Relationships between physical pain and preventive actions against musculoskeletal diseases among dental hygiene students (치위생과 재학생들의 신체통증과 근골격계질환 지식 및 예방행위)

  • The purposes of this study were to investigate the relationships between the degree of physical pain and the knowledge and preventive actions of musculoskeletal diseases among dental hygiene students with a potential risk of contracting those diseases and thus to help them prevent them. A survey was taken among 207 sophomores and juniors specializing in dental hygiene at S1 and S2, Gyeonggi Province in the area of physical pain, knowledge of musculoskeletal diseases, and preventive actions against them. The findings were as follows: 1. The mean scores of the sophomore in the knowledge of musculoskeletal diseases were $6.20{\pm}2.21$, and those of the juniors were $6.72{\pm}2.29$. The mean scores of the sophomores and juniors in the preventive actions against musculoskeletal diseases were $22.50{\pm}2.37$ and $22.29{\pm}3.01$, respectively. 2. The subjects displayed severe physical pain in the lower back, shoulder, and neck in the descending order and medium physical pain in the neck, shoulder, and lower back in the descending order. 3. The higher level knowledge of musculoskeletal diseases they had, the less physical pain they felt. And there were significant differences among the shoulder, lower back, hip, ankle, and foot. 4. There were relationships between physical pain and the preventive actions against musculoskeletal diseases in "placing the hands at the height of the elbows during treatment", "reducing such positions as bending and extending during treatment", "narrowing the distance with the patient", "taking regular breaks during treatment for recovery", "trying not to incline the neck, back, arm, and wrist as much as possible", and "trying to keep the torso in the neutral position." 5. As for the education about musculoskeletal diseases, 74 sophomores(88.10%) and 102 juniors(89.74%) answered they received no such education. The results suggest that there should be some instructions to help dental hygiene students practice the preventive actions against musculoskeletal diseases and further prevention programs against those diseases.

  • Werner's syndrome is a rare autosomal recessive disorder manifesting as premature aging. It is also known to be characterized by a high frequency of malignant tumors, especially sarcomas. However, Werner's syndrome may be not only a premature aging disease but also a cancer syndrome, because the malignant tumors in these patients are different from those of normal population with respect to involved site, histological type, and age of onset. Recent studies found Werner's syndrome was caused by a mutation of Werner helicase suggesting that WRN helicase may participate in metabolism and repair of DNA. And a dysfunction of WRN helicase may induce the genomic instability causing somatic mutations. Further studies of Werner's syndrome associated with sarcoma might give much informations about the normal aging process and the pathogenesis of sarcomas.

  • 수공구 사용시 과도한 힘은 작업성 근골격계 질환을 일으킬 수 있는 주요 원인중 하나이다. 이와 관련하여, 수공구 파지시 인체 내부에 부과되는 근력과 외적으로 작용된 힘 간의 비율을 이해하는 것이 중요하며, 이는 근육에 부과되는 과도한 힘을 최소화 시키고, 작업에 필요한 힘의 효율성을 극대화 시키는데 필수적이라 할 수 있다. 이러한 비율과 관련하여 많은 연구가 되어 왔으나, 대부분 수리적 인체역학적 모델과 같은 간접적 추정 방법에 의거하고 있는 실정이다. 이러한 인체역학적 모델을 검증하고 개선하기 위하여 해부용 팔 (cadaver)을 활용한 직접적인 근력과 악력 측정이 필수적이다. 본 연구에서는 이러한 해부용 팔을 이용하여 상지 굴근(hand flexor)을 자동으로 제어하고 근력과 함께 악력을 측정할 수 있는 Hand Motion Simulator를 개발하고, 이를 통하여 다양한 사이즈의 손잡이 파지시 요구되는 근력과 외적으로 적용된 악력을 비교함으로써 수공구 손잡이 사이즈에 따른 근력의 효율성에 대하여 측정을 해 보았다. 또한, 적용된 굴근 (FDP & FDS) 간의 힘 비율에 따른 파지법의 차이를 조사해 보았다. 내부에 주어진 근력은 외부로 작용된 악력보다 5.3배 높은 부하가 작용하였으며 이러한 수공구 손잡이 파지시 힘의 효율성 역시 FDP 와 FDS 간의 비율이 3:2 였을 때, 그리고 손잡이 크기가 작을수록 높은 결과를 보였다. 반대로 손잡이의 크기가 커질수록 힘의 효율성은 저하되었다. 또한, 손가락 관절 각도의 경우 FDP와 FDS간의 비율에 따라 상이한 자세를 나타내었다. FDP 굴근의 비율이 높을 경우 손가락 끝마디 관절 (DIP) 의 굴곡을 보였으며, FDS 굴근 비율이 높을 경우 손가락 두 번째 관절(PIP)의 굴곡을 보였다. 본 연구를 통한 결과는 추후 상지작업자에 대한 근골격계 질환 예방 기준안 마련 및 수공구 설계를 위한 기초자료로 활용이 가능할 것으로 보인다.

