A CLINICAL STUDY ON BOOT CANAL TREATED PATIENTS (근관치료 환자에 대한 임상적 고찰)

  • In order to observe the frequency of visits and the distribution of endodontically treated patients according to age, sex, diagnosis, type of teeth, the author had surveyed the 17,250 outpatients of the Infirmary of the College Dentistry, Yonsei University from January 1976 to December 1978. 773 cases, 660 patients among 728 endodontically treated patients were studied. The obtained results were as follow; 1. The average frequency of visits were 5.95 in uppers and 5.88 in lowers. 2. The average frequency of visits were 5.79 in males and 6.02 in females. 3. At the age of under 14, the average frequency of visits were 7.42 in uppers and 6.76 in lowers. 4. According to diagnoses, the average frequency of visits in periapical granuloma or cyst were 2.80 in uppers and 3.16 in lowers. 5. The number of endodontically treated teeth had revealed as follow; upper incisors 27.94%, lower molars 25.87%, and upper molars 15.13% 6. The number of endodontically treated teeth by diagnoses were as follow; periapical abscess 45.27%, pulpit is 37.51%, tooth fracture 8.40%. 7. The number of endodontically treated teeth were 51.99% in the age of from 20 to 34.

Quality of Life of Hepatocellular Carcinoma Patients with Transarterial Chemoembolization (간동맥화학색전술 치료를 받는 간세포암 환자의 삶의 질 관련요인)

  • Purpose: The aim of this study was to examine the quality of life (QOL) and its associated factors in hepatocellular carcinoma patients with Transarterial Chemoembolization. Methods: A descriptive correlational study was used, 106 hepatocellular cancer patients participated through convenient sampling. Data were collected by self-report questionniares or face to face interviews during the period from June to December in 2014. Satisfaction for pain control, perceived health status, professional support and QOL were measured by Assessment of Patient Satisfaction for Pain Management, Health Self-Rating, Relationship Questionnaire, Functional Assessment Cancer Therapy-General. Results: The mean age of participants was 63.7 years and most of them were male (86.8%) and married (84.9%). The mean score of QOL was 67.42. QOL was significantly different by religion. Number of other disease and perceived health status were positively correlated to QOL. Pain intensity was negatively correlated to QOL. As a result of stepwise multiple regression analysis, perceived health status and religion were explained 41.0% (p<.001) of the variance in QOL. Conclusion: Perceived health status should be considered when developing nursing intervention to improve QOL among hepatocellular cancer patients with Transarterial Chemoembolization.

Predictors of Taking Tuberculosis Drugs for Health Care Workers with Latent Tuberculosis Infection (잠복결핵감염 의료기관 종사자의 결핵약 복용 예측요인)

  • Purpose: The purpose of this study was to investigate the predictors of tuberculosis (TB) drugs in health care workers diagnosed with latent tuberculosis Infection (LTBI). Methods: This study was a descriptive correlation study. Data were collected by using a self-report questionnaire. The collected data were analyzed by -test, t-test, and logistic regression analysis. Results: There were involved 160 participants, 35 (21.9%) who took the TB drugs, and 125 (73.8%) who did not take the TB drugs. The predictors on taking TB drugs in health care workers with LTBI were subjective norms (OR=3.33, p<.001), perceived sensitivity (OR=2.67, p=.026), perceived barrier (OR=0.14, p=.014), and unmarried (OR=4.69, p=.006) than married, health care worker category 2 group (OR=5.84, p=.015) and 1 group (OR=4.25, p=.022) than 3, 4 group, sleep over 7 hours (OR=4.11, p=.022) than less 7 hours sleep. Conclusion: In order to promote the use of TB drugs in health care workers with LTBI, it is necessary that take intervention strategies to increase the subjective norms and perceived sensitivity and to decrease the perceived barriers.

Effect of Program for Pain Reduction in Patients with Rheumatoid Arthritis: A Systematic Review (류마티스 관절염 환자의 통증 감소를 위한 프로그램의 효과: 체계적 문헌고찰)

  • Purpose: The purpose of this study was to establish the foundations for the application of appropriate programs for pain reduction in patients with rheumatoid arthritis. Methods: The literature on patients with rheumatoid arthritis were found on an electronic search site for South Korean academic papers. The search terms "rheumatism" and "rheumatoid arthritis" were used independently. The combined terms "rheumatism and pain" and "rheumatoid arthritis and pain" were also used. For the meta-analysis, the R version 3.5.1 program was utilized. Results: The meta-analysis of eight papers showed a large effect size of -4.11. The programs were most effective in the order of aquatic exercise, aromatherapy, self-help education, muscle strength exercises, and tai chi. Conclusion: This study could provide the basis for presenting appropriate programs for pain management in patients with rheumatoid arthritis.

