2D AND 3D STRUCTURAL STUDY OF RETE RIDGE IN ORAL MUCOSA AND SKIN PADDLE OF VARIOUS FREE FLAPS (구강내 점막과 유리피판에 사용되는 피부의 rete ridge에 관한 2차원 및 3차원적 구조 연구)

  • Objects : With the advancement of tissue engineering techniques, the effort to develop bioartificial mucosa have been actively delivered. The problem we met with this technique is the lack of mechanical strength between kerationocyte layer and dermal layer, where in the normal skin and mucosa, they are tightly bound with rete ridge structure. The purpose of this study is to understand the 2D and 3D structure of rete ridge of mucosa and skin paddle for rendering more biomimetic structure to the artificial mucosa. Materials and Methods : Oral mucosa and skin from the patients who received the oral surgery and maxillofacial reconstruction were harvested. The epidermis was separated from the dermis after treating with dispase for 12-16 hours. H&E staining was performed for 2D(dimensional) structure study and confocal LASER and SEM study were performed for 3D structure. Mean height(Sc) and arithmetic mean deviation(Sa) of all surface height were calculated. Results : The average height of rete ridge of skin flap was between $67.14{\mu}m$ and $194.55{\mu}m$. That of oral mucosa was between $146.26{\mu}m$ and $167.51{\mu}m$. Pressure bearing area and attached gingiva of oral mucosa showed deeper rete ridges. Conclusion : To obtain the adequate strength of artificially cultured keratinocyte skin and mucosa flap, it is necessary to imitate the original skin and mucosa structure, especially rete ridge. Through this study, 2D and 3D rete ridge structure of normal mucosa and skin was obtained. These results can be used as basis for substrate morphology for keratinocytes culture.

Antimicrobial Activities of Propolis against Oral Candidiasis by Candida Albicans -Effect of Microbial Inhibition Using Propolis- (구강 캔디다증 알비칸스에 대한 프로 폴리스의 항균 활동 -프로폴리스를 이용한 미생물 억제 효과-)

  • Propolis is an extremely safe natural antimicrobial substance that has been reported to have powerful antibacterial efficacy. The aim of this study was to evaluate the inhibitory effects of propolis against Candida albicans (C. albicans). Propolis was collected from the honey bee Apis mellifera. The strain of C. albicans was cultivated overnight in liquid media incubated at $37^{\circ}C$. The antimicrobial activity was investigated using phosphate buffered saline (PBS), 3% sodium hypochlorite (NaOCl), 0.1% chorhexidine (CHX), and propolis extracts ($5{\mu}l/ml$, $10{\mu}l/ml$). C. albicans were sensitive to 3% NaOCl, 0.1% CHX, and propolis ($5{\mu}l/ml$, $10{\mu}l/ml$) with zones of inhibition of 15, 14.5, 16, and 17 mm, respectively. The CFU of PBS, 3% NaOCl, 0.1% CHX, $5{\mu}l/ml$ and $10{\mu}l/ml$ of propolis led a 1, 7, 7, 5 and 7-log reduction. Among the groups tested, C. albicans was most sensitive to $10{\mu}l/ml$ of propolis, which showed the largest inhibition zones. Therefore, propolis can be a new antimicrobial therapy for oral mucosa disease in traditional medicine.

MANAGEMENT OF ORAL MUCOSITIS OWING TO CHEMICAL BURN BY INTOXICATION OF AGRICULTURAL CHEMICALS(GRAMOXON) : REPORT OF CASES (농약(Gramoxon)중독에 의한 화상으로 발생된 구강점막염 치험)

  • Chemical burns onto oral mucosa which are infrequent, may result from contact with a wide variety of chemical agents. The degree of injury depends on the chemical, its concentration, duration of contact, and the natural penetrability and resistance of the tissues involved. Chemicals do not usually "burn" in that they do not cause destruction by hyperthermic activity. Rather, they damage tissue by causing coagulation of protein by one of several processes, reduction, oxidation, desiccation, corrosion, or vesication. Paraquat(Gramoxon) is the most frequently agricultural chemicals that induce the severe toxic reactions onto the organs of human body in Korea. The toxic reaction are composed of pulmonary edema and fibrosis, formation of hyaline membrane, inflammatory reaction and bleeding tendency, owing to the cell damage by the production of superoxide radicals. The contents of essential treatment in paraquat intoxication are commonly airway and breathing maintenance, gastric lavage, much hydration and diuresis, hemoperfusion and medications for the removal of the chemicals and the prevention of various complications. The sedative oral dressings, such as, orabase ointment application, warm saline gargling, lidocaine viscous gargling and oral gargling by the mixed solutions(tetracycline, prednisolone and 10% dextrose water) are important for the improvement of chemical oral mucositis and the comfortable feeding of diet. The authors managed properly two cases of oral chemical mucositis that were occurred by the incorrect use of agricultural chemicals(paraquat) and report the cases with the review of literatures about care of the chemical intoxication and oral mucositis.

