - $EndoVac^{(R)}$; group 4 - conventional method, patency: group 4 - $EndoActivator^{(R)}$, patency; group 6 - $EndoVac^{(R)}$, patency. All teeth were irrigated with sodium hypochlorite. After the root canal irrigation, the teeth were split in bucco-lingual aspect. Percentage of the root canal surface coverage with residual $Ca(OH)_2$ until 3 mm from working length was analyzed using Image Pro Plus ver. 4.0. Statistical analysis was performed using the One-way ANOVA, t-test and Scheffe's post-hoc test. Conventional groups had significantly more $Ca(OH)_2$ debris than $EndoActivator^{(R)}$, $EndoVac^{(R)}$ groups. There was no significant difference between $EndoActivator^{(R)}$ and $EndoVac^{(R)}$ groups. Groups with patency file showed more effective in removing $Ca(OH)_2$ paste than no patency groups. but. it was no significant difference. This study showed that $EndoActivator^{(R)}$ and $EndoVac^{(R)}$ systems were more effective in removing $Ca(OH)_2$ paste from the apical third of the root canal than conventional method.

  • 최근 치근단 와동 형성 시 초음파 또는 음파 기구를 이용하는 것이 보편화되어 있고 여러 측면에서 평가되어져 왔다. 본 연구의 목적은 혐기성 세균을 이용한 미세 누출 모델을 이용하여 초음파와 음파 기구를 사용, 치근단 와동 형성을 하였을 때 두 기구의 사용이 충전된 치근단 와동의 미세 누출에 미치는 영향을 비교 평가하는 것이다. 48개의 단근치의 근관을 crown-down방법을 이용 Profile로 .06 black까지 근관 성형을 시행하고 수직 가압법을 사용하여 gutta-percha와 AH26 sealer를 이용하여 근관 충전을 시행하였다. 각 충전된 시편의 치근단 3mm를 절제하였다. 각 시편은 이미로 4군으로 나누었으며 1군에서는 치근단 와동을 초음파 기구로 형성하였고 2군에서는 음파 기구로 형성하였다. 3군은 음성대조군으로 4군은 양성대조군으로 분류하였다. 형성된 치근단 와동은 공기 분사침을 이용하여 건조 후 super EBA 시멘트로 충전하였으며 Anaerobic chamber에서 Fusobacterium nucleatum (VPI 10197)을 사용한 혐기성 세균 미세누출 모델을 이용하여 30일간 미세 누출을 관찰한 결과, 초음파 기구를 사용한 군에서는 80%에서 음파 기구를 사용한 군에서는 75%에서 미세누출이 나타났으나 통계적으로 유의성 있는 차이는 보이지 않았다.

  • The purposes of this study were to evaluate the degree of cavity cleanliness and to observe cavity wall morphology when root-end retrograde cavity preparation was done with ultrasonics. Root resections were done on 20 extracted human maxillary central incisors after canal filling with gutta-percha, and retrocavities were prepared using a slow-speed round bur as a control, and stainless steel ultrasonic tips of power settings of 2 and 6 ($Miniendo^{TM}$, EIE, SA, USA) as experimentals. The degree of the remaining cavity debris and smear layer, and wall morphology were evaluated under the scanning electron microscope. The results were as follows : Cavity prepared with ultrasonics of either power setting showed significantly less smear layer than did slow-speed preparations (p<0.01). However, there was no significant difference in canal debris (p<0.05). Cavity prepared with ultrasonics showed hatcheted appearance of wall, while slow-speed preparation showed relatively plain one.

COMPARISON OF APICAL SEAL WITH OR WITHOUT THE USE OF DENTIN ADHESIVE SYSTEM (상아질 접착제 사용 여부에 따른 근관 밀폐효과의 비교)

  • The purpose of this study was to compare the sealing ability of root canal obturation with or without the use of dentin adhesive system. Forty extracted human teeth with one canal were selected and decoronated. The teeth were divided into two Groups. The obturation procedure of Group 1 was the same as that of Group 2 with the exception of dentin adhesive system. Group 2 were obturated with dentin adhesive system, AH-26, and gutta-percha. After obturation, the teeth were immersed in methylene blue solution for 84 hours. The teeth were sectioned horizontally at 1.5 mm (Level 1), 2.0 mm (Level 2) 2.5 mm (Level 3) from the root apex using a low speed microtome. Distance of dye-penetrated surface and total dentinal surface were measured using SigmaScan Pro 5.0, and the ratio of dye-penetrated distance to the total dentinal distance was analyzed statistically by Mann-Whitney U-test. 1. In both groups, the mean leakage ratio was decreased cervically. 2. At level 1, there was no significant difference between group 1 and group 2 (p > 0.05). 3. At level 2 and 3, group 1 showed significantly higher mean leakage ratio than group 2 (p < 0.05). The results suggest that using dentin adhesive system in root canal obturation procedure reduces the apical microleakage.

