Current Research Trends on Prevalence, Correlates with Cognitive Function, and Intervention on Sarcopenia in Community-dwelling Older Adults: Systematic Review (지역사회 거주 노인의 근감소증 유병률, 인지기능과의 관계 및 중재연구 동향: 체계적 문헌고찰)

  • This systematic review was performed to investigate relationship between sarcopenia and cognitive function among community-dwelling older adults. Studies which reported prevalence rates of sarcopenia in community-dwelling older adults, relationships between sarcopenia and cognitive dysfunction, or effective interventions in alleviated symptoms of sarcopenia were selected. Of 43 studies reviewed, prevalence rate of sarcopenia ranged from 2.5-42.4% in 25 studies, and 4 studies revealed a significantly positive correlation between sarcopenia and cognitive dysfunction. Among 12 randomized-control trials (RCTs), exercise intervention was used in 2 studies; nutrition intervention in 6; and 4 studies included both the exercise and nutrition interventions. All interventional studies reported positive effects on muscle mass, muscle strength, or physical function. However, only 6 studies met the high quality criteria of Risk of Bias, and there was no study which evaluated cognitive function as an outcome measure. Therefore, this study draws attention to the lack of high-quality RCTs and exclusion of cognitive function as an outcome measure in the research of sarcopenia. A comprehensive and sustained system including standardized sarcopenia screening, assessments of cognitive functioning, and evidence-based intervention programs is needed in order to prevent and manage sarcopenia amongst older adults in the community.

Association between nutritional status, sarcopenia, and frailty in rural elders (농촌 지역 노인의 영양 상태, 근감소증과, 노쇠의 연관성)

  • Objectives: Frailty and sarcopenia are recent important concepts in elder health care. Sarcopenia is the most important factor influencing frailty, and exercise and nutritional status are known to affect sarcopenia. The purpose of this study was to identify the relationship between nutritional status, sarcopenia, and frailty. Methods: This study was a cross-sectional design. The subjects of this study were 411 elderly people aged 65 or older from 10 villages in Gyeongnam. The survey tools were the K-FRAIL for frailty, the GDS-SF for depression, the SARC-F questionnaire for sarcopenia, and the DETERMINE for nutritional status. Frequency analysis, the chi-square test, and multiple logistic regression analysis were performed using the SPSS 25.0 program. Results: As a result of the chi-square test, there was a significant difference in the nutritional status and the frailty proportion (p<0.001), and there was a significant difference between frailty and suspected sarcopenia (p<0.001). After adjustment, nutritional status was significantly associated with sarcopenia (OR=2.946, p<0.001). In addition, nutritional status was significantly associated with frailty (OR=2.958, p<0.001), and sarcopenia also had a significant effect on frailty (OR=5.898, p<0.001). Finally, even after including sarcopenia, nutritional status had a significant effect on frailty (OR=2.246, p=0.002). Conclusions: Nutritional status can have both a direct effect on frailty and an indirect effect through sarcopenia, and it was found that sarcopenia also affects frailty. Therefore, it is necessary to evaluate sarcopenia and nutritional status and to evaluate their levels in the elderly and to take appropriate interventions.

  • Sarcopenia is an increasingly common disease among the elder that has recently received attention. Although the causes of sarcopenia are diverse, aging, dietary habits, lack of exercise are the one of the major factors. As the causes of sarcopenia are diverse, it is important to develop strategies for prevention and treatment. However, predicting sarcopnia accuartely is difficult due to the variety of factors involved. Here, machine learning can significantly improve the accuracy and convenience of predicting sarcopenia. However, since lifestyle habits and biological data are vast, using data without preprocessing may be inappropriate in terms of time complexity and accuracy. This paper reviews recent literature on sarcopnia and its causes, focusing on preprocessing the data to be used in sarcopnia prediction machine learning accrodingly.

