근거기반실무를 활용한 응급간호 수업 운영의 효과 (Effect of Emergency Nursing Class using Evidence-Based Practice)

  • 본 연구의 목적은 근거기반실무를 활용한 응급간호 수업 운영의 효과를 확인하기 위함이다. 본 연구는 단일군 전후설계연구이다. 본 연구는 일개 대학교의 3학년 간호대학생 54명을 대상으로 시행되었으며, 자료수집기간은 2019년 9월 2일부터 12월 2일이었다. 근거기반실무를 활용한 응급간호 수업은 근거기반실무의 5단계를 활용하여, 13차시로 진행되었다. 수업 운영은 강의, 팀기반학습, 사례기반학습으로 구성하였다. 자료분석은 SPSS/WIN 21.0 프로그램을 이용하였으며, 기술통계와 paired t-test를 이용하여 분석하였다. 근거기반실무를 활용한 응급간호 수업 후, 근거기반실무 신념(t=-2.76, p=.008), 근거기반실무 역량(t=-5.035, p<.001)은 통계적으로 유의하게 향상되었다. 반면 비판적사고성향은 통계적으로 유의한 차이가 없었다. 연구 결과는 근거기반실무를 활용한 응급간호 수업이 간호 학생의 근거기반실무 신념과 역량을 향상시키는 효과적인 교육전략임을 확인할 수 있었다.

간호조직의 지식관리, 간호사의 근거기반실무 신념 및 역량이 근거기반 의사결정에 미치는 영향 (Knowledge Management, Beliefs, and Competence on Evidence-Based Practice, Evidence-Based Decision Making of Nurses in General Hospitals)

  • Purpose: The purpose of this study was to explore how knowledge management of hospital and nurses' beliefs and competences on evidence-based practice can affect evidence-based decision making. Methods: In this descriptive study, a total of 184 nurses who were working in the five general hospitals participated. The data were collected through a self-administered questionnaire in September, 2014. Data were analyzed using descriptive statistics, independent t-test, one-way ANOVA, Pearson correlation coefficient, and step-wise multiple regression with SPSS/WIN Statistics 21.0 program. Results: Evidence-based decision making was correlated with EBP beliefs (r=.55, p<.001), EBP competence (r=.57, p<.001), and knowledge management (r=.50, p<.001). Hierarchical regression analysis showed that EBP beliefs (${\beta}=.18$, p=.005), EBP competence (${\beta}=.37$, p<.001), organizational knowledge management (${\beta}=.27$, p<.001) explained 48.6% of evidence based decision making (p<.001). Conclusion: The study results indicated that evidence-based practice competences, organizational knowledge management, and evidence-based practice beliefs were important factors on evidence-based decision making. In order to improve evidence-based practice among nurses through organizational knowledge management, EBP beliefs and competence at individual level need to be considered and incorporated into any systemic training of EBP.

프리셉터 역할을 경험한 간호사의 근거기반실무에 대한 신념, 지식 및 수행 수준 (Level of Beliefs, Knowledge and Performance for Evidence-Based Practice among Nurses Experienced in Preceptor Role)

  • Purpose: As Evidence-Based Practice (EBP) has increasingly been proven as a means of cost-effective and higher-quality healthcare, its successful implementing are challenging. This study done to identify EBP beliefs, knowledge and performance among nurses experienced as a preceptor. Method: A descriptive cross-sectional survey was conducted with a convenience sample of 249 preceptor nurses working in 9 general hospitals in Korea. Reliable and valid questionnaires (EBP beliefs scale, Evidence Based Practice Questionnaire, Research-related activities) were used and the data were analyzed using SPSS win 17.0. Result: Perceived beliefs on EBP were relatively positive (mean score 3.57 out of 5), and the level of knowledge was moderate (4.21 out of 7). However, performance of EBP was low (3.82 out of 7). Regularity in reading research journals and searching evidences using core web-database were rarely conducted. Statistically significant correlations were found between beliefs, knowledge and performance of EBP (all p<.05). Conclusions: This result indicates that education and training programs to facilitate EBP performance are needed among preceptor nurses.