Management of Traumatized Gingival Wound Using Tissue Adhesivein Dental Hygiene Practice (치위생 임상에서 조직접착제를 활용한 외상성 치은열창의 처치)

  • The purpose of this paper is to present the potential application of tissue adhesive in the management of traumatized gingival wound in clinical dental hygiene practice. Cyanoacrylate adhesive has been used for closure of superficial laceration without suturing, which is available in periodontal and oral surgery. Small gingival or mucosal lacerations may occur by improper or excessive instrumentation of the dental hygienist during scaling and root planing procedure. In this circumstances, tissue adhesive is very effective, simple, and convenient method as an alternative to conventional wound closure by suturing. The tissue adhesive consists of monomeric n-butyl-2-cyanoacrylate, which polymerizes quickly in contact with tissue fluid. The sutureless treatment of gingival laceration with tissue adhesive has advantages of good esthetic results, less trauma, time saving, antibacterial and hemostatic effects. In addition, local anesthesia as well as re-visit for dressing and removal of suture are not required. Use of tissue adhesive could be beneficial to both dental hygienist and patient in the management of procedural error.

DERMOID CYST IN AN INFANTILE UPPER LIP (영아의 상순에 발생한 유피낭종)

  • Dermoid cyst is a developmental cyst that is lined by epidermis-like epithelium and contains dermal adnexal structures in the cyst wall. Dermoid cyst most occur in the eye brow although occasionally develop in other locations such as midline of the floor of the mouth, tongue, lip and buccal mucosa. The lesion is slow growing and painless without lympadenopathy. The contents of the dermoid cyst can be caseous or sebaceous. The size can vary from a few millimeters to 12cm in diameter. Histologically, dermoid cyst is lined by epidermis and adnexaes such as sweat gland, sebaceous glands and hair follicles are present in the cyst wall. The 2-year-old female of this case visited Department of Pediatric Dentistry, College of Dentistry, Yonsei University with a chief complaint of the emergence of mass on her upper lip. An excisional biopsy was carried out for a histological examination and sebaceous gland was observed in the cyst wall. The lesion was diagnosed as dermoid cyst. Dermoid cyst does not recur if complete excision is accomplished.

365 nm LED laser treatment on beagle for gingival whitening without gum dermabrasion (잇몸 박피술 없는 치은 미백을 위한 비글에서 365 nm LED 레이저 치료)

  • Purpose: Gingival whitening is one of dental treatment purposes which is close to treating aesthetic disorders. Initial gingival whitening treatment was done by dermabrasion using a high power Diode Laser. However, this treatment method cannot be free from any infection or pain after the treatment. Therefore, we have decided to progress gingival whitening treatment using a low power LED laser. Materials and Methods: The laser was irradiated on pork meat then the safety of output power, temperature change and skin denaturalization was measured. Bison 365 nm LED laser was irradiated on oral mucosal pigment of a 15 - 20 kg beagle for 15 min for 1 - 2 weeks, one or two times each. Any pigment loss was checked through Hematoxyline-Eosin staining. Results: The melanin pigments at the area of 365 nm LED Laser irradiation were decreased. Conclusion: The 365 nm LED Laser proposed in this study is considered to compensate the bleaching effect achieved by either using Diode laser or surgical methods.

Applying the New Technology for Making Pontic Ridge Lap in Posterior Bridge Restoration (대체 신기법을 적용한 구치부 교의치 pontic ridge lap 제작방법)

  • The purpose of this study is to investigate the production method of posterior bridges pontic ridge lap type which prevents the infection in bridge pontic base and is able to cleanse itself, in the process of producing final prothesis that maintains healthy mucous membrane of oral cavity and interproximal papilla, minimizing diastema, is aesthetic and has no effect on pronunciation. New technology is applied to make optimal pontic base which prevent inflammation and clean itself and its products were clinically evaluated in 10 places of dental clinics in busan and gyeongnam. The making of posterior 3 unit bridge pontic base, it was presented as the new technology of forming ridge lab type and to carry out clinical validation, existing conventional method and the new technology were compared. Pontic base made with the existing conventional method cause infection and other periodontal disease by 96% but the pontic base made with the new technology cause infection and other periodontal disease by 3%. Remains of food cause infection and other periodontal disease 100% by the existing conventional method and 91% by the new technology, showing a distinct difference. However, after a gargle, the new technology had low 13%. Additionally, the pontic base made with the existing conventional method showed 71% of chance, the new technology method showed 8% of chance in terms of self-cleansing.