Elderly Sarcopenia and Vitamin B Deficiency: A Relationship? (비타민 B 결핍에 의한 노인성 근감소증)

  • Sarcopenia is a leading cause of increased medical and nursing care costs among the elderly. In Korea, preventive measures for sarcopenia are mostly targeted toward the general elderly population without specific diseases. However, it is also necessary to implement measures for elderly individuals living in nursing homes and hospitals, where the prevalence of sarcopenia is high. Currently, computed tomography and/or magnetic resonance imaging are considered standard diagnostic tools. However, their complexity and time-consuming nature make them unsuitable for clinical use. The exact pathophysiological mechanisms of sarcopenia are unclear, as they involve various molecular biological pathways, including decreased exercise, protein and nutrient intake, changes in testosterone and growth hormone, and inflammation. Sarcopenia symptoms can lead to several diseases, such as osteoporosis, fractures, dementia, diabetes, and cardiovascular disease. Vitamin B deficiency is a significant factor in sarcopenia induction, with B vitamins being directly involved in energy and protein metabolism and nerve function. Vitamin B deficiency can lead to neuromuscular and neurogenic disorders, which often overlap with sarcopenia. Suboptimal intake of B vitamins, malabsorption, and anorexia are common among the elderly. This study aims to provide information on the role of water-soluble B vitamins in preventing and controlling muscle mass loss and deterioration among the elderly with sarcopenia. In addition, we discuss the potential of myokines from the B vitamin family in modulating sarcopenia.

Association Metabolic Syndrome with Sarcopenia in Korean Stroke Patients : Korean National Health and Nutrition Examination Survey Data(2008-2011) (뇌졸중 유병자의 대사증후군과 근감소증의 관련성: 국민건강조사(2008년-2011년)자료를 근거로)

  • The purpose of this study was to examine the association metabolic syndrome with sarcopenia in Korean stroke patients. We used the Korean National Health and Nutrition Examination Survey data from 2008 to 2011 and enrolled a total of 316 stroke patients older than 40 years. Data were analyzed by Rao-Scott ${\chi}^2-test$, generalized linear model and composite sample multiple logistic regression. The prevalence of sarcopenia was 47.3% in men and 46.3% in women. The prevalence of metabolic syndrome was 50.3% in men and 73.6% in women. After adjusting for age, smoking status, alcohol consumption, exercise, education, income, sroke sequla and stroke duration, men with sarcopenia had increased risk of metabolic syndrome (95% CI: 2.454-18.482, p<.001). This finding can be used to develop evidence-based health promotion program to prevent stroke reccurance for stroke patients.

Pre-clinical Models and Exercise Effects for Sarcopenia and Frailty (근감소증과 노쇠의 전임상 모델 및 운동 효과)

  • The purpose of this review is to summarize current knowledge regarding animal sarcopenia and frailty models and their diagnosis indexes. In addition, we introduce the effects of exercise interventions on sarcopenia and frailty syndrome. Data collection and analysis (fifteen published articles from 2005~2017) were conducted by using keywords' sarcopenia index, frailty index, exercise and mice, and so on' in academic search engines such as Google scholar and Pubmed. Sarcopenia and frailty are the representative syndromes in elderly peoples which those symptoms can be effectively prevented or delayed by extremely adjusted long term exercise interventions (The combined oxidative and resistant exercise program might be ideal.).

  • 근감소증은 영양부족, 운동량 감소 그리고 노화 등으로 정상적인 근육의 양과 근력 및 근 기능이 감소하는 질환을 말한다. 근감소증은 보편적으로 유럽 근감소증 실무그룹분석(EWGSOP)에서 정의한 측정 방법을 따른다. 본 논문에서는 근감소증 진단을 위한 영상 분할 모델을 개발하고 외부검증하는 방법에 대해서 제안한다. 우리는 CT 영상에서 L3 영역을 선별하여 자동으로 근육, 피하지방, 내장지방을 분할할 수 있는 인공지능 모델을 U-Net을 사용하여 개발하였다. 또한 모델의 성능을 평가하기 위해서 분할영역의 IOU(Intersection over Union)를 계산하여 내부검증을 진행하였으며, 타 병원의 데이터를 이용하여 같은 방법으로 외부검증을 진행한 결과를 보인다. 검증 결과를 토대로 문제점과 해결방안에 대해서 고찰하고 보완하고자 했다.