INFLUENCES OF APICOECTOMY AND RETROGRADE CAVITY PREPARATION METHODS ON THE APICAL LEAKAGE (치근단절제 및 역충전와동 형성방법이 치근단누출에 미치는 영향)

  • The purpose of this study was to evaluate the influence of root resection and retrograde cavity preparation methods on the apical leakage in endodontic surgery. To investigate the effect of various root resection and retrograde cavity preparation methods on the apical leakage, 71 roots of extracted human maxillary anterior teeth and 44 mesiobuccal roots of extracted human maxillary first molars were used. Root canals of the all the specimens were prepared with step-back technique and filled with gutta-percha by lateral condensation method. Three millimeters of each root was resected at a 45 degree angle or perpendicular to the long axis of the tooth according to the groups. Retrograde cavities were prepared with ultrasonic instruments or a slow-speed round bur, and occlusal access cavities were filled with zinc oxide eugenol cement. Three coats of clear nail polish were placed on the lateral and coronal surfaces of the specimens except the apical cut one millimeter. All the specimens were immerged in 2% methylene blue solution for 7 days in an incubator at $37^{\circ}C$. The teeth were dissolved in 14 ml of 35% nitric acid solution and the dye present within the root canal system was returned to solution. The leakage of dye was quantitatively measured via spectrophotometric method. The obtained data were analysed statistically using two-way ANOVA and Duncans Multiple Range Test. The results were as follows: 1. No statistically significant difference was observed between ultrasonic retrograde cavity preparation method and slow-speed round bur technique, without apical bevel (p>0.05). 2. Ultrasonic retrograde preparation method showed significantly less apical leakage than slow-speed round bur technique, with bevel (p<0.0001). 3. No statistically significant difference was found between beveled resected root surface and non-beveled resected root surface, with ultrasonic technique (p>0.05). 4. Non-beveled resected root surface showed significantly less apical leakage than beveled resected root surface, with slow-speed round bur technique (p<0.0001). 5. No statistically significant difference in apical leakage was found between the group of retrograde cavity prepared parallel to the long axis of the tooth and the group of one prepared perpendicular to the long axis of the tooth (p>0.05). 6. Regarding isthmus preparation, ultrasonic retrograde preparation method showed significantly less apical leakage than slow-speed round bur technique, in the mesiobuccal root of maxillary molar, without bevel (p<0.0001).

The effect of MTAD as a final root canal irrigants on the coronal bacterial leakage of obturated root canals (최종 근관세척제로서의 MTAD 근관세척제가 치관부 세균미세누출에 미치는 영향)

  • The purpose of this study was to evaluate the effects of MTAD, EDTA and sodium hypochlorite(NaOCl) as final irrigants on coronal leakage resistance to Enterococcus faecalis. Forty extracted human maxillary molars were used in this experiment. The teeth were randomly divided into positive control group (Group 1; n = 5), negative control group (Group 2; n = 5) and three experimental groups (n = 30). In Group 3 (n = 10), the root canals were irrigated with sodium hypochlorite. In Group 4 (n = 10) and 5 (n = 10), the root canals were irrigated with sodium hypochlorite and rinsed with EDTA and MTAD, respectively. The teeth in each group were cleaned and shaped to #40 profile with .04 taper, and obturated with gutta-percha and AH-26 root canal sealer. The coronal portion of each tooth was placed in contact with inoculum of Enterococcus faecalis in Brain Heart Infusion (BHI) culture media. Each root tip was placed in a vial containing sterile culture media. The vials were placed in anaerobic chamber and observed everyday for turbidity for 180 days. Statistical analysis was performed using Fisher's Exact Test. After 180 days, Group 3, 4, and 5 showed 7, 4 and 5 leaking samples respectively. The differences in leakage resistance were not statistically significant among Group 3, 4 and 5.

One way ANOVA and Scheffe's post-hoc test were performed for statistical analysis. Group 1 showed the least apical leakage and there was no statistical significance between Group 2, 3, 4. According to the results, the self adhesive resin cement is possible to use as sealer instead of primer & sealant when root canal filled by Resilon.
