Digital Mammography as a Screening Tool in Korea (국가암검진사업에서 디지털 유방촬영술의 현황과 과제)

  • More than 4 million women undergo breast cancer (BC) screening with mammography each year in Korea. Digital mammography (DM) was introduced in 2000, and it has been reported to have a higher diagnostic accuracy than screen-film mammography (SFM) or computed radiography (CR) in women with dense breasts. According to a study using data from the National Cancer Screening Program for BC, the diagnostic accuracy of DM was higher than those of SFM and CR, regardless of age, breast density, and screening round. Currently, despite high supply rate among OECD countries, the distribution of DM equipment is approximately 35% in Korea. For quick replacement with DM, it will be necessary to improve its fee for the National Health Insurance and support an educational program for radiologists. In addition, efforts should be made to increase the accessibility of DM.

The status of breast cancer screening of women at a breast clinic in a small city in Korea -Using medical records- (중소도시 일 유방클리닉 방문 여성의 유방암 검진현황 -의무기록을 이용한 후향적 조사연구-)

  • The status of breast cancer screening of women at a breast clinic in a small city in Korea-Using medical records Purpose: This study was conducted to investigate breast cancer incidence by age, including those under 40, and to emphasize the importance of early breast cancer screening for young breast cancer patients. Method: The study was designed as a retrospective survey method. We investigated the medical charts of 483 patients diagnosed with breast cancer among 23,200 visitors to local breast clinic from May 1, 2010 to April 30, 2020. Results: The average age of 483 patients was 47.9, with 36% in their 40s and 28.6% under 40. Among the patients, 5.4% had a family history of breast cancer, and 70.8% were pre-menopausal. The most common reason for visiting the clinic was the mass(54.2%). The size of tumors was less than 2cm (56%), followed by 2-5cm (43.4%) and more than 5cm (5.6%). The size of tumors was smaller in the asymptomatic case than in the symptomatic case. In the pathologic results, invasive ductal cancer was the most common at 80.3%. Conclusions: Breast cancer screening program should be activated even for young women under the age of 40. We suggest educating the importance of breast cancer screening, and lowering the age of national breast cancer screening program.

Convergence Study on Factors that Influence Cancer Screening Rate in Korea and Japan (한일간 암검진 수검율에 영향을 미치는 정책적 요인에 대한 융합연구)

  • The purpose of this convergence study was to analysis the current status of the cancer screening and background healthcare systems in Korea and Japan. First, Cancer Screening Program is coordinated well with National Health Insurance Service(NHIS) under a unified insurer system in Korea. But in Japan, there are over 3,500 insurer and coordinating a comprehensive strategy for cancer screening promotion has been very difficult. Second, Rate of cancer screening is influenced by public healthcare system. In Korea public healthcare does not cover a wide rage of services. Almost free cancer screening provides high incentive to participation. It is very important to understand the impacts of each healthcare system when designing an efficient cancer screening system.

Effect of Gastric Cancer Screening on Patients with Gastric Cancer: A Nationwide Population-based Study (위암 환자에서 국가암검진의 효과)

  • Background: This study was performed to evaluate the effect of gastric cancer screening through analysis of screening-related data. Methods: We investigated claims data of gastric cancer from 2009 to 2015. We evaluated whether the screening was performed to prior to registration as patients with gastric cancer. The effect of gastric cancer screening was also analyzed by gender. Results: We collected total 196,293 patients with gastric cancer. 74% of them had previous experience of gastric cancer screening. In patients with screening, early gastric cancer was 33.4% and advanced gastric cancer was 17.3%. 22,548 (15.5%) patients were diagnosed with gastric cancer within 2 years after screening. In the case of patients without screening, early gastric cancer was 15.1% and advanced gastric cancer was 25.3%. In case of men, 76% of them confirmed gastric cancer through screening, and 70.2% of women confirmed the gastric cancer. In both men and women, the rate of early gastric cancer was higher among those with screening than those without screening. Conclusion: In this study, we were able to indirectly confirm the stage shift of gastric cancer screening. However, within 2 years after screening, not a few patients with gastric cancer were diagnosed. Therefore, more studies are warranted to in the future.

  • 본 논문에서는 변화하는 ICT 기술을 기반으로 의료정보의 빅데이터화를 통해 건강권에 대해 장애인과 비장애인을 중심으로 살펴본다. 전국민 대상의 국가건강검진 중 암 검진에 수검율을 조사하고 여성 장애인의 수검 요인을 분석해 장애인의 건강권 보장이 가능한지 살펴본다. 국가암검진 중 30대 이상 여성을 중심으로 진행되는 자궁경부암 수검 요인을 건강행태적 요인으로 분석한 결과 통계적으로 유의한 차이를 보였다.

Cervical Cancer Screening Factors in Women With Disabilities Using Big Data Analysis -Focused on National Cancer Screening Subjects- (빅데이터 분석을 이용한 여성 장애인의 자궁경부암 수검 요인 -국가암 검진 대상자 중심-)

  • Despite the fact that Korea has supported national cancer screening worldwide for a long time, the cancer screening screening rate is low compared to other health screenings and infant screening. In particular, women with disabilities show a lower screening rate for cervical cancer screening compared to women without disabilities. Therefore, from 2011 to 2019, this study investigated the trends in cervical cancer screening rates of women with disabilities and non-disabled women aged 30 and older, and analyzed the factors affecting cervical cancer screening of women with disabilities at the individual and regional level through the cervical cancer screening rates among regions. The purpose of this study is to identify the factors that affect regional variation by dividing into regions. From 2011~2019, the cervical cancer screening rate of people with disabilities was the highest in Daejeon and the lowest in Sejong and Jeju, and the gap in cervical cancer screening rate of people with disabilities by region was analyzed to be up to 13.1%.