상급종합병원 신규간호사와 프리셉터 간호사의 근거기반실무에 대한 신념, 간호실무지침에 대한 중요도와 수행도 (Belief in Evidence-Based Practice, Awareness of Importance and Performance of Nursing Practice Guidelines among Novice Nurses and Preceptors in a Tertiary General Hospital)

  • Purpose: This study was to investigate the belief in evidence-based practice, awareness of importance and performance of intravenous infusion and pressure ulcer evidence-based practice guidelines among nurses in a tertiary general hospital. Methods: The subjects of this study were 217 nurses working in a tertiary general hospital. Data collection was performed between February 11 and February 25, 2022. Data analysis was conducted descriptive statistics, t-test, hierarchical regression analysis, and Importance-Performance Analysis. Results: The mean score of belief for evidence-based practice among novice nurses was 3.34 out of 5, while preceptor nurses scored a mean of 3.41 out of 5. There was no significant difference in belief scores between novice nurses and preceptor nurses (t=-1.21, p=.227). The factors influencing the performance of evidence-based practice guidelines for intravenous infusion were belief in evidence-based practice (β=.14, p=.009) and importance of intravenous infusion (β=.51, p<.001), and the factors influencing the performance of evidence-based practice guidelines for pressure ulcer were belief in evidence-based practice (β=.15, p=.002) and importance of pressure ulcer (β=.65, p<.001). Importance-Performance Analysis of the evidence-based practice guidelines of two groups were used to identify common and different items. Conclusion: To improve the performance of evidence-based practice guidelines, it is necessary to enhance the evidence-based practice belief and importance of evidence-based practice guidelines. In particular, evidence-based practice should be provided to improve nursing quality through education on items of low-importance and low-performance and items of high-importance but low-performance guidelines identified through Importance-Performance Analysis.

임상간호사의 근거기반실무 신념과 연구 관련 행동의도에 따른 온 요법 간호 (Heat Application According to Nurse's Belief on Evidence-Based Practice and Behavioral Intention Related to Research)

  • Purpose: This study was done to compare heat applications by nurses in accordance to belief in evidence-based practice and behavior intention related to research. Methods: A cross-section survey design was used. Participants were 228 nurses from 5 institutions who completed the research questionnaire. Data were collected during July and August 2015. Results: The nurses reported that duration of heat applications was 2.5 minutes to 90 minutes. Frequently used heat application devices, in order of frequency, were rubber bag, gel pack and red clay pack. Most of the nurses (78%) responded that advice from colleagues was the most frequently used basic evidence for heat application. There was a statistically significant difference for the necessity of heat application practice guidelines between the high evidence-based practice belief cluster and the low cluster ($x^2$=15.39, p<.001). Conclusion: There were difficulties in providing consistent nursing interventions because of practical differences and absence of evidence-based guidelines for heat application. The researchers recommend that basic studies with various instruments be conducted and proper practice guidelines developed for heat application.

일개 대학병원 임상간호사의 근거기반 간호실무 신념, 역량, 조직의 지원에 관한 조사 (Nurses' Evidence-Based Practice Beliefs and Competencies, and Organizational Supports)

  • Purpose: The aim of this study was to assess nurses' evidence-based practice (EBP) beliefs and competencies, and organizational supports to develop EBP. Methods: A descriptive study was conducted using a survey of clinical nurses at a tertiary hospital in South Korea. Results: The survey was distributed to a total of 1,413 nurses and was completed by 1,318 nurses. There were significant differences in the EBP beliefs and competencies, and the perception of organizational supports among nurses at different educational levels. The EBP beliefs, EBP competencies, and organizational supports had a positive correlation with each other. EBP competencies were the highest in nurses with less than 3-years of clinical experience, and the perception of organization supports were the highest in nurses with more than 10-years of clinical experience. Conclusion: The findings suggest that educational programs, training, and organizational supports are recommended for facilitating successful EBP among nurses.