Study on the Musculoskeletal Subjective Symptoms Disorders and Health Promotion Behavior of Daily Plant Construction Workers in One Area (일 지역 일용직 플랜트 건설근로자의 근골격계 질환 자각증상과 건강증진행위)

  • Purpose: This research is a correlative study to assess the relationships of musculoskeletal subjective symptoms disorders and health promotion behavior of the daily plant construction workers. Methods: The subjects were comprised of 306 daily plant construction workers by self-reported questionnaires on October 19, 2011. The instruments used for this study were musculoskeletal subjective symptoms disorders (NIOSH) and health promotion lifestyle profile II (HPLP-II). The data were analyzed using SPSS/WIN 14.0. Results: The scores of health promotion behavior of daily plant construction workers were 2.13, and the scores of health promotion behavior were significantly different depending on gender (t=-2.51, p=.013), religion (t=3.10, p=.002), owning house (t=2.59, p= .010), exercise (t=6.01, p<.001), time to rest during work (t=2.21, p=.027), and work satisfaction (F=8.97, p<.001). The musculoskeletal subjective symptoms disorders attack rate was 90.5%. The results of the correlative study to assess relationships showed significant negative correlation between musculoskeletal subjective symptoms disorders and health promotion behavior (r=-1.45, p=.011). Conclusion: The results of the study showed that it was lower level of health promotion behavior level. Therefore, to increase the health promotion behavior of the daily plant construction workers, a great deal of efforts are required at the level of the government, employers and workers.

The Effects of Stretching Exercise Education on Female Workers' Self-efficacy, Health Belief and Practical Intention for Preventing Musculoskeletal Diseases (스트레칭체조에 대한 교육이 여성근로자의 근골격계질환 예방을 위한 자기효능감, 건강신념 및 실천의지에 미치는 효과)

  • Purpose: The purpose of this study is to examine the effect of stretching exercise education on female workers' behavior for preventing musculoskeletal diseases. Method: This study educated occupational health nurses working in Seoul and Kyunggi area for stretching exercise methods and then had them educate female workers in each workplace for the stretching exercise methods. The subjects of this study were 205 female workers and the research period was from June to October 2003. Results: The female workers' self-efficacy on stretching exercise increased significantly. The female workers' perceived susceptibility on stretching exercise increased significantly. As for change in practical intention on preventive behavior for musculoskeletal diseases, those who replied to have a plan to exercise regularly and to want to participate in stretching exercise class increased significantly. According to whether to have subjective musculoskeletal symptoms or not, self-efficacy increased significantly after education in both workers with such symptoms and those without, and perceived susceptibility and cue to action increased significantly after education in workers without such symptoms. Conclusion: It is necessary to develop programs to increase self-efficacy and health belief and to apply them to female workers to prevent musculoskeletal diseases. In addition, it is necessary to provide repeated education for female workers' steady practice of preventive behavior.

Job Stress and Musculoskeletal Disorder in Seoul City's School Foodservice Employees (서울시 학교급식시설 조리종사자의 직무 스트레스와 근골격계질환)

  • Purpose: School foodservice employees (SFEs) could be exposed to the risk of musculoskeletal disease and of job stress due to their job characteristics. This study was to evaluate the level of job stress and the prevalence of work-related musculoskeletal symptoms (WRMS) in Seoul city's SFEs, and to determine associations between job stress and WRMS. Methods: The study design was cross-sectional, and 975 SFEs were recruited. Self-administered questionnaire included the 'Korean occupational stress scale-short form' and the 'KOSHA GUIDE H-9-2012' instrument to evaluate the job stress and WRMS, respectively. SFEs' medians of job stress were compared to the reference values of published study in Korean workers. Results: The participants reported greater levels of job demand and physical environment than the general Korean population. WRMS were reported in 89.0% of participants at any body part, and 41.1% were presumed to need for medical intervention. High levels of job demand and of physical environment were significantly associated with WRMS. Conclusion: Subscales of job demand and physical environment were relatively high in SFEs and those were related to the occurrence of WRMS. To reduce the WRMS prevalence, a job stress management program focused on job demand and physical environment may be required.

Relationships between Work-related Psychosocial and Acculturative Factors and Work-related Musculoskeletal Disorders among Korean-Chinese Migrant Workers Living in Korea (조선족 근로자의 직업 관련성 사회 심리적 요인, 문화적응 요인과 직업 관련성 근골격계 질환의 관계)

  • Purpose: The purpose of the study >was to examine the prevalence of work-related musculoskeletal disorders(WMSDs) and to identify the relationship between work-related psychosocial and acculturative factors and WMSDs among Korean-Chinese workers living in Korea. Methods: A cross-sectional survey was conducted with 195 Korean-Chinese workers who have worked full-time for the past 6 months. A structured questionnaire, including measures for musculoskeletal symptoms, physical demand, work-related psychosocial(job demand, job control, interpersonal conflict), acculturative (acculturation strategy and acculturative stress), and personal factors, was used. Descriptive statistics and logistic regression analysis were conducted using the SPSS WIN 17.0. Results: The prevalence of WMSDs was 64.1%. In comparison to Korean workers, job demand of this group was lower while both interpersonal conflict and lack of job control were higher among them. Integration was the most commonly used acculturation strategy and mean scores of acculturative stress were below the median. Acculturative stress was positively correlated with integration and assimilation while negatively correlated with separation and marginalization. Interpersonal conflict and integration appeared to be related to WMSDs among male and female Korean-Chinese workers, respectively. Conclusion: The WMSD interventions need to be planned in consideration of different influences of work-related psychosocial and acculturative factors on WMSDs by gender.

