R&D Capability Analysis of Domestic Fire-fighting Safety and Rescue Research Program (국내 소방안전 및 구조구급 R&D 역량 분석)

  • In this study, the budget investment state of the technology classification structure in a fire-fighting and safety research development program and the capability of a fire-fighting safety research program were analyzed between 2007 and 2013 with support from the ministry of public safety. The Ministry of public safety and security's fire safety research funding has shown a gradual increasing tendency. More 80% of the budget was invested in fire safety and disaster response technologies and the fire safety and special disaster prevention response technology of R&D. From the results of analyzing the capacity of fire-fighting safety and rescue safety, it was found to have a moderate level of 3.5 on a 7 point scale basis. To develop the Ministry of Public Safety and Security's fire safety research in the future, the flexibility and reasonableness of the budget/legal/institutional framework and support should be ensured to improve the operational capability of resources. In addition, a proliferation program is necessary for the commercialization and industrialization of research results.

The Effect of a Mechanical Chest Compressions for Out-of-hospital Advanced Cardiac Life Support (병원 전 전문심장소생술을 위한 기계적 가슴압박기의 효과)

  • The purpose of this study is to evaluate the quality of chest compression by conducting comparison research between mechanical chest compressor(LUCAS) and manuale cardiopulmonary resuscitation(CPR) in a out-of-hospital environment and suggest effective advanced cardiac life support using mechanical chest compressors. For this, a out-of-hospital cardiac arrest was simulated with a team of 3 ambulance workers, and manuale CPR and CPR using LUCAS were performed on site and during transport in an ambulance. The research results are as follows: the comparison of manuale CPR between on site and in an ambulance revealed that on-site manuale CPR showed significant differences in the average compression depth, compression rate, and relaxation rate. Second, the comparison between manuale CPR and LUCAS in an ambulance showed significant differences in the average compression depth, compression rate, the number of compression per minute.

Effect of a Triage Education Program on Accuracy of Triage -Focused on 119 Emergency Medical Service Team- (중증도 분류 교육 프로그램이 중증도 분류 정확성에 미치는 효과 -119구급대원을 중심으로-)

  • The study was conducted to determine the effectiveness of the triage training program utilizing pre-and post-training experiments designed for 119 emergency medical services teams. Objectives: This study evaluated the effectiveness of triage training programs on the accuracy of triage performed by 119 emergency medical services team staff who participated in the triage training program. Behavior: Participants in this study included 119 of the 166 EMS staff. In this program, a modified START triage consisting of a 20-minute theoretical presentation was presented to the participants. Data were analyzed using SPSS 21.0. Results: A significant increase in triage accuracy for 119 EMS teams(p<.001). And undertriage showed a significant decrease(p<.001). In addition, overtriage showed a decrease but was not statistically significant. Conclusions: The results obtained from this study showed that the triage training program was effective in improving the accuracy of the triage of multiple injury patients or disaster victims when presented to the 119 emergency medical services team. Therefore, these results suggest that it would be helpful to add triage training to the fire department's formal training program.

From Patients and Caregivers 119 Rescue party's Experiences for Violence in the Pre-Hospital Emergency Medical Field (환자 및 보호자로부터 구급대원이 경험한 병원 전 응급의료현장 폭력실태)

  • The purpose of this study was to investigate 119 rescue party's violence experiences and to analyze their responses of violence experiences based on the pre-hospital emergency medical field. Data were collected from the self-administered survey of 226 rescue party in Jeollabuk-do area in Korea from January 1 through March 2, 2009. The data were analyzed by descriptive statistics and logistic regression analysis using SPSS Win 12.0. The results were as follows: first, all of 119 rescue unit in the pre-hospital emergency medical field reported that they got violence experiences more than once a week, and 62% of rescue team were exposed to physical violence. Patient's factors that caused violence were drug abuse or alcohol(56.2%), on the other hand rescue party's factors were shortage of human resources. Second, 42% of the respondents replied that they did not have any educational experiences for prevention and coping methods about violence within 5 years. 77.4% of those surveyed indicated that they wanted to receive specific educations which can prevent and cope with violence. Third, the mean value of total violent response scores was 2.53, and emotional response score was 3.2, social response score was 2.22, and physical response score was 2.17. Fourth, violent response score related to general characteristics proposed that physical(t=-2.08, p=o.38), emotional(t=-7.13, p=.006), and total responses(t=-4.764, p=.000) were statistically significant. And emotional(t=4.257, p=.000) and total responses(t=8.1330, p=.000) related to age were also statistically significant. Finally, among current tenure qualification scores, social response(t=9.987, p=.012) and total score(t=8.130, p=.000) were statistically significant. Between fire departments, violent response score suggested that physical(t=9.987, p=.000), emotional(t=2.433, p=.012), social(t=6.738, p=.000), and total score(t=5.943, p=.000) were statistically significant.

