Diagnostic Performance of the Intraoral Radiographs on the Interproximal Dental Caries (구내방사선 사진상의 인접면 치아우식진단능 평가)

  • The purpose of this study was to evaluate the diagnostic performance of the senior dental students for the proximal dental caries on intraoral radiographs and to compare it with the dental hospital residents, the reference group. It was also investigated the diagnostic performance according to the carious lesion depth. Thirty-five intraoral periapical and bitewing radiographs with 213 proximal surfaces included in this study were selected from the dental patients at Chonnam National University Hospital. The observers were 181 senior dental students from 5 dental schools and 40 dentists who were second year resident from 5 dental hospitals. They were asked to evaluate the presence or the absence of the proximal dental caries. The results were as follows: 1. The mean of the hitting rate for the overall observers was 184.51 surfaces and the diagnostic accuracy was 86.62%. 2. The diagnostic performance of the sound proximal tooth surfaces was very high, i.e., 91.5% true negative rate and 8.5% false positive rate. 3. The diagnostic performance of the dentist group was higher than the student group(P<0.05). 4. The proximal dental caries perceptibility increased as the lesion depth increased significantly(P<0.001) except no difference between the carious lesion depth III and IV (P>0.001).

A STUDY ON SKELETAL RELAPSE PATTERNS FOLLOWING ORTHOGNATHIC SURGERY OF CLASS III PATIENTS : COMPARISON BETWEEN SSRO AND IVRO (제 III급 부정교합자의 악교정 수술후 골격적 재발 양상에 관한 연구 : 구내 시상 분할 골절단술과 구내 상행지 수직 골절단술의 비교)

  • To evaluate the relapse pattern and long-term stabilities depanding on surgical methods following orthognathic surgery of Cl III patients, the author selected 24 subjects(10 male, 14 female) operated by SSRO and 26 subjects(10 male,16 female) operated by IVRO. Each subject took four lateral cephalograms : just before surgery(T1), within 48hrs after surgery(T2), 4-8 wks after surgery(T3), 6 month or more after surgery(T4), and the landmarks were digitized. The differences of relapse patterns in each interval between two groups were compared and the significance of correlation among the variables of each group was tested. The obtained results are as follows ; 1. Horizontal early relapse was forward movement of mandible in SSRO group, as compared to the backward movement in IVRO group, and there was a statistical significance between the two groups. 2. Vertical early and late relapses were decreases in anterior facial height in both groups and there was no statistical significance between the two groups. 3. There was a statistical significance in negative correlation between mandibular horizontal late relapse and surgical change of articular angle in SSRO group. 4. There was a statistical significance in negative correlation between amount of mandibular set-back and mandibular horizontal early relapse in both groups.

Induction of Micronuclei in Human and Mouse Lymphocytes Irradiated with Gamma Radiation and Effect of Panax ginseng C.A. Meyer (마우스와 사람 림프구에서 방사선에 의한 미소핵의 형성 및 고려인삼의 효과)

  • The frequencies of ${\gamma}$-ray-induced micronuclei (MN) in cytokinesis-blocked (CB) lymphocytes at several doses were measured in three donors of human and C57BL/6 mice. Measurements performed after irradiation showed a dose-related increases in MN frequency in each of the donors studied. The relative sensitivity of mouse in spleen lymphocytes (SLs) compared with human peripheral blood lymphocytes (PBLs) was estimated by best fitting linear-quadratic model based on the radiation-induced MN data over the range from 0 cGy to 400 cGy. In the case of MN frequency with 0.2 per CB cell, the relative sensitivity of mouse SLs was 1.67. Compared with the radiation-induced MN formation in the PBLs of human, the SLs of mouse were more radiosensitive. Using this MN assay with human PBLs and mouse SLs, studies were performed to determine whether the water fraction of ginseng (Panax ginseng C.A.Meyer) against radiation-induced MN in human PBLs after in vitro irradiation (3Gy) and in SLs of C57BL/6 mice after in vivo irradiation (3Gy). The frequency of MN in human PBLs was reduced by water fraction of ginseng (0.5mg/ml of medium) both pre-and post treatment (p<0.0l) in vitro. In addition, the frequency of MN in mouse SLs was also reduced by pretreatment of ginseng (2mg/ml of drinking water for 7days) in vivo. The data suggested that the ginseng may reduce cell damage caused by ${\gamma}$-rays in vitro and in vivo. Further studies are needed to characterize better the protective nature of ginseng extract, its fractions and compounds.

