Needle Type of Hybrid Temperature Probe for Both Diagnosis and Treatment of Musculoskeletal Pain Syndrome (근골격계 통증질환의 진단과 치료를 위한 주사바늘형 복합온도 프로브의 개발)

  • This paper describes the development of needle type probe that measures temperature and injects medicine for both diagnosis and treatment of musculoskeletal pain syndrome (MPS). The size of trigger points is from several micrometers to millimeter. Therefore, it is required to develop a medical device that is capable of not only finding the trigger points by temperature measurement, but also injecting medicine at the exact location for treatment. To challenge these difficulties, thermocouple was fabricated on the surface of a needle using metal deposition process. Special type of stainless-constantan thermocouple was achieved from the stainless body of a needle itself and deposited constantan metal film. In particular, parylene coating enables to limit the temperature sensitive area to the end of the needle tip. Fabricated needle type probe produces $3.25mV/^{\circ}C$ of thermoelectric sensitivity and compared its performance with commercial T-type thermocouple in animal muscle sample.

Effects of Auricular Acupressure Therapy on Musculoskeletal Pain, Depression and Sleep of the Elderly in Long-term Care Facilities (이압요법이 시설거주노인의 근골격계 통증, 우울 및 수면에 미치는 효과)

  • Purpose: The purpose of this study is to examine and evaluate the effects of auricular acupressure on musculoskeletal pain, depression and sleep of the elderly who are institutionalized in long-term care facilities. Methods: The research was conducted in a non-equivalent control group and non-synchronized design with data collected from October to December, 2016. The subjects were from long-term care facilities for the elderly in D city and divided into an experimental group (24 subjects) and a control group (25 subjects). Results: After the intervention, the experimental group showed significant improvement in degree of musculoskeletal pain on time ($x^2=23.89$, p<.001) and degree of depression on time and group ($x^2=37.42$, p<.001, U=122.0, p<001) and degree of sleep on time ($x^2=33.62$, p<.001). Thus, the suggested hypothesis is partially supported. Conclusion: Auricular acupressure therapy is expected to be a practical and efficient nursing intervention for the elderly institutionalized in long-term care facilities.

Prevalence of Musculoskeletal Symptom in Rural Farmers (농촌지역주민의 근골격계 증상 경험률)

  • This study was performed to examine the annual experience rate of musculoskeletal symptom and its related factors in rural farmers. Six-hundred and sixty one rural farmers were investigated by questionnaire survey through interview from July 1 to July 31 in 2000. The results were as follows; The rural farmers who had experienced musculoskeletal symptom during recent 1 year was 82.7% in male, 92.1% in female. The annual experience rate of musculoskeletal symptom by part of the body in male were turned out as back; 42.3% leg and knee; 36.4%, shoulder; 21.4%, arm and hand; 12.7%, elbow; 7.7%, hip; 6.8% neck; 6.4%, ankle and foot; 6.4% in order. The annual experience rate of musculoskeletal symptom by part of the body in female were turned out as back; 59.4%, leg and knee; 48.5%, shoulder; 20.2%, arm and hand; 13.8%, hip; 10.7%, neck; 6.8%, ankle and foot; 6.3%, elbow; 5.0% in order. The annual experience rate of musculoskeletal symptom, back pain, and pain on leg and knee were significantly higher in female. In male, the experience rate of musculoskeletal symptom was different significantly according to educational level, family size, economic status, status of smoking, duration of farming, type of major agricultural affairs. By the multiple logistic regression analysis for experience of musculoskeletal symptom, family size and economic status were the significant variables. It was higher in male with no or one family living together and low economic status. In female, the experience rate of musculoskeletal symptom was different significantly according to body mass index, perceived health status, type of agricultural affairs. By the multiple logistic regression analysis for experience of musculoskeletal symptom, body mass index, perceived health status, and type of major agricultural affairs were the significant variables. It was higher as female farmer's body mass index was increasing and when female farmer's perceived health status was not healthier, and it was lower when female farmer did livestock farming as major agricultural affair. The experience rate of musculoskeletal symptom was higher in the busy season for farmers and treatment methods for musculoskeletal symptom were physical therapy, medication, herb medical treatment, etc. In conclusion, because most of farmers experience musculoskeletal symptom, continuous systematic study on musculoskeletal symptom of farmers and active effort to prevent and manage musculoskeletal symptom of farmers are necessary.