Effects of Ethyl Chloride Spray before Arteriovenous Fistula Puncture on Pain, Depression, and Noncompliance of Hemodialysis (혈액투석 환자의 동정맥루 천자 시 에틸클로라이드 스프레이 적용이 통증, 우울 및 치료 지시 불이행에 미치는 효과)

  • Purpose: This study was conducted to examine the effects of ethyl chloride spray during arteriovenous fistula puncture on pain, depression, and noncompliance in hemodialysis patients. Methods: This study used a randomized controlled trial design on adults with chronic renal failure who received hemodialysis treatment through arteriovenous fistula. Ethyl chloride spray was applied to 20 subjects in the experimental group during arteriovenous fistula puncture, and a placebo spray was applied to 20 subjects in the control group; the intervention was conducted 12 times for 4 weeks. A total of 33 participants were used in the final analysis. The outcomes were measured using the face pain rating scale, Center for Epidemiological Studies Depression Scale (CES-D), and modified United States Renal Data System tools. Results: As a result of the pre-homogeneity test between the two groups, it was confirmed that the sexes were not homogeneous. Therefore, sex was treated as a covariate and analyzed. Puncture pain was significantly reduced in the experimental group (p<.001). However there was no significant difference between the groups in depression and noncompliance. Conclusion: Ethyl chloride spray was convenient and effective pain management intervention for both hemodialysis patients and medical staff.

The Effects of Sling Exercise on Fall Risk Score, Ankle Dorsiflexion and Balance in Community-Dwelling Elderly Women (슬링운동이 여성노인의 낙상위험도, 족관절배측굴곡력 및 균형에 미치는 효과)

  • Purpose: The aim of this study was to examine the effects of sling exercise on fall risk score, ankle dorsiflexion and balance in community-dwelling elderly women. Method: A 6-week prospective study was conducted to examine the effects of sling exercise. Participants were required to attend their assigned exercise classes three times a week for 6 weeks. Result: After the 6 week study period, PPA fall-risk scores were reduced by 0.90 for the exercise group, which was a significant change (p<.001). Dorsiflexion strength increased significantly (p<.01) by 1.56 kg after the sling exercise. There was a 2.0 cm-increase (p<.05) in FRT and 0.38 second-improvement (p<.01) in TUGT. A 7.88 second-increase in One-leg standing with Eyes Open and a 3.12 second-increase in One-leg standing with Eyes Closed were reported during the 6-week intervention period. Conclusion: The 6-week sling exercise reduced falls risk score significantly (p<=.001) in community-dwelling elderly women by improving fall risk related factors such as reaction time, balance and strength.

Effects of Stress, Resilience, and Focusing Manner on Quality of Life in Burn Rehabilitation Patients (재활치료 중인 화상 환자의 스트레스, 극복력, 포커싱적 태도가 삶의 질에 미치는 영향)

  • Purpose: The aim of this study was to investigate the degree of stress, resilience, focusing manner, and quality of life among burn rehabilitation patients. Furthermore, it seeks to identify factors affecting the quality of life in this patient group. Methods: The participants of this study were 129 burn patients, aged 19 or older, who received treatment at the rehabilitation of a university hospital in Seoul. Data were collected from June 22 to August 31, 2022 through self-report questionnaires covering stress, resilience, focusing manner and quality of life.The data were analyzed using the SPSS 29.0 program. Results: Multiple regression analysis showed that education level, cause of burn, area of burn, pain, stress, and focusing manner significantly affected the quality of life, explaining 74.5% of the variance. Conclusion: In particular, focusing manner was a major factor affecting the quality of life in burn rehabilitation patients. Therefore, there is a need to develop intervention programs aimed at increasing focusing manner in this patient group.

Preferences for Care Near the End of Life among Hospital Employees (일 대학병원종사자의 생애말기 치료 선호도)

  • Purpose: The purpose of this study was to investigate end-of-life care preferences of employees working in a university hospital. Methods: Of 650 eligible employees that were approached, 607 employees (386 nurses, 93 physicians, and 128 general staff) completed the Korean version of Preferences for Care Near the End of Life (PCEOL-K). Results: Among 5 dimensions of the PECOL-K, "Pain" was the most preferred care dimension and "Decision making by health care professional" was the least preferred care dimension. The item that received the highest mean score was "I want to let nature guide my dying and I do not want my life to be artificially prolonged in any way", and the lowest item was "I want health care providers to make all decisions about my care". As preferred care near the end of life, nurses gave lower scores to the life sustaining treatment and decision making by health care profession than physicians and general staff. Compared to physicians and nurses, general staff preferred the decision making by health care professional and by family. Conclusion: The results show that adequate pain relief is the most preferred care at the end of life among hospital employees and non-medical personnel preferred decision making by others.

  • Infective endocarditis is a life-threatening disease, although it is relatively uncommon. Substantial morbidity and mortality result from this infection. Therefore, primary prevention of endocarditis whenever possible is very important. The American Heart Association updated recommendations for the prevention of infective endocarditis in individuals at risk for this disease in 1997. But, utilization of antibiotic prophylaxis for patients at risk does not provide absolute immunity from infection. This report presents the case of infective endocarditis that occurred in spite of appropriate antibiotic prophylaxis in the ventricular septal defect(VSD) patient.

  • Trauma to the tooth is the most common accidents in pediatric dentistry and tooth fracture occurs frequently. Fracture is classified into crown fracture, root fracture, and crown-root fracture which involves both. Also, it is classified into simple or complicated fracture depending on whether the pulp is exposed or not. When the fracture is extended down to the subgingival level following the long axis of the root, or when more than 1/3 of the root is involved, extraction is the primary treatment. But alternative treatment such as extruding the root fragment with orthodontic force and restoring it, or intentionally extracting the tooth and replanting it to a position which it can be restored. This is a case report on intentional replantation of a traumatized maxillary central incisor with crown-root fracture of a patient with mixed dentition. The teeth was extracted and immediate endodontic treatment, retrograde filling and resin restoration were carried out.
