Sinus floor augmentation at the time of tooth removal (상악 구치부의 발치와 동시에 시행하는 상악동저 증강술을 이용한 임프란트 증례)

  • Rapid crestal bone resorption following maxillary tooth loss is further accentuated in the posterior regions because of pneumatization and enlargement of the maxillary sinuses. A treatment rationale that allows preservation and augmentation of vertical available bone at the time of posterior maxil-lary tooth extraction may offer numerous therapeutic benefits which are more short courses of ther-apy and no needs of additional surgical augmentation. The present study comprised 3 patients who had 4 posterior maxillary teeth with no evident bone between the tooth apex and sinus floor, as estimated through preoperative radiographic analysis. Sinus floor augmentation at the time of tooth extraction was chosen for the ltreatment of these patients. After the tooth was carefully extracted, the empty alveolus was thoroughly debrided and a trephine approach was performed. Particulated autogenous bone was gently pushed beyond the empty alveolus to elevate the sinus membrane using an osteotome. The distance between bone crest and si-nus floor was radiographically estimated 4 months after the first procedure. Another procedure was then carried out to place the implants of 11 mm length without another augmentation procedure. All implant were clinically stable, with no sign of infection. The presented surgical procedure performed at the time of extraction of posterior maxillary teeth in close proximity to the sinus floor allowed placement of implants of proper length.

  • 지금까지 여러 가지 형태의 기기를 이용하여 보고된 사람의 교합력은 그 최대치가 매우 다양하다. 본 연구에서는, 교합력을 측정하기 위하여 새로운 측정기기를 개발하였다. 본 기기를 이용하여 전치부 교합력 측정 시에는 대칭적으로 최대한 물도록 하악을 유도하여 최대 교합력을 측정하였고, 구치부는 편측으로 물도록 하악을 유도하여 최대 교합력을 측정하였다. 교합 시 치아간 수직고경은 15.1mm 였다. 측정기기 내 strain gauge를 사용하여 전기저항의 변화를 힘으로 전환하였다. 마이크로 프로세서가 수치를 계산하면 액정화면에 수치가 표시된다. 실험실 내 기기교정 검사에서는 50kg과 100kg의 부하를 가하였다. 개발된 교합력 측정기기의 임상적 신뢰도를 시험하기 위해 건강한 치과대학 재학생 10명을 대상으로 최대 교합력을 측정 하였다. 이 새로운 측정기기로 측정된 교합력은 이전연구에서 기록되었던 것보다 더 높은 수치가 나왔다. 또한 제1대구치와 전치부에서 최대 교합력을 측정한 경우 실험자 내와 실험자 간에 통계적으로 유의한 신뢰성을 얻을 수 있었다. 본 기기는 안정적이고 조작성이 쉽다고 판명되었다. 그러므로, 본 기기는 더 큰 연구 집단의 선별검사에 매우 유용하게 사용할 수 있을 것이며, 또한 치주질환이 있는 치아나 임플란트, 악관절 장애를 가진 환자의 교합력 측정과 같이 특정된 경우의 교합력 측정에도 유용할 것이다.

RECURRENT CEMENTOBLASTOMA IN LEFT MANDIBLE : A CASE REPORT (좌측 하악 구치부에 발생한 재발성 백악모세포종의 치험례)

  • Cementoblastoma is a benign ectomesenchymal odontogenic neoplasm that forms a mass of cementum or cementum-like tissue continuous with the tooth root. Cementoblastoma represents 1% to 6.2% of all odontogenic tumors, and occurs more than 75% arise in the mandible, with most cases arising in the molar and premolar regions. About 75% arises before the age of 30. Radiographically, it appears as a radiopaque mass with a thin radiolucent rim attached to the roots of a tooth. The recurrence rate is 37% in the current study and cortical expansion and perforation of the cortex are common findings in lesions that subsequently recurred. It is apparent that recurrence rate depends largely on the completeness of removal than any other factor. Here we present a 20 year old, female case of an unusual multiple recurrent cementoblastoma around a previous lesion which had been surgically excised 2 years ago.

RETROSPECTIVE STUDY OF SINTERED POROUS-SURFACED DENTAL IMPLANTS IN RESTORING THE POSTERIOR MAXILLA (상악 구치부에 식립된 엔도포아 임플란트의 후향적 연구)

  • Purpose: The purpose of this retrospective report was to analyze long-term survival rate of sintered porous-surfaced dental implant ($Endopore^{(R)}$ Dental Implant system, Innova Corporation, Toronto, ON, Canada). Methods: 61 partially edentulous patients were received a total of 127 Endopore dental implants in the maxilla. Of the 127 implants, 24 implants were restored with individual (ie, non-splinted) crowns, while 103 implants were splinted to other implants. Medical records and radiographs were evaluated and analyzed by the cumulative survival rate, location of implants, implants length and diameter, crown/implant ratio and whether the implant was splinted. Chi squire test was used statistically. Result: Of the 127 implants, 8 implants (6.3%) were removed and and cumulative survival rate was 93.7%. Conclusion: Endopore implants showed satisfactory results after up to 8 years function periods in the edentulous posterior maxilla.