Prolotherapy in Orthopedic Field (정형외과 영역에서의 증식치료)

  • To describe the background, mechanism, clinical results and complications of prolotheapy based on the literature review. Prolotherapy is a minimally invasive injection-based treatment of chronic musculoskeletal pain, including ligament and joint laxity. The mechanism of this injection-based technique is to initiate a local inflammatory response with resultant tissue healing. The used proliferants are classified by bio-mechanism to act in three different ways as osmotic, irritants, and chemotatics. The most commonly used proliferant is hyperosmolar (10~25%) dextrose to act by osmotic rupture of cells. High resolution ultrasound imaging of musculoskeletal structure provide a more accurate diagnosis. Also ultrasound-guided intervention provides a more high efficacy and low rate of complications. The most common complication is local pain at the injected site, that is self-limited and good responsive to anti-inflammatory agents. Other complications are rare. It is reported that prolotherapy appears safe when performed by an experienced clinician. Prolotherapy has grown in popularity and has received significant recent attention. However there are limited evidence-based data supporting the indication and efficacy of prolotherapy in the treatment of chronic musculoskeletal pain or soft tissue injuries. Future studies are necessary to determine whether prolotherapy can play an independent and definitive role in a treatment for chronic musculoskeletal pain.

근골력계(누적외상성) 질환의 업무상 질병 인정기준 비교(한국, 일본, 미국)

  • 우리나라에서 근골격계 질환으로 인한 업무상 질병 인정 건수가 매년 급격히 늘고 있다. 근골격계 질환이란 생산직과 사무직에서 단순반복작업으로 인하여 목, 어깨, 팔꿈치, 손목, 손 등의 근육, 건, 신경 등 주변조직에 스트레스가 누적되면서 손상을 입혀 만성적 통증이나 감각이상 등을 나타내는 질환으로,2001년에는 산재로 승인된 질환자 수가 1,598명으로 전년보다 58%가 증가하는 등 우리나라에서 중요한 직업병의 하나로 자리매김하고 있다. 그러나 우리나라에서는 재해판정기준은 있으나 구체적인 인정요건에 대한 지침이 정립되지 않고 있어 업무상 질병으로 인정할 때 어떻게 판단하여야 하는지에 대한 문제가 심각해질 우려가 있다. 따라서 본고에서는 한국, 일본, 미국의 업무상 질병 인정기준을 비교하고 그중에서 작업관련성에 대한 각국의 ㅊ차이를 비교함으로서 어떤 문제가 해결되어야 할 것인지를 논하고자 한다.

Effect of seat height of chair on muscle activity of erector spinae and rectus abdominis (의자 좌면 높이가 척추 세움근과 배곧은근의 근 활성도에 미치는 영향)

  • This study examined the effects of the seat height of a chair on the muscle activity of the erector spinae and rectus abdominis. Thirty healthy subjects were asked to sit on chairs at three different seat heights. The muscle activities of both the erector spinae and rectus abdominis were measured by surface electromyography. The data were analyzed by repeated one way ANOVA and the muscle activity was compared according to the seat height. The alpha level was set to 0.05. The results showed that the muscle activities of the erector spinae were not significantly different among the three seat heights. The muscle activities of the rectus abdominis were significantly different among the three seat heights. Both the rectus abdominis muscle activities were significantly greater in the low seat height than the other seat heights. These results showed that the seat height of the chair affects the muscle activities of the rectus abdominis muscle, leading to musculoskeletal pain, such as low back pain. Therefore, the seat height of a chair with a correct sitting position is important for preventing musculoskeletal pain.

Impact of Musculoskeletal Pain on Pedestrian Crossing among the Aged (노인의 근육뼈대계 통증이 횡단보도 보행에 미치는 영향)

  • This study attempted to examine the impact of musculoskeletal pain on pedestrian crossing using a sample that consisted of 282 aged people. The research method involved an examination of the musculoskeletal pain of the elderly using a musculoskeletal questionnaire. The subjects were also told to walk across four- and six-lane pedestrian crossings while their gait velocity, step count, and step time were measured. The research results for pain by body regions indicated that waist pain had the highest complaint rate(33.3%). When there was musculoskeletal pain in the waist and leg/foot, gait velocity also decreased and step count and step time increased. However, usage of a cane turned out to have a positive impact on pedestrian crossing. Nevertheless, no significant relationship between waist and leg/foot pain and pedestrian crossing characteristics was found. We expect there will be continuous further studies on the subject of diverse physical problems of the aged and pedestrian crossing.

