Dental Care Utilization Patterns and Its Related Factors of the Rural Residents (경상북도 일부 농촌지역 주민의 치과의료이용양상 및 관련요인)

  • Objectives: This study was conducted to analyze the dental care utilization patterns and related factors of the rural residents. Methods: The data collected by interview and self-administered questionnaire survey of 524 peoples of Seongju county in Gyeongsanbuk-do. The summarized results are as follows. Results: The rate of persons who experienced the oral disease was 52.5% during 1 year and it was at most in the age group of 40-49. The rate of persons who had experienced the oral disease were investigated according to general characteristics, perception of oral health, being of regular treatment facility. Therefore the rate of persons who had experienced the oral disease was significantly higher the younger peoples, worse oral health status and being of the regular treatment source than the other groups. During 1 year period, 64.0% of the cases had treated the perceived oral disease, 36.0% did no action at all during last year. Among respondents, 49.4% had treated their oral disease at dental clinics, 8.0% had treated at community health center or subcenter and remains did not treated at all. The results of logistic regression analysis suggested that statistically significant factors in dental health care utilization were educational level, degree of pain, oral health status and regular treatment facility. Therefore the dental health care utilization rate was higher at groups with the high educational level, serious pain, better oral health status and being of the regular treatment source than other groups. 45.5% of the rural residents did not treat their oral disease immediately due to the no identified need, limitation of time(19.2%), economic limitation(19.2%), and geographical limitation(9.0%). Conclusions: In consideration of above findings, we may conclude that oral health community program to prevent oral diseases should be intensified, oral health education to raise oral health knowledge should be performed periodically.

Effect of Obesity and Psychological Stress on Oral Health (비만과 스트레스가 구강건강에 미치는 영향)

  • The purpose of this study is to understand the effects of obesity and stress on oral disease in Korean adults by using the data of the Fifth Korea National Health and Nutrition Examination Survey. The subjects of this study are 4,627 adults at the age of 19 to 64, and the data were analyzed by using SPSS Windows ver.18.0. The results are as follows: Obesity was higher in men and the age of 40 to 64 than the other. And the group with lower education or lower income level tended to show significantly higher obesity (p<0.05). As for stress, women, the age of 19 to 39 and the subjects with higher academic career tended to indicate significantly higher stress (p<0.05). As for the effects of obesity on oral disease, there were no significant difference between body mass index (BMI) or waist-hip ratio (WHR) and oral disease but there was statistically significant difference between stress and temporomandibular joint disorder (TMD) (p<0.05). Even when the subjects with stress were obese or abdominally obese, there was no significant difference in oral disease (p>0.05). As to correlation among variables, there were correlations between BMI and WHR, stress and TMD, masticatory problems and periodontal disease or TMD (p<0.05). About the effects of general characteristics, obesity, and stress on oral disease, age was the variable influencing TMD, and age and abdominal obesity were the variables influencing masticatory problems (p<0.05). In this society, the environment surrounding individuals is fairly complex, and the concept of health including quality of life has more complex meaning than in the past. Various factors are influencing obesity and stress, and they are also influencing oral health and behaviors. Accordingly, it will be needed not only to make efforts to reduce obesity and stress but also to employ approaches from different perspectives to improve oral health.

Treatment Outcome and Prognosis of the Outpatients with Orofacial Pain (구강안면통증 환자의 치료결과와 예후에 관한 연구)

  • The purpose of this study was to evaluate treatment outcome and prognosis of the patients with orofacial pain disorders who visited for treatment in the Department of Oral Medicine, Dankook University Dental Hospital from January 2002 to December 2004. Orofacial pain disorders were categorized into TMD(myogenous, arthrogenous and muscle-joint combined TMDs), neuropathic pain disorder, oral soft tissue disease and complex condition simultaneously having more and two aforementioned categories and treatment period, method and treatment outcome were evaluated. The results of this study were as follows; 1. Average longevity of treatment period was the longest in the neuropathic pain, followed by soft tissue disease, complex conditions, arthrogenous TMD, muscle-joint combined TMD and myogenous TMD in order. 2. When treatment methods were largely categorized into pharmacologic, physical and oral appliance therapy, pharmacologic therapy was used the most frequently for the patients with neuropathic pain or oral soft tissue diseases, oral appliance therapy for those with arthrogenous TMD and physical therapy for those with myogenous TMD. 3. Of physical therapeutic methods used in our clinic, EAST and microwave was employed the most frequently in the patients with myogenous TMD, ultrasound for those with arthogenous TMD and LLLT for those with neuropathic pain or oral soft tissue disease. 4. In comparison with change of pain after treatment, there existed a tendency that pain from neuropathic pain disorders persisted while pain from TMD was getting better or totally disappeared. 5. Concerning the change of mouth opening range in the TMD subgroups, there was no significant difference among the subgroups but significant difference existed among opening ranges, indicating comfortable maximum mouth opening increased the most following treatment. Improvement of active range of mouth opening was the most considerable in those with disc displacement without reduction. It can be said on the basis of the findings from this study that various treatments currently used for the orofacial pain showed good results with TMD in regards with pain control and improvement of function, suggestive of favorable prognosis, while neuropathic pain or soft tissue disease was the clinical conditions difficult to resolve, requiring a long and persistent treatment.