Establishment an Environment for Smart Emergency Information Service (스마트 구급 정보 서비스를 위한 환경 구축)

  • Emergency care provides primary care and stability for the patient in the early stages. It saves a patient's life and minimizes the risk until that treated in a hospital. In the recent years, An attempts have been made to convergence medical technology and IT technology to overcome the limitations of temporal, spatial, and medical technology applications. In this study, we established an environment for emergency information services. The developed smart application for emergency rescue activities support can easily identify emergency situation and First Aid. Then, it can be transmitted to a remote medical guidance doctor so that emergency treatment and hospital response can be performed quickly.

  • To help emergency medical dispatchers receive rapid and accurate identification and corrective action status determination of an emergency call, and to support the automatic processing of a voice recognition system to the Korean emergency medical dispatch system, emergency call records were analyzed. Furthermore, a list of Korean symptoms expression were produced and the characteristics of the symptoms that appear on the actual wording of the telephone records were identified. This language list and its characteristics will be useful for training emergency medical dispatchers.

An Empirical Study on Emergency Medical Care Transportation Policy (응급의료 이송정책에 관한 실증적 연구)

  • This research made a survey to 119 EMT laying stress on general contents connected with job in a frame of mutually organic cooperation system between the processes, composing Emergency Medical Care Transportation Policy in Korea, as a step before hospital, of happening emergency patients, 119 first-aid service of the spot, transportation of patients, construction of communication network etc.. As a result of analysis to that, it is found that there must be systematic devices which makes EMT not to be caught on medical dispute, a modernization of emergency equipments, professional first-aid agents, a proper personnel arrangement. Consequently, it suggests policy plan focusing on structural and functional aspect to improve an Emergency Medical Care Transportation system into a realistic one.

A study on the preceptor role awareness of senior 119 paramedics in the field (선임구급대원의 구급현장 프리셉터 역할인식에 대한 연구)

  • Purpose: The aim of this study was to provide basic data to aid in the effective adoption of the preceptor system for new paramedics by assessing the preceptor role awareness among senior 119 paramedics in the field. Methods: A questionnaire was administered to 182 senior paramedics who each had more than 3 years field experience. The questionnaire measuring preceptor role awareness was composed of 64 items with responses based on a 5 point Likert scale. Data were collected from July 31, 2018 to August 29, 2018, and were analyzed with IBM SPSS version 24.0. Results: The average score for preceptor role awareness was 3.87 (supervisor = 3.95, resource person = 3.88, role model = 3.85, and instruction designer = 3.81). There were significant differences in preceptor role awareness among the participants based on competence (p =.002) and clinical skill (p =.000). Also, there were high positive correlations between preceptor role awareness and its subdivision (p <.01). Conclusion: For effective operation of the preceptor system in the future, the preceptors' role as designers of instruction and role models should be enhanced, Also, future preceptors are very important in the improvement of competence and clinical skill in the preceptor system. Therefore, it is necessary to provide training that enables new preceptors to effectively adopt and operate the preceptor system.

Design of a hospital assignment scheme for ambulances based on minimum cost maximum flow algorithm (최소비용 최대유량 알고리즘에 기반한 구급차의 환자이송 병원 배정 기법의 설계)

  • This paper presents a design and analyzes the performance of a hospital assignment and reassignment scheme for emergency rescue services based on minimum cost maximum flow algorithm. It consists of flow graph building, link capacity updating, and allocation discovering steps. The efficiency of the algorithm makes it possible to reallocate hospitals even in case of dynamic changes in the number of patients or hospitals. The performance measurement result obtained from a prototype implementation shows that the proposed scheme can reduce the transport time requirement miss by up to 24%.

Convergent Influence Effect on the High - Quality CPR of 119 Paramedics by Wearing Personal Protective Equipment by Level (구급대원의 수준별 개인용 보호구 착용에 따른 심폐소생술 품질에 미치는 융합적인 영향)

  • The purpose of this study is to investigate the effect of wearing clothes, Level B, and Level C PPE on normal CPR using mannequins. The paramedics who participated in this experiment were 20 paramedics with more than 5 years of experience in firefighting area C. It was found that chest compressions in the process that hand off time was 11.9 seconds in the uniform wearing group, 11.4 seconds in the level C PPE wearing group, and 13.1 seconds in the SCBA wearing group. In other words, wearing personal protective equipment prevents the movement of paramedics and uses compensatory movements, which may increase the difficulty of efficient first aid. If this situation persists, it may cause fatigue and damage to the body of paramedics. Rescue and paramedics should wear level personal protective equipment in case of emergencies and conduct rescue and CPR training.