Root resection of compromised molars (치근절제술을 이용한 구치부의 치료)

  • Purpose: Studies reported controversial results about prognosis of root resection with 0%-38% failure rate. The purpose of this study is to consider clinical efficacy of root resection. Materials and Methods: This study reported three cases of root resection which were performed on different sites. In first case, lower right first molar showed pus discharge and 7 mm of probing pocket depth. It was diagnosed as fracture of mesial root. Hemisection of mesial root was performed. In second case, upper right first molar showed severe bone loss on disto-buccal root and 7 mm of probing pocket. Resection of disto-buccal root was performed In third case, lower left second molar showed severe bone loss on distal root including apex and 15mm of probing pocket depth. Hemisection of distal root was performed. Result: In these three cases of root resection, compromised molars were treated successfully and conserved. Conclusion: Root resection is a successful procedure treating compromised multi-rooted teeth and conserving teeth when proper case selection is performed.

CENTRAL GRANULAR CELL ODONTOGENIC TUMOR(CGCOT): A CASE REPORT INCLUDING LIGHT MICROSCOPY, IMMUNOHISTOCHEMISTRY AND LITERATURE REVIEW (상악 구치부에 발생한 Central Granular Cell Odontogenic Tumor(CGCOT)의 치험례)

  • Central granular cell odontogenic tumor(CGCOT) is a very rare lesion that consists of densely packed granular cells with numerous scattered strands of odontogenic epithelium interspersed throughout the tissue. CGCOT was initially reported in 1962 by Cough et al as central granular cell ameloblastic fibroma. But, recently, this term is inappropriate because of histologic and chronologic differences. CGCOT is usually present as painless swellings. Radiographs show a well-demarcated radiolucent or mixed radiopaque-radiolucent lesion. The average age on presentation of CGCOT is 47.3 and women are 75% more likely to develop this lesion than men. The tumor only occur in tooth bearing areas of the jaw with 88% of cases occurring in the mandible and 12% involving the maxilla, usually in an equal distribution between the caninepremolar-molar areas. This tumor is benign, and care is effected by localized surgical excision. We report an additional case of CGCOT that occurred in the Rt. Maxillar premolar/molar region of a 32-year old man with literature review.

Fixed hybrid prosthesis (고정성 하이브리드 수복)

  • Often there are edentulous patients with severe vertical resorption of the alveolar ridge so that implant placement in the mandibular posterior is impossible. In this case, implants can be placed in the anterior portion of the mandible and the patient can be treated with an implant assisted overdenture, but the inherent discomfort of the removable prosthesis can not be overcome. However, if implants are appropriately placed between the mental foramen of both sides and a fixed hybrid prosthesis is fabricated to the patient, the patient will be satisfied without the inconvenience of the removable prosthesis. However, the treatment plan should be well designed to prevent disadvantages and expected complications of fixed hybrid prosthesis. Based on my clinical experience, I will explain treatment plan and protocol in detail.

  • Digital dentistry is becoming a mainstream of dental practice. We are already used to intraoral scan, guided implant surgery, manufacture of prosthesis (CAD-CAM) in our daily practice; those are not sensational anymore. However, treatment planning with digital simulations is not spotlighted enough. In this report, a digital mockup was utilized to visualize the final goal of maxillary rehabilitation including all anterior and posterior teeth, and persuade a patient who wanted restoration of only anterior teeth. We will also discuss the use of digital mockup and its actual copy as the guide of altered vertical dimension through the clinical process.

Full Mouth Rehabilitation in a Patient with Limited Restorable Space (수복 공간이 부족한 환자에서의 완전구강회복)

  • Loss of posterior support may cause overloading and excessive wear of remaining teeth. Moreover, the extrusion of antagonistic teeth leads to the destruction of the occlusal plane. The loss of vertical dimension of occlusion (VDO) also emerges clinically, which may bring the loss of esthetic appearance and function. These patients who suffer from the loss of posterior support, often require vigorous periodontal treatments (osteotomy, crown lengthening) and extensive oral rehabilitation. Sixty three years old female patient visited for the prosthetic treatment of the posterior edentulous area. She had no other systematic disease and parafuctional habits for prosthetic treatment. Intraoral and radiographic examinations were done. The evaluation of VDO and vertical dimension of rest position were evaluated for proper prosthetic procedures and diagnostic wax up was done. As a result of diagnosis, VDO was increased by 2 mm considering the loss of VDO and space for the prosthetic treatment. After the pretreatments, initial preparation of teeth and provisionalization were carried out. Six weeks later of provisionalizaion, final preparation and impression was performed. Using the duralay resin copings, jaw relation was registered. The master cast was mounted and definitive restoration was fabricated. After the evaluation of esthetic and function, pick up impression for clinical remounting was done. Lucia jig was made for new jaw relation and occlusal adjustment on the articulator. Definitive restoration was delivered and the patient was periodically recalled for additional occlusal adjustment. From this case, the satisfactory functional and esthetic results through full mouth rehabilitation with increase vertical dimension were achieved.

Orthognathic surgery on Skeletal Class III patiens with collapsed vertical dimension: case report (수직고경이 붕괴된 골격성 III급 부정교합자의 악교정 수술 치험례)

  • Patients who lost posterior teeth due to periodontitis or dental caries have collapsed vertical dimension, unstable occlusion and change of the mandibular position. In particular, patients in orthognathic surgery, clinician should re-establish the pre-operative stable position of mandibular condyle in articular fossa and favorable vertical dimension for high post-operative stability of mandible. Therefore, interdisciplinary approach and co-operation, including prosthetics, orthodontics, oral and maxillofacial surgeon, from diagnosis and treatment plan is important to get a good outcome. This case report was patients who had collapsed occlusal plane due to severe dental caries on maxillary molars with skeletal Class III malocclusion. Before orthognathic surgery, resetting of maxillary occlusal plane with temporary removable denture was performed. Then successful multidisciplinary approach was done and lead to acceptable clinical outcome.