The Effects of Musculoskeletal Symptom and Burden Work on Presenteeism among Gastrointestinal Endoscopy Unit Nurses (소화기 내시경실 간호사의 근골격계질환 증상, 근골격계부담작업이 프리젠티즘에 미치는 영향)

  • Purpose: This study aimed to identify the effects of musculoskeletal symptoms and burden on presenteeism among nurses in a gastrointestinal endoscopy unit. Methods: This was an observational cross-sectional study. Data were collected through self-reported questionnaires administered to 140 nurses working in the gastrointestinal endoscopy unit of a hospital located in Busan metropolitan city. Results: The body part with the most musculoskeletal symptoms was the back (73.2%), and the most common musculoskeletal burden work was "when you have to stand or maintain the same posture for a long time in a lead apron protection clothes." The factors most related to work impairment were working hours, musculoskeletal symptoms, and musculoskeletal burden, with an explanatory power of 63.3%. Factors affecting perceived productivity were working hours and musculoskeletal symptoms, with an explanatory power of 29.2 %. Conclusion: To reduce work impairment and increase the perceived productivity of nurses in gastrointestinal endoscopy units, various programs and improved working environments are needed that can improve musculoskeletal symptoms and reduce musculoskeletal burden.

Related factors on musculoskeletal symptoms in selected Korean female office and blue-collar workers (우리나라 일부 사무직과 생산직 여성근로자에서 근골격계 자각증상과 관련요인)

  • Objectives : To investigate the related factors on musculoskeletal symptoms among Korean female workers. Methods : A total of 1,875 female workers (1,002 office workers and 873 blue-collar workers) was analyzed. They were randomly selected among female workers by cluster sampling and interviewed by well trained interviewer, who filled out a structured questionnaire on the musculoskeletal symptoms. Other factors such as general and occupational characteristics including workplace location, employment condition, work duration, and working condition were also investigated. Results : Current smokers and drinkers were significantly more common in blue-collar workers than in office workers. Those working full time were significantly more common in blue-collar workers than in office workers. Those working over 8 hours per day were also popular in office workers than in blue-collar workers. Subjective musculoskeletal symptoms were significantly higher in the type of work including excessive repetitive work, work using awkward stretching hand upwards, work using repetitive wrist movements, static work using awkward hand posture, and heavy manual lifting and handling works. The mean score of the symptoms were significantly higher in blue-collar than in office workers. The significantly different working factors between two groups were work duration, employment condition and daily work hours. Conclusion : We suggest that the type of work including awkward and repetitive works as well as job type may be one of the most important factors related to musculoskeletal symptoms of female workers in the workplace. Preventive strategy will be needed to prevent the negative impact on health status of working women in the workplace.

Factors Affecting Quality of life in the Elderly with Chronic Musculoskeletal Pain (근골격계 만성통증 노인의 삶의 질 영향요인)

  • Purpose: This study aimed to identify factors affecting the quality of life of the elderly people with chronic musculoskeletal pain. Methods: The data were collected from 307 older adults aged 65 years or older with chronic musculoskeletal pain, who visited senior welfare centers in two cities. We used self-rated questionnaires including NRS for pain, WHOQOL-BREF for quality of life, Pain Response Inventory for coping responses to pain, and MSPSS for social support. Stepwise multiple regression analysis were performed using SPSS/WIN 23.0 to identify factors affecting the study subjects' quality of life. Results: The regression model explained 43% of quality of life, which was statistically significant (F=34.11, p<.001). Educational level of high school (${\beta}=.13$, p=.006), pain (${\beta}=-.13$, p=.013), restriction of function (${\beta}=-.13$, p=.028), accommodative pain coping (${\beta}=.24$, p<.001), family support (${\beta}=.18$, p<.001), colleague's support (${\beta}=.25$, p<.001), and perceived health status (${\beta}=.25$, p<.001) were identified as influential factors on subjects' quality of life. Conclusion: Developing integrative interventions is necessary to improve accommodative pain coping skills and to engage family and colleague in support for positive perception of older adults' health status and management of symptoms.