Cytokinesis-blocked micronuclei in the human peripheral lymphocytes following low dose γ-rays irradiation (저선량의 감마선 피폭된 사람 말초 임파구의 미소핵을 이용한 방사선 생물학적 피폭선량 측정법 연구)

  • To determine if micronucleus (MN) assay could be used to predict the absorbed dose of victims after accidental radiation exposure, we carried out to assess the absorbed dose depending on the numerical changes of MN in human peripheral blood lymphocytes after $^{60}Co\;{\gamma}-rays$ exposure in the range of 0.25 to 1 Gy, respectively. The MNs were observed at very low doses, and the numerical changes according to doses. Satisfactory dose-effect calibration curve is observed after low dose irradiation of human lymphocytes in vitro. When plotting on a linear scale against radiation dose, the line of best fit was $Y=(0.02{\pm}0.0009)+(0.033{\pm}0.010)D+(0.012{\pm}0.012)D^2$. The dose-response curve for MN induction immediately after irradiation was linear-quadratic and has a significant relationship between the frequencies of MN and dose. These data show a trend towards increase of the numbers of MN with increasing dose. The number of MN in lymphocytes that were observed in the control group is $0.1610{\pm}0.0093/cell$. Accordingly, MN assay in human peripheral lymphocytes could be a useful in viva model for studying radio-protective drug sensitivity or screening test, microdosimertic indicator and radiation-induced target organ injury. Since MN assay is simple, rapid and reproducible, it will also be a biodosimetric indicator for individual dose assessment after accidental exposure.

COMPARISON OF RELATIONSHIPS BETWEEN ANTRAL FLOOR AND MAXILLARY ROOT APEX IN BISECTING AND PANORAMIC TECHNIQUES (구내법과 파노라마법에 의한 상악치근과 상악동저의 관계 비교)

  • This study was performed to compare the difference of intraoral bisecting and panoramic techniques in evaluating the relationship of antral floor and maxillary roots. The vertical distances from maxillary root apices to antral floor were measured on both orthopantomograms and bisecting projections obtained from fifty subjects. The results were as follows, 1. Tooth lengths measured on orthopantomogram were larger than on bisecting projection and the magnification ratios were 1.08-1.17. 2. The dimensions from maxillary root apices to antral floor measured on orthopantomogram were larger than on bisecting projection. 3. The above results held true regardless of age and sex.

  • Purpose: To determine whether the mandibular radiomorphometric indices in panoramic radiography are correlated with the bone mineral density of Cu-equivalent images in intraoral film. Materials and Methods: The bone mineral density (BMD) of the mandibular premolar area was measured in the Cu-equivalent image of intraoral film. The Panoramic Mandibular Index (PMI) and Mandibular Cortical Width (MCW) were measured in panoramic radiographs of six dry mandibles, and the Pearson correlation between PMI, MCW, and BMD were tested. Results: There were no significant correlations between PMI and BMD (r = 0.280), nor between MCW and BMD (r =0.237). Conclusion: The results show that PMI and MCW were poor diagnostic indicators of mandibular BMD in the six dry mandibles used in this study. The correlationship between the mandibular radiomorphometric indices (PMI and MCW) and mandibular BMD needs to be researched further using large in vivo patient samples.