NON-SURGICAL TREATMENT WITH TYING OF MUCOCELE (봉합사를 이용한 점액종의 비외과적 처치)

  • Mucocele is a mucous retention phenomenon which is caused by a laceration to the duct of minor salivary glands causing extravasation of mucin into the connective tissue forming a cyst-like space. Sialolithiasis of minor salivary glands and chronic obstruction of salivary glands may also cause such a phenomenon. Mucocele is a smooth, rounded sessile mass with diameters varying from 1 to 15mm of sudden appearance. Mucocele tying directly beneath the mucosa may rupture spontaneously and decrease in size, but frequently recurs. Lower lip is most frequently affected, and the mouth floor and buccal vestibule may also be affected. Enucleation of the cyst is needed and removal of minor salivary glands, marsupialization and cryotherapy may also be done. The mucocele frequently recurs after its removal. A 1-year-old female patient visited the hospital with a complaint of a swelling on the lower lip since 4 months before. She had no pain history but 4 months ago, fell and such symptom appeared since then. On her first visit, a bullous solid, opaque lesion of 5mm in diameter was noted. Treatment choice of surgical approach and nonsurgical approach were explained to the guardian. Considering the patient's age, the guardian agreed to a nonsurgical procedure. Treatment was carried out by tieing 3-0 silk to the base of the lesion. One week later, the tie loosened and was re-tied. A week later, the mucocele disappeared. Mucocele on the lower lip may be usually be treated by surgical removal, but this may traumatize the surrounding minor salivary gland causing it to recur. Also, surgicial removal may induce an ischemic change causing sialometaplasia. In case of young patients or children with management problems, non-surgical methods such as this tie method may be used. This tie method does not need any local anesthesia and has no pain, no secondary infection, and low bleeding tendency.

  • The Dandy-Walker syndrome is a relatively rare congenital malformation occurring about one in 25,000-35,000 pregnancies. It is characterized by hydrocephalus, cystic dilatation of the fourth ventricle, complete or partial absence of the cerebellar vermis, and other various extra-CNS malformations. We report a dental caries treatment of a 4-year old girl with Dandy-Walker syndrome and partial trisomy 9q. The patient visited Seoul National University Dental Hospital due to multiple caries. The cause of multiple caries is mainly presumed as patient's eating habit caused by her general condition. She was still using milk bottle because she was having difficulties swallowing solid food due to breathing problem while eating. The treatment was performed under general anesthesia considering patient's condition; delayed development, very poor cooperation and respiratory problem. The dental procedure was successfully conducted and there were no postoperative complications. In consideration of her eating habit and oral hygiene problem, regular check appointment was recommended.

NT-proBNP as a useful tool in diagnosing incomplete Kawasaki disease (불완전 가와사끼병에서 NT-proBNP의 진단적 역할)

  • Purpose : To determine the efficacy of the N-terminal fragment of B-type natriuretic peptide (NT-proBNP) as a useful diagnostic method in children with incomplete Kawasaki disease (KD). Methods : Ninety-six patients who were diagnosed as having KD between January 2008 and June 2009 were enrolled in the study. American Heart Association recommendations for diagnosis were used, and patients were divided into the complete KD and incomplete KD groups. Blood tests including NT-proBNP were performed on admission day. Nineteen patients who had other febrile diseases other than KD were enrolled as control. Results : Thirty-three patients (34%) had incomplete KD. Change in the lips and oral cavity and conjunctivitis were the most common clinical features, but their frequency was lower than complete KD (76% vs 98%, 76% vs 90%). Patients with incomplete KD exhibited significantly higher NT-proBNP level than that of control ($1,407.7{\pm}1633.5pg/mL$ vs $126.2{\pm}135.5pg/mL$, $P$<0.001). An NT-proBNP cutoff value of 158 pg/mL provided a sensitivity of 81% and a specificity of 74% for diagnosis of incomplete KD. Conclusion : NT-proBNP assay can be clinically useful for the diagnosis of incomplete KD, if the patient has persistent fever, change in the lips and oral cavity, and conjunctivitis, and if the patient with those symptoms is suspected to have incomplete KD.