Comparison of pain, fatigue, and Achilles tendon in female college students wearing high heels and flat shoes: A Preliminary Study (여대생의 하이힐 착용에 따른 하지 통증, 피로, 아킬레스건의 융복합 비교: 예비연구)

  • The purpose of this study is to analyze the differences of the lower back pain, fatigue, and Achilles tendon thickness according to wearing and not wearing high heels in order to understand symptoms of musculoskeletal diseases caused by wearing high heels in college students. lower extremity pain fatigue and achilles tendon thickness were measured in high heel wear group (n=19) and non high heels wear group (n=23). Participants' general characteristics and the lower extremity pain, fatigue, and Achilles tendon thickness were analyzed using mean, standard deviation. Differences in lower extremity pain, fatigue, and achilles tendon thickness were analyzed by independent t-test. The results showed that the difference between the participants' lower extremity pain (t=2.28, p=.028), right achilles tendon thickness (t=2.30, p=.027) and left achilles tendon thickness (t= 3.89, p<.001) The results of this study show that convergence approach can be applied as a basis for health problems in the structure and function of musculoskeletal disorders associated with wearing high heels. In the future, follow up observation according to the wearing of high heels in the same subject will be needed.

Comparison of the Effects of an App-based and Poster-based Self-managed Workplace Stretching Program on Musculoskeletal Symptoms of Workers in Small Manufacturing Businesses (소규모 제조업 사업장 노동자의 근골격계질환 증상관리를 위한 앱 기반과 포스터 기반 자가관리 작업장 스트레칭 프로그램의 효과 비교)

  • Purpose: This study aimed to compare the effects of an 8 week, self-managed, app-based and poster-based stretching program on musculoskeletal symptoms, flexibility, stretching frequency, self-efficacy, social support, and musculoskeletal disorder knowledge in small manufacturing business workers. Methods: This was a cluster randomized, two-group pretest-posttest design. Workers were assigned to either an app-based (n=20) or a poster-based (n=25) stretching intervention. Both groups received an educational class. The app group also received mobile phone text messages and an app with stretching videos, stretching alarms, stretching records, and information on musculoskeletal disorders. The poster group received workplace stretching posters. Data were collected from April to September 2018 and analyzed with the 𝑥2 test, paired t-test, and independent t-test. Results: There was only a significant difference in social support. Significant increase in flexibility and musculoskeletal symptoms were noted for both groups, but social support and musculoskeletal disorder knowledge were significantly changed only in the poster group. More than half of the workers practiced stretching at least 3 times a week. Conclusion: The 8 week, self-managed, workplace stretching program was effective to increase flexibility and stretching frequency to at least 3 times a week. However, effective interventions for musculoskeletal symptoms could not be identified.

Development of Work-related Musculoskeletal Disorder Questionnaire Using Receiver Operating Characteristic Analysis (Receiver Operating Characteristic 분석법을 이용한 업무관련성 근골격계질환 설문지 개발)

  • Objectives: Receive Operating Characteristic(ROC) curve with the area under the ROC curve(AUC) is one of the most popular indicator to evaluate the criterion validity of the measurement tool. This study was conducted to develop a standardized questionnaire to discriminate workers at high-risk of work-related musculoskeletal disorders using ROC analysis. Methods: The diagnostic results determined by rehabilitation medicine specialists in 370 persons(89 shipyard CAD workers, 113 telephone directory assistant operators, 79 women with occupation, and 89 housewives) were compared with participant's own replies to 'the questionnair on the worker's subjective physical symptoms'(Kwon, 1996). The AUC's from four models with different methods in item selection and weighting were compared with each other. These 4 models were applied to 225 persons, working in an assembly line of motor vehicle, for the purpose of AUC reliability test. Results: In a weighted model with 11 items, the AUC was 0.8155 in the primary study population, and 0.8026 in the secondary study population(p=0.3780). It was superior in the aspects of discriminability, reliability and convenience. A new questionnaire of musculoskeletal disorder could be constructed by this model. Conclusion: A more valid questionnaire with a small number of items and the quantitative weight scores useful for the relative comparisons are the main results of this study. While the absolute reference value applicable to the wide range of populations was not estimated, the basic intent of this study, developing a surveillance fool through quantitative validation of the measures, would serve for the systematic disease prevention activities.