A STUDY OF THE RESOLUTION OF DENTAL INTRAORAL X-RAY MACHINES (치과 구내 X-선 촬영기의 사용연한에 따른 해상능에 관한 연구)

  • The purpose of this study was to assess the resolution and focal spot size of dental X-ray machines. Fifty dental X-ray machines were selected for measuring resolution and focal spot size. These machines were used in general dental clinics. The time of installation of the X-ray machine varies from 1 year to 10 years. The resolution of these machines was measured with the test pattern. The focal spot size of these machines was measured with the star test pattern. The following results were obtained: 1. The resolution of dental intraoral X-ray machines was not significantly changed in ten years. 2. The focal spot size of dental intraoral X-ray machines was not significantly increased in ten years. The statistical analysis between the mean focal spot size and nominal focal spot size was significant at the 0.05 level about the more than 3 years used machines.

A absorbed and effective dose from the full-mouth periapical radiography using portable dental x-ray machine and panoramic radiography (ORIGINAL ARTICLE - 이동형 구내방사선촬영기로 촬영한 치근단 방사선촬영과 파노라마방사선촬영의 흡수선량과 유효선량 평가)

  • Purpose: The purpose of this study was to measure the absorbed dose and to calculate the effective dose for full-mouth periapical radiography using the portable dental x-ray machine and panoramic radiography Material and Method: Thermoluminescent chips were placed at 25sites throughout the layers of the head and neck of a tissue-equivalent human skull phantom. The man phantom was exposed with the portable dental x-ray machine and panoramic unit. During full-mouth periapical radiography the exposure setting was 60 kVp, 2 mA and 0.15 ~ 0.25 seconds, while during panoramic radiography the selected exposure setting was 72 kVp, 8 mA and 18 seconds. Absorbed dose measurements were obtained and equivalent doses to individual organs were summed using ICRP 103 to calculate of effective dose. Result: In the full-mouth periapical radiography, the highest absorbed dose was recorded at the mandible body follow with submandibular glands and cheek. Using panoramic unit, the highest absorbed dose was parotid glands and the following was back of neck and submandibular glands. The effective dose in full-mouth periapical radiography using portable dental x-ray machine was 46 ${\mu}Sv$. In panoramic radiography, the effective dose was 38 ${\mu}pSv$. Conclusion: It was recommended to panoramic radiography for general check in the head and neck area because that the effect dose in the panoramic radiography was lower than the dose in the full-mouth periapical radiography using portable dental x-ray machine.

Reduction of Entrance Surface Dose Depending on Shielding Methods for Panoramagraphy (파노라마 X선 검사시 차폐방법에 따른 Entrance Surface Dose 저감)

  • Panoramagraphy was the second most used intraoral radiography utilized in Korea, resulting in 17.8% in university dental hospitals, 24.8% in dental hospitals, and 31.4% in dental clinics. Depending on increased demand like orthodontics and implant, panoromagraphy tends to consistently increase. This study were used lead glasses and lead shielding to reduce unnecessary radiation to the eyeballs and thyroid. ESD was 41.4% when radiation was shielded with the lead glasses while reducing 47.3% of ESD by shielding the X-ray tube area with shielding lead. There was no statistically significant difference. The lead glasses is appropriated to reduce unnecessary radiation exposure to the eyeballs.

A RADIOLOGICAL STUDY ON THE ADDITIONAL ROOT OF THE MANDIBULAR FIRST MOLARS AND PREMOLARS (하악 제1대구치와 소구치의 과잉치근에 관한 방사선학적 연구)

  • The purpose of this study was to evaluate the prevalence and distribution of additonal root in the mandibular 1st molars and premolars by means of the analysis of the full mouth periapical radiographs in 6,082 patients visited the Dental Infirmary of Kyungpook National University Hospital from March 1989 to February 1993. The results were as follows: 1. The incidence of additional root in the mandibular 1st molars was revealed to be 9.32% in total examined patients, and there was a higher prevalence in males(I1.35%) than in females(7.46%). And bilateral occurrence was revealed to be 4.26%. 2. The incidence of additional root in the mandibular premolars was to be 3.57% in total examined patients, and there was a higher prevalence in males(4.28%) than in females(2.91%). And bilateral occurrence in the 1st premolars was revealed to be 1.53%.