The Correlation between Diabetes and Periodontitis in Korean Adults (한국 성인의 당뇨병과 치주질환의 관련성)

  • The purpose of this study was to evaluate the correlation between diabetes and periodontal diseases in Korean adults based upon the data of the 2007 Korea National Health and Nutrition Examination Survey. The subjects of this study were 190 adults aged ${\geq}19years$ whose CPI(Community Periodontal Index) and diabetes related medial histories including its component-specific examination data had been available, and the results of this study are summarized as follows: 1. Diabetes related characteristics associated with the prevalence of periodontal diseases include diabetes education, diabetes treatment, and glycosylated hemoglobin (HbA1c). The incidence rate of periodontitis was higher in the subjects who had not received diabetes education or treatment or who showed at least 7.0 of glycosylated hemoglobin (HbA1c) levels (apt to be uncontrolled). 2. General health behaviors associated with the prevalence of periodontal diseases include smoking and drinking. The incidence rate of periodontitis was higher in those who were currently smokers or who had higher frequencies of drinking per week. 3. Oral health behaviors associated with the prevalence of periodontal diseases include the history of oral health examination in the previous year, the use of proxabrush or dental floss, and perceived oral health status. The incidence rate of periodontitis was higher in those who had not received oral health examinations in the previous year, who had used neither proxabrush nor dental floss or who perceived that their own oral health was poor.

Effects of Metabolic Syndrome on Periodontal diseases in Korean Adults (한국 성인의 대사증후군이 치주질환에 미치는 영향)

  • This study aimed to evaluate the correlation between metabolic syndrome and periodontal in the adult Korean population on the basis of the 2009 Korea National Health and Nutrition Examination Survey(NHANES) data. Of 19-year-old or older adult patients suffering from their disease diagnosed as metabolic syndrome, 1,315 subjects whose community periodontal index(CPI) information was fully available were finally analyzed, and the following conclusions were drawn. 1. It was found that of sociodemographic characteristics, age and educational background correlated to the prevalence of periodontal diseases. With increasing age and poorer educational background, the rates of periodontal diseases increased(p<0.001). 2. It was revealed that of general health behaviors, smoking, alcohol drinking and BMI correlated with the prevalence of periodontal diseases. Higher rates of periodontal diseases were shown in the group of patients who were current smokers(p<0.05), had the higher frequency of drinking per week(p<0.001), and had higher BMI(p<0.005). 3. It was ascertained that of oral health behaviors, the history of oral health examination in the previous year, the use of proxabrush or dental floss, and perceived(i.e., subjective) oral health status correlated to the prevalence of periodontal diseases. The higher rates of periodontal diseases were seen in those who had no history of oral health examination in the previous year(p<0.001), had used neither proxabrush nor dental floss(p<0.05), and perceived that their own oral health was poor(p<0.001).

Relationship Between Oral Contraceptive Use and Periodontal Disease in Korean Women (제4기 국민건강영양조사 자료에 근거한 한국 여성의 경구피임약 복용과 치주질환의 관련성)

  • This study aims to evaluate the influence of oral contraceptive(OC) on periodontal disease. Research data was used the results derived from the Fourth Korean National Health and Nutrition Examination Survey(KNHANES), and 1,101 Korean women, aged 19 to 50 years with non-pregnant and premenopausal, were selected. The chi-square test and logistic regression analysis were performed to identify the relationship between OC use and periodontal disease, according to factors of demographic and socioeconomic, oral health behavior, OC use period, and periodontal status. In the results, OC use was statistically related with age, marital status and smoking. No significant differences were found in between the gingival condition and current OC use. The prevalence of periodontal disease increased with age, but did not have significant correlations with period of OC usage, smoking and oral health behavior. Final analysis was indicated that OC use is not associated with periodontal disease, regardless of adjustment for confounding variables.

Relationship between National Exam Stress and Subjective Oral Symptom in Allied Health Students (보건계열 대학생의 국가시험 스트레스와 주관적 구강이상증상)

  • The aim of this study was to confirm for relationship between national exam stress and oral health status on allied health students. For this, 829 of allied health students were surveyed in four college and university located in Chungbuk and Kyungbuk. Survey tool were constituted general characteristic(4 items), national exam stress(16 items) and oral health status(34 items). The result of study, national exam stress of female were higher than male and that of department of dental hygiene, nursing, emergency medical service students were higher than other students(p<0.05). Subjective oral symptom were higher on high stress group than other groups(p<0.05). Relationship between stress and subjective oral symptom were related each other(p<0.01). Therefore it is necessary to education for manage oral symptom affected national exam stress.

  • 골화석증(Osteopetrosis)은 전신의 뼈가 대리석 모양으로 골 경화를 일으키며 골수강의 폐쇄 및 골의 취약성을 보이는 드문 질환이다. 성인형(Adult form)은 우성 유전되며 경미한 증상을 나타낸다. 반면에 유아형(Infantile form)은 열성 유전되면서 심각한 증상을 나타내고, 조기에 사망에 이를 수도 있다. 골화석증은 골격계, 조혈계, 신경계에서 다양한 증상을 유발한다. 본 증례의 목적은 드물게 발생하는 유아형 골화석증을 가진 2 명의 환아를 치료한 후, 그 질환과 치과적 관리에 대해 고찰하고자 함이다. 첫 번째 증례는 골화석증으로 진단된 5세 8개월 남환이었다. 임상 및 방사선 검진 결과 악골 및 치아의 변형을 나타내었으나, 특별한 병적 소견이 없어 예방 치료 및 주기적인 검진 시행하였다. 두 번째 증례는 중증도의 치아우식증을 가진 환아로 의료진과의 협조 하에 수혈한 후 발치하였다. 유아형 골화석증은 다양한 구강 합병증을 유발하며, 질환의 심각성에 의해 치료 자체도 어렵다. 치과의사로서 골화석증 환아를 관리할 때에는 건전한 구강 상태를 유지하기 위한 지속적인 관심과 예방 치료가 필요하며, 아무리 간단한 발치나 치과 술식이더라도 의료진과 협조를 해야 한다.