  • 의료영상기반의 인공지능 연구는 질환의 조기진단 및 예측 분야에 눈부신 기술발전이 되어왔다. 근감소증 질환은 다양한 기저질환을 기반으로 발생하며, 특히 60대 이상은 30%의 유병율을 갖는다. 해당 질환은 임상적인 진단 방법의 발달과 임상 결과가 알려지면서 관심이 증가하고 있다. 최근 근감소증 진단방법 중의 하나로 CT 또는 MR 의료영상을 통한 진단방법이 제시되었다. 본 논문에서는 인공지능을 기반으로 하여, 근감소증을 진단하기 위해 척추부위 중 Lumbar 3 영역의 근육, 지방 영역의 영상분할 모델을 제시하고자 한다. 이를 위해 인공지능 영상분할 모델을 개발하는 과정과 그 근육과 지방의 영상분할 결과를 보인다. 본 논문에서 제시한 영상분할모델을 통해 근감소증을 빠르게 진단할 수 있을 것으로 기대한다.

Sarcopenia and Smoking: Systematic Review and Meta-analysis (근감소증과 흡연: 체계적 문헌고찰 및 메타분석)

  • This study performed meta-analysis of published articles to examine the relationship between sarcopenia and smoking. We searched the PubMed, EMBASE, Cochrane library, and RISS database sup to January 2017 using search terms such as sarcopenia AND (smoking OR tobacco OR cigarette). A total of 25 articles were included in the analysis (seven cohort studies and 18 cross-sectional studies). Incidence of sarcopenia in smokers and non-smokers was analyzed by the random effects model. Incidence of sarcopenia was OR 1.49 (95% CI 1.21, 1.84, p<.001, I2 = 88.20%), Asian (OR 1.28, 95% CI 0.91, 1.78, p=0.150, I2=85.32%), and Western (OR 1.65, 95% CI 1.25, 2.17, p<.001, I2=91.08%), Western smokers showed a significantly higher incidence of sarcopenia:males and females with OR 1.25(95% CI 1.11, 1.40, p<.001, 12=87.4%) and 1.80(95% CI 0.98, 3.30, p=.057, 12=89.1 %) respectively. Male smokers showed significantly higher incidence of sarcopenia. Based on the results of the study, smoking in Western males is related to sarcopenia. However, since the criteria for the diagnosis of sarcopenia and the criteria for smoking were different, cumulative individual clinical studies applying uniform assessment for the evaluation of sarcopenia and smoking status are needed in the future.

Convergence Factors Affecting Sarcopenia in Middle-Aged and Older Women in Korea: A Cross Sectional Study by Using 5th KNHANES (한국 중년 이후 여성의 근감소증에 영향을 미치는 융합적 요인: 제 5기 국민건강영양조사 자료를 활용한 단면조사 연구)

  • This study was conducted to identify the factors affecting sarcopenia in middle-aged and older women in Korea. This is a cross sectional study by using 5th Korea National Health and Nutrition Examination Survey data. The data were analyzed by using the SPSS 26.0 program. The prevalence of sarcopenia was 10.8%. There were significant differences according to ASM, age, education, current job, marital status, perceived health status, energy intake, protein intake, BMI, body fat, alcohol drinking, menopause, central obesity, impaired fasting glucose and chronic disease. Woman with central obesity and osteoarthritis increased risk of sarcopenia each 4.15 times(p<.001), 3.06 times(p=.041) and energy intake decreased risk of sarcopenia 0.99 times(p=.043). In order to prevent and manage sarcopenia, strategies for managing central obesity and osteoarthritis in mddile-aged and older woman and adequate food intake are needed.
